Flavobacterium Indologenes에 의한 각막염 1예

목적 : Flavobacterium indologenes는 기회감염의 형태로 매우 드물게 각막염을 일으키는 것으로 알려져 있다. 저자들은 당뇨병의 과거력을 가진 1안에서 Flavobacterium indologenes에 의한 각막염을 경험하여 이를 보고하고자 한다. 대상과 방법 : 각막염이 있었던 1안에 대해 병력청취, 세극등현미경검사, 도말 및 배양검사, 항생제감수성검사 등을 시행하였다. 결과 : 내원시 시행한 각막찰과 도말배양검사에서 Flavobacterium indologenes가 검출되었다. 항생제감수성검사상 감수성이 있다고 나타난 Piperacillin을 점안하여 치료하였고 치료후 시력의 호전과 증상의 개선을 보였다. 결론 : 전신질환이나 안면역성이상이 동반되는 등의 기회감염이 증가하는 상황에서 발생한 각막염의 경우 원인균으로 Flavobacterium indologenes을 고려해 볼 만 하다. Purpose: Flavobacterium indologenes is known to cause keratitis very rarely. Authors have experienced 1 case of keratitis from Flavobacterium indologenes with history of diabetes mellitus, thereby reporting it. Methods: History taking, slit lamp examination, staining and culture, sensitivity test about antibiotics were performed on 1 case of keratitis. Results: Flavobacterium indologenes was detected in staining and culture that was performed on the first visit. Piperacillin was used based on the sensitivity test about antibiotics. Improvement of corneal lesion and symptom was observed with the use of piperacillin. Conclusions: Flavobacterium indologenes can be considered as a casual pathogen in keratitis with condition susceptible to opportunistic infection such as systemic illness or abnormal ocular immunity.

Purpose: Persistent pupillary membrane(PPM) can be classified as four types by means of its morphology. Type 2, which is attached to the lens, is very rare and can cause visual disability. We report 3 cases of type 2 PPM that shows a good visual acuity and cosmetic improvement after removal. Methods: We chose 3 female who showed a poor visual outcome and abnormal shaped pupil since a child. All of 3 case could be examined a hyperplastic persistent pupillary membrane by slit lamp biomicroscopy. Treatment was a surgical removal of PPM, and a surgical removal of PPM and cataract operation in one case of PPM with cataract. Results: All of 3 patient showed a good visual acuity and cosmetic improvement, Conclusion: Surgical management should be also considered to good visual acuity and cosmetic improvement to PPM observed in adult eye.
