DDAVP는 항이뇨 호르몬 유사체로 야뇨증 치료에 사용되고 있다. 그러나 주로 고령의 환자에게 DDAVP 투여 시 저나 트륨혈증이 생긴 예가 다양하게 보고되고 있다. 본 증례에서는 고령의 심부전 환자에서 장기간 DDAVP 투여로 중증 저나트륨혈증과 그로 인한 동기능부전이 발생하였으나 DDAVP를 중단하고 생리 식염수 정주 치료 등의 저나트륨혈증 교정 후에 증상 및 동기능이 모두 호전된 예를 경험하였기에이를 문헌고찰과 함께 보고하는 바이다. Desmopressin diacetate arginine vasopressin (DDAVP) is a synthetic analogue of the arginine vasopressin that is widely used in the treatment of diabetes insipidus, nocturnal enuresis, and polyuria. Although it is generally well-tolerated, DDAVP can cause hyponatremia, especially in elderly patients. There are many reports of DDAVP-induced hyponatremia, but there has been only one case report in which sinus node dysfunction was caused by severe hyponatremia. Here we report a case of sick sinus syndrome that occurred during an episode of severe hyponatremia induced by chronic use of desmopressin in a 91-year-old man who had nocturnal enuresis. T.A.I.D.A 시나리오 플래닝 방법과 교차영향분석을 이용한 U-Eco City 개발과 미래유망기술의 R&D 전략 수립에 관한 연구

A pan-government research and development plan is being prepared to establish the so-called U-Eco City as the national growth- driving industry. This plan will be made possible along with an increasing predictability in the image of future society, survey and prediction in technology demand, and the composition of technology road-map, and the distribution of resources. U-Eco City technologies are a human-centered, creative concept of city through which all the modern technologies such as city design, information-communication technology, ubiquitus technology, environment technology are fused or compounded to construct a new urban space. Thus it is demanded that limited resources in research development fund, research personnel and facilities be efficiently and tactically invested in the technology development in accordance with the principle of choice and concentration. I conducted the research in two stages. In the first step, with using the scenario planning method as the technology forecasting method, bright prospect technology field in the future society and the next prospect technology of future U-Eco City through technology prediction research are chosen. In the second step, results of chosen next prospect technology analyze into setting the priority of future technology in U-Eco City with the Analytic Hierarchy Process (AHP), method of multi-standards decision making, which is using extensively in public area.

Massive pulmonary embolism (PE) is associated with poor prognosis and high mortality. Moreover, patients with massive PE who present with shock have mortality rates ranging from 30% to 50%. Thrombolysis should be administered to patients with massive PE unless there are absolute contraindications to its use. However, treatment failure still occurs, and there is no consensus for the management of massive PE with cardiopulmonary arrest with regard to thrombolysis. In this study, two cases of massive PE with cardiopulmonary arrest are described, both of which were successfully treated with thrombolysis and hemodynamic support, which was administered by extracorporeal membrane oxygenation (ECMO). This report suggests that ECMO may provide safe and adequate cardiopulmonary support in patients with massive PE with refractory thrombolysis and cardiopulmonary arrest. Chest pain and dyspnea are the dominant manifestations in various acute chest conditions. Electrocardiography is one of the most important diagnostic tools used in the differential diagnosis of these conditions. In the majority of cases, electrocardiographic abnormalities suggest cardiac problems. However, there are many non-cardiovascular diseases that may affect electrocardiography. We experienced a case of a 60-year-old man who developed spontaneous pneumothorax associated ST-segment elevation on electrocardiography during the treatment of acute exacerbation of chronic obstructive pulmonary disease. Our case reminds us of the importance of meticulous physical examination as well as accurate analysis of uncommon electrocardiographic alterations.

As the BIM has been issued in AEC industry, its applications have been expanding from design review/clash detection to BIM based engineering and construction management. However, the practical applications in construction site have not meet expectations, due to high infra cost, different processes, lack of information for decision-making on construction phase, etc. Therefore this study offered a BIM application lessons-learned of free form building project for applying BIM in construction site. Also we performed a feasibility study on BIM based cloud computing service for reducing BIM infra cost, and proposed suggestions for expanding BIM application in construction site

This study prepared a basic framework for the development and improvement of Korean Dental Technicians' Licensing Examination, based on actual test questions. A peer review was conducted to ensure relevance to current practices in dental technology. For the statistical analysis, 1000 dental laboratory technicians were selected; specialists in dental laboratory technology (laboratory owners, educators, etc.) were involved in creating valid and reliable questions. Results indicated that examination subjects should be divided into three categories: basic dental laboratory theory, dental laboratory specialties, and a practical examination. To ensure relevance to current practice, there should be less emphasis on basic dental laboratory theory, including health-related laws, and more emphasis on dental laboratory specialties. Introduction to dental anatomy should be separated from oral anatomy and tooth morphology; and fixed prosthodontics should be separated from crown and bridge technology and dental ceramics technology. Removable orthodontic appliance technology should be renamed 'orthodontic laboratory technology'. There should be less questions related to health related law, oral anatomy, dental hygiene, dental materials science and inlay, while the distribution ratio of questions related to tooth morphology should be maintained. There should be a decrease in the distribution ratio of questions related to crown and bridge technology, dental ceramics technology, complete dentures and removable partial dentures technology, and orthodontic laboratory technology. In the practical examination, the current multiple choice test should be replaced with tooth carving using wax or plaster. In dental laboratory specialties, subjects related to contemporary dental laboratory technology should be included in the test items.
