Metabolic Alterations in Parkinson’s Disease after Thalamotomy, as Revealed by 1H MR Spectroscopy

Objective: To determine, using proton magnetic resonance spectroscopy (1H MRS) whether thalamotomy in patients with Parkinson's disease gives rise to significant changes in regional brain metabolism. Materials and Methods: Fifteen patients each underwent stereotactic thalamotomy for the control of medically refractory parkinsonian tremor. Single-voxel 1H MRS was performed on a 1.5T unit using a STEAM sequence (TR/TM/TE, 2000/14/20 msec), and spectra were obtained from substantia nigra, thalamus and putamen areas, with volumes of interest of 7-8ml, before and after thalamotomy. NAA/Cho, NAA/Cr and Cho/Cr metabolite ratios were calculated from relative peak area measurements, and any changes were recorded and assessed. Results: In the substantia nigra and thalamus, NAA/Cho ratios were generally low. In the substantia nigra of 80% of patients (12/15) who showed clinical improvement, decreased NAA/Cho ratios were observed in selected voxels after thalamic surgery (p < 0.05). In the thalamus of 67% of such patients (10/15), significant decreases were also noted (p < 0.05). Conclusion: Our results suggest that the NAA/Cho ratio may be a valuable criterion for the evaluation of Parkinson's disease patients who show clinical improvement following surgery. By highlighting variations in this ratio, 1H MRS may help lead to a better understanding of the pathophysiologic processes occurring in those with Parkinson's disease.

마음인문학에서의 공유(共有, sharing)의 의미는 웹2.0 시대에 개인과 개인 사이의 사회적 관계를 뜻하는 것으로, 특정 관계를 가지고 있는 사람들 간에 존재하는 연결선을 말한다. 즉 마음인문학의 지식자원을 사람들 간에 연결할 수 있도록 하는 매체를 연구하고 실현하는 연구이다. 본 연구에서는 마음인문학 공유분과의 과제와 전망을 통해 마음인문학이 인류 정신문명의 새로운 패러다임을 전개하는 학문으로 성장 발전하고 개인뿐만 아니라 세계인들이 마음인문학의 심층적 연구 성과를 공유하고 활용할 수 있도록 하는데 있다. 즉, 새로운 조화의 정신문명을 창출하기 위해 마음인문학의 공유분과에서 10년 동안 어떻게 연구할 것인지를 단계적으로 제시하고자 한다. 첫째로는 마음인문학 콘텐츠 기반 구축이다. 마음인문학 콘텐츠 자료의 기반 구축으로 이론과 실천의 균형 있는 결합을 통해 마음인문학 콘텐츠 개발의 핵심적 가치를 이끌 것이다. 둘째로는 마음인문학 전자문화지도의 제작 및 적용이다. 새로운 공유 패러다임으로서의 마음인문학, 나아가 마음인문학의 그 변화와 인식 속에서 어떻게 자리매김할 수 있을지, 그리고 궁극적으로는 우리의 시도가 위기에 빠진 인문학의 실용적 대안 모델 가운데 하나가 될 수 있는지를 모색하기 위하여 연구를 진행할 것이다. 셋째로는 마음인문학 전자문화지도 확충 및 유통이다. 마음인문학 전자문화지도를 전문분류체계에 따라 구축해 시간과 공간을 뛰어넘어 살아 숨 쉬는 정보이자 생활문화로 만들어 가는 것이다. 이를 위해 공유분과에서는 마음인문학 디지털 지식자원 구축을 활성화하고 더 나아가 상품으로써 생산, 유통 및 소비될 수 있도록 마음인문학 디지털 지식자원이 다른 디지털 지식자원과 차별되는 정체성을 지니도록 하여야 한다. The Mind Humanities ‘sharing’ is defined as a networking line of group members who share specific goals. It also means social relationships among individuals in the era of web 2.0. In other words, it is considered as a study which enables social members to connect or share knowledge resources of the Mind Humanities. This research aims to develop the Mind Humanities to be a human science presenting a new paradigm of spiritual civilization of mankind through studying issues and prospect of the Mind Humanities sharing. First, this study will draw a core value of the Mind Humanities by combining their theory and practice in building contents of the Mind Humanities. Second, it focuses on producing and adapting electronic cultural atlas of the Mind Humanities, which is thought to find out the change and awareness of a new paradigm of the Mind Humanities sharing and explore a practical alternative model of endangered humanities. Third, it centers on the development and distribution of electronic cultural atlas of the Mind Humanities, which can enhance the quality of daily life beyond time and space. The ‘sharing’ department of the Mind Humanities will try to make built digital knowledge resources be unique products which are produced, circulated and consumed, and bear their unique identity different from other digital knowledge resources.

Using 1H magnetic resonance spectroscopy (MRS), we quantitatively evaluated correlation representing linear relationship between the metabolite peak area ratios associated with poor shimming conditions. The inadequate shimming due to linear shim offsets directly affected overall MR spectral quality as well as peak area for each metabolite. Three major peaks such as N-acetylaspartate (NAA), creatine (Cr), choline (Cho) were used as a reference for data analysis. Despite considerable variations of metabolite peak area, a significant correlation between the metabolite peak area ratios relative to Cr was established while the correlation between the peak area ratios relative to Cho and NAA was not. The present study suggested that metabolite peak area ratios based on the metabolite of Cr could be an acceptable quantification method even under the poor shimming in clinical MRS examination. 영어논문
