The purpose of this study is to review the operative management and outcome of operation for Crohn's disease. The medical records of 17 patients who underwent operations for Crohn's disease at Seoul National University Children's Hospital from January of 1988 to June of 2005 were reviewed. The male-to -female ratio was 1.8: 1. The median age at the onset of symptoms and the time of diagnosis was 9 years 6 months and 11 years 6 months respectively. The median time interval from diagnosis to operation was 2 years and 1 month (0 month~8 years). The ileocolic or ileocecal region was the most common site of involvement. The indications for operation were intractable symptoms (8 cases) and obstruction or stricture (7 cases). The median postoperative hospitalized days were 14.4 days (8~35 days). Five patients (29 %) experienced postoperative complications. Symptom free state or symptom relief was observed in 11 cases after surgery and 6 cases had intermittent episodes of remissions and recurrences. In pediatric Crohn's disease patients who present with intractable symptoms despite medical treatment or develop surgical complications, symptom free state or symptom relief can be achieved by minimal resection of the diseased segment.

Purpose: To investigate the differentiation of rat bone marrow-derived mesenchymal stem cells (MSCs) into hepatocytes by cell transplantation using fibrin gels in a 70% hepatectomized rat model. Methods: MSCs were isolated from Sprague-Dawley rats. MSCs (1.5×107 cells) were mixed with fibrin gels and injected immediately into the abdominal cavity of 70% hepatectomized rats. Fibrin-gels consisted of 500 IU/ml of thrombin and 90 mg/ml of fibrinogen. Transplanted MSCs in the fibrin scaffold were retrieved from surgically opened peritoneal cavities of rats on days 5, 10, 15, and 21 after the operation. The specimens were analyzed histologically and immunohistochemically. Results: On H&E staining, MSCs from hepatectomized rats had changed to a round shape, while MSCs of the control group kept their spindle shape. When the fibrin matrix was biodegraded at day 15, the morphology of the MSCs had changed to hepatocyte-like cells without sinusoids and the hepatocyte-like cells had formed a three-dimensional tissue permitting cell-to-cell contacts within the matrix. On immunohistochemistry, MSCs expressed the hepatocyte markers cytokeratin 18, albumin, and alpha-fetoprotein, after 15 days of transplantation. Conclusion: When bone marrow-derived MSCs are transplanted using fibrin gels in the 70% hepatectomized rat, MSCs differentiate into hepatocyte-like cells and are conglomerated so that they form three-dimensional tissue-like hepatocytes without sinusoids.
幼弱白鼠 肝細胞內 DNA, RNA 및 PAS 陽性物質에 미치는 Chlorambucil과 Ginseng Saponin의 相互作用에 關한 組織化學的 硏究

The effects of interaction between chlorambucil and crude ginseng extract (ginseng saponin) had teen investigated on liver cell of 90 experimental weanling albino rats from 22 healthy primiparous mother rats by means of histochemical detection as RNA, DNA and PAS positive materials 1. Severe regressive changes of the liver tissue, especially on the portal area, induced by intraperitoneal administration of single dose of chlorambucil rated 0.2㎎/0.1㏄ per 20㎎ of body weight had been protected or recovered from the degeneration by following administration of single dose of the crude ginseng extract rated 1.0㎎/0.1㏄ per 20㎎ of the body weight of young. 2. An abrupt declination of PAS positive material in hepatocyte due to use of chlorambucil was contrasted with a gradual inclination of it induced by use of crude ginseng extract, and the effect of the latter appears to be contributed for recovering the lower PAS reaction into that of the normal on the former hepatic cellular change. 3. A declination of stain property or RNA to Azure B was resulted from the use of chlorambucil, while that of DNA was not affected until 24 hours and later it was increased by 36 hours after the administration. 4. A slight decrease of the staining in both the DNA and RNA was recovered the property by 48 hours after the administration of crude ginseng extract. 5. On concomitant administration of both the chlorambucil and crude ginseng extract the staining property of the RNA to Azure B was shown a decrease in the initial stage, and was recovered in that of the normal immediately thereafter without influencing that for DNA.
