        Personalized Medicine의 보험급여 현황과 쟁점

        최근 Personalized Medicine (PM)은 바이오 마 커 진단을 통해 환자를 선별하여 치료하는 맞춤의료법으로 주목을 받고 있다. PM의 종류는 사용 목적에 따라 치료 효과를 높이기 위한 것 과 부작용을 줄이기 위한 것으로 구분할 수 있고, 질병군별로 분류하면 암, 심혈관계 질환, 에이즈, 레트로 바이러스 치료제로 나눌 수 있다. PM은 선별된 그룹에게 치료 반응 률 이 높기 때 문에 효과적인 치료를 가능하게 하고 부작용은 줄이는 강점을 가지고 있다. PM의 이러한 장 점 때문에 의약품 시장에서 PM의 개발은 점차 두드러지고 있고 앞으로 더 많은 PM 의약품이 개발될 것으로 예상된다. 그러나 PM은 새로운 기회를 맞이하는 동시에 새로운 도전에 직면 하 고 있다. 보다 높은 질의 보건의료 서비스를 원하는 환자의 수요를 바탕으로 PM을 개발하여 그 가치를 인정받고자 하는 제약사의 입장과 PM의 경제적 가치의 불확실성 때문에 보험급여를 꺼리는 보험자의 입장 사이에 갈등이 일어날 가능성이 생긴 것이다. 따라서 앞으로 PM의 특성을 잘 파악하여 그 특성에 맞는 보험급여 평가 방법에 대해 논의할 필요가 있을 것이다. Customized medical therapy, Personalized Medicine (PM) which selects the patients based on genetic variation is now spotlighted. If the type of PM is divided by the usage, its usage can be divided into increasing its therapeutic effects and reducing the side effects. Type of PM can also be divided into treatment groups such as anticancer drug, cardiovascular agents, and AIDS antiretroviral drugs. The strengths of PM are increasing therapeutic effects and reducing side effects. These advantages of PM provide sufficient incentives for pharmaceutical companies to develop more PM. However, there are both opportunities and challenges for healthcare reimbursement policy and emerging market. Even with the recognition of certain clinical advantages of PM, payers may be reluctant to invest in PM because of embedded uncertainty on resource savings. It will therefore be necessary to develop reimbursement schemes which reflect the characteristics of PM at the system level. This review is a first step, hopefully, toward such a development.

        한국복지패널 종단자료로 살펴본 건강보험료 체납에 영향을 미치는 요인

        우리나라 건강보험 재정의 대부분은 보험료의 징수를 통하여 조달된다. 그러나 질병에 걸릴 가능성이 낮고 경제적 여력이 부족한 가구는 건강보험료를 장기 체납할 확률이 높고 높은 체납금에 대한 해결능력이 낮다. 본 연구는 한국복지패널 5개년 자료를 이용하여 중위 소득 60%를 기준으로 저소득가구와 일반가구의 건강보험료 체납의 결정요인을 확인하였다. 건강보험료 6개월 이상 체납의 결정요인을 선행 요인, 가능 요인, 질병욕구 요인으로 세분하여 확률효과 로짓모형으로 분석하였다. 일반가구와 비교하여 저소득가구는 체납률이 높고 평균 체납기간이 길었다. 체납의 결정요인은 젊은 가구주, 도시 거주, 지역가입자, 낮은 재산 규모 및 낮은 질병욕구 등인데 이 요인들의 영향은 일반가구보다 저소득가구에서 더 높았다. 본 연구의 결과는 취약계층의 건강보험료 체납 실태를 확인함으로써 현재 활발하게 논의되고 있는 건강보험료 부과체계 개편과 저소득층 지원제도 등 건강보험료 체납가구가 사각지대에 방치되지 않도록 하는 정책 설계를 위한 기초 자료를 제공한다. Contribution evasion of the National Health Insurance (NHI) is a critical problem in Korea which has a social insurance system mostly funded by compulsory contribution. The low-income and healthy population tends to evade NHI contribution and become ineligible to receive benefits. The objective of the present study was to investigate factors associated with the evasion of NHI contribution among low-income households and general households using the Korea Welfare Panel Study data from 2008 to 2012. Among those eligible for NHI, we defined those with income lower than 60 percent of median household income level as low-income households. Random effect logistic models were used for statistical analysis. The results showed that low-income households more often evaded NHI contribution for a longer period of time than general households. The contribution evasion was associated with young breadwinner, living in urban areas, the self-employed insured, less wealthy households, and low disease needs in both groups, and the odds of evasion were higher among low-income households than general households. In conclusion, it is necessary to improve the contribution rating system including contribution reduction on low-income households in order to keep them eligible to receive the benefits of the NHI.

        In this paper, a ROS-based autonomous driving framework designed by team ACCA from the School of Mechanical Engineering, Soongsil University, for the 2021 international college student creative car challenge held in K-city. The autonomous vehicle’s chassis used in this challenge is equipped with Velodyne 3D lidar, a Sick down-looking 2D lidar, Xsens MTi-30 AHRS, CCD camera, webcam, and PC-based controller. First, before the challenge in K-city, we evaluated the ROS-package-based SLAM such as LIO-SAM in a ring-shaped road environment on the campus of Soongsil University. After the successful SLAN and mapping process, the hdl_localization, which is a 3D lidar-based real-time 3D localization package, is used to estimate the global pose with respect to the global frame using NDT scan matching. For lane detection, traffic sign, and traffic signal recognition, the two well-known DNN models are utilized. Based on experimental results from both simulation and an actual autonomous vehicle platform, the Point Instance Network (PINet) for lane detection shows 88% of test accuracy, and the YOLO V4 for the traffic light and sign recognition offers 95% test accuracy.

        자연과 포트폴리오 적용 수업이 초등학생의 과학 정의적 특성과 포트폴리오 인식에 미치는 영향

        The purpose of this study is to examine the effects of the Portfolio applied science instruction on the students' scientific affective domain and perceptions of portfolio in elementary schools. Portfolio applied science instruction of the 6th grade science unit 'Environment pollution and Nature protection' was developed for this study. Traditional instruction was implemented to the control group and portfolio applied science instruction was implemented to the experimental group. Pretests of the scientific affective domain were administered to both groups. The treatment was given for about seven weeks for both groups. Instruments about scientific affective domain were administered to both groups. A questionnaire on perception of portfolio applied science instruction was given to the experimental group after the treatment. The results were analyzed using t-test on the students' scientific affective domain. The results of this study are as follows: 1. Portfolio applied science instruction program for elementary schools was developed. Students themselves determine the portfolio learning goal in a portfolio applied science instruction. Students construct the portfolio and they evaluate themselves and other colleagues. Also teachers go on portfolio applied science instruction considering portfolio purpose, concepts, evaluation. 2. There was not a statistically meaningful difference between an experimental group and a control group on the students' scientific affective domain. In three sub categories of a scientific affective domain, the science perception, the interest on science and scientific attitude, there were not statistically meaningful difference among them. 3. As the results of the questionnaire on perceptions of portfolio, they didn't understand it very well but after learning portfolio, they showed positive attitude to perceptions of portfolio. Students in portfolio applied science instruction like more the portfolio applied science instruction than general instruction. 4. Portfolio applied science instruction has an useful value as a method of teaching and evaluation. Students and teachers can produce various portfolios products in portfolio applied science instruction. As a conclusion, portfolio applied science instruction was not statistically meaningful on the students' scientific affective domain, but it gives positive effects on perceptions of portfolio in elementary schools. Therefore, portfolio has an educational value as a method of teaching and evaluation for students' growth. In the future, teachers and students must have interaction and feedback in portfolio applied science instruction.
