Osteochondral lesion usually occurs in the elbow, knee and ankle joints. Many articles about osteochondral lesion of the talus in the ankle joint have been reported. We experienced a rare case of partially detached osteochondral lesion of the tibial plafond treated with excision and multiple drilling.

Recently, the cases of consignment medical treatments are increasing every year. 1726 cases of consignment medical treatments from January to December 2010 were analyzed for this study. A purpose of this study is to find ways to improve consignment medical treatments. A hand injury is ranked as the most frequent illness/disease in 2009, and CRPS is ranked as the 4th. These diseases, compared to a benign tumor(3rd) and a malignant tumor(5th), have higher chance of being cured in armed forces hospitals, when human resources and facilities are supplemented. In order to treat hand injuries at the Armed Forces Capital Hospital, two medical specialists took training courses at a civilian hospital. Treatment of patients with hand injuries started in May 2010. The total number of consignment medical treatment in 2010 were 805, and among those, 48 cases were related to hand injury treatment(5.96%). The percentage decreased by 1/3 compared to that of 2009. Therefore, as confirmed by this study, it is urgent that human resources and equipments are adequately provided in certain medical fields. This kind of effort will not only lead to a cost decrease in consignment treatment but also enhance a status of armed forces hospitals.

상완골 두 치환술을 이용한 회전근 개 파열 관절증의 치료

목적: 회전근 개 파열 관절증의 치료로 상완골 두 치환술의 효용성을 조사하고자 한다. 대상 및 방법: 1997년부터 2003년까지 회전근 개 파열 관절증으로 상완골 두 치환술을 받은 11예를 대상으로 하였다. 평균 연령은 69세였고, 9예가 여자였으며 평균 추시기간은 44개월이었다. 동통, 일상생활 능력, 견관절 운동 범위 및 환자의 만족도에 대해 수술 전과 후를 조사하였다. 결과: 동통은 수술 전 9.0에서 2.2로 감소하였으며 전방 거상은 평균 40도에서 82도로, 외회전은 평균 8.2도에서 30도로 증가하였다. 평균 ASES 점수는 22점에서 72점으로 증가하였으며 총 11예 중 9예(82%)에서 동통 완화 및 일상생활 능력의 향상에 대해 만족한 결과를 나타냈다. 결론: 회전근 개 파열 관절증의 치료로 상완골 두 치환술은 동통 완화 및 기능적 활동의 개선에 효과적인 수술방법이라고 생각한다. Purpose : To assess the effectiveness of a prosthetic replacement of the humeral head in rotator cuff tear arthropathy patients. Materials and Methods : 11 patients underwent a prosthetic replacement of the humeral head for rotator cuff tear arthropathy between 1997 and 2003. The average age was 69 years and 9 out of the 11 patients were women. The average follow-up was 44 months. The level of pain, daily activities, range of motion and patient's satisfaction were assessed both preoperatively and postoperatively. Results : The mean pain score decreased from 9.0 preoperatively to 2.2 postoperatively. The mean active forward elevation increased from 40° to 82° and the mean external rotation increased from 8.2° to 30°. The mean ASES score improved from 22 to 72. Nine patients were satisfied with the surgical outcome, particularly in terms of pain relief and the improvement in their daily activities. Conclusion : A prosthetic replacement of the humeral head is an effective treatment option for pain relief as well as for improving the functional activity in rotator cuff tear arthropathy.
