        복부 대동맥류의 임상적 경험

        A total and consecutive 87 patients underwent aortic valve replacement[AVR with the St. Jude Medical prosthesis between 1984 and 1993. Age ranged from 14 to 66 years[mean:38.6$\pm$ 14.0 years .Twenty-one patients [24.1% had undergone previous valve replacement. There were 8 early deaths with an operative mortality rate of 9.2% [7.6% for primary AVR and 14.3 % for re-replacement AVR . Seventy-nine early survivors were,followed for a total of 309.1 patient-years[mean:3.9$\pm$ 2.5 years . A late mortality rate was 5.1% [4 patients or a linearized incidence of 1.294 %/patient-year. All were anticoagulated with coumadin to maintain the international normal ized ratio[INR between 1.5 and 2.5. One patient experienced thromboembolism[0.324%/patient-year , and none did bleeding. Endocarditis occurred in one[0.324%/patient-year . Paravalvular leak was the most frequent complication and was experienced by 8 patients[2.588%/patient-year , and 5 of them required re-replacement AVR[1.618 %/patient year of reoperation rate . There was no structural failure of the prosthesis. Actuarial survival including operative death was 83.9%$\pm$ 4.6% at 10 years.The actuarial estimates of freedom from thromboembolism and of freedom from late death and all complications were 95.1% $\pm$ 4.8 % and 81.4% $\pm$ 6.1%, respectively, at 10 years. These clinical results suggest that less intensive anticoagulation may be allowed for patients of AVR with the St. Jude Medical valve with low incidences of both thromboembolic and bleeding complications.

        Localized pleural mesothelioma -수술치험 1예-

        Pleural mesothelioma is the tumor of the cell of mesodermal origin lining the pleura. It is relatively uncommon tumor and its localized form is much rarer than diffuse form. Authors experienced a localized mesothelioma in a patient who was 44 year old male worker at copper plumbing field for 20 years, and admitted due to incidentally found abnormal chest X-ray. Exploratory thoracotomy was done and a 23 x 16 x 8 cm sized solitary mass was resected with adherent right middle lobe. Low grade malignancy of the pleural mesothelioma was confirmed by the pathology. We report the case with a literature review.

      • 도시철도 지하 수 · 변전 시스템의 철공진 현상에 대한 사례연구

        전력 계통 망에서 외란 현상들은 전력 품질에 영향을 주는 중요한 요소로 인식되고 있다. 이러한 장애 요소들 중에서, 철공진 현상은 수·변전 시스템의 전력 품질을 저하시키고 시스템의 안정상 문제점을 일으킬 수 있는 외란 중 한가지이다. 철공진 현상은 전력계통 운영중에 나타나 과도한 전압과 전류를 수반하는 복잡하고 특이한 현상 중 하나이며, 계기용 변압기의 소손, 금속산화물 저항의 파괴 그리고 전력 계통과 전력기기에 심각한 결과를 초래할 수 있다. 본 논문은 도시철도 수변전 시스템에서 발생되는 철공진 현상을 이론적인 모델링을 통해 해석하였으며, 실제 현장에서 발생한 문제점을 바탕으로 실증적인 해결방안을 연구하였다. 그리고, 지하화된 수·배전 방식을 채택하고 있는 도시철도 전력 계통에서 철공진에 의한 계기용 변성기의 손상 사례와 이에 대한 대책을 제시하였다. Power disturbances have become an important factor on the increase through electrical networks. Ferroresonance is one of these disturbances that can occur on distribution systems, affecting power quality and security problems. Ferroresonance is a particularly complex case of disturbance that involves high levels of over-voltage and over-current distortion in operation, of which phenomenon can be a devastating blow to voltage transformers,power system and electric equipment. This paper is to present an analysis of Ferroresonance, its theoretical modeling aspect, and to conduct research of ironing it out based on an empirical study in the operating field. Furthermore, actual cases that are the destroyed voltage transformers owing to Ferroresonance are depicted. In addition, the appropriate preventive solution for unpredicted accidents may come about within the underground power distribution of urban rail way substation is proposed.

        소지역 단위의 주택시장 불안정 진원지 파악에 관한 연구

        본 연구에서는 서울특별시를 대상으로 아파트 실거래가 자료를 활용하여 주택가격의 급상승기와 급하락기의 법정동별(소지역 단위) 주택시장의 이질성을 파악하고, 거래밀도분석을 통하여 거래집중지역의 거래주택 특성을 개략적으로 도출하였다. 나아가 주택시장의 불안정을 유발하는 진원지로서 국지적 핫스팟지역과 콜드스팟을 탐지하였다. 분석결과, 시군구 단위의 주택가격지수는 하위주택시장의 특성을 평활화하는 문제가 발견되었으며 이를 근거로 법정동별 주택시장의 이질성과 소규모 단위의 기초통계 생산의 필요성을 도출할 수 있었다. 또한, 커널밀도추정과 핫스팟 분석을 통해 거래가 집중적으로 발생하고 주택가격의 불안정을 유발하는 진원지를 파악할 수 있었다. 더구나 두 분기별로 결과를 비교하면 주택정책과 주택시장 상황에 따라 그 진원지가 변화하는 것으로 나타났다. 향후, 분석범위지역을 확대하여 서울 이외의 지역에 대한 공간통계기법에 대한 검증이 요구되며, 거래집중지역의 선정 시 통계적인 접근방법, 핫스팟으로 탐지된 지역의 검증과정 및 집중적인 분석이 필요할 것이다. 이 논문은 지리정보시스템을 활용하여 실거래 자료의 다양한 분석 방법을 적용한 의의가 있다. Fundamental purpose of this paper is to identify spatial origins of housing market unstability based on the small spatial unit. The hot-spot and cold-spot areas derived from spatial statistical techniques were considered as the origin of market unstability. For this purpose, market price data of Seoul was collected and analyzed using real transaction data. The main findings are follows. First, it was proved that the current statistical unit is too large to identify origin areas of market unstability because it contains different market price trends in the same unit of analysis. Therefore, a smaller unit of statistical area, such as the primary administration unit, is necessary to identify more accurate market trend. Second, through kernel density estimation and hotspot analysis, it was able to find the origins of market unstability in the region where transactions occur intensively. Based on these findings, this paper recommends some policy directions to utilize the market price data more effectively.

        공공 및 민간개발 주체별 주거지 개발의 특성 비교

        The primary purposes of this study is to identify the characteristic of residential land development in urban areas to derive some useful implications for urban development policy. The characteristics of the urban development by public and private sector were identified by comparing main principles of smart growth, such as high density development, jobs-housing balance, transit-oriented development. The results tell us that the location of residential site development by public sector is farther away from existing urban area and main roads than that of by private sector. Furthermore, public sector is more responsible for open space loss than private sector. Finally, planned developments by public sector provide more green area than unplanned private development. Based on these findings, this study suggests that public sector should identify Korean type of smart growth principles and apply them in urban development.

        최근 5년간의 흉부손상 경험 -481 예-

        From January 1981 through December 1985, 481 thoracic civilian injuries were reviewed in the Department of Thoracic Surgery, Paik Hospital in Seoul. Sixty two percent of the injuries were caused by traffic accident, 18% fall down, 15% blunt trauma, 2% crushing injury, 2% stab wound, and 0.4% gunshot wound. Peak incidence of the trauma victim was fourth and fifth decades revealing 22% and 27% respectively. Sex ratio was 3.5:1 with male predominance. Elapsed time before admission was less than one hour in 36% and one to six hour in 30%. The types of the injuries were as follows: Non-penetrating injuries were the most part of the wounded, 97.6%. Rib fracture was the most common lesion occupying 292 patient out of 481 [61%]. Of these 292 patients, 72% was multiple rib fracture. The incidence of hemothorax or hemopneumothorax was 19% [102 patients] [Table 4]. Most common associated condition was head injuries, 98 patients [14%]. Thoracoabdominal injuries were seen in 31 patients [0.6%]. Tube thoracostomy was the definitive measures in the 20% of the wounded. Open thoractomy was performed in 5%. Additional procedures for the associated condition were done in the 16% of the cases, for example, reduction of long bone fracture and trephination for the head injury. Among 481 wounded, fatal complication occurred in 13 patients [2.7%]. This paper has also compared two series of patients according to period; one from 1970 to 1980 and the present series [Table 8]. Conclusively, the fatal complications or trauma death may be reduced by the effort 1] rapid transport of the victim, 2] initial correction or resuscitative measures of the circulatory and ventilatory deficit 3] early decision of definitive thoracostomy or thoracotomy and 4] proper prioritizing for the care of the multiple critically injured patient.

        남부평야지 이모작에서 벼 무논점파 재배시 파종기에 따른 생육 및 수량

        Field experiments were conducted to identify the safe seeding date for rice wet hill direct seeding on puddled paddy surface after barley or wheat cultivation in Honam plain area. Two Korean bred-cultivars, Unkwangbyeo (early maturing) and Dongjin2 (mid-late maturing), were seeded 4 times by 10-day interval from May 25 to June 25 in 2012 and 2013. Seedling stand decreased as seeded later, but it was in optimal range in all the seeding dates. Heading date of the last seeding of Unkwangbyeo on June 25 was August 28, and the last seeding of Dongjing2 on June 25 was September 2, which was within the limit date of heading (September 3). Panicle numbers per m2 were not significantly different until the third seeding on June 15, but it decreased in the seeding date on June 25. Number of grains per panicle decreased as seeded later. Milled rice yield and head rice ratio was also decreased as seeded later, and were the lowest in the last seeding date on June 25. Protein content of Unkwangbyeo was similar regardless of seeding dates, but it increased at later seeding dates in Dongjin2. According to the above results, the seeding date would be recommended within June 25 for Unkwangbyeo, June 15 for Dongjin2 in wet hill direct seeding on puddled paddy in Honam double cropping area.

        연구보문 : 자연과학; 벼 재배양식간 간단관개에 따른 물 절약 효과

        To verify the effect of intermittent irrigation on water saving, two irrigation regimes, intermittent irrigation (II) and conventional irrigation (CI), were applied to machine transplanting (TP) and wet-hill-seeding (WHS) in 2009 and 2010. In intermittent irrigation plots, alternate flooding was applied from 20 days after transplanting and from 30 days after direct seeding. Conventional irrigation was imposed by continuous flooding including broken irrigation. The amount of irrigation water in II plots was 22% and 24% lower than that in CI plots of machine transplanting and wet-hill-seeding cultivation, respectively. Water productivity of intermittent irrigation plots ranged from 0.73 and 0.79. There were no significant different in plant growth and milled rice yield between the irrigation methods.
