        후방경유요추 수술 후 근육 관련 효소의 변화

        Objective:Creatine phosphokinase-MM(CPK-MM) and lactate dehydrogenase(LDH) are well known indices of muscular injury. To know the degree of muscular injury during spinal surgery, the author report serial measurement of serum CPK-MM and LDH4 level. Methods : The authors investigated 23 patients who underwent lumbar spinal surgery without bone fusion or instrumentation. Peripheral venous blood samples were serially collected 1 day before surgery, operation day, postoperative 3rd, 7th, 14th and 28th day. Postoperative back pain was measured by visual analogue scale. We evaluated the relationship between number of operation level, serial changes of serum CPK-MM, LDH4, duration of surgery and postoperative back pain. Results : CPK-MM activity was higher after surgery than before it and reached at maximal level on the postoperative 3rd day, and it was returned to normal level on the postoperative 7th day. The score of postoperative back pain scale was the highest on the postoperative 3rd day. CPK-MM activity was significantly correlated with operation level, duration of surgery, and postoperative back pain(P<0.05). In contrast, serum level of LDH4 showed no significant correlation with duration of surgery(P>0.05). Conclusion : Postoperative muscle injury is inevitable in all patient who underwent spinal surgery, and these injuries are related extent of exposure and duration of surgery. To reduce muscle injury and postoperative back pain, less invasive and shorter surgery in time is recommend. Key words:Creatine phosphokinase MM;lactate dehydrogenase;Postoperative back pain.

        In the treatment of tuberculous spondylitis, posterior spinal fusion is more simple and safe procedures than anterior spinal fusion. We performed bilateral anterior spinal fusion with autologous iliac bone graft and concomitant with psoas muscle abscess drainage and antituberculosis drug. As a result, patient was possible early ambulation and he had no recurrence, no complication such as resorption of bone fragment and kyphotic angle change. This article describeds the advantage of bilateral anterior approach in tuberculous spondylitis wish psoas abscess.

      • 다형성 교모세포종에서 광범위 전적출술후 단기 재발된 치험 1예

        Glioblasty/ma Multiforme(GBM) is the one of the most common malignant tumors in central nervous system(CNS). The incidence of the tumor is about 12% of all CNS tumors and about 50% of all intracranial tumors. Though the development of the microsurgical technique, radiosurgical technique, and chemotherapy works effectively, complete cure of the GBM has not been achieved. It has been known that the prognosis of GBM depends on the extent of surgical tumor removal, remained tumor volume, radiation dose, and chemotherapy. Not only patient's age but also tumor characteristics such as MRI enhancement, angiogenesis and necrosis are also very important prognostic factors. Authors experienced very aggressive GBM in aged woman, who received gross total tumor removal, but refused other therapeutic motalities postoperatively. Very rapid recurrence of GBM and global cerebral edema in aged woman should be considered fatal. This case demonstrates the effectiveness of aggressive removal and the limited usefulness of neurosurgical tools in GBM recurrence. Authors would suggest that the early diagnostic effort and the verified neurosurgical therapy for malignant tumor are still strongly requested to make better prognosis.

        점유율을 이용한 대기행렬길이 추정 알고리즘 개발

        본 연구는 대기행렬길이를 보다 정확하게 측정하는데 목적을 두고 있다. 현재 서울시 실시간 신호제어시스템(COSMOS)에서 적용되는 대기행렬산출 알고리즘은 평균차량길이와 점유시간을 이용하여 속도를 산출하고 이를 다시 밀도 개념의 정체도로 환산하여 대기행렬길이를 구하고 있다. 평균차량길이에 의한 속도산출로 인하여 오차가 발생되고, 운영자가 지정해 주어야하는 값이 산재되어 그 값을 최적화하기가 어려워 정확한 대기행렬길이를 구하기가 어렵다. 따라서 본 연구에서는 정지선으로부터의 거리와 대기검지기에서부터 수집되는 점유율 간의 관계를 밝혀, 이를 대기행렬길이 산출 알고리즘에 적용하였다. 대기행렬길이 추정 알고리즘을 사용할 경우 루프 검지기에서 많은 가공단계가 필요하지 않은 기본 데이터인 점유율을 이용하여 대기행렬길이를 예측할 수 있다고 할 수가 있다. 그리고 운영자 지정값이 없어 현장 적용에 있어서 그 값에 대한 최적화 작업이 필요가 없어 쉽게 적용할 수 있는 장점이 있다. 개발된 대기행렬길이 추정알고리즘을 COSMOS시스템이 운영중인 사이트(Site)에 적용할 결과 현재 사용 중인 대기행렬길이 추정 알고리즘 보다 우수한 것을 확인 할 수가 있었다. 하지만, 본 연구에서 개발된 대기행렬길이 추정 알고리즘을 일반적으로 적용하기 위해서는 다양한 현장 및 경우에 대하여 적용하여 검증을 하여야 할 것이다. 이에 관련한 연구가 향후 진행될 경우 현재 대기행렬길이 추정 알고리즘 보다 적용이 쉽고, 정확한 값을 얻는 알고리즘으로 완성될 것으로 기대한다. The purpose of this research is how to estimate the traffic queue length in the signal intersection accurately. The current traffic queue length algorithm in COSMOS has been using the congestion diagram which comes from the speed of an average separated vehicle - using average vehicle length and the occupancy time from loop detectors. So some errors were occurred by the speed estimation method using average vehicle lengths. And Operators had been difficult to optimize some variables for measuring the traffic queue length estimation algorithm in COSMOS. Therefore the traffic queue length estimation algorithm on the basis of the relation between distances and occupancy rates from loop detectors was developed in this thesis. This thesis had the advantage of using occupancy rates which came out from loop detectors easily and no need to optimize some variables for the established algorithm in COSMOS. And the results of testing this algorithm in some sites which had installed COSMOS system showed better results than COSMOS system's results. But it was noted that further studies which carry it out in various sites and under various cases are necessary for applying to actual intersections.
