        Long-term Observation of Gastric Adenocarcinoma of Fundic Gland Mucosa Type before and after Helicobacter pylori Eradication: a Case Report

        Gastric adenocarcinoma of the fundic gland mucosa type (GA-FGM) was proposed as a new variant of gastric adenocarcinoma of the fundic gland type (GA-FG). However, at present, the influence of Helicobacter pylori and the speed of progression and degree of malignancy in GA-FGM remain unclear. Herein, we report the first case of intramucosal GA-FGM that was endoscopically observed before and after H. pylori eradication over 15 years. The lesion showed the same tumor size with no submucosal invasion and a low MIB-1 labeling index 15 years after its detection using endoscopy. The endoscopic morphology changed from 0-IIa before H. pylori eradication to 0-IIa+IIc and then 0-I after H. pylori eradication. These findings suggest that the unaltered tumor size reflects low-grade malignancy and slow growth, and that the endoscopic morphology is influenced by H. pylori eradication.

        Atmosphere-forest Exchange of Ammoniacal Nitrogen in a Subalpine Deciduous Forest in Central Japan during a Summer Week

        The present study aimed to investigate the diurnal variations in air concentrations and exchange fluxes of ammoniacal nitrogen (NHx: ammonia (NH_3) and particulate ammonium) in a subalpine deciduous forest in central Japan during a week in summer. The NH_3 concentrations (0.50 μg N m^-3 on average)showed a clear circadian variation, i.e., high and low in the daytime and nighttime, respectively. The concentration of particulate ammonium in the coarse fractions was extremely low, whereas that for the PM2.5 fraction was relatively high (0.55 μg N m^-3 on average). The main inorganic ion components of PM2.5 at the study site were ammonium and sulfate. The exchange fluxes of NHx were bidirectional. Both the maximum and minimum values occurred in the daytime, i.e., 0.39 mg N m^-2 hr^-1 of downward flux and 0.11 mg N m^-2 hr^-1 of upward flux for NH_3 and 0.25 mg N m^-2 hr^-1 of downward flux and 0.13 mg N m^-2 hr^-1 of upward flux for PM2.5 ammonium. The exchange fluxes of NHx at night could be considered as zero. The mean deposition velocity during the research period was almost zero for both NH_3 and PM2.5 ammonium. The atmosphere-forest exchange of NHx in the forest during the study period was balanced. The remarkably large deposition of NHx was attributable to meteorological events such as showers the night before that thoroughly washed the forest canopy and subsequent clear skies in the morning,which enhanced convection. The cleaning effect of rainfall and the rapid change in convection in the early morning should be monitored to evaluate and generalize the gas and particle exchange in a forest.

        miniTAO/ANIR Paα SURVEY OF LOCAL LIRGs

        ANIR (Atacama Near InfraRed camera) is a near infrared camera for the University of Tokyo Atacama 1m telescope, installed at the summit of Co. Chajnantor (5,640 m altitude) in northern Chile. The high altitude and extremely low water vapor (PWV = 0.5 mm) of the site enable us to perform observation of hydrogen $Pa{\alpha}$ emission line at $1.8751{\mu}m$. Since its first light observation in June 2009, we have been carrying out a $Pa{\alpha}$ narrow-band imaging survey of nearby luminous infrared galaxies (LIRGs), and have obtained $Pa{\alpha}$ for 38 nearby LIRGs listed in AKARI/FIS-PSC at the velocity of recession between 2,800 km/s and 8,100 km/s. LIRGs are affected by a large amount of dust extinction ($A_V$~ 3 mag), produced by their active star formation activities. Because $Pa{\alpha}$ is the strongest hydrogen recombination line in the infrared wavelength ranges, it is a good and direct tracer of dust-enshrouded star forming regions, and enables us to probe the star formation activities in LIRGs. We find that LIRGs have two star-forming modes. The origin of the two modes probably come from differences between merging stage and/or star-forming process.

      • Can Conventional Magnetic Resonance Imaging Substitute Three-Dimensional Magnetic Resonance Imaging in the Diagnosis of Lumbar Foraminal Stenosis?

        Study Design: Retrospective radiological comparative design.Purpose: To investigate whether conventional magnetic resonance imaging (MRI) could substitute three-dimensional (3D)-MRI for the calculation of the foraminal stenotic ratio (FSR) and clarification of which patients can be assessed more accurately using 3D-MRI.Overview of Literature: Previous studies have indicated that 3D-MRI is useful for diagnosing lumbar foraminal stenosis. The FSR obtained using 3D-MRI, described as the ratio of stenosis length, characterized by perineural fat obliteration, to the length of the entire foramen, could indicate the stenosis severity; however, this method is time-consuming and expensive. The FSR also can be calculated using conventional MRI.Methods: We investigated 154 foramina at L5–S1 in 77 patients. All the patients had degenerative lumbar disorders and had undergone both conventional MRI and 3D-MRI during the same visit. Differences between the FSRs calculated from conventional and 3D-MRI reconstructions and any correlations with the plain radiography findings were assessed.Results: In foramina that had a FSR of <50% on conventional MRI, the difference between the FSR obtained using conventional MRI and 3D-MRI was 5.1%, with a correlation coefficient of 0.777. For foramina with a FSR ≥50% on conventional MRI, the difference was 20.2%, with a correlation coefficient of 0.54. FSR obtained using 3D-MRI was significantly greater in patients who required surgery than in those who were successfully treated with conservative methods (88% and 42%, respectively). Segments with spondylolisthesis or lateral wedging showed higher FSRs than those without these conditions on both types of MRI.Conclusions: FSRs <50% obtained using conventional MRI were sufficiently reliable; however, the results were inaccurate for FSRs ≥50%. Patients with high FSRs on 3D-MRI were more likely to require surgical treatment. Therefore, 3D-MRI is recommended in patients with suspected stenosis detected using conventional MRI or plain radiographs.

        Deposition of Aerosols on Leaves in a Cool-temperate Larch Forest in Northern Hokkaido, Japan

        Aerosol concentrations at the CC-Lag site in the Teshio Experimental Forest increased from winter to spring and sometimes showed extremely high values associated with Kosa and/or forest-fire events. The range and mean of the mass concentrations of aerosol chemical species were as follows: total particulate mass, 1.2-29, 5.0; elemental carbon, 0.061-2.2, 0.43; organic carbon, 0.059-3.5, 0.79; and sulfate, 0.12-6.2, 1.8 ${\mu}g/m^3$. The total masses of the deposited particles on hybrid larch and on bamboo leaves were approximately 35 and 30 ${\mu}g/cm^2$, respectively. The amounts of soil particles on the leaves were 6 ${\mu}g/cm^2$ for the upper part of hybrid larch, 2 ${\mu}g/cm^2$ for the lower part of hybrid larch, and 1 ${\mu}g/cm^2$ for Sasa bamboo leaves. The amounts of deposited black carbon were 2.3 ${\mu}g/cm^2$ for the upper part of hybrid larch, 0.6 ${\mu}g/cm^2$ for the lower part of hybrid larch, and 0.2 ${\mu}g/cm^2$ for Sasa bamboo leaves. Half of the total deposited particular mass was attached on the hybrid larch; however, most of the total deposited mass was adhered on the Sasa bamboo leaves. Regardless of the species, there tend to be more deposited particles on the leaves in the upper part than in the lower part, with only a few meters height difference. Comparing the composition of the deposited particles to that of the atmospheric aerosols without any size cut, the fractions of water-soluble material sulfate and sea salt in the deposited aerosols were about one tenth and one hundredth lower than that in the aerosols, respectively. On the basis of the measured concentration and the deposited amount on leaves, the deposition velocity of black carbon was estimated to be approximately 0.5 cm/s.

        Possible Prevention of Neonatal Death: A Regional Population-Based Study in Japan

        Purpose: The neonatal mortality rate in Japan has currently been at the lowest level in the world. However, it is unclear whether there are still some potentially preventable neonatal deaths. We, therefore, aimed to examine the backgrounds of neonatal death and the possibilities of prevention in a region of Japan. Materials and Methods: This is a population-based study of neonatal death in Shiga Prefecture of Japan. Results: The 103 neonatal deaths in our prefecture between 2007 and 2011 were included. After reviewing by a peer-review team, we classified the backgrounds of these neonatal deaths and analyzed end-of-life care approaches associated with prenatal diagnosis. Furthermore, we evaluated the possibilities of preventable neonatal death, suggesting specific recommendations for its prevention. We analyzed 102 (99%) of the neonatal deaths. Congenital malformations and extreme prematurity were the first and the second most common causes of death, respectively. More than half of the congenital abnormalities (59%) including malformationsand chromosome abnormality had been diagnosed before births. We had 22 neonates with non-intensive care including eighteen cases with congenital abnormality and four with extreme prematurity. Twenty three cases were judged to have had some possibility of prevention with one having had a strong possibility of prevention. Among specific recommendations of preventableneonatal death, more than half of them were for obstetricians. Conclusion: There is room to reduce neonatal deaths in Japan. Prevention of neonatal death requires grater prenatal care by obstetriciansbefore birth rather than improved neonatal care by neonatologists after birth.

        Prompt gamma detection for range verification in proton therapy

        It is an on-going challenge to verify the proton range in situ during proton therapy. Since the protons stop in target tissue, measurement of gamma-rays emitted either promptly from nuclear de-excitation or in pair from positron annihilation is the feasible method to monitor the proton range in-vivo. Using the technique of gamma collimation, we empirically demonstrated that the proton range and prompt gamma distribution are well correlated in the therapy energy range, and that measuring prompt gammas is a viable method for the clinical application. However, this collimation technique appears not to be applicable to passively scattered proton beams. The device chosen for gamma imaging in 2D is an electron tracking Compton camera, which images single-emission photons employing a gas chamber to induce Compton scattering. Images of prompt gammas were attained at the proton beam energy of 140 MeV. Measurements showed that gamma image in the energy range of 800e2000 keV provides a better match with the proton range compared to the image by lower energy gammas.
