        The influence of hypophosphatemia on outcomes of low- and high-intensity continuous renal replacement therapy in critically ill patients with acute kidney injury

        Background: The purpose of this study was to assess the role of hypophosphatemia in major clinical outcomes of patients treated with low- or high-intensity continuous renal replacement therapy (CRRT). Methods: We performed a retrospective analysis of data collected from 492 patients. We divided patients into two CRRT groups based on treatment intensity (greater than or equal to or less than 40 mL/kg/hour of effluent generation) and measured serum phosphate level daily during CRRT. Results: We obtained a total of 1,440 phosphate measurements on days 0, 1, and 2 and identified 39 patients (7.9%), 74 patients (15.0%), and 114 patients (23.1%) with hypophosphatemia on each of these respective days. In patients treated with low-intensity CRRT, there were 23 episodes of hypophosphatemia/1,000 patient days, compared with 83 episodes/1,000 patient days in patients who received high-intensity CRRT (P < 0.01). Multiple Cox proportional hazards analysis showed that Acute Physiology and Chronic Health Evaluation (APACHE) III score, utilization of vasoactive drugs, and arterial pH on the second day of CRRT were significant predictors of mortality, while serum phosphate level was not a significant contributor to mortality. Conclusion: APACHE score, use of vasoactive drugs, and arterial pH on the second CRRT day were identified as significant predictors of mortality. Hypophosphatemia might not be a major risk factor of increased mortality in patients treated with CRRT.

        Evaluating the Safety and effectivenesS in adult KorEaN patients treated with Tolvaptan for management of autosomal domInAnt poLycystic kidney disease (ESSENTIAL): short-term outcomes during the titration period

        Background: Tolvaptan reduces height-adjusted total kidney volume (htTKV) and renal function decline in autosomal dominant polycystic kidney disease (ADPKD). This study was aimed at investigating the efficacy and safety of tolvaptan in Korean patients with ADPKD during the titration period. Methods: This study is a multicenter, single-arm, open-label phase 4 study. We enrolled 108 patients with ADPKD (age, 19-50 years) with an estimated glomerular filtration rate (eGFR) of >30 mL/min/1.73 ㎡ and factors defined as indicative of rapid disease progression. After tolvaptan titration, we evaluated efficacy and side effects and assessed factors associated with the effects. Results: After titration for 4 weeks, eGFR and htTKV decreased by 6.4 ± 7.9 mL/min/1.73 ㎡ and 16 ± 45 mL/m, respectively. No serious adverse drug reactions were observed during the titration period. The greatest eGFR decline was observed in the first week, with a starting tolvaptan dose of 45 mg. Multivariate linear regression for htTKV decline showed that the greater the change in urine osmolality (Uosm), the greater the decrease in htTKV (β, 0.436; p = 0.009) in the 1D group stratified by the Mayo Clinic image classification. Higher baseline eGFR was related to a higher htTKV reduction rate in the 1E group (β, -0.642; p = 0.009). Conclusion: We observed short-term effects and safety during the tolvaptan titration period. The decline of htTKV can be predicted as a short-term effect of tolvaptan by observing Uosm changes from baseline to end of titration in 1D and baseline eGFR in 1E groups.

        Self-concept molds choice experiences among multiple alternatives: An fMRI study

        This study addresses the relationship between individual differences regarding self-concept, measured by Self Concept Clarity (SCC) scale and Relational-Interdependent Self-Construal and relationships (RISC) scale, and diverse affective consequences after choice behavior. We hypothesized that lower self-concept clarity and higher relational interdependence would be related to increased susceptibility to choice context-for example, how a choice set is constructed based on one's initial preference. We examined how variations in a choice set can produce different affective consequences after making choices, and investigated the underlying neural mechanism using fMRI. In this experiment, participants first rated their preferences for art posters, and made a series of choices from a presented set. After the choice task, they completed post-choice measures including preferences for the chosen posters, as well as measures of their self-concept clarity and relational interdependence. Our behavioral results demonstrated that when participants faced more conflicting choice context, self-concept clarity was related to more positive affective consequences after choice, whereas relational interdependence was correlated with a lower second-rated attractiveness of the chosen option. The neuroimaging analysis of choice-making revealed that self-concept clarity and the degree of their relational interdependence served as modulators in shaping how one perceives and experiences the same decision-making process. These results have theoretical and practical importance in that it is one of the first studies investigating the influence of the individual differences regarding self-concept on value-based decision making process among diverse choice set contexts.

        Estimation of Reward Probability in the Fronto-parietal Functional Network: An fMRI Study

        We investigated the neural representation of reward probability recognition and its neural connectivity with other regions of the brain. Using functional magnetic resonance imaging (fMRI), we used a simple guessing task with different probabilities of obtaining rewards across trials to assay local and global regions processing reward probability. The results of whole brain analysis demonstrated that lateral prefrontal cortex, inferior parietal lobe, and postcentral gyrus were activated during probability-based decision making. Specifically, the higher the expected value was, the more these regions were activated. Fronto-parietal connectivity, comprising inferior parietal regions and right lateral prefrontal cortex, conjointly engaged during high reward probability recognition compared to low reward condition, regardless of whether the reward information was extrinsically presented. Finally, the result of a regression analysis identified that cortico-subcortical connectivity was strengthened during the high reward anticipation for the subjects with higher cognitive impulsivity. Our findings demonstrate that interregional functional involvement is involved in valuation based on reward probability and that personality trait such as cognitive impulsivity plays a role in modulating the connectivity among different brain regions.

        The Effects of Generative Concept Map on Science Learning Achievement and Cognitive Load

        This study investigated the effect of generative concept maps according to learning achievements and cognitive load. A total of 78 students in the first grade of middle school participated in this study. Before the experimental treatment was implemented, students had to fill out a questionnaire assessing prior knowledge. The study was designed where all the students were presented the same learning contents regarding photosynthesis; however, the two experimental groups were provided with different concept map methods: a learner-generative concept map (GCM) and an instructor-provided concept map (PCM). GCM students were asked to make a concept map by themselves in small groups while they are reading material. PCM students were instructed to study in small groups in order to read the material; however, they were provided a concept map developed by their teacher. The control group (CG) had the teacher present the learning contents in traditional lecture format with no accompanying concept map. The results show that there were significant differences in the achievements among the groups. CG showed higher achievement than both the experimental groups. There was also a significant difference in cognitive load. Although the GCM group did not obtain higher achievement than the other groups, the GCM group showed higher mental effort and lower physical fatigue than the other groups. The GCM group might have invested more effort to find and connect ideas when drawing their concept map with peers which is unlike the conditions for the PCM group and CG. In conclusion, we should consider applying GCM in teaching and learning design in order to increase learning achievement and decrease extraneous cognitive load.

        Spectrofluorimetric Determination of Bisphosphonates in Biological Sample with a Fluorescent Chemosensor, NadDPA-2Zn2+

        The accurate determination of bisphosphonate levels in bone and biological fluids is important in both clinical and pharmacological/toxicological studies; however, the quantitative analysis of the bisphosphonate is difficult because its concentration is quite low in most of biological sample. A novel fluorescent chemosensor (FCS)-based measurement method of bisphosphaonate levels using Naphta-diDPA-2Zn2+ (NadDPA-2Zn2+, DPA = dipycolylamine), an excellent FCS previously used for detecting PPi, was developed. By the FCS method, the concentration of bisphosphonates having no fluorophores can be determined analyzed with sufficient sensitivity. The results of this study indicate that the FCS-based measurement can be a useful method to analyze bisphosphonates in biological samples.

        염증성 장질환에서의 신장요로계 질환

        The incidence of inflammatory bowel disease (IBD) is increasing rapidly and extra-intestinal manifestations in IBD are also increasing. The prevalence of renal and urinary involvement in IBD ranges from 4-23%. Nephrolithiasis is the most common urinary complication in IBD patients. Parenchymal renal disease is rare but has been well documented and presents most commonly as glomerulonephritis or tubulointerstitial nephritis. The overall morbidity of IBD-related renal manifestations is significant. Therefore, a high index of clinical suspicion and optimal monitoring of the renal function are needed for the early diagnosis and prevention of IBD-related renal manifestations and complications.
