Garlic (Allium sativum L.) is a widely distributed plant used in all parts of the world not only as a spice but also as a popular remedy for several thousand years Recent studies have shown that garlic components may regulate the many metabolism in liver. Therefore, the present work was undertaken to study the effect of garlicjuice, allicin fraction and heat-treated allicin fraction on the hepatic glutathione s-transferase and glutathione peroxidase which protect against toxic compound and lipid peroxide in liver. The results obtained were summarized as follows. By the treatment of garlic juice, glutathione s-transferase and glutathione peroxidase activity was increased in hepatic cytosolic fraction compare with control group. The enzyme activtiy is more increased by treatment with boiled garlic juice compared with fresh garlic juice in rats. The effect of allicin fraction and heat-treated allicin fraction on the hepatic glutathione s-transferase was observed that these components inhibited the enzyme activity in vitro, whereas the enzyme activity was strongly inhibited heat-treated allicin fraction than allicin fraction. These observation led us to conclude that the garlic components which are a stable substance in heat may regulate the hepatic glutathione s-transferase and glutathione peroxidase activity.
Metal stain on monolithic zirconia restoration: A case report

In restorative treatment using fixed dental prostheses, dentists should select appropriate restoration material among various types of dental materials. The strength, marginal fit, esthetics, wear resistance, biocompatibility, and cost are important factors in the choice of restoration materials. The present case showed a surface stain on a monolithic zirconia restoration that was due to wear between the monolithic zirconia restoration and the base metal alloy restoration. This phenomenon was confirmed by surface roughness measurement and electron probe micro-analysis.

Background: Tolvaptan reduces height-adjusted total kidney volume (htTKV) and renal function decline in autosomal dominant polycystic kidney disease (ADPKD). This study was aimed at investigating the efficacy and safety of tolvaptan in Korean patients with ADPKD during the titration period. Methods: This study is a multicenter, single-arm, open-label phase 4 study. We enrolled 108 patients with ADPKD (age, 19-50 years) with an estimated glomerular filtration rate (eGFR) of >30 mL/min/1.73 ㎡ and factors defined as indicative of rapid disease progression. After tolvaptan titration, we evaluated efficacy and side effects and assessed factors associated with the effects. Results: After titration for 4 weeks, eGFR and htTKV decreased by 6.4 ± 7.9 mL/min/1.73 ㎡ and 16 ± 45 mL/m, respectively. No serious adverse drug reactions were observed during the titration period. The greatest eGFR decline was observed in the first week, with a starting tolvaptan dose of 45 mg. Multivariate linear regression for htTKV decline showed that the greater the change in urine osmolality (Uosm), the greater the decrease in htTKV (β, 0.436; p = 0.009) in the 1D group stratified by the Mayo Clinic image classification. Higher baseline eGFR was related to a higher htTKV reduction rate in the 1E group (β, -0.642; p = 0.009). Conclusion: We observed short-term effects and safety during the tolvaptan titration period. The decline of htTKV can be predicted as a short-term effect of tolvaptan by observing Uosm changes from baseline to end of titration in 1D and baseline eGFR in 1E groups.
Full mouth fixed implant rehabilitation in a patient with generalized aggressive periodontitis

BACKGROUND. Generalized aggressive periodontitis (GAP) is a destructive periodontal disease that can develop in young age. Only a few cases of full mouth rehabilitation, using dental implants, have been reported in a patient with aggressive periodontitis. CASE DESCRIPTION. This clinical report describes the treatment procedures and results of full mouth rehabilitation in a patient with aggressive periodontitis. After all teeth were extracted, 6 implants were placed in the maxilla and mandible, respectively. Fixed detachable implant prostheses were made. The patient was satisfied with the final results. She was followed for 10 months postloading. CLINICAL IMPLICATION. For a long-term success, continuous maintenance care is critical, as the contributing factors of the disease (such as immune factors or periodontal pathogens) may not be controlled adequately.
Expression of standard CD44 in human colorectal carcinoma: Association with prognosis

<P>The aim of the present study was to evaluate the expression of standard CD44 (CD44s) in colorectal cancer (CRC), its relationship with clinicopathological characteristics, and its potential prognostic significance. CD44s levels were measured on immunohistochemistry in tumors and surrounding normal mucosa from 74 patients with primary colorectal carcinomas. The patients were followed for a median period of 37 months. Expression of CD44s in primary tumor and surrounding normal mucosa tissues was demonstrated in 100% (74/74) and 37.9% (28/74), respectively. The expression of CD44s in tumors was significantly associated with the depth of invasion (<I>P</I> = 0.034) and lymph node involvement (<I>P</I> = 0.031). A significant difference was observed between the overall survival and level of tumor CD44s expression, especially for stage IV carcinoma (<I>P</I> = 0.038). Multivariate analysis indicated that TNM stage (<I>P</I> = 0.020) and tumor CD44s expression (<I>P</I> = 0.008) were independent predictors of overall survival in adenocarcinomas. CD44s overexpression may be an independent unfavorable prognostic factor for overall survival in advanced CRC, especially stage IV disease. Further investigation, however, is necessary to assess the biological roles of CD44 in CRC, and validate their possible value as novel therapeutic targets.</P>
In vitro evaluation of the polishing effect and optical properties of monolithic zirconia

<P><B>Abstract</B></P> <P><B>Statement of problem</B></P> <P>The relationship between surface roughness and the optical characteristics of zirconia prostheses may be affected by the type of shaded block.</P> <P><B>Purpose</B></P> <P>The purpose of this in vitro study was to investigate changes in surface roughness and brightness of monolithic zirconia blocks of different shades after polishing.</P> <P><B>Material and methods</B></P> <P>The surface roughness averages of 3 different kinds of shaded zirconia blocks (Zenostarof T0 [T0], Zenostar sun [Ts], and Zenostar sun chroma [Tsc]) were compared after surface polishing. Fifteen specimens of differing thicknesses were produced per block. (1 mm and 3 mm). Surface morphology and compositions were analyzed by using filed emission scanning electron microscopy (FE-SEM) and energy dispersive spectrometry. A 1-way ANOVA and the post hoc Scheffé test were performed (α=.05). The correlations between surface roughness and brightness were determined using the Pearson correlation analysis (α=.01).</P> <P><B>Results</B></P> <P>After phased polishing, Ts had the highest average roughness, followed by T0 and then Tsc. A statistically significant difference in roughness was observed between Ts and Tsc (<I>P</I><.05). Ts exhibited multiple distinct layers in the FE-SEM image. Carbon was detected in the Ts specimens only. Brightness was highest in T0, followed by Ts and then Tsc. A positive correlation was observed between brightness and surface roughness for all specimens (<I>P</I><.01). This correlation was stronger in the thicker specimens.</P> <P><B>Conclusions</B></P> <P>The results of this study suggest that polishing affects monolithic zirconia differently depending on shade. The polishing of monolithic zirconia reduces both surface roughness and brightness. The optical properties of monolithic zirconia vary by thickness and can be opaque or translucent.</P>

Purpose: This study evaluated the functional and oncological outcomes of proximal gastrectomy (PG) in comparison with total gastrectomy (TG) for upper-third early gastric cancer (EGC). Materials and Methods: The medical records of upper-third EGC patients who had undergone PG (n=192) or TG (n=157) were reviewed. The PG group was further subdivided into patients who had undergone conventional open PG (cPG; n=157) or modified laparoscopy-assisted PG (mLAPG; n=35). Patients who had undergone mLAPG had a longer portion of their intra-abdominal esophagus preserved than patients who had undergone cPG. Surgical morbidity, recurrence, long-term nutritional status, and the incidence of reflux esophagitis were compared between the groups. Results: The rate of postoperative complications was significantly lower for PG than TG (16.7% vs. 31.2%), but the five-year overall survival rate was comparable between the two groups (99.3% vs. 96.3%). Postoperative levels of hemoglobin and albumin were significantly higher for patients who had undergone PG. However, the incidence of reflux esophagitis was higher for PG than for TG (37.4% vs. 3.7%; P<0.001). mLAPG was related to a lower incidence of reflux esophagitis after PG (P<0.001). Conclusions: Compared to TG, PG showed an advantage in terms of postoperative morbidity and nutrition, and there was a comparable prognosis between the two procedures. Preserving the intra-abdominal esophagus may lower the incidence of reflux esophagitis associated with PG.
