Characteristics of Reaction of Salivary α-amylase to Duty Hours by Ages of Hospital Nurse

It is known that work load affects hypothalamus pituitary adrenocortical axis(HPA axis). It was studied that job stress-related factors affected neuroendocrine in nurses, however, which was not quantitatively evaluated. Especially, the shifts of nurses may bring about physical, emotional problems, and this working type may induce a syndrome of shift maladjustment. Therefore, this study was aimed to grasp the effect that job stress on duty hours by ages of nurses has on α-amylase. This study was conducted for the hospital in City D which consisted of a general ward, an emergency center, and an intensive care unit and which had three-shift system, and from a total of 50 nurses who were working at the hospital at daytime and nighttime and who were in their twenties(25) and in their thirties(25), Salivary α-amylase was extracted every two hours and analyzed. As the results of analysis, the change in characteristics of α-amylase by ages in the nurses in their 20s and 30s showed a significant difference(p<0.05), and as the change in the eigen value of α-amylase was highest in the nurses in their thirties between 11 and 13 o`clock, it could be observed that the activation ratio of α-amylase, stress reactant, increased with the activation of sympathetic nerve due to the mental stress.

The work load of nurses had an effect of awakening and increased the concentration of α-amylase, and the change in the level of job stress by ages could be identified. Therefore, to improve the level of mental health of nurses and the service to patients, the stress management by ages and by time should be considered important and it is necessary to proceed the study for various types of job to generalize the results of this study later.

The purpose of this study is to examine the green management evaluation criteria and Employee Awareness and analyze their impact on management performance. A survey was conducted using a questionnaire form that was given to companies with fewer than 300 regular workers and less than 30 billion won in sales. The three-step mediated effect analysis proposed by Baran and Kenny was performed to verify the roles of the parameters. The research results are summarized as follows. First, awareness of greenhouse gases and environmental pollution play a mediating role affecting the management performance. Second, awareness of harmful chemical substances play a mediating role affecting the management performance. Lastly, The survey subjects were limited to SMEs in Chungcheong province and thus, this may not be applicable to all companies.
Extremely Rare CT and MRI Findings of Peritoneal Leiomyoma Mimicking Hepatic Mass: A Case Report

Leiomyoma is a common benign tumor from smooth muscle cells, mostly in the uterus. Peritoneal leiomyomas (PLs) are extremely rare and mostly reported as disseminated peritoneal leiomyomatosis. However, to the best of out knowledge, radiologic findings of isolated PL are not reported in English literature. Herein, we introduce the radiologic findings of PL mimicking hepatic mass in a 34-year-old female. CT showed a mass with curvilinear heterogeneous enhancement at the liver’s peripheral area. On MRI, the mass showed gradual and heterogeneous enhancement on gadoxetic acid-enhanced MRI and diffusion restriction. The radiologic diagnosis was a benign hepatic tumor, such as degenerated hemangioma, adenoma, and inflammatory myofibroblastic tumor; however, the mass was diagnosed as PL pathologically.

Aims: Sarcopenia and osteoporosis are frequent complications in patients with liver cirrhosis. The aim of this study was to investigate the prognostic impact of skeletal muscle mass and bone mineral density in these patients. Methods: We included retrospectively cirrhotic patients who underwent both computed tomography scans (CT) and hepatic venous pressure gradient (HVPG) measurement between December 2009 and March 2015. Single transverse CT image at the level of the third lumbar vertebra (L3) were used to evaluate skeletal muscle area, visceral and subcutaneous fat. The mean CT attenuation of the trabecular bone of the vertebral body in L4 (L4<sub>HU</sub>) was measured. Results: A total of 160 cirrhotic patients were enrolled. The mean age of the patients was 52.9 years with men predominating (n=116, 72.5%) and most of the cause was alcohol (62.5%). Forty-nine patients (30.6%) died during a median follow-up period of 39.6 months (range, 3.6-87.6). In multivariate analyses, HVPG (hazard ratio [HR] 1.069, 95% confidence interval [CI] 1.011-1.130; P=0.018), Model for End-Stage Liver Disease (MELD) scores (HR 1.081, 95% CI 1.014-1.153; P=0.018), presence of sarcopenia (HR 1.859, 95% CI 1.004- 3.443; P=0.049) and L4<sub>HU</sub> (HR 1.839 for L4<sub>HU</sub> < 145 HU, 95% CI 1.013-3.338; P=0.045) were independently associated with long term mortality. In patients with low MELD score (<10), HVPG (HR 1.161, 95% CI 1.040-1.296; P=0.008) and L4<sub>HU</sub> (HR 4.173 for L4<sub>HU</sub> < 145 HU, 95% CI 1.501-11.598; P=0.006) were independent predictors of mortality, but and presence of sarcopenia was not significant. Conclusions: Muscle mass and bone mineral density, together with the known MELD and HVPG, were associated with longterm prognosis in cirrhotic patients. Especially, in low MELD score, low bone mineral density was independently associated with long-term mortality. It is necessary to identify prognostic roles of liver-muscle-bone according to the degree of cirrhosis.
