        전화추후관리가 항암요법을 받는 암환자의 자가간호수행 및 가족원 역할스트레스에 미치는 효과

        Purpose: This study was conducted from January to March, 2004 to examine the effect of telephone follow-up on the performance of self-care in cancer patients undergoing chemotherapy and on role stress of family caregivers. Method: Research design was a nonequivalent control group non-synchronized design. Seventy-two participants were assigned to either the experimental group (18 cancer patients, 18 family caregivers) or the control group (18 cancer patients, 18 family caregivers). Data were collected before and after the intervention and were analyzed with paired t-test, t-test, Mann-Whitney U Test & Wilcoxon Signed Ranks Test. Results: Performance of self-care in the experimental group undergoing telephone follow-up was significantly higher than that of the control group (t=8.016, p=0.000). Role stress of family members in the experimental group was also significantly higher than that of the control group (t=2.133, p=0.042). Conclusion: This results suggest that the telephone follow-up is effective for cancer patients undergoing chemotherapy and their family caregivers. Telephone follow-up can be recommended as an effective nursing intervention for self-care performance in cancer patients undergoing chemotherapy and to reduce role stress of family caregivers.

        Management of carbapenem-resistant Acineto-bacter baumannii epidemic in an intensive care unit using multifaceted intervention strategy

        Background/Aims: We evaluated the effects of a multifaceted intervention for controlling a carbapenem-resistant Acinetobacter baumannii (CRAB) epidemic in an intensive care unit (ICU) with no restrictions on carbapenem use. Methods: This study was conducted in a medical ICU between April 2012 and June 2016. The baseline infection control programs included surveillance cultures, contact precautions, and environmental cleaning. However, increases in the detection of CRAB isolates beginning in May 2013 led to the implementation of a new protocol, consisting of universal glove and gown use with daily chlorhexidine bathing for all patients in combination with baseline programs. The efficacy of the intervention was subjected to interrupted time series (ITS) analysis. Results: The multifaceted intervention led to a decrease in the incidence of CRAB from 17.65 to 0.89 cases per 1,000 patient-days, during the study period (p < 0.001). ITS analysis revealed a significant change in the trend of CRAB incidence (-0.413, p < 0.001). Over this same period, mean monthly use of carbapenems increased from 143.54 ± 95.73 to 204.95 ± 78.43 doses per 1,000 patient-days (p = 0.018). Conclusions: Our multifaceted intervention reduced the incidence of CRAB acquisition in an ICU where carbapenem use was not restricted.

        이압요법이 병원여성근로자의 월경곤란증에 미치는 효과

        Purpose: This study was done to provide support material to traditional nursing interventions through investigation of the effect of Auricular acupressure therapy for female staff in the hospital who have dysmenorrhea but cannot be absent from work even with severe symptoms affecting their work and therefore cannot get proper pain management. Method: The participants in this study were 62 women were working in the G-university hospital in Jin-ju, assigned to experimental (30) and 32 control groups (32). The study method was a quasi-experimental, nonequivalent control group non-synchronized design. The experimental group received acupressure treatment (a pin was attached to tape) on the special stimulation points in the inside of the ear, 5 times for 5 seconds, morning, afternoon, and night. Tape without the pressure pin was applied to the same points for women in the control group to evaluate placebo effect. Data were collected from May to November 2007 and were analyzed using SPSS PC+ 12 version. Descriptive statistics, t-test, x2 -test, Fisher's exact test, paired t-test, were used to analyze the data. Results: The experimental group showed a significant decrease in dysmenorrhea compared to the control group (t=-2.743, p<.01). In the sub-part, the experimental group showed menstruation related pain (t=-3.726, p<.01), menstruation related to behavioral change (t=-2.421, p<.05) and symptoms however, there was no significant difference related to attention deficit, water retention, and negative image. Conclusion: Application of auricular acupressure therapy was approved for this study and although auricular acupressure therapy was not effective for all of the symptoms, it was effective for dysmenorrhea.

        Pharmacokinetics and tissue distribution of ginsenoside Rh2 and Rg3 epimers after oral administration of BST204, a purified ginseng dry extract, in rats

        <P><OL><LI><P>BST204, a purified ginseng dry extract containing a high concentration of racemic Rh2 and Rg3 mixtures, is being developed for supportive care use in cancer patients in Korea. This study investigates the pharmacokinetics and tissue distribution of BST204 in rats.</P></LI><LI><P>After oral administration of BST204, only the <I>S</I> epimers, S-Rh2 and S-Rg3, could be determined in rat plasma. The poor absorption of the <I>R</I>-epimers, R-Rh2 and R-Rg3, may be attributed to lower membrane permeability and extensive intestinal oxygenation and/or deglycosylation into metabolites. The AUC and <I>C</I><SUB>max</SUB> values of both S-Rh2 and S-Rg3 after BST204 oral administration were proportional to the administered BST204 doses ranged from 400 mg/kg to 2000 mg/kg, which suggested linear pharmacokinetic properties.</P></LI><LI><P>There were no statistically significant differences in the pharmacokinetics of S-Rh2 and S-Rg3 after oral administration of pure S-Rh2 (31.5 mg/kg) and S-Rg3 (68 mg/kg) compared with oral administration of BST204, 1000 mg/kg. These indicated that the presence of other components of BST204 extract did not influence the pharmacokinetic behavior of S-Rh2 and S-Rg3.</P></LI><LI><P>After oral dosing of BST204, S-Rh2 and S-Rg3 were distributed mainly to the liver and gastrointestinal tract in rats.</P></LI><LI><P>Our finding may help to understand pharmacokinetic characteristics of S-Rh2, R-Rh2, S-Rg3, and R-Rg3, comprehensively, and provide useful information in clinical application of BST204.</P></LI></OL></P>
