Objective To analyze the preoperative diagnostic clues to ovarian pregnancy (OP). Methods This study conducted a retrospective chart review of 23 patients with OP and 46 patients with tubal pregnancy (TP) from October 1, 2003 to September 31, 2016 in Hanyang University Hospital. Results There were no significant differences in demographic and clinical characteristics between the two groups. The presence of an ectopic gestational sac and hemoperitoneum was significantly higher in the TP group (13.0% vs. 95.7%, P=0.000; 13.0% vs. 54.3%, P=0.001, respectively) in preoperative ultrasonogram. The OP group had more ruptured ectopic gestational sacs than the TP group (73.9% vs. 45.7%, P=0.039) in surgical findings. Conclusion For the patients in whom a gestational sac is not detected in the uterus or the fallopian tubes, it is important to be aware of the possibility of OP and rupture of an ovarian gestational sac to promote early diagnosis and surgical intervention.
Laparoscopic resection of retroperitoneal mesothelial inclusion cyst mimicking an ovarian tumor

Retroperitoneal inclusion cysts are uncommon mesothelium-lined cysts seen in the pelvis of reproductive-age women. It presents as a pelvic mass or with pelvic pain and may be misdiagnosed as an ovarian malignancy. Although benign, local recurrences are common, often requiring repeated surgeries. We report a case of retroperitoneal inclusion cysts, which was diagnosed clinically as benign multilocular cyst of ovary; however, it turned out to be retroperitoneal mesothelial inclusion cyst on histopathology. A 55-year-old postmenopausal woman referred to our outpatient department with intermittent lower abdominal pain. Results of routine blood investigation and CA-125 were within normal limits. A pelvic MRI showed a cystic ovarian tumor, approximately 7 cm in diameter with numerous septated small cysts. We performed a laparoscopic surgery on April 19, 2016. First, we removed extensive adhesions on pelvic wall. After confirming no abnormalities in both ovaries, we opened the peritoneum to investigate retroperitoneal space. There was a cystic multiloculted mass firmly attached to the right ureter and the pouch of Douglas. The lesion was friable and to avoid rupture of the cyst and spillage of the cystic fluid, meticulous care was taken. Complete resection of the lesion was performed. Postoperative recovery was good and the histopathology showed benign cystic mesothelial lesion. We present this case along with a video clip.

<B>ABSTRACT</B><P>Four phosphoproteins, of 34, 43, 50, and 84 kDa, with common amino termini are synthesized via alternative splicing from the UL112-113 region of the human cytomegalovirus genome. Although genetic studies provided evidence that both the UL112 and UL113 loci in the viral genome are required for efficient viral replication, whether the four proteins play specific roles or cooperate in replication is not understood. Here we present evidence, using in vitro and in vivo coimmunoprecipitation assays, that the four UL112-113 proteins both self-interact and interact with each other. A mapping study of the 84-kDa protein showed that the N-terminal region encompassing amino acids 1 to 125, which is shared in all UL112-113 proteins and highly conserved among betaherpesviruses, is required for both self-interaction and nuclear localization as foci. Further localization studies revealed that, unlike the 43-, 50-, and 84-kDa proteins, which were distributed as nuclear punctate forms, the 34-kDa form was located predominantly in the cytoplasm. However, when all four proteins were coexpressed simultaneously, all of the UL112-113 proteins were efficiently localized to the promyelocytic leukemia oncogenic domains. We also found that the ability of the UL112-113 proteins to relocate UL44 (the viral polymerase processivity factor) to prereplication foci relied on self-interaction and reached maximal levels when the four proteins were coexpressed. Therefore, our data suggest that interactions occurring among UL112-113 proteins via their shared N-terminal regions are important to both their intranuclear targeting and the recruitment of UL44 to subnuclear sites for viral replication.</P>
Provenance Studies for Prehistoric Obsidian by Using Mössbauer Spectroscopy

The iron compounds in minerals of obsidians from the continental Far East, Mount Baekdu (Baedusan) near the Sino-Korean border and Kyushu in southwest Japan were studied using 57Fe Mössbauer spectroscopy. The mineral Pyroxene (FeSiO3) in obsidian was found in the metamorphic rocks of Mount Beakdusan and Kyushu. However, the mineral biotite in obsidian was found in Kyushu only. The chemical compounds of the microlites in obsidians from Mount Baekdu are quite different from those in the Kyushu obsidians. The Mössbauer spectrum measured on the Baekdusan obsidian was fitted using two sextets, two ferrous iron doublets, and a ferric iron doublet tentatively assigned to clinopyroxene. Two sextets result from the nonstoichiometric magnetite. In contrast, Kyushu obsidians consisted of two sextets, a ferrous iron doublet for clinopyroxene, and a ferric iron doublet tentatively associated with biotite. In mineralogical and geochemical studies on the Baekdusan and Kyushu obsidians, the origination of the magma compositions and the crystallization environments were confirmed to have been different.

Clinical symptoms of herpes zoster include red rash, burning pain, tingling or extreme sensitivity in the affected area, usually limited to one side of the body. There might be a fever or a headache. The pain of acute herpes zoster may be severe, but it is usually temporary. Some of old patients are likely to develop postherpetic neuralgia. We treated a 73 year-old male patient who had severe pain and a headache after acute herpes zoster. In the point of Differentiation of Syndromes(辯證), this subject was diasgnosed as wind-heat syndrome(風熱證) and was administered Bangpungtongsung-san. For the purpose of making the pain easier, we used the western medication as well. After six days of treatment, pain and the other symptoms improved. Besides postherpetic neuralgia dissipated. Based on this experience, both oriental medicine and western medicine have a good effect on acute herpes zoster.
Suppressive Effect of Curcuma Zedoaria Roscoe on Pulmonary Metastasis of B16 Melanoma Cells

Objective: We examined the antimetastatic effect of Curcuma zedoaria Roscoe (CZ) on pulmonary metastasis of B 16 cells. Methods: For 6 weeks, Zedoariae Rhizoma made from dried CZ were dissolved in distilled water and administered to mice 2 weeks before they were injected with B]6 melanoma cells. Mice were given CZ at doses of 250 and 500 mg/kg, and were compared for lung weight, survival days, and NO production. Results: Intake of CZ throughout the experiment extended the average survival time. Intake after B16 cell injection slightly prolonged survival time, but intake before B]6 cell injection did not influence life span. We examined the effect of CZ on macrophage function by measuring NO production. After the macrophages were given CZ for 6 weeks, the amount of NO generated by the macrophages stimulated with LPS in culture medium increased. NO generated by the macrophages also served as a cytotoxic factor against B16 melanoma cells. B16 melanoma-conditioned medium reduced NO production by macrophages. However, CZ treatment reversed the reduction in NO production by the conditioned medium significantly. Conclusion : These findings may suggest that macrophage function-modulating activity by CZ appears to underlie its antimetastatic activity, which leads to a decrease in the number of lung metastatic surface nodules and the extension of life span.

<P>Our group found that the treatment of embryos with histone deacetylase inhibitors (HDACi), including trichostatin A, Scriptaid, suberoylanilide hydroxamic acid, and oxamflatin, after cloning by somatic cell nuclear transfer (SCNT) resulted in significantly improved efficiency. Although many researchers have investigated the use of HDACi treatment to improve the quality of cloned mouse embryos, the mechanism underlying this treatment has not been completely understood. We believe that the effect of HDACi on embryonic gene activation (EGA) is important for normal development of cloned embryos. In the present study, using highly sensitive fluorescence in situ hybridization (FISH) with probes complementary to mouse rDNA, the effect of Scriptaid on the onset of rRNA synthesis was examined in cloned embryos. In addition, to determine how Scriptaid affects pre-rRNA processing machinery in SCNT embryos with activated rDNA transcription, functional nucleolar formation was analyzed in detail by combined assessment of rRNA synthesis and nucleolar protein allocation in preimplantation embryos. In this experiment, at least part of the rRNA localization by FISH was substituted by 5-bromouridine 5'-triphosphate staining after alpha-amanitin treatment. The results show that in the late 2-cell stage, a number of SCNT embryos initiated transcriptional activation while having one blastomere showing inactivated rRNA transcription and another blastomere showing activated rRNA transcription and despite both nuclei being in interphase. In addition, in some SCNT embryos, the same nuclei contained a mixture of inactively and actively transcribed rRNA, which was rarely observed in intracytoplasmic sperm injection embryos. This asynchronous transcription induced a delay of one cell cycle in SCNT embryo activation of functional nucleoli. Scriptaid can overcome this failure in the timely onset of embryonic gene transcription by activation of rRNA genes and promotion of nucleolar protein allocation during the early phase of EGA.</P>
