Background: In super acute phase after the Great East Japan Earthquake (11 March 2011), Disaster medical association team (DMAT) consisted of emergency physicians acted in disaster areas in the East Coast of Japan. On the other hand, these areas needed chronic care support by general physicians because of the enormous destructions of the medical facilities by the tsunami. So then, the Japanese Red Cross Society had sent disaster emergency outpatient assistance doctors to Isinomaki Red Cross Hospital, was intact because it was moved away from the Pacifi c Ocean three years ago. Methods: Determination of specialism by direct hearing and information from home page of each Red Cross Hospitals. Results: Cumulative 81 medical doctors were 38 internal medicine physicians, trainees 15, emergency physicians 12, surgeon 6, pediatrician 3, orthopedic surgeon 3, anesthesiologist 2, obstetrician 2, from April to August 2011. At fi rst 6 dispatched doctors were resident and did emergency outpatient work in Ishinomaki Red Cross Hospital for 6 days and dipatched. A lot of Cases of pneumonia and bronchial asthma due to debris and rubble by Tsunami were seen. All dispatch doctors corresponded to primary surgery including fractures and trauma, pediatric diseases, and posttraumatic stress disorder due to earthquake itself, tsunami damage, and radiation problem of Fukushima. Conclusions: The role of general physicians is very important in chronic phase of disaster medical assistance. Medical facilities in the area with many earthquakes should be moved apart from the sea in order to avoid the damage of tsunami.
Novel mutations of CDKN1C in Japanese patients with Beckwith-Wiedemann syndrome

Beckwith-Wiedemann syndrome (BWS) is an imprinting-related human disease that is characterized by macrosomia, macroglossia, abdominal wall defects, and variable minor features. BWS is caused by several genetic/epigenetic alterations, such as loss of methylation at KvDMR1,gain of methylation at H19-DMR, paternal uniparental disomy of chromosome 11, CDKN1C mutations, and structural abnormalities of chromosome 11. CDKN1C is an imprinted gene with maternal preferential expression, encoding for a cyclin-dependent kinase (CDK) inhibitor. Mutations in CDKN1C are found in 40 % of familial BWS cases with dominant maternal transmission and in *5 % of sporadic cases. In this study, we searched for CDKN1C mutations in 37BWS cases that had no evidence for other alterations. We found five mutations—four novel and one known—from a total of six patients. Four were maternally inherited and one was a de novo mutation. Two frame-shift mutations and one nonsense mutation abolished the QT domain, containing a PCNA-binding domain and a nuclear localization signal. Two missense mutations occurred in the CDK inhibitory domain,diminishing its inhibitory function. The above-mentioned mutations were predicted by in silico analysis to lead to loss of function; therefore, we strongly suspect that such anomalies are causative in the etiology of BWS.
Characteristic evaluations of microwave absorbers using dielectric and magnetic composite materials

A microwave absorber for use in an electronic toll collection(ETC) device, having a central frequency of 5.8 GHz, is fabricated using a composite of magnetic (iron, Fe (coated by Fe3O4), made from Carbonyl Iron, Fe(CO)5) and dielectric (titanium oxide, TiO2) material. The reflection loss in free space, using a circularly polarized wave, was −20 dB or more around a central frequency of 5.8 GHz at incident angles of up to 60o, which satisfies the necessary conditions for ETC use in JAPAN. In this study, we explain the characteristic evaluations of microwave absorbers by changing the weight ratio of Fe/TiO2 composite materials (Fe/TiO2 = 100/0-0/100) and incidence characteristics of the transverse electric wave (TE wave), the transverse magnetic wave (TM wave) and the circularly polarized wave (CP wave) in a free-space without a characteristic deterioration. In addition, we show the experimental results by changing the arrangement of microwave absorber that also satisfied a necessary condition for ETC use. Current Status of the Diagnosis and Management of Pancreatic Neuroendocrine Tumors in Japan

The epidemiology of pancreatic neuroendocrine neoplasms (PNENs) in Asia has been clarified through epidemiological studies, including one conducted in Japan, and subsequently another in South Korea. As endoscopic ultrasonography (EUS) has become more widely accessible, endoscopic ultrasound-fine needle aspiration (EUS-FNA) has been performed in pancreatic tumors for which the clinical course was only monitored previously. This has enabled accurate diagnosis of pancreatic tumors based on the 2010 WHO classification; as a result, the number of patients with an accurate diagnosis has increased. Although surgery has been the standard therapy for PNENs, new treatment options have become available in Japan for the treatment of advanced or inoperable PNENs; of particular note is the recent introduction of molecular target drugs (such as everolimus and sunitinib) and streptozocin. Treatment for progressive PNENs needs to be selected for each patient with consideration of the performance status, degree of tumor differentiation, tumor mass, and proliferation rate. Somatostatin receptor (SSTR)-2 is expressed in many patients with neuroendocrine tumor. Somatostatin receptor scintigraphy (SRS), which can visualize SSTR-2 expression, has been approved in Japan. The SRS will be a useful diagnostic tool for locating neuroendocrine neoplasms, detecting distant metastasis, and evaluating therapy outcomes. In this manuscript, we review the latest diagnostic methods and treatments for PNENs.

31yrs. old female known to have hypertension and dyslipidemia for 1 yr.presented with h/o weight loss around 10 kg for 1 yr,palpiations,diaphoresis and hypertensive urgencyBp 210/110 mmhg)and also found to have diabetes (newly diagnosed)further investigations shows large adrenal mass (left side)suspicion of phaechromocytoma and urine catecholamoines was high, urology was consulted and she underwent left adrenalectomy and biopsy confi rm the diagnosis of malignant phaechromcytoma. Repeat Urine catecholamines which was quite high on presentation later normalized after surgey. Now she is under active follow up with endocrinologist and on antihypertensive medications very minimal doses and for her diabetes she is not on any medications as well after surgery.
