        H7N9 Avian Influenza Emergency Management in China -Temperature and Live Poultry Markets Analysis with Responding Policy Study -

        Influenza A virus subtype H7N9 has received great attention since the first human case was reported at the beginning of this year. The viral characteristics have been quickly brought into research process. Here, the effect of temperature and live poultry market has been considered on the basis of analysis of existing data. As expected, both temperature and live poultry market has significant impact on the spread of this epidemic. Finally, the effect of government reactions and medical treatment on response was discussed. It indicated that sufficient and rapid reactions did not always lead to a perfect outcome which proved the influence of temperature and live poultry market potentially. Research highlights: The effect of temperature on H7N9 avian influenza is proved by analyzing the latitude and temperature of affected area. The effect of live poultry markets on H7N9 avian influenza is confirmed by analyzing the relationship between the number of live poultry markets in affected area and incidence of H7N9 human infection. The effect of government actions and medical treatment is studied and it turned out the result is not in accordance with actual efforts.

        The Study of International Cooperation on Emergencies

        International cooperation on emergencies is significant to the response of these emergencies. Firstly, the connotation and characteristics of international cooperation on emergencies were analyzed. Secondly, the typical types of emergencies in need of international cooperation, the types of countries involved in international cooperation and the typical types of international cooperation modes on emergencies were analyzed in order to explain the mechanism of international cooperation. Finally, some strategies of international cooperation on emergencies were proposed. The result shows that, in order to have a better international cooperation, international cooperation modes on emergencies should be determined according to the types of emergencies, the situations of countries and organizations involved in it.

        Tea consumption is associated with a reduced risk of coronary heart disease in female but not male populations in Guangzhou, China

        BACKGROUND/OBJECTIVES: The association between tea consumption and risk of coronary heart disease (CHD) remains controversial. This study aimed to determine whether tea consumption has an effect on CHD risk in Chinese adults. SUBJECTS/METHODS: In this hospital-based case-control study, 267 cases of CHD and 235 non-CHD controls were enrolled. Blood samples from all cases were examined. Cardiac function indices (left ventricular ejection fraction, left ventricular end-diastolic dimension, lactate dehydrogenase, and creatine kinase of the muscle or brain type), blood lipid index (high-density lipoprotein cholesterol), and blood coagulation function indices (fibrinogen and activated partial thromboplastin time) were recorded. Tea consumption of study participants was assessed by a specifically designed questionnaire. The baseline characteristics of the study populations were recorded, and CHD-related biomarkers were detected. Differences in baseline characteristics of the study participants were examined using t-tests for continuous variables and chi-squared tests for categorical variables. Unconditional logistic regression was used to measure the association between tea and CHD. RESULTS: There were significant differences in cardiac function indices, blood lipid index, and blood coagulation indices between CHD cases and controls (P < 0.05). We found tea consumption reduced CHD risk in female participants (adjusted odds ratio (OR) = 0.484, 95% CI: 0.242-0.968, P = 0.0403). Regarding the type of tea consumed, the risk of CHD was reduced in women who drank partially fermented tea (adjusted OR = 0.210, 95% CI: 0.084-0.522, P = 0.0008). Analytic results for the amount of tea consumed per unit time showed CHD risk was reduced in women who consumed 1-2 cups of tea per day (adjusted OR = 0.291, 95% CI: 0.131-0.643, P = 0.0023). A tea-drinking frequency of > 6 days/week was beneficial for CHD prevention (adjusted OR = 0.183, 95% CI: 0.049-0.679, P = 0.0112). When analyzed according to the duration of tea consumption, the risk of CHD was reduced in participants who had been drinking tea for 10-20 years (adjusted OR = 0.360, 95% CI: 0.137-0.946, P = 0.0382). CONCLUSIONS: Tea consumption is associated with a reduced risk of CHD in female but not male populations in Guangzhou.

      • A Cross-Cultural Comparative Study of Cultural Characteristics and Informa tion Search Behaviours of Business Travellers

        This study examines the information searching of business travelers and their behavior based on the cultural differences. Despite growing academic interest in business travel i n recent years, there has been little investigation of the information search behaviors of business travelers in a cross-culture context. The above themes are rational assumed that information search behaviour are necessary to understand. To address the identified gap s in the literature, the proposed relationships are examined and presented through using chi-square method with a survey sample. The research sample included Japanese, English and Chinese business travellers. A self-administrated questionnaire was distributed to 211 business travelers attending international conventions in Taipei. The research findings reveal that information preferences of business travelers are significantly differed from the three recognized groups in the case of Taiwan. Some significant differences in information search behaviors were recognized amongst the three recognized respondent groups. Marketing implications were also proposed and presented.

        Traditional information system and managerial mechanism cannot meet the new environment protection regulations; therefore, a new solution must be set up, else it may run the risk of losing the market and the value of product brand.This research focuses on the solutions provided by the network and communication industry, manufacture of the green products, and the establishment of a Web-based green collaboration management system, which incorporates the environment protection requests into the product development and green supply chain activities. There are three goals: (1) For the design of green products, green components and product management are set up for the purpose of de-toxin and ease of disassemble; (2) For the green supply chain, the management of hazardous substances, product recycle and disassemble are set up; (3) For the product recycling management, the recycling information is feedback for the product update and collaboration operation. The result of research can provide information for the integration of workflow and information system currently in use, provide RoHS and WEEE management solutions, and set up green collaboration information system, so that green products, brand leadership, and environment protection endeavor can be assured.

        Development and Validation of an Integrated Healthy Workplace Management Model in Taiwan

        Background: Impacts of exposure are generally monitored and recorded after injuries or illness occur. Yet, absence of conventional after-the-effect impacts (i.e., lagging indicators), tend to focus on physical health and injuries, and fail to inform if workers are not exposed to safety and health hazards. In contrast to lagging indicators, leading indicators are proactive, preventive, and predictive indexes that offer insights how effective safety and health. The present study is to validate an extended Voluntary Protection Programs (VPP) that consists of six leading indicators. Methods: Questionnaires were distributed to 13 organizations (response rate ¼ 93.1%, 1,439 responses) in Taiwan. Cronbach a, multiple linear regression and canonical correlation were used to test the reliability of the extended Voluntary Protection Programs (VPP) which consists of six leading indicators (safe climate, transformational leadership, organizational justice, organizational support, hazard prevention and control, and training). Criteria-related validation strategy was applied to examine relationships of six leading indicators with six criteria (perceived health, burnout, depression, job satisfaction, job performance, and life satisfaction). Results: The results showed that the Cronbach’s a of six leading indicators ranged from 0.87 to 0.92. The canonical correlation analysis indicated a positive correlation between the six leading indicators and criteria (1st canonical function: correlation ¼ 0.647, square correlation ¼ 0.419, p < 0.001). Conclusions: The present study validates the extended VPP framework that focuses on promoting safety and physical and mental health. Results further provides applications of the extended VPP framework to promote workers’ safety and health.

        Sofosbuvir/velpatasvir plus ribavirin for Child-Pugh B and Child-Pugh C hepatitis C virus-related cirrhosis

        Background/Aims: Real-world studies assessing the effectiveness and safety of sofosbuvir/velpatasvir (SOF/VEL) plus ribavirin (RBV) for Child-Pugh B/C hepatitis C virus (HCV)-related cirrhosis are limited. Methods: We included 107 patients with Child-Pugh B/C HCV-related cirrhosis receiving SOF/VEL plus RBV for 12 weeks in Taiwan. The sustained virologic response rates at off-treatment week 12 (SVR<sub>12</sub>) for the evaluable population (EP), modified EP, and per-protocol population (PP) were assessed. The safety profiles were reported. Results: The SVR<sub>12</sub> rates in the EP, modified EP and PP were 89.7% (95% confidence interval [CI], 82.5-94.2%), 94.1% (95% CI, 87.8-97.3%), and 100% (95% CI, 96.2-100%). Number of patients who failed to achieve SVR<sub>12</sub> were attributed to virologic failures. The SVR<sub>12</sub> rates were comparable regardless of patient characteristics. One patient discontinued treatment because of adverse events (AEs). Twenty-four patients had serious AEs and six died, but none were related to SOF/VEL or RBV. Among the 96 patients achieving SVR<sub>12</sub>, 84.4% and 64.6% had improved Child-Pugh and model for endstage liver disease (MELD) scores. Multivariate analysis revealed that a baseline MELD score ≥15 was associated with an improved MELD score of ≥3 (odds ratio, 4.13; 95% CI, 1.16-14.71; P=0.02). Patients with chronic kidney disease (CKD) stage 1 had more significant estimated glomerular filtration rate declines than patients with CKD stage 2 (-0.42 mL/min/1.73 ㎡/month; P=0.01) or stage 3 (-0.56 mL/min/1.73 ㎡/month; P<0.001). Conclusions: SOF/VEL plus RBV for 12 weeks is efficacious and well-tolerated for Child-Pugh B/C HCV-related cirrhosis. (Clin Mol Hepatol 2021;27:575-588)

        A retrospective analysis of 20-year data of the surgical management of ulcerative colitis patients in Taiwan: a study of Taiwan Society of Inflammatory Bowel Disease

        Background/Aims: With the recent progress in medical treatment, surgery still plays a necessary and important role in treating ulcerative colitis (UC) patients. In this study, we analyzed the surgical results and outcomes of UC in Taiwan in the recent 20 years, via a multi-center study through the collaboration of Taiwan Society of IBD. Methods: A retrospective analysis of surgery data of UC patients from January 1, 1995, through December 31, 2014, in 6 Taiwan major medical centers was conducted. The patients’ demographic data, indications for surgery, and outcome details were recorded and analyzed. Results: The data of 87 UC patients who received surgical treatment were recorded. The median post-operative follow-up duration was 51.1 months and ranged from 0.4 to 300 months. The mean age at UC diagnosis was 45.3±16.0 years and that at operation was 48.5±15.2 years. The 3 leading indications for surgical intervention were uncontrolled bleeding (16.1%), perforation (13.8%), and intractability (12.6%). In total, 27.6% of surgeries were performed in an emergency setting. Total or subtotal colectomy with rectal preservation (41.4%) was the most common operation. There were 6 mortalities, all due to sepsis. Emergency operation and low pre-operative albumin level were significantly associated with poor survival (P =0.013 and 0.034, respectively). Conclusions: In the past 20 years, there was no significant change in the indications for surgery in UC patients. Emergency surgeries and low pre-operative albumin level were associated with poor survival. Therefore, an optimal timing of elective surgery for people with poorly controlled UC is paramount.

        Prediction of the Duration to Next Admission for an Acute Affective Episode in Patients with Bipolar I Disorder

        Objective: Predicting disease relapse and early intervention could reduce symptom severity. We attempted to identify potential indicators that predict the duration to next admission for an acute affective episode in patients with bipolar I disorder. Methods: We mathematically defined the duration to next psychiatric admission and performed single-variate regressions using historical data of 101 patients with bipolar I disorder to screen for potential variables for further multivariate regressions. Results: Age of onset, total psychiatric admissions, length of lithium use, and carbamazepine use during the psychiatric hospitalization contributed to the next psychiatric admission duration positively. The all-in-one found that hyperlipidemia during the psychiatric hospitalization demonstrated a negative contribution to the duration to next psychiatric admission; the last duration to psychiatric admission, lithium and carbamazepine uses during the psychiatric hospitalization, and heart rate on the discharge day positively contributed to the duration to next admission. Conclusion: We identified essential variables that may predict the duration of bipolar I patients’ next psychiatric admission. The correlation of a faster heartbeat and a normal lipid profile in delaying the next onset highlights the importance of managing these parameters when treating bipolar I disorder.
