        Influence of late removal after treatment on the removal torque of microimplants

        Objective: To compare the removal torque of microimplants upon post-use removal and post-retention removal and to assess the influencing factors. Methods: The sample group included 241 patients (age, 30.25 ± 12.2 years) with 568 microimplants. They were divided into the post-use (microimplants removed immediately after use or treatment) and post-retention (microimplants removed during the retention period) removal groups. The removal torque in both groups was assessed according to sex, age, placement site and method, and microimplant size. Pearson correlation and multiple linear regression analyses were performed for evaluating variables influencing the removal torque. Results: The mean period of total in-bone stay of microimplants in the post-retention removal group (1,237 days) was approximately two times longer than that in the post-use removal group (656.28 days). The removal torques in the post-retention removal group (range, 4–5 N cm) were also higher than those in the post-use removal group. The mandible and pre-drilling groups demonstrated higher placement and removal torques than did the maxilla and no-drilling groups, respectively. In the no-drilling post-use removal group, the placement torque and microimplant length positively correlated with the removal torque. In the post-retention removal group, unloading in-bone stay period and microimplant diameter positively correlated with the removal torque in the no-drilling and pre-drilling methods, respectively. Conclusions: The removal torques differed according to the orthodontic loading and removal time of microimplants. With prolonged retention of microimplants inserted using the no-drilling method, the removal torque was clinically acceptable and positively correlated with the unloading in-bone stay period.

        기성품과 맞춤형 임플란트 지대주 나사의 풀림 토크 비교

        연구 목적: 본 연구의 목적은 임플란트 지대주 연결에 사용되는 기성품 나사와 맞춤형 나사의 풀림 토크를 비교하는 것이다. 연구 재료 및 방법: 세가지 임플란트 시스템에(Osstem, Astra, Zimmer) 대해 고정체와 지대주의 연결에 기성품 나사군(대조군) 과 맞춤형 나사군(실험군)으로 총 6군(n = 6)으로 나누었다. 조임 토크조절은 디지털 토크 측정기를 이용하였으며 각 임플란트 제조사가 추천한 조임 토크 값을 적용하였다. 체결 10분 후 동일한 조임 토크를 다시 적용하고 5분 후에 풀림 토크력을 측정하였다. 이 과정을 10회 반복 측정하였다. 반복 하중 실험에서는 6개 군(n = 3)에 대해 체결10분 후 2차 조임 토크를 적용하고 30도 경사로 50 N 하중으로 총 1,000,000번 반복 하중을 가하였다. 반복 하중 적용 이후 풀림 토크값을 측정하였다. 통계 방법으로는 10회 반복 측정에서 풀림 토크값의 차이를 비교하기 위해 repeated measures of ANOVA test (α=.05)를 사용하였고, 반복 하중 후 풀림 토크값의 차이를 비교하기 위해서 각 시스템별 Independant t-test로 통계 처리하였다. 결과: 모든 군에서 반복 횟수가 증가할수록 풀림 토크값이 유의성 있게 감소하는 것으로 나타났다(P< .05). 10회 반복 측정 실험에서는 세 종류의 임플란트에서 대조군(기성품 나사)과 실험군 (맞춤형 나사) 간에 풀림 토크값은 유의차가 없었다(P> .05). 반복 하중 실험에서 세가지 시스템에서 대조군과 실험군 간의 풀림 토크력은 유의한 차이가 없었다(P> .05). 결론: 반복 측정된 풀림력 검사와 반복 하중을 적용 후 풀림력 검사에서 맞춤형 나사와 기성품 나사의 풀림력은 유의한 차이가 없었다. Purpose: The purpose of this study is to compare the removal torque between prefabricated and customized implant abutment screw. Materials and methods: Three types of implant system (Osstem, Astra, Zimmer) were used. For each system, prefabricated abutment screw (control group) and customized abutment screw (test group) were used to connect the fixture and the abutment (n = 6). Digital torque gauze was used to control the tightening torque and the screws were tightened under each manufacturer's recommendation. 10 minutes after the connection the same tightening torque was applied, and 5 minutes after the second connection, the removal torque was measured. This procedure was repeated 10 times. In the cyclic loading test, 10 minutes after the first connection to the 6 groups (n = 3), the same tightening torque was applied, and a total of 1,000,000 time loading was applied at 30 degree angle to long axis with 50 N load. Repeated measures of ANOVA test (α=.05) was used as statistics to evaluate the effect of repeated loading number on the removal torque. Independent t-test was used to evaluate the difference in removal torque after cyclic loading. Results: The removal torque significantly decreased as the number of loading repetition increased (P<.05). In the 10 time repetition test, there was no significant difference between the prefabricated and customized implant abutment screw of the 3 implant system (P<.05). Also in the cyclic loading test, there was no significant difference between the prefabricated and customized implant abutment screw of the 3 implant system (P<.05). Conclusion: Within the limitation of this study, there was no significant difference in the removal torque between the prefabricated abutment screw and customized abutment screws.

        A STUDY ON THE CORRELATION BETWEEN IMPLANT STABILITY VALUES AND INITIAL INSERTION TORQUE

        Statement of problem. Osseointegration is important mechanism of dental implant but it is not easy to evaluate. Indirect measurement is non-invasive and clinically applicable but they need more study about correlation between indirect values and degree of osseointegration. Purpose. The aims of this study were to evaluate the coefficient of correlation between indirect measurement and direct measurement under different healing time, and assessment of effect of initial insertion torque to the implant stability. Material and Methods. 20 rabbits received 3 implants on each side of tibia. Three kinds of implants (machined surface implant, Sandblasted with Large grit and Acid etched implant, Resorbable Blast Media treated implant) were used. During the surgery implant insertion torque were measured with $Osseocare^{TM}$. After the 1, 4, 8, 12 weeks of healing time, animals were sacrificed and stability values (Implant Stability Quotient with $Osstell^{TM}$, removal torque with torque gauge) were measured. Results. The Bone quality of rabbit tibia was classified into 2 groups according to the insertion torque. Resonance frequency analysis (ISQ) and removal torque showed positive correlation until $4^{th}$ week (r=0.555, p=0.040). After $8^{th}$ week (r=0.011, p=0.970) the correlation became weak and it turned negative at $12^{th}$ week (r=-0.074, p=0.801). Insertion torque and ISQ showed changing correlation upon the healing time ($1^{st}$ week: r=0.301, p=0.033, $4^{th}$ week: r=-0.429, p=0.018, $8^{th}$ week: r=0.032, p=0.865, $12^{th}$ week: r=-0.398, p=0.029). Insertion torque and removal torque has positive correlation but it was not statistically significant ($1^{st}$ week: r=0.410, p=0.129, $4^{th}$ week: r=0.156, p=0.578, $8^{th}$ week: r=0.236, p=0.398, $12^{th}$ week: r=0.260, p=0.350). Conclusion. In this study, bone quality may affect the degree of osseointegration positively during healing time and correlation between ISQ and degree of osseointegration can be different according to the healing time and bone quality.

        Titanium Implant의 Removal Torque에 관한 연구

        The concept of biologic attachment of load-bearing implants has developed over the past decades as an alternative to the difficulties associated with long term implantation using mechanical fixation and bone cement. The choice of implant material is also as critical an element as site preparation or insertion procedure. The properties of implants that affect host tissue responses are not limited to chemical composition alone, but also include shape, surface characteristics, site of implantation, and mechanical interaction with host tissues. Initial mechanical interlocking prevents micromotion and may be a prerequisite for direct bone apposition. A hard tightening of screws does not necessarily mean a stronger fixation and final tightening of the fixtures is dependent on the experience of the operator. Removal torque is lower than insertion torque. The purpose of this study was to investigate differences in the removal torques at the bone-implant interface of polished and sandblasted Titanium. This experiment will give insight into important factors that must be considered when interpreting in vivo screwing forces on implants during the connection of the transmucosal abutments. We evaluated the significance of different surface textures by comparison of the withdrawal forces necessary for removal of otherwise identical rough and polished implants of Titanium and also evaluated interfacial response on the light microscopic level to implant surface. And the priority of the area of insertion on osseointegration were evaluated. 9 Titanium implants - among them, 3 were for the developmental - of either a smooth or rough surface finish were inserted in the dog mandible in the right side. 3 months later Kanon Torque Gauge was used to unscrew the implants. The results were as follows : 1. No significant difference was seen in the removal torque due to variation in surface treatment, 23 Ncm for the sandblasted and 23.33 Ncm for the polished surface (p>0.05). 2. Implants in the anterior (25 Ncm) mandible showed better resistance to unscrewing in comparison to ones in the posterior (18 Ncm) region (p<0.05). 3. Developmental fixtures (22 Ncm) had similar pullout strength to the control group (p>0.05).

        DEVELOPMENT OF PREDICTABLE STABILITY TEST FOR ASSESSMENT OF OPTIMUM LOADING TIME IN DENTAL IMPLANT

        STATEMENT OF PROBLEM: The application of a simple, clinically applicable noninvasive test to assess implant stability are considered highly desirable. So far there is still a controversy about correlation of various tests and implant stability. PURPOSE: In order to assess implant stability, the development of a new method is critical. It's possible to assess implant stability by calculating energy and angular momentum during implant installation. The purpose of this study is to evaluate the correlation of energy and implant stability. MATERIAL AND METHODS: Twenty three implants were installed in two different types of pig bone. Type I bone was retrieved from the distal aspect of the rib, with more cortical bone. Type II bone came from a more proximal region with less cortical components and a higher content of bone marrow and spongeous trabeculae. Insertion torque, removal torque, ISQ values and angular momentum and energy were measured. Pearson Correlation test was done to analyze the relation between RFA, maximum insertion torque, mean insertion torque, bone type, energy and removal torque. RESULTS: Type I bone showed higher removal torque than type II bone. Energy value was significantly correlated with maximum insertion torque and mean insertion torque. RFA values were related with insertion torques but the significance was lower than Energy value. CONCLUSION: Within the limitation of this study energy values were considered clinically predictable method to measure the implant stability.

        교정용 미니 임플랜트의 cutting flute의 길이 및 형태에 따른 식립 및 제거 토크의 비교

        Objective: The purpose of this study was to evaluate the effect of length and shape of cutting flute on mechanical properties of orthodontic mini-implants. Methods: Three types of mini-implants with different flute patterns (Type A with 2.6 mm long flute, Type B with 3.9 mm long and straight flute, Type C with 3.9 mm long and helical flute) were inserted into the biomechanical test blocks (Sawbones Inc., USA) with 2 mm and 4 mm cortical bone thicknesses to test insertion and removal torque. Results: In 4 mm cortical bone thickness, Type C mini-implants showed highest maximum insertion torque, then Type A and Type B in order. Type C also showed shortest total insertion time and highest maximum removal torque, but Type A and B didn't showed statistically significant difference in insertion time and removal torque. In 2 mm cortical bone thickness, there were no significant difference in total insertion time and maximum removal torque in three types of mini-implants, but maximum insertion torque of Type A was higher than two other Types of mini-implants. Conclusions: Consideration about length and shape of cutting flute of mini-implant is also required when the placement site has thick cortical bone. 본 연구는 self-tapping screw의 특징적 구조인 cutting flute의 길이 및 형태 변화에 따른 미니 임플랜트의 식립 및 제거 토크를 비교해보기 위해 시행되었다. 외경, 내경, 길이 및 pitch 등 다른 조건이 모두 동일하고 cutting flute의 형태만 다른 세 종류의 미니 임플랜트를 사용하였다(A군; 2.6 mm 길이의 flute가 형성된 그룹, B군; 3.9 mm의 길이이면서 직선형의 flute를 갖는 그룹, C군; 3.9 mm의 길이이면서 나선형의 flute를 갖는 그룹). 골밀도를 균일하게 하기 위해 실험용 인공골(Sawbones Inc., USA)을 사용하였으며, 수직력 및 회전속도를 일정하게 하기 위해 구동식 토크 시험기를 이용하였다. 그 결과 다음과 같은 결론을 얻었다. 총 삽입 시간은 2 mm 두께의 피질골 시편에서는 세 군간에 차이를 보이지 않으나, 4 mm 두께의 피질골 시편에서는 C군 > B군 > A군 순으로 짧은 삽입 시간을 보였다. 최대 삽입 토크는 2 mm 두께의 시편에서는 A군이 다른 두 군에 비해 높은 값을 보이나, 4 mm 두께의 피질골 시편에서는 C군 > A군 > B군 순으로 높은 값을 보였다. 최대 제거 토크는 2 mm 두께의 피질골 시편에서는 세 군 사이에 차이가 없으나, 4 mm 두께의 피질골 시편에서는 C군이 다른 두 군에 비해 높은 값을 보여주었다. 이상의 결과로 보아 피질골이 두꺼운 부위에 미니 임플랜트를 식립할 경우 cutting flute의 길이 및 형태 또한 고려하여 미니 임플랜트를 선택하는 것이 바람직하다.

        Influence of the implant abutment types and the dynamic loading on initial screw loosening

        PURPOSE. This study examined the effects of the abutment types and dynamic loading on the stability of implant prostheses with three types of implant abutments prepared using different fabrication methods by measuring removal torque both before and after dynamic loading. MATERIALS AND METHODS. Three groups of abutments were produced using different types of fabrication methods; stock abutment, gold cast abutment, and CAD/CAM custom abutment. A customized jig was fabricated to apply the load at 30° to the long axis. The implant fixtures were fixed to the jig, and connected to the abutments with a 30 Ncm tightening torque. A sine curved dynamic load was applied for 105 cycles between 25 and 250 N at 14 Hz. Removal torque before loading and after loading were evaluated. The SPSS was used for statistical analysis of the results. A Kruskal-Wallis test was performed to compare screw loosening between the abutment systems. A Wilcoxon signed-rank test was performed to compare screw loosening between before and after loading in each group (α=0.05). RESULTS. Removal torque value before loading and after loading was the highest in stock abutment, which was then followed by gold cast abutment and CAD/CAM custom abutment, but there were no significant differences. CONCLUSION. The abutment types did not have a significant influence on short term screw loosening. On the other hand, after 105 cycles dynamic loading, CAD/CAM custom abutment affected the initial screw loosening, but stock abutment and gold cast abutment did not.

        Joint stability of internal conical connection abutments with or without hexagon indexes: an in vitro study

        Purpose: The purpose of this study was to compare the axial displacement of the hexagonal and conical abutment in internal conical connection implant after screw tightening and cyclic loading. Materials and Methods: Internal conical connection implants were divided into two groups (n = 10): group HEX, hexagonal abutment; and group CON, conical 2-piece abutments. The axial displacement and removal torque values were measured after 30 Ncm torque tightening and 250N loading test of 100,000 cycles. The Student t test with 5% significance level was used to evaluate the data. Results: HEX group demonstrated significantly higher axial displacement values after 30 Ncm tightening in comparison to the CON group (P < 0.05). No significant difference was found in axial displacement after cyclic loading (P = 0.052). Removal torque loss before and after the cyclic loading both revealed no significant difference between groups (P = 0.057 and P = 0.138). Removal torque value decreased after cyclic loading in both groups (P < 0.05). Conclusion: Overall, both abutment with or without hexagon index presented similar biomechanical performance except HEX group demonstrated significantly more axial displacement after applying tightening torque.         The effect of various thread designs on the initial stability of taper implants

        STATEMENT OF PROBLEM. Primary stability at the time of implant placement is related to the level of primary bone contact. The level of bone contact with implant is affected by thread design, surgical procedure and bone quality, etc. PURPOSE. The aim of this study was to compare the initial stability of the various taper implants according to the thread designs, half of which were engaged to inferior cortical wall of type IV bone (Group 1) and the rest of which were not engaged to inferior cortical wall (Group 2) by measuring the implant stability quotient (ISQ) and the removal torque value (RTV). MATERIAL AND METHODS. In this study, 6 different implant fixtures with 10 mm length were installed. In order to simulate the sinus inferior wall of type IV bone, one side cortical bone of swine rib was removed. 6 different implants were installed in the same bone block following manufacturer’s recommended procedures. Total 10 bone blocks were made for each group. The height of Group 1 bone block was 10 mm for engagement and that of group 2 was 13 mm. The initial stability was measured with ISQ value using Osstell mentor® and with removal torque using MGT50 torque gauge. RESULTS. In this study, we found the following results. 1. In Group 1 with fixtures engaged to the inferior cortical wall, there was no significant difference in RTV and ISQ value among the 6 types of implants. 2. In Group 2 with fixtures not engaged to the inferior cortical wall, there was significant difference in RTV and ISQ value among the 6 types of implants (P < .05). 3. There was significant difference in RTV and ISQ value according to whether fixtures were engaged to the inferior cortical wall or not (P < .05). 4. Under-drilling made RTV and ISQ value increase significantly in the NT implants which had lower RTV and ISQ value in Group 2 (P < .05). CONCLUSIONS. Without being engaged to the inferior cortical wall fixtures had initial stability affected by implant types. Also in poor quality bone, under-drilling improved initial stability.
