        이차성 구순비 변형에서 비교정술을 이용한 비변위의 교정

        The secondary cleft lip nose deformities show complex deformities of nasal cartilage, septum, nasal bone, piriform aperture and maxilla. The external characteristic of cleft lip nose deformity is conspicuously shown by alar deformities. The nasal septum and bone deformities on the cleft side have a tendency to deviate totally toward the non-cleft side. Occasionally the nasal septum is dislocated from the vomer and the anterior nasal spine and nasal septum are displaced toward the noncleft side. The anatomical structure of the nasal cartilage in the cleft lip nose is well known. There are also many methods of treatment for nasal cartilage deformities. But the anatomical structures as well as treatment methods for deformities of the nasal bone & septum in the secondary cleft lip nose deformity have rarely been reported. The author intends to explain the anatomical characteristics of deformities of the nasal bone, maxilla, and nasal septum and to suggest surgical treatment methods accordind to 98cases reported recently. The anatomic characteristics are deviation of nasal bony pyramid, deviation of anterior nasal spine and vomer, crooked septum, septal dislocation sometimes, turbinate hypertrophy. The surgical methods for radical corrective rhinoplasty include bilateral osteotomies of the lateral pyramids of the nasal bone, infracturing, sepal dislocation and fixation of the septum, turbinecomy, and submucosal resection, if needed. We performed 98 cases and follow-up period was from 6 months to 4 years. We have gained the results of median location of deviated nasal skeleton, improved nasal airway, good nasal symmetry. In conclusion, secondary cleft lip nose deformities are deviation of nasal bony pyramid, deviation of anterior nasal spine and vomer, crooked septum, septal dislocation sometimes, turbinate hypertrophy. Radical corrective rhinoplasty is needed for median location of deviated nasal skeleton, improved nasal airway, good nasal symmetry.

        세움대 이식(Spreader Graft)을 이용한 비중격 상연 변형에 의한 비만곡증의 교정

        High septal deformities exert pressure on upper lateral cartilages and nasal bones and cause external deviation of the nose. However, detection of high septal deformities causing deviated noses is very difficult if a detailed intranasal examination is not performed. As well, the high septal border is a difficult area to approach via the endonasal rhinoplasty and is the weakest portion in the septum resulting in much difficulty in correcting deformities and in frequent later recurrence of deviation secondary to the healing process. In order to solve the above problems in 33 cases of deviated nose associated with high septal deformities, the authors evaluated deformities in the high septal border after separation of the septum from upper lateral cartilages and nasal bones. The high septal border is centralized by scoring after resection of the mid or lower-septal portion. And spreader grafts were applied to the convex sides of deviated septal borders to stabilize the high septal border and to prevent long-term recurrence of deviation. Also, to balance the traction force of side walls, the length of both upper lateral cartilages and nasal bones were equalized by resection in the longer side and grafting in the shorter side. Clinical follow-up ranged from 3 to 31 months. There was no surgical or septal complication. Also, there was no incomplete correction, recurrence of deviation, or compromized nasal support. All patients except 2 were satisfied with the aesthetic and functional results. We found that separation of septum from side walls allows detection and correction of higher septal deformities and that spreader graft allows long-term support and prevents later recurrence of deviation.

        Calculation of Tooth Thickness Errors and Its Adjustment on Meshing Backlash of Harmonic Drive

        Meshing backlash mainly determines the transmission accuracy of harmonic drive, which is an important index in design. In this paper, a design method for short tooth involute profile is proposed. By intercepting part of the tooth profile, meshing interference of the addendum can be effectively avoided. Based on the dimensions of over pins of circular spline and flexspline, a calculation method of tooth thickness deviation is proposed, and the positive and negative tooth thickness deviation are defined. Involute functions need not be calculated by this method, which simplifies the solution process. By discretizing the teeth profile points of circular spline and flexspline, a calculation method of time-varying backlash is proposed, the variation of backlash in meshing in and out is analyzed, and the influence law of tooth thickness deviation on backlash is studied. On this basis, an algorithm for adjusting the radial deformation to compensate for the backlash is proposed. The results show that the excessive backlash in the meshing area can be reduced by increasing the radial deformation. When meshing interference occurs, the interference-free meshing can be realized by decreasing the radial deformation.

        Precision assembly simulation of skin model shapes accounting for contact deformation and geometric deviations for statistical tolerance analysis method

        Tolerance analysis methods, which are important to achieve a balance between manufacturing costs and functional requirements, have been studied and improved over the last 30 years. This paper proposes a new method to improve the precision of assembly simulation for statistical assembly simulation; by investigating the effect of the geometric deviations and mechanical behaviors of assembly contact on the stack-up assembly deviation by combining the recent theories on the tolerance analysis of rigid bodies and local contact deformation. To improve the precision of the tolerance analysis, real parts with form defects were simulated with the help of non-nominal skin model shapes. The SMSs were first considered to be rigid, and a quadratic optimization-based method for the rigid body displacement was used to calculate the initial contact points of all the SMSs. Next, contact deformation, following Hertzian contact theory, occurred at the initial contact points, which changed the relative positions of the SMSs in the subsequent interactions. The iteration process was convergent, and the final simulation for tolerance analysis could be generated after several iterations. Case studies demonstrated the practicality of the proposed method. Their actual assembly deviations were simulated by combining the effects of geometric deviations and local surface deformations. Since the proposed tolerance analysis framework based on statistical assembly simulation, it is expected to control the functional requirements of mechanical assemblies with much rough tolerance design and help optimize the tolerance in the future.

        클락킹 봉합 단일 기법을 이용한 연골성 사비의 교정

        Background and Objectives Deviated nose is a commonly encountered deformity, and the correction of deviated nose is the most difficult part of septorhinoplasty. Generally, additional cartilaginous graft is required for straightening the deviated segment. However, there are many cases where sufficient cartilage grafts are not available due to revision surgery or cases with small cartilage. In such cases, tilted cartilaginous dorsum should be corrected without grafting. The purpose of this study is to evaluate the usefulness of clocking suture single technique for cartilaginous deviation by analyzing the surgical results of cases who underwent corrective rhinoplasty without any cartilaginous graft. Subjects and Method From June 2009 to June 2016, 43 patients with tilted cartilaginous dorsum were corrected with clocking suture single technique. Cases underwent additional graft for straightening dorsum were excluded. The patients’ medical records and facial photographs were analyzed to assess surgical outcomes and complications. Surgical outcomes were graded as excellent, fair and poor according to patients’ satisfaction and evaluation by two physicians. Results Mean follow up period was 15.4 (5-39) months. There were 39 patients (90.7%) with excellent result, 3 patients (7.0%) with fair results, and 1 patient (2.3%) with poor result. Augmentation was performed with expanded polytetrafluoroethylene or septal cartilage in 26 patients (60.5%) and dorsal hump was removed in 22 patients (51.2%). Concomitant augmentation didn’t affect surgical results. No postoperative complication such as infection or inflammation were found. Conclusion The clocking suture single technique is an effective surgical method for the correction of dorsal cartilaginous deviation. Korean J Otorhinolaryngol-Head Neck Surg 2017;60(7):342-6

        The soil response calculation is described, by which, threw the fictive path of stress, the stress-deformation diagrams are determined, considering the nonlinear soil behavior. The calculation are lead incrementally, by which is shown that in the presented soil model (modified Cam Clay), considering the influence of overconsolidated soil pressure OCR, the number of calculation steps may, but not necessarily, have a sufficient influence on the value of failure load and definite soil deformation. The simplicity and the practicalness of the procedure, the enables modeling the complex relations in soil.

        한국인 사비 환자의 치료

        Background and Objectives The surgical techniques based on the classification system based on Western individuals for the correction of deviated nose should be modified because of the differences in the anatomy of the nose for Korean individuals. To evaluate the usefulness of a classification system that has been designed for the esthetics and deviated nose of Korean individuals, we used that to analyze the surgical outcomes of rhinoplasty for deviated nose for 76 Korean patients. Subjects and Method Seventy six patients who underwent rhinoplasty for deviated nose between January 2010 and June 2016 were reviewed retrospectively. All patients were classified with Jang’s classification system and surgical techniques were analyzed according to the classification. Surgical outcomes were evaluated based on the preoperative and postoperative photo taken of the patient’s nose. The degree of nasal deviation, tip projection index, nasofrontal angle and columellar-labial angle were measured. Results Deviated nose was classified into type I (34%), type II (26%), type III (13%), type IV (9%), and type V (17%). Preoperative and postoperative anthropometric measurements revealed that the mean degree of deviation changed from 8.84° to 3.01° (p<0.01); the tip projection index changed from 0.54 to 0.58 (p<0.01), and the degree of nasofrontal angle changed from 131.55° to 133.14° (p<0.01). Aside from the residual deviations, complications included a shifting of the dorsal graft, an inverted V-deformity from the depression of the middorsum after the spreader graft and incomplete hump removal. Conclusion Analysis based on the said classification system and personal esthetics showed good surgical outcomes for rhinoplasty to correct deviated nose. Therefore, it would be helpful to treat Korean patients for the correction of deviated nose by using both classification system and personal esthetics that are based on Korean individuals.         Thermal Deformation Compensation of Laser Tracker Relocating in Aircraft Assembly

        During aircraft assembly, measurements from either a single or multiple laser tracker with diff erent positions are transformedto an assembly coordinate system by measuring the enhanced reference system (ERS) points via a rigid-body registrationmethod. The location deviations of the ERS points resulting from thermal deformations have become the major error sourcein registration. To improve the accuracy of measurement, a thermal deformation compensation method, based on precisethermal deformation theory, is proposed in this study. Thermal deformation coeffi cients of ERS points in X-, Y-, and Z-axesare calculated separately, and an iterative algorithm is developed to correct the actual locations of the ERS points andestimate the optimal coordinate transformation parameters. A case study indicated that, with our proposed novel thermaldeformation compensation method, the average registration error is decreased by 9.2–38.7%, and the maximum registrationerror is deceased by 12.3–38.6%.

The effectiveness of the improved deformed exponential-type potential is tested by simulating the inter-nuclear interaction potential curves for the state of, state of, state of and state of molecules.
