        Clinical Significance of Serum Autoantibodies in Idiopathic Interstitial Pneumonia

        Although autoantibodies are routinely screened in patients with idiopathic interstitial pneumonia, there are no reliable data on their clinical usefulness. The aim of this study was to investigate the prognostic value of autoantibodies for predicting the development of new connective tissue disease in these patients and also mortality. We conducted retrospective analysis of the baseline, and follow-up data for 688 patients with idiopathic interstitial pneumonia (526 with idiopathic pulmonary fibrosis, 85 with nonspecific interstitial pneumonia, and 77 with cryptogenic organizing pneumonia) at one single tertiary referral center. The median follow-up period was 33.6 months. Antinuclear antibody was positive in 34.5% of all subjects, rheumatoid factor in 13.2%, and other specific autoantibodies were positive between 0.7%-6.8% of the cases. No significant difference in patient survival was found between the autoantibody-positive and -negative groups. However, the presence of autoantibodies, especially antinuclear antibody with a titer higher than 1:320, was a significant predictor for the future development of new connective tissue diseases (relative risk, 6.4), although the incidence was low (3.8% of all subjects during follow-up). In conclusion, autoantibodies are significant predictors for new connective tissue disease development, although they have no prognostic value.

        원형탈모증의 철 결핍 관련성 및 자가면역항체 양성률 , 갑상선 기능 이상 빈도에 관한 연구

        N/A Background:Iron deficiency is known as an aetiological factor in diffuse alopecia and androgen-dependent alopecia, especially in females. In alopecia areata, a few studies have been performed to evaluate the role of iron deficiency, but the results were not conclusive. Alopecia areata is suspected to be an autoimmune disease and increased frequencies of thyroid disease, vitiligo, and autoantibodies have been demonstrated. Objective:The purpose of this study is to assess the role of iron deficiency in alopecia areata and to study the frequency of abnormal thyroid function tests and autoantibodies in Korean patients with alopecia areata. Method:A total of 40 (32 premenopausal and 8 postmenopausal) female and 33 male patients with patchy alopecia areata were included. Hematologic tests, thyroid function tests and autoantibody tests were performed. Result:About 65% of premenopausal female patients with alopecia areata had low level of serum ferritin below 20 ng/ml. This percentage was increased to 87% with low limit of 40 ng/ml, and to 100% with low limit of 70 ng/ml in premenopausal women, but all postmenopausal patients showed normal ferritin level. About one-fifth of premenopausal female patients had low serum iron level and one-tenth had low hemoglobin level. Serum ferritin level was significantly lower in premenopausal women (21.5±12.4 ng/ml) than in postmenopausal women (71.1±47.1 ng/ml) (p<0.05). Hematologic test abnormalities were very infrequent in male patients with alopecia areata. Thyroid function tests were normal in most of the patients. Frequency of antinuclear antibody was 30% in female patients with alopecia areata, which was higher than 9% in male patients. Frequencies of antimicrosomal and antithyroglobulin antibodies were rather low and they were similar in female and male patients. Conclusion:Our results suggest possible role of ferritin deficiency in premenopausal female patients with alopecia areata. Abnormalities in thyroid function tests or autoantibodies were not a major problem in alopecia areata patients. (Korean J Dermatol 2002;40(1):8~13)

        Uterine blood flow indices, antinuclear autoantibodies and unexplained recurrent miscarriage

        Objective To study the correlation between 2D and 3D uterine flow indexes and the presence or the absence of antinuclear antibodies (ANA) in women with unexplained recurrent miscarriage (uRM). Methods Fifty-two subjects (26 uRM and 26 control women) underwent 2D Doppler measurement of pulsatility index and resistance index of the uterine arteries in both the follicular and midluteal phase of the cycle. Additionally, 3D ultrasonography determination of vascularisation index, flow index, and vascularisation flow index was carried out with the aid of the VOCAL technique. Serum assay for the presence of ANA was performed in all women. Results Pulsatility index of ANA+ uRM women was higher than that of ANA- uRM women and control ANA+ and ANAwomen, both in the follicular and in the midluteal phase of the cycle. Vascularisation index in ANA- uRM women was significantly higher than that in ANA+ control women. Flow index in uRM ANA+ women was significantly lower than that of each of the other groups. Conclusion ANA might be involved in uRM by determining an impairment in uterine blood flow hemodynamic, particularly in uterine blood flow intensity and uterine artery impedance.
