        Intravenous Tenecteplase for Acute Ischemic Stroke Within 4.5–24 Hours of Onset (ROSE-TNK): A Phase 2, Randomized, Multicenter Study

        Background and Purpose Intravenous tenecteplase (TNK) efficacy has not been well demonstrated in acute ischemic stroke (AIS) beyond 4.5 hours after onset. This study aimed to determine the effect of intravenous TNK for AIS within 4.5 to 24 hours of onset. Methods In this pilot trial, eligible AIS patients with diffusion-weighted imaging (DWI)-fluid attenuated inversion recovery (FLAIR) mismatch were randomly allocated to intravenous TNK (0.25 mg/kg) or standard care within 4.5–24 hours of onset. The primary endpoint was excellent functional outcome at 90 days (modified Rankin Scale [mRS] score of 0–1). The primary safety endpoint was symptomatic intracranial hemorrhage (sICH). Results Of the randomly assigned 80 patients, the primary endpoint occurred in 52.5% (21/40) of TNK group and 50.0% (20/40) of control group, with no significant difference (unadjusted odds ratio, 1.11; 95% confidence interval 0.46–2.66; <i>P</i>=0.82). More early neurological improvement occurred in TNK group than in control group (11 vs. 3, <i>P</i>=0.03), but no significant differences were found in other secondary endpoints, such as mRS 0–2 at 90 days, shift analysis of mRS at 90 days, and change in National Institutes of Health Stroke Scale score at 24 hours and 7 days. There were no cases of sICH in this trial; however, asymptomatic intracranial hemorrhage occurred in 3 of the 40 patients (7.5%) in the TNK group. Conclusion This phase 2, randomized, multicenter study suggests that intravenous TNK within 4.5–24 hours of onset may be safe and feasible in AIS patients with a DWI-FLAIR mismatch.

      • Supply Chain Model Based on Concurrent Negotiation Particle Swarm

        Due to the uncertainty of the green product market, the manufacturers assume enormous R&D and manufacturing risks. Under such background, the problem regarding the cooperation between the manufacture and the retailer becomes an important factor influencing the decisions and the profits of the two parties. In allusion to such problem, the revenue sharing contract is proposed in this article to promote the deep cooperation between the manufacturer and the retailer. Firstly, in consideration of such factors as product greenness and risk avoidance, the centralized decision-marking model and the manufacturer predominated Stackelberg game model are established, wherein the supply chain efficiency of the latter model is discovered to be less than that of the former model. Therefore, the revenue sharing contract is proposed to coordinate the green supply chain so as to make the overall profit and efficiency of the supply chain reach the corresponding level of the centralized decision-making model. Finally, the numerical experiment is carried out to verify the effectiveness and the reliability of the proposed revenue sharing contract. The experiment result shows that the revenue contract can well coordinate the cooperative relationship between the manufacturer and the retailer and meanwhile maximally improve the utilities and the profits for the two parties.

      • Complex Regulatory Network of MicroRNAs, Transcription Factors, Gene Alterations in Adrenocortical Cancer

        Several lines of evidence indicate that cancer is a multistep process. To survey the mechanisms involving gene alteration and miRNAs in adrenocortical cancer, we focused on transcriptional factors as a point of penetration to build a regulatory network. We derived three level networks: differentially expressed; related; and global. A topology network ws then set up for development of adrenocortical cancer. In this network, we found that some pathways with differentially expressed elements (genetic and miRNA) showed some self-adaption relations, such as EGFR. The differentially expressed elements partially uncovered mechanistic changes for adrenocortical cancer which should guide medical researchers to further achieve pertinent research.

      • Current Evidence on the Relationship Between Two Polymorphisms in the NBS1 Gene and Breast Cancer Risk: a Meta-analysis

        Introduction: Published studies on the association between Nijmegen breakage syndrome 1(NBS1) gene polymorphisms and breast cancer risk have been inconclusive, and a meta-analysis was therefore performed for clarification. Methods: Eligible articles were identified by a search of MEDLINE and EMBASE bibliographic databases for the period up to March 2012. The presence of between-study heterogeneity was investigated using the chi-square-based Cochran's Q statistic test. When there was statistical heterogeneity, the random effects model was chosen; otherwise, fixed effects estimates were reported as an alternative approach. Results: A total of 11 eligible articles (14 case-control studies) were identified, nine case-control studies were for the 657del5 mutation (7,534 breast cancer cases, 14,034 controls) and five case-control studies were for the I171V mutation (3,273 breast cancer cases, 4,004 controls). Our analysis results indicated that the 657del5 mutation was associated with breast cancer risk (carriers vs. non-carriers: pooled OR =2.63, 95% CI: 1.76-3.93), whereas the I171V mutation was not (carriers vs. non-carriers: pooled OR =1.52, 95% CI: 0.70-3.28). Conclusion: The present meta-analysis suggests that the 657del5 gene mutation in the NBS1 gene plays a role in breast cancer risk, while the I171V mutation does not exert a significant influence.

        Seismic Stability Analysis of Slope Reinforced by Frame Anchors Considering Prestress

        Taking the slope reinforced by frame anchors as the research object, it is assumed that the potential sliding surface of the slope is an arc. Based on the limit equilibrium theory, the seismic stability analysis model of frame anchor reinforced slope considering anchor prestress is established. This study considers prestress as uniformly distributed forces acting on the slope surface and calculates the additional stress induced by prestressing within the slope to investigate its reinforcement effect on slopes. Thus, the stability of the slope is analyzed and calculated. On this basis, the functional relationship between the center position coordinates of the potential sliding surface and the safety factor is established. Using the optimization algorithm toolbox in Matlab, the possible center position area is dynamically searched, and the minimum safety factor and its corresponding center position coordinates are obtained. Taking slope engineering as an example, the calculated results are compared with the finite element calculation results. The results show that the calculation result regarding the anchor prestress as the uniformly distributed force is reliable, and the anchor prestress can significantly enhance slope stability. This calculation method applies to slope engineering in homogeneous soil with circular sliding surfaces.

        Enhanced Transfection of Polyplexes Based on Pluronic-Polypropylenimine Dendrimer for Gene Transfer

        Third generation cationic dendritic polymeric polypropyleneimine (PPI) was modified by Pluronic P123 and investigated for gene delivery. The cytotoxicity of P123-PPI was evaluated by the MTT assay and shown to be much lower than that of PPI alone. P123-PPI and PPI can both condense plasmid DNA into nanoparticles with a size of approximately 100 nm and a zeta potential of about 15 mV at the N/P ratio 20:1. The nanoparticles can protect plasmid DNA from being digested by DNase I at a concentration of 0.4 U/μg DNA. The nanoparticles were resistant to dissociation induced by 50% fetal bovine serum and 75 μg/mL sodium heparin. The transfection efficiency of SPC-A1 cells using P123-PPI/DNA nanoparticles was much higher than the transfection utilizing PPI/DNA nanoparticles. The addition of free P123 during the preparation of P123-PPI/DNA nanoparticles could significantly enhance the transfection efficiency in the presence of 10% fetal bovine serum. Therefore, P123-PPI/DNA complex nanoparticles may be a safe, efficient and promising cationic conjugate for gene delivery.

        Effects of Temperature, Illumination, and Sodium Ascorbate on Browning of Green Tea Infusion

        Browning of tea infusion is an obstructive factor influencing shelf life of ready-to-drink green tea. Effects of temperature and illumination on the browning of green tea infusion were investigated. It was shown that both elevated temperature and illumination led to the browning of green tea infusion, but temperature had greater effect on infusion color and level of catechins than illumination. The levels of unoxidized catechins such as (-)-epigallocatechin gallate (EGCg), (-)-epigallocatechin (EGC), (-)-epicatechin gallate (ECg), (-)-epicatechin (EC), and total catechins remaining in the tea infusion were significantly correlated to color parameters of the tea infusion. Sodium ascorbate inhibited the infusion browning by suppressing the oxidation of tea catechins and it is considered to be a more suitable preservative for prolonging shelf life of ready-to-drink green tea than ascorbic acid because it has less effect on tea taste. The effects of temperature and illumination on the epimerization of catechins were also discussed.

        Establishment of a novel myocarditis mouse model based on cyclosporine A

        Background: Myocarditis is a myocardial injury that can easily cause adolescent death. Traditional research models of animal invasion with viral components, lipopolysaccharide (LPS) or porcine myocardial myosin, among others, have the shortcomings of potential biological safety hazards and high animal mortality. Objective: To explore the construction of a novel myocarditis model with cyclosporine A and the potential genes and pathways associated with it. Methods: BALB/c mice were used in this study, and cyclosporin A and LPS were injected into the peritoneal cavity of mice. The successful establishment of the model was assessed by detecting serum myocardial injury markers and inflammatory factors levels, HE, IHC staining, and RT-qPCR methods. Key genes were obtained using the GSE35182 dataset from the GEO database and validated with the RT-qPCR method. Results: We found that a large number of inflammatory cells infiltrated the myocardium of mice in each group of Cyclosporin A constructed model, while the expression of inflammatory factor indicators was increased, and this model has the characteristics of high degree of local inflammation in myocardial tissue, low mortality, and safe and non-toxic treatment. Using GSE35182 data, we selected 18 Hub genes and validated Hub genes in myocardial tissue with RT-qPCR and found that multiple signaling pathways such as Toll-likereceptor signaling pathway(TLRs), Rap1 signal pathway(Rap1), and Chemokine signaling pathway may be involved in the development of myocarditis. Conclusion: Cyclosporin A can construct a new myocarditis model, and TLRs, Chemokines and Rap1 signaling pathways may be the core pathways of myocarditis.

        The detection efficiency study of NaI(Tl) scintillation detector with the different numbers of SiPMs

        SiPMs are generally coupled into whole columns in gamma energy spectrum measurement, but the relationship between the distribution of whole SiPM columns and the energy resolution of the measured energy spectra is rarely reported. In this work, ∅ 3 3 inch NaI scintillator is placed on an 8 8 SiPM array, and the energy resolution of the 137Cs peak at 662 keV corresponding to the g-ray is selected as a reference. Each SiPM is switched to explore the influence of the number of SiPM arrays, distribution position, and reflective layer on the energy resolution of SiPMs. Results show that without coupling, the energy resolution is greatly improved when the number of SiPMs ranges from 4 to 32. However, after 32 slices (the area covered by SiPMs relative to the scintillator reaches 25.9%), the improvement in energy resolution and total pulse count is not obvious. In addition, the position of SiPMs relative to the scintillator does not exert much impact on the energy resolution. Results also indicate that by adding a reflective film (ESR), the energy resolution of the tested group increases by 10.38% on average. This work can provide a reference for the design and application of miniaturized SiPM gamma spectrometers
