<P><B>Objectives</B></P><P>To evaluate the mobile health applications (apps) developed by a single tertiary hospital in Korea with a particular focus on quality and patient safety.</P><P><B>Methods</B></P><P>Twenty-three mobile health apps developed by Asan Medical Center were selected for analysis after exclusion of the apps without any relationship with healthcare or clinical workflow, the apps for individual usage, and the mobile Web apps. Two clinical informaticians independently evaluated the apps with respect to the six aims for quality improvement suggested by the United States Institute of Medicine. All discrepancies were resolved after discussion by the two reviewers. The six aims observed in the apps were reviewed and compared by target users.</P><P><B>Results</B></P><P>Eleven apps targeted patients, the other 12 were designed for healthcare providers. Among the apps for patients, one app also had functions for healthcare providers. 'My cancer diary' and 'My chart in my hand' apps matched all the six aims. Of the six aims, <I>Timeliness</I> was the most frequently observed (20 apps), and <I>Equity</I> was the least observed (6 apps). <I>Timeliness</I> (10/11 vs. 10/12) and <I>Patient safety</I> (10/11 vs. 9/12) were frequently observed in both groups. In the apps for patients, <I>Patient-centeredness</I> (10/11 vs. 2/12) and <I>Equity</I> (6/11 vs. 0/12) were more frequent but <I>Efficiency</I> (5/11 vs. 10/12) was less frequent.</P><P><B>Conclusions</B></P><P>Most of the six aims were observed in the apps, but the extent of coverage varied. Further studies, evaluating the extent to which they improve quality are needed.</P>

<P>Herein, we report a novel method for H<SUB>2</SUB>O<SUB>2</SUB> detection based on a single plasmonic nanoprobe <I>via</I> cytochrome <I>c</I> (Cyt <I>c</I>)-mediated plasmon resonance energy transfer (PRET). Dynamic spectral changes were observed in the fingerprint quenching dip of a single plasmonic nanoprobe in response to changes in the redox state of Cyt <I>c</I>, induced by H<SUB>2</SUB>O<SUB>2</SUB>. Based on the changes in the spectral profile of the single plasmonic nanoprobe, H<SUB>2</SUB>O<SUB>2</SUB> was successfully detected in a wide concentration range from 100 mM to 10 nM, including physiologically relevant micromolar and nanomolar concentrations.</P> <P>Graphic Abstract</P><P>We report a novel method for H<SUB>2</SUB>O<SUB>2</SUB> detection based on a single plasmonic nanoprobe <I>via</I> cytochrome <I>c</I>-mediated plasmon resonance energy transfer. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c5cc05327g'> </P>

Background: Lung cancer is associated with significant psychological distress, including fear of progression (FoP). Because insomnia and depression are highly prevalent and associated with FoP, we examined the association between FoP, insomnia, and depression in cancer patients. Furthermore, we tested the mediation effect of cancer-related dysfunctional beliefs about sleep (C-DBS) on this association. Methods: We analyzed data collected from patients with surgically resected non-small cell lung cancer from a single-center randomized controlled study investigating digital healthcare applications. Baseline demographic and clinical variables were collected. In addition, selfreported questionnaires including the Fear of Progression Questionnaire-Short Form, Patients Health Questionnaire-9 items (PHQ-9), Insomnia Severity Index, and C-DBS were administered. Results: Among the 320 enrolled patients with lung cancer, a regression model showed that FoP was predicted by age (β = −0.13, P = 0.007), PHQ-9 (β = 0.35, P < 0.001), and C-DBS (β = 0.28, P < 0.001). Insomnia did not directly influence FoP, but C-DBS mediated the association. Depression directly influenced FoP, but C-DBS did not mediate this association. Conclusion: Among patients with surgically resected lung cancer, C-DBS mediated the effects of severity of insomnia on FoP. Depression directly influenced FoP, but C-DBS did not influence this association. To reduce FoP among patients with lung cancer, C-DBS should be addressed in the cognitive behavioral therapy module.

Background/Aims: Functional dyspepsia (FD) has long been regarded as a syndrome because its pathophysiology is multifactorial. However, recent reports have provided evidence that changes in the duodenal ecosystem may be the key. This study aimed to identify several gastrointestinal factors and biomarkers associated with FD, specifically changes in the duodenal ecosystem that may be key to understanding its pathophysiology. Methods: In this case-control study, 28 participants (12 with FD and 16 healthy control individuals) were assessed for dietary nutrients, gastrointestinal symptom severity, immunological status of the duodenal mucosa, and microbiome composition from oral, duodenal, and fecal samples. Integrated data were analyzed using immunohistochemistry, real-time polymerase chain reaction, 16S rRNA sequencing, and network analysis. Results: Duodenal mucosal inflammation and impaired expression of tight junction proteins were confirmed in patients with FD. The relative abundance of duodenal Streptococcus (p=0.014) and reductions in stool Butyricicoccus (p=0.047) were confirmed. These changes in the gut microbiota were both correlated with symptom severity. Changes in dietary micronutrients, such as higher intake of valine, were associated with improved intestinal barrier function and microbiota. Conclusions: This study emphasizes the relationships among dietary nutrition, oral and gut microbiota, symptoms of FD, impaired function of the duodenal barrier, and inflammation. Assessing low-grade inflammation or increased permeability in the duodenal mucosa, along with changes in the abundance of stool Butyricicoccus, is anticipated to serve as effective biomarkers for enhancing the objectivity of FD diagnosis and monitoring.
Porokeratosis ptychotropica: a case report

Porokeratosis ptychotropica is an uncommon form of porokeratosis, which was initially described in 1995. It is clinically characterized by symmetrical reddish to brown-colored hyperkeratotic, verrucous, or psoriasiform plaques on the perianal and gluteal regions. The lesions tend to integrate and expand centrally, with small peripheral satellite lesions. Early skin biopsy and appropriate diagnosis are essential because malignant change occurs in 7.5% of porokeratotic lesions. Conventional treatment options include topical steroid, retinoid, imiquimod, 5-fluorouracil, isotretinoin, excimer laser, photodynamic therapy, intralesional steroid or bleomycin injection, cryotherapy, carbon dioxide (CO2) laser, and dermatome and excision, but none seem to achieve complete clearance. A 68-year-old woman presented with diffuse hyperkeratotic scaly lichenoid plaques on the buttocks that had persisted for several years. A skin biopsy of the buttocks revealed multiple cornoid lamellae and intense hyperkeratosis. There were some dyskeratotic cells beneath the cornoid lamellae and the granular layer was absent. Porokeratosis ptychotropica was diagnosed based on the characteristic clinical appearance and typical histopathological manifestations. She was treated with a CO2 laser in one session and topical application of urea and imiquimod cream for 1 month. The lesions slightly improved at the 1-month follow-up. We herein present a rare case of porokeratosis ptychotropica.

The heat from bulbs of automotive lamp is transferred to internal materials in the form of conduction, convection and radiation. The transferred heat causes deformation of lamp materials when it exceeds the heat-resistance limits. To reduce the effort from trial and error by failing the heat-resistance test after designing and producing lamps, using a simulation program is necessary at the design stage. A new in-house heat-resistance prediction program with less errors is going to be developed, which covers conduction, convection and radiation based on experiments. The experimental facility is designed, according to variables, such as bulb powers, heat-resistant distance, boundary conditions, etc. Key parameters were set to select and perform experimental cases. The surface average temperatures were calculated, and the temperature distribution patterns of each surface were found. As a result of the experiment, the temperature distribution of all data values was different at temperature, but the shape was very similar. This shows similar heat transfer patterns throughout the data. In the case of forced convection, the temperature distribution was lower depending on the atmosphere temperature, bulb distance, and box size compared to the natural convection. By securing reliable data, algorithm development and verification program based on the experiment is possible.
