To determine the recovery time from noise-induced temporary threshold shift(TTS), a prospective field study was conducted at three worksites where workers are known to be exposed high level of noise. Subjects were selected according to answers on a questionnaire which inquired about otological history and previous noise exposure, including avocational, military and occupational exposures. After excluding employee with past otologic problems, recent exposure to high level noise, and under medications, total 92 employees participated in the study. Among 92 participants, complete consecutive audiometric examinations were carried out at 0∼2 hours, 5∼7 hours, 14∼16 hours after worktime noise exposure on 26 participants wearing hearing protectors and 22 participants wearing no protective devices. The difference between the hearing level 0∼2 hours after noise exposure and 5∼7 hours is statistically significant by paired t-test(p<0.01). The median recovery times calculated from the data of 22 participants wearing no protective hearing devices are 15.6 hours at 4000Hz, and 7.7 hours, 10.3 hours, 8.4 hours at 1000Hz, 2000Hz and 8000Hz respectively. These data suggest that when measuring the pure tone audiometry for noise exposed workers, at least 16 hours noise-free interval is required.

Group occupational health service programme started in 1990 is one of the measures to cope with limited human and financial resources in occupational health. The programme has expanded rapidly to include 52 institutions, private as well as public, all over the country. In spite of its potential impact on health of employees and practice of occupational health in small and medium sized industries, comprehensive evaluation in terms of quality has not been tried. This study has aims to develop the criteria to assess the quality of newly developed group occupational health service programme, and to investigate the quality of institutions, and finally to develop policies for the quality improvement. 1) Criteria development : By defining occupational health services, in particular for small and medium sized industries, as one of the primary health care, followings are included as core elements of qualitative occupational health programme; accessibility, continuity, comprehensiveness, technical quality, intersectoral collaboration, emphasis on preventive services, community participation, and adequacy. Again each element is divided into five major components of national health system infrastructure developed by the World Health Organization; development of health resources, organized arrangement of resources, delivery of health care, economic support, and management. In turn, each component is categorized into three aspects of quality assessment, structure, process and outcome. Expert panel selected several criteria for each category to evaluate the programme. Criteria were modified according to each group of interviewees, to produce two sets of questionnaire, one for chief operating officer and another for nurses in the institutions, and the chief operating officer and workers in the workplace. 2) Subject : Of all 52 institutions, 25 voluntarily participated in the survey. At individual institution, chief operating officer and practicing nurses were interviewed in depth. After intensive education for interviewees, every interviewees, every interview was performed with standardized guideline and questionnaire. The quality of the 'Group occupational health service programme' was found to be lower than expected. Especially in continuity, comprehensiveness, technical quality, community participation and adequacy, lower quality in structural aspect was commonly identified throughout all the institutions,. Quality in terms of accessibility and continuity highly varied among institutions., To improve quality of the programme, more comprehensive and systematic programme such as accreditation has to be introduced. In addition, human resources, governmental fund and information systems for individual workers are to be developed. As a long range plan, integration of occupational health services into the national health systems and pooling of financial resources and planned allocation should be considered.

Objectives: To assess knowledge of HPV and attitudes towards HPV vaccination among the general female population, government officials, and healthcare providers in China to assist the development of an effective national HPV vaccination program. Methods: A cross-sectional epidemiologic survey was conducted across 21 urban and rural sites in China using a short questionnaire. 763 government officials, 760 healthcare providers, and 11,681 women aged 15-59 years were included in the final analysis. Data were analyzed using standard descriptive statistics and logistic regression. Results: Knowledge of HPV among the general female population was low; only 24% had heard of HPV. Less than 20% of healthcare providers recognized sexually na$\ddot{i}$ve women as the most appropriate population for HPV vaccination. There was high acceptance of the HPV vaccine for all categories of respondents. Only 6% of women were willing to pay more than US $300 for the vaccine. Conclusions: Aggressive education is necessary to increase knowledge of HPV and its vaccine. Further proof of vaccine safety and efficacy and government subsidies combined with increased awareness could facilitate development and implementation of HPV vaccination in China.

Under the COVID-19 pandemic, interest in immune enhancement is increasing. Although the immune-enhancing activity of plants of the genus Hibiscus has been reported, there is no study on the immune-enhancing activity of H. syriacus. Thus, in this study, we investigated the immune-enhancing activity of Hibiscus syriacus leaves (HSL) in mouse macrophages, RAW264.7 cells, and immunosuppressed mice. HSL increased the production of immunostimulatory factors such as nitric oxide (NO), inducible nitric oxide synthase (iNOS), interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α) and activated the phagocytosis in RAW264.7 cells. The HSL-mediated production of immunostimulatory factors was dependent on toll-like receptor 4 (TLR4), p38, and c-Jun N-terminal kinase (JNK) in RAW264.7 cells. In the immunosuppressed mouse model, HSL increased the spleen index, the levels of the cytokines, and the numbers of lymphocytes, neutrophils, and monocytes. Taken together, HSL may be considered to have immune-enhancing activity and be expected to be used as a potential immune-enhancing agent.
S-413 Giant functioning parathyroid cyst presenting with exertional dyspnea and mild dysphagia

Background: The differential diagnosis of cystic cervical masses includes thyroid cyst, thymic cyst, thyroglossal duct cyst, branchial cleft cyst, bronchogenic cyst, lymphangioma, and parathyroid cyst (PC). PC is one of the less common causes of cervical masses. Large PCs can manifest with compressive symptoms of the surrounding tissues. Functioning parathyroid cysts are a rare cause of primary hyperparathyroidism and are often mistaken for thyroid cysts. We report a case where a functioning parathyroid cyst presented as a mediastinal mass to emphasize the potential pitfalls associated with their diagnosis and management.?Case: A 59-year-old male patient visited our clinic for an assessment of dyspnea on exertion and mild dysphagia, symptoms that had begun 6 months prior. Chest computed tomography and neck ultrasound scanning revealed a large mediastinal cystic mass of approximately 10.8 × 5.1 cm in size, located inferior to the left thyroid lobe, causing right-lateral displacement of the large vessels, trachea, and esophagus. We performed fine needle aspiration for the cystic mass, wherein we aspirated approximately 100 cc of fluid with the appearance of old blood and tiny crystals. We also measured the parathyroid hormone (PTH) level of the aspirated fluid to be >5000 pg/mL, suggesting that the fluid originated from the parathyroid gland. The patient presented with primary hyperparathyroidism and elevated levels of calcium and PTH. He subsequently underwent successful parathyroidectomy without any postoperative morbidity, which resulted in complete clinical resolution of the presenting symptoms.?Conclusions: The present study reported the case of a man with a giant functional PC extending from the lower neck to the superior mediastinum. Fine needle aspiration and the detection of increased levels of PTH or thyroglobulin in the fluid of the cyst is a valuable diagnostic tool to confirm cystic neck lesions. Functional hyperparathyroidism should always be considered when parathyroid cyst is confirmed, with surgical excision being the treatment of choice in functioning giant PC.

A variety of biopharmaceuticals such as antibodies, proteins, and gene/cell therapeutics have been developed in recent decades and demands for them have increased dramatically every year. To meet such demands, it is essential to cultivate cells that produce biopharmaceuticals in large-scale bioreactors and it is very important to optimize culture conditions for massive production of biopharmaceuticals. Scale-down model, using multi- and small size (less than 100 mL) bioreactors, has been an effective tool for applying DOE (Design of Experiment) and QbD (Quality by Design) for process optimization of large-scale cell cultures. In this study, a newly developed single-use multi-bioreactor system that can control pH and DO (Dissolved Oxygen) with optical sensors, and culture temperature of each bioreactor which is expandable to 24 bioreactors is introduced. And the evaluation of biological and physical performances of this single-use multi-bioreactor system are explored. Ultimately, this research implies that the newly developed cell culture system can be a practical tool for optimization of cell culture process in the biopharmaceutics industry.

Allium hookeri (Liliaceae) has been received the increasing attention as a bioactive resource due to its potent biological activities including anti-oxidant, anti-obesity, anti-microbial and lipid-regulating activities. The beneficial effects of A. hookeri are known contributed from the high content of organosulfur compounds in A. hookeri. Though a variety of articles demonstrated that A. hookeri contains 'saponin' as a bioactive constituent, the scientific evidence to prove it was limited. In the present study, we have attempted to identify saponin contained in A. hookeri through chromatographic isolation and NMR spectroscopic methods. As a result, a spirostane-type steroidal saponin (1) has been successfully isolated from the methanolic extract of A. hookeri roots. The structure of 1 was elucidated by extensive 1D and 2D spectroscopic methods including <sup>1</sup>H-NMR, <sup>13</sup>C-NMR, <sup>1</sup>H-<sup>1</sup>H COSY, HSQC, HMBC and NOESY; identified as (3β, 22R, 25S)-spirost-5-en-3yl O-6-deoxy-α-L-mannopyranosyl-(1→4)-O-6-deoxy-α-L-mannopyranosyl-(1→4)-O-[6-deoxy-α-L-mannopyranosyl-(1→2)]-β-D-gluco pyranoside. 1 showed the significant inhibitory activity on mushroom tyrosinase with IC<sub>50</sub> values of 248.7 μM while the inhibition on alpha-glucosidase was not significant.
