Multiple system atrophy (MSA) is an adult-onset, progressive neurodegenerative disorder. Patients with MSA show various phenotypes during the course of their illness, including parkinsonism, cerebellar ataxia, autonomic failure, and pyramidal signs. Patients with MSA sometimes present with isolated autonomic failure or motor symptoms/ signs. The median duration from onset to the concomitant appearance of motor and autonomic symptoms is approximately 2 years but can range up to 14 years. As the presence of both motor and autonomic symptoms is essential for the current diagnostic criteria, early diagnosis is difficult when patients present with isolated autonomic failure or motor symptoms/signs. In contrast, patients with MSA may show severe autonomic failure and die before the presentation of motor symptoms/signs, which are currently required for the diagnosis of MSA. Recent studies have also revealed that patients with MSA may show nonsupporting features of MSA such as dementia, hallucinations, and vertical gaze palsy. To establish early diagnostic criteria and clinically definitive categorization for the successful development of disease-modifying therapy or symptomatic interventions for MSA, research should focus on the isolated phase and atypical symptoms to develop specific clinical, imaging, and fluid biomarkers that satisfy the requirements for objectivity, for semi- or quantitative measurements, and for uncomplicated, worldwide availability. Several novel techniques, such as automated compartmentalization of the brain into multiple parcels for the quantification of gray and white matter volumes on an individual basis and the visualization of α-synuclein and other candidate serum and cerebrospinal fluid biomarkers, may be promising for the early and clinically definitive diagnosis of MSA.

Pollinators play an important role in the production of many agricultural products. Honeybees, Apis mellifera L., are leading pollinators, but the number of honeybees in the world is declining. Finding alternatives is beginning to be important. In the present study, we compared the longevity of Lucilia sericata (Meigen, 1826), which is used as a pollinator, in cages in the laboratory and in a vinyl greenhouse under controlled feeding conditions. First, we showed that the longevity of the flies was significantly extended (c.a. 40 days) in the laboratory when water and sugar were supplied compared to the cases without sugar and/or water. Second, we found that the average longevity was 20 days with water and sugar in cages kept in a vinyl greenhouse during the summer. Finally, we released marked flies into a vinyl greenhouse where strawberries were cultivated without feeding water or sugar in the spring. As a result, fewer than 10% of the flies survived 10 days after release. Based on the result, we discuss the use of this species as a pollinator in the context of its survival rate.

Study Design: Clinical case series.Purpose: This study aimed to report dynamization–posterior lumbar interbody fusion (PLIF), our surgical treatment for hemodialysisrelated spondyloarthropathy (HSA), and investigate patients’ postoperative course within 2 years.Overview of Literature: HSA often requires lumbar fusion surgery. Conventional PLIF for HSA may cause progressive destructive changes in the vertebral endplate, leading to progressive cage subsidence, pedicle screw loosening, and pseudoarthrosis. A dynamic stabilization system might be effective in patients with a poor bone quality. Thus, we performed “dynamization–PLIF” in hemodialysis patients with destructive vertebral endplate changes.Methods: We retrospectively examined patients with HSA who underwent dynamization–PLIF at our hospital between April 2010 and March 2018. The radiographic measurements included lumbar lordosis and local lordosis in the fused segment. The evaluation points were before surgery, immediately after surgery, 1 year after surgery, and 2 years after surgery. The preoperative and postoperative radiographic findings were compared using a paired t-test. A p-value of less than 0.05 was considered significant.Results: We included 50 patients (28 males, 22 females). Lumbar lordosis and local lordosis were significantly improved through dynamization– PLIF (lumbar lordosis, 28.4°–35.5°; local lordosis, 2.7°–12.8°; <i>p</i><0.01). The mean local lordosis was maintained throughout the postoperative course at 1- and 2-year follow-up (12.9°–12.8°, p=0.89 and 12.9°–11.8°, <i>p</i>=0.07, respectively). Solid fusion was achieved in 59 (89%) of 66 fused segments. Solid fusion of all fixed segments was achieved in 42 cases (84%). Within 2 years postoperatively, only six cases (12%) were reoperated (two, surgical debridement for surgical site infection; two, reoperation for pedicle screw loosening; one, laminectomy for epidural hematoma; one, additional fusion for adjacent segment disease).Conclusions: Dynamization–PLIF showed local lordosis improvement, a high solid fusion rate, and a low reoperation rate within 2 years of follow-up.

The amorphization of indomethacin (IMC) with cystine (Cys2) was studied by discrete element method (DEM) simulations and principal component analysis (PCA). X-ray powder diffraction (XRPD) analysis suggested that the conversion of crystalline IMC to amorphous state was accelerated by co-grinding with Cys2. XRPD spectra about amorphization of IMC with Cys2 were analyzed by PCA. PCA results suggest that IMC/Cys2 system undergoes two-phase amorphization, as indicated by the 2nd PC score, and that the change in phase depends on the total frictional energy calculated by DEM simulations. Electron spin resonance result revealed that the radical from Cys2 may be related to the amorphized progression of IMC.

In this paper,I will report the cases of children who are able to study at kindergarten or elementary school because they learned how to move by themselves using a moving aid before school age, and I will also discuss the development of a mobility device which allows severely disabled preschoolers to practice moving around by themselves safely and easily at home and institutions.
