Case Reports : A Case of Spontaneous Portal Vein Thrombosis After Splenectomy and its Resolution

Portal vein thrombosis (PVT) is a rare but serious complication of splenectomy that is more common in patients with hematologic disorders (hemolytic anemia, myeloproliferative disease, etc.), but relatively rare when splenectomy is conducted because of trauma. We present the case of a 42-year-old woman, admitted for abdominal trauma, who produced a computed tomography (CT) scan showing a grade IV splenic laceration with perisplenic hematoma and subsequently underwent splenectomy. Preoperatively, a hematologic workup revealed no underlying platelet or coagulation disorder. The patient developed PVT in her anterosuperior branch of the right portal vein and anticoagulation treatment was immediately started. After the event was resolved, no symptoms of PVT were observed, which was confirmed by laboratory and radiologic findings. In summary, we report a case of spontaneous PVT after splenectomy for trauma and its successful resolution with anticoagulation treatment.

혼성체육수업의 남학생 역차별이 체육교사에 대한 동일시, 체육수업의 재미, 몰입에 미치는 영향

본 연구는 중학교 3학년 남학생들 대상으로 혼성체육수업의 역차별이 체육교사 동일시, 체육수업의 재미, 몰입에 어떠한 영향을 미치는가를 분석하는 것을 목적으로 수행되었다. 이를 위해 중학교 3학년 남학생 871명을 연구대상으로 선정하여 연구를 진행하였다. 수집된 자료는 SPSS-WIN 15.0을 이용하여 중다회귀분석을 통해 분석되었고, 다음과 같은 결론을 얻을 수 있었다. 첫째, 혼성체육수업의 처벌, 수업활동의 차별이 체육교사 동일시에 부적인 영향을 미쳤다. 둘째, 혼성체육수업의 역차별이 체육수업 재미에 부적인 영향을 미치는 것으로 나타났는데 평가의 차별은 사회성 함양에 긍정적인 영향을 미쳤다. 셋째, 혼성체육수업의 역차별이 체육수업 몰입에 부적인 영향을 미치는 것으로 나타났는데 평가의 차별은 체육수업과정 통합과 수업행동과 의식의 일치에 긍정적인 영향을 미치는 것으로 나타났다. 마지막으로, 혼성체육수업의 체육수업 재미가 체육수업 몰입에 긍정적인 영향을 미치는 것으로 나타났는데 수업의 용이성이 자기 목적적 경험에는 부적인 영향을 미치는 것으로 제시되었다. The purpose of this study was to analyze how reverse discrimination in coeducational classes affected students` overall enjoyment of P.E. students` personal identification with the P.E. teacher, and student`s flow. For achieving this purpose, some relevant questionnaires were distributed to 871 third grade male middle school students. This study drew the following conclusions. First, different methods of punishment between genders during class activity produced a negative effect on students` trust and identification with the P.E. teacher. Second, reverse discrimination in coeducational classes also produced a negative effect on the enjoyment of P.E., whereas differing standards in evaluation produced a positive effect on students` sociality. Third, while reverse discrimination in coeducational classes had a negative effect on students` absorption in P.E., differing gender requirements in student evaluations produced a positive effect on the accord between class behavior, consciousness, and the integration of the P.E. course. Finally, the overall enjoyment students experienced from a coeducational P.E. class played a positive role that improved student`s flow. The subsequently easier drills and exercises, however, were found to produce a negative effect on the overall experience for male students.

A magnetic fluid separation is applied to the advanced treatment of sewage for removing the suspended solids which discharged as being entrained in the effluent from the final settling pond. In this study, the correlation with the concentration of suspended solids in the sewage and COD, with the solid concentration and turbidity, and the effects of operating parameters on the removal of the pollutant materials from the sewage were investigated. It is expected that COD and turbidity in sewage can be reduced by the removal of suspended solids in the sewage. And in result, turbidity was most strongly affected by the removal, and followed by ICOD and TCOD. In the present study, the concentration of suspended solids, TCOD and turbidity could be kept below 20 g/㎥, 60 g/㎥ and 16 NTU, respectively. The feasibility, therefore, of the magnetic fluid separation as an attractive technology for the advanced treatment of sewage was verified. However a special treatment for the extra reduction of TCOD in the clarified sewage is necessary.

This study aims at providing some basic material to be made use of for establishing plans through which one can reduce job-related stress, through conducting a survey on the sources of stress and then distinguishing its symptoms by the sources. As the methods of the study, we conducted a questionnaire survey of some selected medium enterprises' employees in the National Capital region and analysed the results. The questionnaire was designed by using the dynamic stress model of Cooper and Eaker and consisted of 85 questions to examine the sources of stress, the characteristics of personalities, and symptoms by each type of stress. The total valid respondents were 392 persons indicating 65.3% of the response rate. For analysing the results, a rating scale was used, and the reliability and validity were tested by using Cronbach's alpha coefficient. As the result of the study, those workers in medium enterprises were found to be put under much stress by responsibility, physical environment factors and career development factors and so on. As physical symptoms of job-related stress, eye fatigue, stiffness in their shoulders, headache, worry or anxiety, and waist ache presented most in the respondents. In addition, as behavioral symptoms, chronic fatigue was most indicated. In organizational symptoms, dissatisfaction about the company and lack of self-motivation for the job performance were found to show most. Moreover, the results indicated that the closer personalities to those appearing in persons mostly of type A they had, the more they were put under intensive stress by the vagueness of their roles and responsibility among all the job characteristics.
Synchronous T-cell lymphoma in patient with colon cancer

Colorectal cancer is the third most common malignancy in Korea. In contrast, pericolic or mesenteric lymphoma is relatively rare. We experienced an extremely rare case of synchronous primary colon cancer in the ascending colon with T-cell lymphoma in the pericolic lymph node. A 79-year-old woman presented with complaints of epigastric and right lower abdominal pain combined with anorexia and nausea. Colonoscopic evaluation and biopsy were performed, and the diagnosis was cecal adenocarcinoma. She underwent right hemicolectomy with lymph node dissection. The pathology report revealed adenocarcinoma in cecum with metastasis to 1 regional lymph node out of 37 lymph nodes. In addition, there was malignant angioimmunoblastic T-cell lymphoma in 1 pericolic lymph node. There was no evidence of lymphoma in ileum, cecum and ascending colon, so the possibility of early phase of lymphoma was suggested.
파라쿼트를 투여한 백서의 급성 폐 손상 모델에서 메틸렌블루 투여가 Inducible Nitric Oxide Synthase 유전자 발현에 미치는 효과에 관한 연구

Purpose: This study was designed to determine if methylene blue inhibited the lipid peroxidation, the production of NO, and the gene expression of iNOS in acute lung injury induced by paraquat and if the inhibitory effect was dose dependent. Methods: Female Sprague-Dawley rats were divided into four groups: the control group, the group treated with paraquat only, the group treated with paraquat and a low dose of methylene blue (2 mg/kg), and the group treated with paraquat and a high dose of methylene blue (20mg/kg). Methylene blue was administered via the jugular vein 1 h after paraquat administration, and animals were sacrificed 6 and 24 h after paraquat administration. Malondialdehyde (MDA) as lipid peroxidation, reduced glutathione (GSH) as an antioxidant defense, the plasma NO concentration, and the expression of iNOS mRNA in the lung tissue were measured Results: Lung MDA contents decreased, with no significant difference between the methylene-blue groups and the paraquat-only group. Lung GSH contents were significantly elevated at 24 h in the methylene-blue groups compared with the paraquat-only group. Plasma NO concentrations were significantly reduced at 6 and 24 h in the methylene-blue groups compared with the paraquat-only group. There was also a significant decrease in the plasma NO concentration at 6 h in the high-dose methylene-blue group compared with the low-dose methylene-blue group. The expression of iNOS mRNA in the lung tissue was slightly decreased in the methylene-blue groups. It was also markedly increased at 24 h in the paraquat-only group compared with the methylene-blue groups. The gene expression was relatively decreased in the high-dose methylene-blue group compared with the low-dose methylene-blue group. Conclusion: This study suggests that methylene blue has an inhibitory effect on the plasma NO concentration and the expression of iNOS mRNA in lung injury induced by paraquat. No inhibitory effect of methylene blue on lipid peroxidation or dose-dependent inhibitory effects were clearly shown.
