<P>BACKGROUND: A lighted stylet is an effective alternative to a direct laryngoscope and has been reported to be particularly useful in patients with difficult airways. A high Mallampati class indicates poor visibility of the oropharyngeal structures. Because a lighted stylet does not require direct oropharyngeal visualization, we hypothesized that the lighted stylet would be easier to use than a direct laryngoscope in patients with a high Mallampati score. To examine our hypothesis, we performed a prospective, randomized study comparing a lighted stylet (Surch-Lite) with direct laryngoscopy in patients with high Mallampati scores. Success rate, time required for intubation, and hemodynamic changes were compared. METHODS: Mallampati Class III patients were enrolled and were randomly assigned to the Surch-Lite group (Group SL) or the direct laryngoscopy group (Group DL). Patients' tracheas were intubated with the randomly selected intubation device after induction of general anesthesia. Heart rate (HR) and mean arterial blood pressure were measured immediately before and every 30 s after intubation for 5 min. The time to intubation and success rate were recorded. Postoperative pharyngolaryngeal complaints were also assessed. RESULTS: Thirty patients were enrolled in each group. The success rate on the first attempt was significantly higher in Group SL (29 of 30) than in Group DL (24 of 30). The difference between maximal HR and baseline HR was significantly higher in Group DL (25 +/- 13 bpm) than in Group SL (16 +/- 10 bpm). The change in mean arterial blood pressure was also higher in Group DL (38 +/- 14 mm Hg) than in Group SL (20 +/- 13 mm Hg). The time to intubation was significantly shorter in Group SL (12 +/- 6 s) than in Group DL (17 +/- 12 s). Postoperative pharyngolaryngeal complaints were not significantly different between the two groups. CONCLUSIONS: The Surch-Lite showed a higher success rate on the first intubation attempt and produced an attenuated hemodynamic response to endotracheal intubation of patients with high Mallampati score. Thus, the Surch-Lite is an effective alternative to direct laryngoscopy in these patients.</P>
Cloning and DNA Sequencing of Flagellin Gene of Helicobacter pylori

A λgtll expression library of Helicobacter pylori DNA was screened with flagllin-specific rabbit antiserum for molecular cloning of the flagellin gene of Helicobacter pylori. A positive clone,λHPF4, was obtained and the recombinant antigen expressed from λHPF4 was a fusion protein with the molecular weight of 168kd. Sequence analysis of antigen-encoding DNA ahowed that an open reading frame of 1536 nucletides encodes a polypeptide with a predicted molecular size of 54kd. This open reading frame did not show any homology with flaA gene encoding 56kd protein of Helicobacter pylori amd was confirmed as a uniques sequence through homology searching. The cloned antigen is tentatively supposed to be the carboxy-termonal region of the other flagellin protein of Helicobacter pylori, flaB, with the molecular weight of 58kd.
Flexible Video Authentication Based on Aggregate Signature

In this paper we propose a flexible video authentication scheme based on aggregate signature, which provides authenticity of a digital video by means of cryptographic signature to guarantee right of users. In contrast to previous works, the proposed scheme provides flexible usages on content distribution system, and it allows addition of new contents to the signed contents and deletion of some parts of the signed contents. A modification can be done by content owner or others. Although contents are modified by one or more users, our scheme can guarantee each user's right by aggregation of the each user's signatures. Moreover, the proposed scheme has half size of Digital Signature Algorithm (DSA) with comparable security.
The Role of Teachers in Furthering the Development of Social Competence in Young Children

Establishing and maintaining positive social interaction with peers is important for young children because it will form a foundation for their later social adjustment and academic achievement. Social competence in this study means an increasing awareness and understanding of other human beings as part of the child's world. The purpose of the study was to help four-year-old children to accept other children in their mind. This study had three specific aims: To find the reasons for their difficulties; to examine their changing play behavior; and to observe teachers' roles in helping them to become involved in peer group play. Lack of communication skills and emotional instability were the main difficulties the children faced. Regarding the changing play pattern, they passed through five distinctive stages: Staying alone, showing strong interest in the peer group play, going voluntarily to friends, actively playing with one or two friends, and finally becoming an active member of the peer play group. In improving children's involvement in peer group play the teachers played several important roles: Making them feel safe, giving them an opportunity to experience familiar things, suggesting plays, creating opportunities to become acquainted with other children, and modeling how to play with other children.
Characteristics of Cytosolic Calcium-Independent Phospholipase $A_2$ Isolated from Rat Liver

A calcium-independent phospholipase $A_2$ ($iPLA_2$) was identified from the cytosolic fraction of rat liver cells. On gel filtration chromatography, the $iPLA_2$ activity was eluted as broad peaks of 150 to 500 kDa. The enzyme was maximally active at pH 7.5, retained 75% of its original activity after heating at $50^{\circ}C$ for 5 h, and was inhibited by $Ca^{2+}$, $Mg^{2+}$, and $Zn^{2+}$ ions, but was not affected by $Na^+$ and $K^+$ ions. The enzymatic activity was increased up to 150% by 1 to 4 mM DTT and was inhibited up to 25% by 0.1 to 1 mM PMSF. The $iPLA_2$ activity had preference for the head group of phospholipids, where phosphatidylethanolamine was preferred to phosphatidylcholine. The results suggest that the $iPLA_2$ may be a novel enzyme distinct from the previously reported $iPLA_2s$.

Matrix metalloproteinase-9 (MMP-9) plays a pivotal role in the turnover of extracellular matrix (ECM) and in the migration of normal and tumor cells in response to normal physiologic and numerous pathologic conditions. Here, we show that the transcription of the MMP-9 gene is induced by lipopolysaccharide (LPS) stimulation in cells of a macrophage lineage (RAW 264.7 cells). We provide evidence that the NF-$\kappa$B binding site of the MMP-9 gene contributes to its expression in the LPS-signaling pathway, since mutation of NF-$\kappa$B binding site of MMP-9 promoter leads to a dramatic reduction in MMP-9 promoter activation. In addition, the degradation of l$\kappa$B$\alpha$;, and the presences of myeloid differentiation protein (MyD88) and tumor necrosis factor receptor-associated kinase 6 (TRAF6) were found to be required for LPS-activated MMP-9 expression. Chromatin immunoprecipitation (ChIP) assays showed that functional interaction between NF-$\kappa$B and the MMP-9 promoter element is necessary for LPS-activated MMP-9 induction in RAW 264.7 cells. In conclusion, our observations demonstrate that NF-$\kappa$B contributes to LPS-induced MMP-9 gene expression in a mouse macrophage cell line.

<P>In this paper, a novel RF active bandpass filter (BPF) is proposed and its noise performance is optimized by noise analysis. In the proposed design, a resonator consists of an active capacitance circuit together with a conventional inductor. The active capacitor is made of a field-effect transistor that exhibits negative resistance as well as capacitance. It can, therefore, compensate the loss of an inductor. Whereas the conventional active filters using negative resistance circuits usually have a high noise figure, the proposed active filter shows good noise property achieved by adopting a novel active capacitance circuit, which is proven by noise analysis and the experimental result. The measured second-order active BPF shows bandwidth of 95 MHz, 0.1-dB insertion loss, 0.3-dB ripple, and a noise figure of 2.4 dB at the 1.9-GHz band, which agrees well with the simulated results</P>
