<P>While dietary habits or nutritional intake continue to rank as significant factors influencing the incidence of cancer, there have been considerable scientific uncertainties about who will benefit, but who about will not benefit from nutrition. This might be due to inadequate knowledge about an individual's genetic background, the cumulative effect of nutrients on genetic expression profiles, ambiguous clinical differences between beneficiaries and non‐beneficiaries and the lack of information about active protein induction. During the past 200 years of nutrition research, we have experienced revolutionary advances in both chemistry and genomics. According to the high expectations for tailored medicine, a nutrigenomic approach harboring tremendous potential to change the future of dietary guideline and personal recommendations will provide an essential basis for personalized dietary recommendations to prevent common multifactorial diseases decades before their overt clinical manifestation. In the current review, we introduce our efforts to discover <I>Helicobacter pylori</I> (<I>H. pylori</I>)‐related disease biomarkers applicable for diagnostic, predictive and therapeutic purposes using several kinds of technology. For instance, based on publications showing the <I>in vitro</I> and <I>in vivo</I> efficacy of Korean red ginseng on mitigating <I>H. pylori</I>‐associated gastric atrophy, a nutrigenomic approach allows us to confirm that Korean red ginseng prevents <I>H. pylori</I>‐associated gastric cancer in predictable ways.</P>

Background/Aims: We evaluated whether manometric sub-type is associated with treatment outcome in patients with achalasia treated by peroral endoscopic myotomy (POEM). Methods: High-resolution manometry data and Eckardt scores were collected from 83 cases at two tertiary referral centers where POEM is performed. Manometric tracings were classified according to the three Chicago subtypes. Results: Among the 83 cases, 48 type I, 24 type II, and 11 type III achalasia cases were identified. No difference was found in pre-POEM Eckardt score, basal lower esophageal sphincter (LES) pressure, or integrated relaxation pressure (IRP) among the type I, type II, and type III groups. All three patient groups showed a significant improvement in post- POEM Eckardt score (6.1±2.1 to 1.5±1.5, p=0.001; 6.8±2.2 to 1.2±0.9, p=0.001; 6.6±2.0 to 1.6±1.4, p=0.011), LES pressure (26.1±13.8 to 15.4±6.8, p=0.018; 32.3±19.0 to 19.2±10.4, p=0.003; 36.8±19.2 to 17.5±9.7, p=0.041), and 4s IRP (21.5±11.7 to 12.0±8.7, p=0.007; 24.5±14.8 to 12.0±7.6, p=0.002; 24.0±15.7 to 11.8±7.1, p=0.019) at a median follow-up of 16 months. Conclusions: POEM resulted in a good clinical outcome for all manometric subtypes. (Gut Liver 2017;11:642-647)

With the accumulation of clinical trials demonstrating its efficacy and safety, peroral endoscopic myotomy (POEM) has emerged as a less invasive treatment option for esophageal achalasia compared with laparoscopic Heller myotomy. However, the difficulty in determining the exact extent of myotomy, a critical factor associated with the success and safety of the procedure, remains a limitation. Although the various endoscopic landmarks and ancillary techniques have been applied, none of these has been proven sufficient. As a solution for this limitation, the double-scope POEM technique with a second endoscope to assure the exact length of the submucosal tunnel has been applied since 2014. Before double-scope POEM was introduced, the second endoscope was applied only to confirm the accuracy of the procedure. In the present study, we performed double-scope POEM in the treatment of esophageal achalasia through a novel procedure of simultaneous application of the second endoscope to assist in the conventional POEM procedure.

Background: Radiation proctitis is a common complication of radiation to lower abdomen and pelvis. Different modalities of treatment are available, however the effi cacy is incomplete. We hypothesized that human placenta and adipose-derived mesenchymal stem cells could have therapeutic potential on radiation proctitis. Methods: Placenta and adipose-derived mesenchymal stem cells were locally injected on distal rectal mucosa of female Sprague-Dawley rats within 24hrs of 25Gy rectal irradiation. 1,2,4 weeks later, they were sacrifi ced and the rectum was removed and blood sample was obtained to evaluate various parameters of infi ammation Results: Transplanted stem cell was identifi ed at rectal mucosa during 4 weeks after irradiation. Severe proctitis was provoked after pelvic irradiation refi ected with extensive infi ammatory cell infi ltration, loss of crypt epithelium and collagen deposition. However, treatment with mesenchymal stem cell restored these pathologic indices. In addition, mesenchymal stem cell had anti-infiammatory effect, anti- apoptotic effect. Regenerative property were increased. These effect was more prominent on adipose-derived mesenchymal stem cell. Conclusions: Placenta and adipose derived mesenchymal stem cell transplantation may be an potential therapeutic strategy to treatment of radiation proctitis.

<P>Ni/Cu two-step plating is a promising metallization technique because low contact resistance and improved contact adhesion can be achieved after the Ni annealing process. Also, narrow fingers, which are required for high-efficiency solar cells, can be formed by plating. However, the reliability of contact adhesion is still considered one obstacle to industrializing solar cells with plated metal contacts. In this experiment, the influence of ARC opening methods on plated contact adhesion was investigated because the roughnesses of the Si surfaces produced by using pico-second laser ablation and photolithography may be different. Also, the annealing process was conducted before and after plating Cu/Ag metal stacks. The sequence of the annealing can be significant for efficient production because plating is a wet process while annealing is a dry process. The contact adhesion was measured by using a peel-off test. The test was conducted on a 1.5-mm-wide by a 60 similar to 70- mm-long bus bar area. A 3.2-N/mm adhesion force was recorded as a highest average value along the bus bar.</P>
A high performance Position Sensorless Control System of SRM using Neural Network

In this paper, a high performance position sensorless control scheme of Switched Reluctance Motor (SRM) using Neural Network is presented. The control of an SRM depends on the commutation of the stator phases in synchronism with the rotor position. The position sensing requirement increases the overall cost and complexity.<br/> Moreover, a high performance of motor drive system is required for many industrial applications. From these reasons, variable speed rotor position estimation algorithm is developed. The developed rotor position scheme is realized using TMS320F2812 digital signal processor and 1-hp SRM.

The process of pervaporation in which two components diffuse through a nonporous polymer membrane was modelled when one of the penetrants can exert a plasticization action to the membrane material. Thereat a phenomenological model was employed for describing the plasticization effect on the diffusivities for penetrants in the membrane. The sorption equilibria and permeation fluxes for aqueous ethanol soletions in a chitosan membrane were measured, and the permeation fluxes for water were compared with those predicted by the proposed model. The concentration of sorbed water was linear with its weight fraction (x_A) in the feed solution, whereas the permeation flux of water was affected by the plasticization action of sorbed water to the polymer. This plasticization effect on the diffusion process can be simulated in terms of the proposed phenomenological model.
Clinical Study of Adrenal Incidentaloma in Korea

Background : This study was designed to evaluate the clinical, endocrinological and histological characteristics of adrenal incidentalomas. Methods : Eighty patients (41, males; 38, females; age range 17-80 years) who were diagnosed with adrenal incidentaloma at Korea University Hospital from 1992 to 2003 were studied retrospectively. Results : Endocrinological investigation revealed 16 pheochromocytomas (20%), nine Cushing`s syndromes (11%), eight primary aldosteronism (10%) and 46 non-functioning tumors (58%). Forty-four patients received operations, and biopsies were performed on two patients. Pathologic examination revealed 16 adrenocortical adenomas (20%), five carcinomas (6%), 13 pheochromocytomas (16%), three metastatic cancers (4%), and other tumors (10%). The diameter of the carcinomas (mean: 10.8 cm, range: 5-19 cm) were significantly larger than the diameter of benign adenomas (mean: 2.84 cm, range: 1-6 cm) (p=0.002). According to the receiver operating charactenstic (ROC) curve analysis, the cut-off value of tumor size for discriminate malignant tumor was 4.75 cm (sensitivity 90%, specificity 58%). Twenty-four patients with non-functioning tumors were followed up for a period of 3 to 72 months. During the follow up period, two patients showed an increase in tumor size of more than 1 cm, and one patient developed Cushing`s syndrome. Changes in mass size and function were observed only between 10 and 26 months after the initial diagnosis. Conclusions : These data show that an endocrine evaluation should be performed in all adrenal incidentalomas, and an adrenalectomy is recommended for tumors 5 cm or greater or tumors with adrenocortical hyperfunction. In addition, these tumors should be monitored for changes in mass size and function for a follow up period of approximately 26 months.
