        Acute Exacerbation of Interstitial Pneumonia Other Than Idiopathic Pulmonary Fibrosis

        <P>BACKGROUNDS: Acute exacerbation (AE) in idiopathic pulmonary fibrosis (IPF) is increasingly recognized as a relatively common and highly morbid clinical event. However, clinical data on AE in non-IPF interstitial pneumonia are sparse. This study was performed to find the frequency, clinical features, and outcome of AE in non-IPF interstitial pneumonia. METHODS: Retrospective analysis of 10 patients who satisfied the modified Akira criteria for AE during follow-up of 74 patients with surgical lung biopsy-confirmed idiopathic nonspecific interstitial pneumonia (I-NSIP) and 93 patients with biopsy-confirmed interstitial pneumonia associated with collagen vascular disease (CVD-IP). RESULTS: AE occurred in six patients with I-NSIP (1-year frequency, 4.2%) and in four patients with CVD-IP (rheumatoid arthritis [RA], n = 3; scleroderma, n = 1), with 1-year frequency of 3.3%. Median age was 58 years (range, 47 to 75); six patients were female. AE occurred in two patients immediately after surgical biopsy. Median duration of acute symptom before hospital admission was 10 days (range, 1 to 30). Median ratio of Pao(2) to the fraction of inspired oxygen (Fio(2)) was 172 (range, 107 to 273), and Pao(2)/Fio(2) ratio was < 200 in six patients. Surgical lung biopsy performed at the time of AE in two patients revealed diffuse alveolar damage superimposed on nonspecific interstitial pneumonia pattern. Four patients with I-NSIP survived to discharge and were followed up for 24 months (range, 6 to 121). CONCLUSION: AE occurred in the patients with I-NSIP with apparently better prognosis. In patients with CVD-IP, AE occurred mostly with RA-usual interstitial pneumonia in our small series with poor outcome.</P>

        A Retrospective Population-Based Survival Study of Idiopathic Pulmonary Arterial Hypertension in Korea

        Background: Few studies used nationwide data to assess the survival rates (SRs) and death risk for idiopathic pulmonary arterial hypertension (IPAH; ICD-10 I27.0) in Korea. Methods: IPAH data (N = 9,017; female:male = 6:4) were collected from the National Health Insurance Service in Korea, from 2006 through 2017. The data consisted of primary diagnoses related to IPAH. The Kaplan–Meier method and Cox proportional-hazards analyses were carried out. Results: The mean age was 62.3 (± 19.4) years, 64.2 (± 18.9) years in female and 59.4 (± 19.8) years in male (P < 0.001). The one-, three-, five- and 10-year SRs for IPAH were 89.0%, 79.8%, 72.3% and 57.0%, respectively. The adjusted hazard ratio (HR) of IPAH was 1.81 (95% confidence interval [CI], 1.26–2.59) in 60–69 age group, 3.42 (95% CI, 2.40–4.87) in 70–79, and 7.73 (95% CI, 5.43–11.0) in 80s. Other risk factors were male, low-income status, diabetes, myocardial infarction, atrial fibrillation, ischemic stroke, hemorrhagic stroke, and malignant neoplasm. Conclusion: The 10-year SR of IPAH was 57% in Korea. The HR for IPAH was significantly high in patients with older age and other risk factors.

        아산시 지역에서 과민성 장 증후군의 유병률: 표본인구 설문조사

        목적: 우리나라의 과민성 장 증후군과 배변증상의 유병률을 조사하고, 다른 진단 기준을 적용하였을 때 유병률에 차이가 있는 지를 알아보기 위하여 지역사회 주민을 대상으로 무작위 표본추출조사를 시행하였다. 대상 및 방법: 충청남도 아산시 보건소와 협조하여 11개 면과 2개동을 무작위로 선정한 후, 인구에 비례하여 18세 이상 70세 미만의 남녀 2,024명을 무작위로 표본추출하였다. 조사원이 대상가구를 직접 방문하여 배변증상을 설문지로 조사하였다. 결과: 3회 방문에 부재 중이거나 제외기준에 속한 주민을 제외하고, 1,417명(남 762명, 평균 44±14세, 여 655명, 평균 43±15세) 이 설문지를 완료하였다. 로마기준 II에 의한 과민성 장 증후군의 유병률은 2.2% (95% 신뢰구간 [95% CI], 1.4~3.0)이었고, 로마기준 I으로는 2.8% ([95% CI], 1.9~3.7)였다. 복통과 Manning 기준 중 2가지 이상을 만족하는 과민성 장 증후군의 유병률은 3.6% ([95% CI], 2.6~4.6)였다. Manning 기준과 로마기준 I, 그리고 Manning 기준과 로마기준 II간에는 각각 moderate agreement를 나타내었다(κ=0.48; p<0.001), (κ=0.50; p<0.001). 로마기준 I과 로마기준 II간에는 substantial agreement를 나타내어(κ=0.66; p<0.001) 세 가지 진단기준 중 로마기준 I과 로마기준 II의 경우에 일치의 정도가 가장 높았다. 결론: 로마기준 II를 적용하여 표본추출 설문조사에 의한 우리나라의 과민성 장 증후군의 유병률은 2.2%이며, 적용된 진단기준에 따라 차이가 있었다. Backgr ound/Aims: Irritable bowel syndrome (IBS) is common in Caucasians and Japanese, but its epidemiology has not been studied in random Korean populations. It has been suggested that the variation in the prevalence of IBS may be due to the application of different diagnostic criteria. New criteria for IBS have been proposed (Rome II). The aim of this study was to estimate the prevalence of IBS in a random population in Asan city, Korea by using the Bowel Symptom Questionnaire (BSQ), which was based on the multinational, diagnostic, Rome II criteria for functional bowel disorders, as a measure of GI symptoms and to compare diagnostic criteria. Methods: A cross-sectional survey, using a reliable and valid questionnaire based on the fulfillment of the Rome II criteria was performed in a random population in Asan city, Korea, on residents aged 18~69 yr. The response rate was 78.4% (n=1417). All respondents were interviewed at either their homes or offices by a team of interviewers. Results: The prevalence for BS according to the Rome II, Rome I, and Manning criteria was 2.2% (95% confidence interval [CI], 1.4~3.0), 2.8% (95% CI, 1.9~3.7), and 3.6% (95% CI, 2.6~4.6), respectively. There was moderate agreement between the Manning and Rome II criteria (κ=0.48; p<0.001), and between the Manning and Rome II criteria (κ=0.50; p<0.001), but substantial agreement between the Rome I and Rome II criteria (κ=0.66; p<0.001). The prevalence for abdominal bloating was 4.0% (95% CI, 3.0~5.1), for diarrhea was 0.8% (95% CI, 0.3~1.2), and for constipation was 2.6% (95% CI, 1.8~3.5). Conclusions: The prevalence of IBS based on the Rome II criteria was 2.2%. The prevalence of IBS varied substantially depending on the specific definition of IBS used.       • Isolation and purification of fucoidan fraction in <i>Turbinaria ornata</i> from the Maldives; Inflammation inhibitory potential under LPS stimulated conditions in in-vitro and in-vivo models

        <P><B>Abstract</B></P> <P>Fucoidan, referred to as fucose containing sulfated polysaccharides (FCSP), is a polymer from brown algae cell wall that is reported to exhibit potential anti-inflammatory activity. In the present study, the fucoidans are extracted from <I>Turbinaria ornata</I> (<I>TO</I>) from the Maldives. The method involves enzyme assisted extraction and is modified in order to improve the effectiveness and purity of final product. Purified fucoidan fraction was identified as F10, and its chemical properties were verified via FTIR, <SUP>1</SUP>H NMR and monosaccharide analysis. Selected inflammatory mediators were studied to evaluate the anti-inflammatory potential using RAW 264.7 macrophages. F10 successfully inhibited NO production (IC<SUB>50</SUB> = 30.83 ± 1.02 μg mL<SUP>−1</SUP>). F10 dose-dependently down-regulated iNOS, COX-2, and pro-inflammatory cytokines including PGE<SUB>2</SUB> levels. The in vivo experiments were assisted by zebrafish embryo model. This exhibited reduction in ROS, NO expression levels. To our knowledge, this is the first report to illustrate potential anti-inflammatory activity of FCSPs' extracted from the brown algae <I>T. ornata</I>. Concisely, the results suggest that fucoidan purified from <I>T. ornata</I> increases the macrophage cellular and zebrafish embryo resistance against LPS-induced inflammation. Based on the observations, the fucoidans are promising candidates to be used in the pharmaceutical and cosmeceutical sectors.</P>

      • Genome-wide SNP analysis for phylogenetic relationship in Flammulina velutipes

        NGS data was yielded by using Illumina Hiseq platform. The short reads were filtered by quality score and read length were mapped against the reference genome (KACC42780). Genome-wide reanalyzed data of Flammulina strains were compared against the reference genome to establish a genome-wide single nucleotide polymorphism (SNP). The rate of mapping differences between the strains reflected in the strain variation in its result. The genome-wide SNPs distribution divided into types of homozygous SNP and heterozygous SNP moreover all of the strains demonstrated a wide variation in all of the regions. In the further study of topological relationship between the collected strains, phylogenetic tree was separated into 3 major groups. Group I contained F. velutipes var. related strains of ASI 4062, 4148, 4195. Group Ⅱ contained strains that were different species of ASI 4188 F. elastica, ASI 4190 F. fennae, and ASI 4194 F. rossica. The other 19 strains F. velutipes were classified as a single group. Polymorphic SNPs of F. velutipes strains representing the phylogenetic segregation of whiteand brown-fruiting body forming groups were compared. As previously reported, white gene expression is recessive to brown in fruiting body color gene expression. The white strains produced 131,874 SNPs to be aa type and homozygous from of SNP. 407,947 SNPs were detected as AA, Aa type from the brown-fruiting body of SNP. We constructed a SNP matrix with 8 white strains and 12 brown strains. To develop the molecular marker related in to fruiting body color and geographical origin, we isolated 240 SNPs from the white-and brown-fruiting body forming. To determine the chromosome relationship on polymorphic SNP between Korea and Japan strains producing white-fruiting body, we analyzed that the Korea white strains detected 185,695 SNPs and the Japan white strains produced 263,811 SNPs. Using the constructed SNP matrix with 3 Korea white strains and 3 Japan white strains, the experiment generated 475 SNPs of phylogenetic SNPs from Korea and Japan white-fruiting body. As a result, we regarded as they are potentially related to the white color. White and brown color and origin specific SNPs could be used as an identification marker for selection of F. veluipes strains in the breeding program.

      • Mycelial growth optimization of Armillaria sp. and its bioligical characteristics

        Armillaria spp. have been used for edible and medicinal purposes in Asian countries for a long period of time. This study was carried out to obtain basic data for mycelial growth conditions of Armillaria spp. The growth of seven isolates of Armillaria species (A. gallica, A. mellea, and A. ostoyae) were tested with potato dextrose agar (PDA), Sabouraud dextrose agar (SDA), and malt extract agar (MEA). NIFoS 2319 and 2320 strains were selected because mycelium grew fast and showed fin density. All strains showed higher mycelial growth on PDA or SDA than MEA plates. The optimal temperature for mycelial growth was 25°C and the growth was good between 15°C and 25°C in all strains. Further study regarding their mycelial growth properties in various environmental conditions is needed to use this mushroom as applicable resource for food and pharmaceutical production.

      • Identification of genomic regions associated with white fruitbody forming in Flammulina velutipes

        A total of 25 strains of Flammulina velutipes were analyzed to identify the genomic regions responsible for producing white-fruiting body. NGS data was yielded by using Illumina Hiseq platform. Short reads were filtered by quality score and read length were mapped on the reference genome (KACC42780). Between the white- and brown fruitbody forming strains, we found 9376 SNPs, of which 8178 were non-polymorphic and 1198 were polymorphic. There is a high possibility that SNPs can be detected among the white strains as homozygous because white phenotype is recessive in F. velutipes. Thus, we constructed SNP matrix within 8 white strains. SNPs discovered between mono3 and mono19, the parental monokaryotic strains of 4120 strain (white), were excluded from the candidate. If the genotypes of SNPs detected between white and brown strains were identical with those in mono3 and mono19 strains, they were included in candidate as a priority. As a result, if more than 5 candidates SNPs were localized in single gene, we regarded as they are possibly related to the white color. In F. velutipes genome, chr08: 950kb-2650kb, chr09: 500kb-1400kb, chr09: 2800kb-4350kb, and chr11: 2450kb-3500kb regions were identified to be associated with white fruitbody forming.
