        경한 둔상에 의하여 발생한 경골 동맥의 경골비골동체의 가성동맥류

        주로 고에너지 손상이나 관통상에 의해서 발생하는 가성동맥류는 진단이 지연되면 심각한 결과를 초래할 수 있다. 저자들은 계단에서 넘어져 다친 후 처음에는 단순 타박 및 피부 열상으로 진단되었다가 증상이 악화되어 수상 2주일 후 시행한 자기공명영상 검사와 컴퓨터 단층촬영 혈관 조영술에서 경골 동맥의 경골비골동체의 가성동맥류로 뒤늦게 진단된 63세 남자를 치험하였다. 경한 둔상에 의하여 경골비골동체에 가성동맥류가 발생한 예는 아직까지 국내에 보고된 바 없어 이에 저자들은 문헌고찰과 함께 보고하고자 한다. Pseudoaneurysm which is usually caused by high energy trauma or penetrating injury results in serious complications if the diagnosis is delayed. The authors experienced a 63-year-old patient who slipped on the stairs with simple contusion and skin laceration. But, he was finally diagnosed with pseudoaneurysm of tibioperoneal trunk of tibial artery on magnetic resonance imaging and 3-dimensional computed tomography angiogram performed in two weeks due to aggravation of the symptoms. To the best of my knowledge, pseudoaneurysm of tibio-peroneal trunk caused by minor blunt trauma has not been reported till now in Korea. So we report this case with a review of the relevant literature.

        간과된 흉요추부 삼주 손상의 운명

        흉요추부 골절에서 척추의 불안정성에 대한 정확한 진단은 적절한 치료방법의 선택과 추후 일어날 수 있는 합병증을 예방하기 위해 중요하며, 척추 불안정성에 대한 다양한 연구에서 후방 인대 복합체의 파열 여부가 척추 불안정성을 결정하는 데 매우 중요한 인자로 받아들여지고 있다. 불안정성 흉요추부 골절에 대해 보존적 치료를 할 경우, 흉요추부의 후만 변형을 유발하여 만성 동통의 원인을 제공하고 기능적 손실 및 하지 마비의 합병증이 발생할 수 있기 때문에 수술적 치료의 개념이 잘 정립되어 있으며, 후방 인대 복합체의 손상을 간과하여 보존적 치료를 시행하게 되는 경우에는 결과가 좋지 않은 것으로 되어 있다. 이에 저자들은 타 의료기관에서 불안정성 흉요추부 골절을 간과하여 수술적 치료 시기를 놓치고, 보존적 치료를 시행한 후 지속되는 통증과 척추의 후만 변형이 발생하여 본원으로 전원된 2예를 경험하였기에 간과된 흉요추부 삼주 손상의 운명에 대하여 재차 상기하고자 문헌적 고찰과 함께 보고하는 바이다. In a thoracolumbar fracture, accurate diagnosis of spinal stability is necessary in deciding on appropriate treatment options and for prevention of complications that can subsequently occur. In various reports for spinal stability, rupture of the posterior ligament complex is generally accepted as a very important factor of spinal stabiliby. In cases of conservative treatment for unstable thoracolumbar fractures, the progressive kyphotic deformity can cause chronic pain and functional disability, and neurologic deterioration in severe cases. Therefore, the concept of surgical treatment for unstable thoracolumbar fracture has been well established. We report on two cases of neglected three column injury in the thoracolumbar spine, which were treated conservatively due to misdiagnosis at other hospitals, and finally underwent delayed operation in our hospital because of chronic back pain and progressive kyphotic deformity.

        악성 담관협착에서 Niti-S 스텐트 및 Wallstent 의 비교

        Background/Aims: In order to provide a complete obstacle to tumoral growth, we designed a new self-expandible metal stent fully covered with polyurethane membrane. The purpose cf the study was to compare the safety and effectiveness of a new membrane-covered Niti-S stent versus conventional uncovered Wallstent. Methods: In a prospective trial, 59 patients with malignant extrahepatic biliary obstruction received either a newly developed covered metal stent (Niti-S stent, 30 cases) or an uncovered metal stent (Wallstent, 29 cases) by the endoscopic trans-papillary route. Results: Effective biliary decompression was achieved in all patients initially, and short-term results were similar in both groups. Stent failure was observed in 4 of the 30 patients (13%) after a median period of 165 days in the Niti-S group and in 8 of the 29 patients (28%) after a median period of 152 days in the Wallstent group. The reasons of stent failure were occlusion (n=3) and migration (n=l) in the covered Niti-S stent group and occlusion (n=8) in uncovered Wallstent group. According to the Kaplan-Meier life table analysis, the patency rates after 90, 180, and 360 days were 100%, 90%, and 74% in covered Niti-S stent group and 100%, 73%, and 61% in uncovered Wallstent group respectively. The occluded membrane-covered Niti-S stent was easily removed endoscopically and a new Niti-S stent was reinserted in one patient. Conclusions: The new, membrane-covered Niti-S stent has a lower stent failure rate and a tendency toward long-term patency rate compared to uncovered Wallstent. The covering may effectively prevent tumor ingrowth and it is sometimes possible to remove an occluded Niti-S stent.

      • Re-ignition System using Vacuum Triggered Gap-switch for Synthetic Breaking Test

        The synthetic breaking test method was developed to evaluate the breaking performance of ultra high-voltage circuit breaker and made up of two independent circuits; current source circuit and voltage source circuit. In application of this test method, it is necessary to extend the arc of the test breaker. So, the new re-ignition system using VTGS (Vacuum Triggered Gap-Switch) was constructed to improve the efficiency and reliability of this test. In this re-ignition system, VTGS operates in high vacuum state of $5{\time}10^{17}$torr and control system consists of the triggering device and the air M-G (Motor-Generator). This re-ignition system showed the operating characteristics, such as delay time ($t_d$) and jitter time ($t_j$ not exceeding 5us and 1us respectively, and had the operating voltage of $25\~150kVdc$ at the gap distance of 24mm.

      • Perpendicular Magnetic Anisotropy for CoFeBZr/MgO

        <P>We present the effects of Zr insertion (5%) in Co(65)Fe(20)B(10) on perpendicular magnetic anisotropy due to interfacial magnetization with MgO. Perpendicular magnetization is successfully formed with a stalking structure of Ta/CoFeBZr/MgO. The thickness margin for crossover of perpendicular and in-plane magnetization is increased to 1.52 nm with an enhancement of surface anisotropy energy because of reduced thickness dependence. The Gilbert damping parameter is determined to be as low as 0.004 based on the line width of ferromagnetic resonance. Moreover, the maximum effective anisotropy energy (K<SUB>eff</SUB>) was 1.63 Merg/cm<SUP>3</SUP> for CoFeBZr.</P>

      • 잉어 膽汁에 依한 肝細胞像害

        최근 우리 나라에서 보고되고 있는 잉어의 담즙의 유독성에 대한 병적기전을 알아 보기 위해 저자는 흰쥐에 이스라엘 잉어의 담즙을 경구투여 시킨후 임상적 관찰과 더불어 간조직의 변화를 관찰하였던 바 그 소견을 요약하면 다음과 같다. 잉어의 담즙을 투여한 후 실험동물의 활동성은 뚜렷하게 저하되었으며 수시간 후에는 혈뇨를 보이고 사망하는 흰쥐도 볼 수 있었다. 담즙의 투여량이 2㎖인 군은 1㎖인 군 보다 더 심한 변화를 나타내었다. 잉어담즙으로 인한 가장 현저한 간조직의 변화는 8시간 후부터 관찰된 국소적 spotty necrosis 이었으며 이는 시간이 지날수록 더 심해져서 괴사지역이 서로 융합하게되고 1일이 지난후에는 거의 submassive necrosis에 달하는 정도이었다. 그러나 이 시기에 있어서도 간세포색의 reticulin framework는 비교적 유지되어 있었다. 이와 같은 간세포의 괴사는 3일째부터는 현저한 재생의 경과를 나타내었다. 이와 같은 본 실험의 성적 잉어담즙에 의한 간상해의 병적기전이 간소엽의 심한 괴사에 의한 것이며 따라서 잉어담즙중에는 간독성 물질이 함유되어 있는 것으로 사료된다. The author studied the clinical manifestation and liver changes of rats after oral administration of raw carp bile to investigate the pathogenesis of toxic effects of raw carp bile reported by other authors in Korea. The results obtained were summarized as follows: After the administration of bile, the activity of the experimental animals were markedly decreased and after several hours, hematuria was shown and even the death was found. Compare with 1 ㎖ administration group showed more severe change. The most remarkable liver change by raw carp bile was focal spotty necrosis, which was shown 8 hrs after the administration and this necrotic area extended as time lapses, then was confluent each other and reached the almost submassive necrosis atfer 1 day. But the reticulin framework was relatively well preserved at this point. Like this, necrosis of liver cells revealed regenerative activity from the 3rd day after the administration. From the result of the experimental study, the pathogenesis of liver injury by raw carp bile is caused by severe necrosis of hepatic lobule. This suggests that hepatotoxic agent is caused by severe necrosis of hepatic lobule. This suggests that hepatotoxic agent is contained in the raw carp bile.
