        More than 5-Year Follow-up Results of Two- Level and Three-Level Posterior Fixations of Thoracolumbar Burst Fractures with Load-Sharing Scores of Seven and Eight Points

        Background: The development of pedicle screw-based posterior spinal instrumentation is recognized as one of the major surgical treatment methods for thoracolumbar burst fractures. However, the appropriate level in posterior segment instrumentation is still a point of debate. To assesses the long-term results of two-level and three-level posterior fixations of thoracolumbar burst fractures that have load-sharing scores of 7 and 8 points. Methods: From January 1998 to May 2009, we retrospectively analyzed clinical and radiologic outcomes of 45 patients with thoracolumbar burst fractures of 7 and 8 points in load-sharing classification who were operated on using two-level posterior fixation (one segment above and one segment below: 28 patients, group I) or three-level posterior fixation (two segments above and one segment below: 17 patients, group II). Clinical results included the grade of the fracture using the Frankel classification, and the visual analog score was used to evaluate pain before surgery, immediately after surgery, and during follow-up period. We also evaluated pain and work status at the final follow-up using the Denis pain scale. Results: In all cases, non-union or loosening of implants was not observed. There were two screw breakages in two-level posterior fixation group, but bony union was obtained at the final follow-up. There were no significant differences in loss of anterior vertebral body height, correction loss, or change in adjacent discs. Also, in clinical evaluation, there was no significant difference in the neurological deficit of any patient during the follow-up period. Conclusions: In our study, two-level posterior fixation could be used successfully in selected cases of thoracolumbar burst fractures of 7 and 8 points in the load-sharing classification.

        자동차 부품 소재로 사용하고 있는 S45C는 상용차 브레이크 시스템의 S/CAM 샤프트 제품에 적용 생산하고 있다. 그러나 S/CAM 생산과정에서 여러 공정으로 인한 생산단가의 상승으로 경쟁력이 떨어지고 있어 이전의 생산 방식인 금형의 문제점을 파악하고, 개선하고자 컴퓨터 시뮬레이션을 실시하였으며, 개발된 금형을 이용한 열간단조 부품생산의 과정을 컴퓨터 시뮬레이션을 통하여, 제품의 정확한 성형상태를 예측하고, 단조품의 온도 분포도와 응력 분포도를 분석하였다. In this paper, the computer simulation analyzed the effective plastic strain and temperature behaviors. The quantitative analyses which proposed the effective mold design of S/CAM shaft was executed. The parameters of forging shape that affected on the optimize conditions that was calculated with simple equation were investigated. it is expected that the developed analysis model and design technique would greatly contribute to the drum brake optimal design considering effective plastic strain and temperature affected behaviors. This development could save more than 20% of production cost and reduced failure rate to more than 30%. By improving the life span of mold from 15.000 to 25.000. financial difficulty of company imposed on a mold manufacture could be overcome.

        Cavernous Hemangioma of the Gallbladder: a Case Report

        Cavernous hemangioma of the gallbladder is an extremely rare benign tumor. The tumor has only a few cases being reported in literature. However, to the best of our knowledge, no reports focusing on the MRI findings of cavernous hemangioma of the gallbladder have been published. This study reports a case of gallbladder hemangioma with pathologic and radiologic reviews, including MRI findings.

        Gene expression profiling of HepG2 cells treated with endocrine disrupting chemicals using the HazChem human array V3

        The endocrine system is a system of glands that secrete hormones to regulate the body. This system is disrupted by endocrine disrupting chemicals (EDCs), which are similar to sex hormones. These chemicals function as androgen antagonists or estrogen agonists. To determine the effect of EDCs on the gene expression profile in human cells, we treated HepG2 cells with 3 chemicals (bisphenol A, $17\beta$-estradiol, vinclozolin), and analyzed common gene expression using a custom-made HazChem human array V3. The HazChem human array V3 included a total of 1136 genes, all of which were differentially expressed by exposure to VOCs, PAHs, POPs, and LTCs. Of these genes, 24 genes were commonly expressed after exposure to all 3 chemicals, based on the SAM (Significant Analysis of Microarray) method (q-value<0.5%). These genes were analyzed further and found to be involved in coagulation, hemostasis, wound healing, angiogenesis, homeostasis, cell redox homeostasis, and cell proliferation based on GO function and pathway networks.

      • 이동환경에서 문서관리를 위한 지식관리 시스템의 설계

        Wiki시스템은 공유와 참여의 장점을 갖고 있지만 이로 인해 정확성과 유효성을 보장하지 못하고 잘못된 자료들이 많아 질수 있는 점과 재사용이 어렵다는 취약점이 있다. 그러므로 효율적으로 지식의 획득, 유지, 및 공유할 수 있는 지식관리가 필요하다. 본 논문에서 제안하는 지능형 문서검색 시스템은 메타데이터를 활용하여 지식그룹을 생성하고 랭킹기법을 활용한 지능형 문서검색 시스템을 제안한다. Dokuwiki를 활용한 지능형 문서검색 시스템은 이동성과 효율성 정확성이란 특징을 갖는다. Dokuwiki는 미니 웹 서버에서 실행되기 때문에 Mobile또는 PDA등 이들 첨단 정보통신 기기를 사용한다면 지식관리가 자유롭기 때문에 더욱 편리하고 효율적인 지식관리 시스템이 가능할 것이다.

        Classification of Environmental Toxicants Using HazChem Human Array V2

        Toxicogenomics using microarray technology offers the ability to conduct large-scale detections and quantifications of mRNA transcripts, particularly those associated with alterations in mRNA stability or gene regulation. In this study, we developed the HazChem Human Array V2 using the Agilent Sure-Print technology-based custom array, which is expected to facilitate the identification of environmental toxicants. The array was manufactured using 600 VOCs and PAHs-specific genes identified in previous studies. In order to evaluate the viability of the manufactured HazChem human array V2, we analyzed the gene expression profiles of 9 environmental toxicants (6 VOCs chemicals and 3 PAHs chemicals). As a result, nine toxicants were separated into two chemical types-VOCs and PAHs. After the chip validations with VOCs and PAHs, we conducted an expression profiling comparison of additional chemical groups (POPs and EDCs) using data analysis methods such as hierarchical clustering, 1-way ANOVA, SAM, and PCA. We selected 58 genes that could be classified into four chemical types via statistical methods. Additionally, we selected 63 genes that evidenced significant alterations in expression with all 13 environmental toxicants. These results suggest that the HazChem Human Array V2 will expedite the development of a screening system for environmentally hazardous materials at the level of toxicogenomics in the future.

        Effect of Reduced Graphite Oxide as Substrate for Zinc Oxide to Hydrogen Sulfide Adsorption

        Zinc oxide (ZnO) and reduced graphite oxide (rGO) composites were synthesized and tested as adsorbents for the hydrogen sulfide ($H_2S$) adsorption at mid-to-high (300 to $500^{\circ}C$) temperatures. In order to investigate the critical roles of oxygen containing functional groups, such as hydroxyl, epoxy and carboxyl groups, attached on rGO surface for the $H_2S$ adsorption, various characterization methods (TGA, XRD, FT-IR, SEM and XPS) were conducted. For the reduction process for graphite oxide (GO) to rGO, a microwave irradiation method was used, and it provided a mild reduction environment which can remain substantial amount of oxygen functional groups on rGO surface. Those functional groups were anchoring and holding nano-sized ZnO onto the 2D rGO surface; and it prevented the aggregation effect on the ZnO particles even at high temperature ranges. Therefore, the $H_2S$ adsorption capacity had been increased about 3.5 times than the pure ZnO. ZnO(산화아연)와 rGO(환원 흑연산화물, reduced graphite oxide)로 구성된 복합체를 제조하여 중저온 영역($300-500^{\circ}C$)에서 $H_2S$(황화수소) 흡착실험을 수행하였다. rGO에 붙어있는 수산화기, 에폭시기, 그리고 카르복실기와 같은 산소를 포함하는 관능기들이 $H_2S$흡착에 미치는 영향을 조사하기 위해서 다양한 특성분석(TGA, XRD, FT-IR, SEM, 그리고 XPS)을 실시하였다. GO(흑연산화물, graphite oxide)를 rGO로 환원시키기 위해서 마이크로파 조사법을 사용하였다. 마이크로파 조사법에 의한 환원공정에서는 온화한 환원분위기를 조성하여 rGO 표면에 상당량의 산소 관능기들이 남아있는 것을 확인하였다. 이러한 관능기들은 나노 크기의 ZnO가 2D rGO 표면에 균일하게 부착되도록 유도하여 고온 영역에서도 ZnO의 응집 및 소결이 일어나는 것을 방지하는 효과가 있다. 이로 인해 ZnO/rGO 복합체는 순수한 ZnO와 비교하여 3.5배 정도의 흡착량을 보여주었다.

        Fluoroscopy-Guided Percutaneous Transthoracic Pleural Forceps Biopsy in Patients With Exudative Pleural Effusion

        Objective: This study aimed to evaluate the diagnostic performance and procedural characteristics of fluoroscopy-guided percutaneous transthoracic pleural forceps biopsy (PTPFB) in patients with exudative pleural effusion. Materials and Methods: Patients with exudative pleural effusion who underwent PTPFB between May 1, 2014, and February 28, 2023, were included in this retrospective study. The interval between percutaneous catheter drainage (PCD) and PTPFB, number of biopsies, procedural time, and procedure-related complications were evaluated. The sensitivity, specificity, and accuracy of diagnosing malignancy were computed for pleural cytology using PCD drainage, PTPFB, and combined PTPFB and pleural cytology. Results: Seventy-one patients, comprising 50 male and 21 female (mean age, 69.5 ± 15.3 years), were included in this study. The final diagnoses were benign lesions in 48 patients (67.6%) and malignant in 23 patients (32.4%). The overall interval between PCD and biopsy was 2.4 ± 3.7 days. The interval between PCD and biopsy in the group that underwent delayed PTPFB was 5.2 ± 3.9 days. The mean number of biopsies was 4.5 ± 1.3. The mean procedural time was 4.4 ± 2.1 minutes. Minor bleeding complications were reported in one patient (1.4%). The sensitivity, specificity, and accuracy for pleural cytology, PTPFB, and combined PTPFB and pleural cytology were 47.8% (11/23), 100% (48/48), and 83.1% (59/71), respectively; 65.2% (15/23), 100% (48/48), and 88.7% (63/71), respectively; and 78.3% (18/23), 100% (48/48), and 93.0% (66/71), respectively. The sensitivity and accuracy of cytology combined with PTPFB were significantly higher than those of cytological testing alone (P = 0.008 and 0.001, respectively). Conclusion: Fluoroscopy-guided PTPFB is an accurate and safe diagnostic technique for patients with exudative pleural effusion, with acceptable diagnostic performance, low complication rates, and reasonable procedural times.
