        유두종증을 동반하지 않은 융합성 망상 유두종증

        Confluent and reticulated papillomatosis (CRP) was first described as a relatively rare dermatosis. CRP is characterized by slightly hyperkeratotic pigmented papules that are confluent in the center and reticulated at the periphery, located on the neck, abdomen, intermammary, and interscapular regions. Histopathologically, lesions typically reveal orthohyperkeratosis, papillomatosis, and focal acanthosis. A 20-year-old man presented with a 2-year history of slightly pruritic lesions on the chest, abdomen, back, and upper extremities. The lesions were brownish papules and plaques in a reticulated and coalescing pattern. Histologically, a basket-weave pattern of orthohyperkeratosis and mild acanthosis were seen. However, there was no significant papillomatosis, even in multiple sections. Periodic acid Schiff stain was negative for fungi. The patient was treated with doxycycline, 200 mg daily, for 4 weeks. The lesions completely resolved and did not recur during 12 months of follow-up. We report a rare case of CRP without papillomatosis. (Korean J Dermatol 2011;49(11):1010∼1013)

        흰쥐에서 반복적 스트레스가 GABA성 신경전달에 미치는 영향

        연구목적 : 일반적으로 생체는 각종 스트레스에 반응하여 적응(adaptation) 기전이 나타난다. 이러한 적응 기전과 관련하여 포유동물의 중요한 억제성 신경전달계인 GABA성 신경전달계의 스트레스성 자극에 대한 반응양상에 대한 연구가 진행되고 있다. 신경전달의 변화양상은 부하된 스트레스의 종류 및 기간에 따라 다양하게 나타나므로 더욱 상세한 연구를 필요로 한다. 방 법 : 본 연구에서는 하루 2시간의 부동화 스트레스를 반복적으로 2주간 부하한 흰쥐의 뇌조직에서 GABA성 신경전달의 변화 양상을 관찰하고자 하였다. 수용체 결합반응을 통하여 GABA 수용체의 각 component별 배위자들의 각 수용체에 대한 결합 반응의 변화 양상을 정상 대조군와 비교하여 보고, 유의한 변화가 있는 부분의 상세한 검정을 위하여 포화결합반응을 통하여 친화력(Kd) 및 최대결합력(Bmax)에서의 변화양상을 검색하였다. 뿐만 아니라 GABA성 신경전달에 관여하는 중요한 신경전달체인 내인성 GABA의 함량과 대사효소인 GABA transaminase의 활성도의 변화양상도 검색하였다. 결 과 : 연구결과 반복적인 부동화 스트레스는 GABA 수용체 효현제인 [³H]muscimol의 흰쥐 뇌조직 GABA 수용체에 대한 결합도에는 별다른 영향을 미치지 않았다. 대뇌피질조직의 GABA 대사효소인 GABA transaminase 활성도를 감소시켰으나, 피질조직외의 다른 조직의 GABA transaminase 활성도나 내인성 GABA의 농도에는 별다른 영향을 미치지 않았다. 반복적인 부동화 스트레스를 부하한 흰쥐의 대뇌피질, 해마 및 시상하부 조직에서는 [³H]flunitrazepam의 수용체 결합도가 감소하였으며, 포화결합 반응결과 수용체의 수가 감소하였음을 확인하였다. 결 론 : 이상의 결과를 요약하면, 흰쥐에게 부하한 반복적인 부동화 스트레스는 뇌조직의 benzodiazepine 수용체의 수를 감소시킴으로써 GABA성 신경전달을 감소시키며, 이러한 GABA성 신경전달계의 변환은 생체에서 스트레스로 인한 행동장애의 한 기전이 될 수 있을 것으로 생각되었다. Objectives : Changes of GABAergic neurotransmission in response to the application of different types of environmental stress have been the subject of research for over two decades. However, the nature of the changes induced by stress appear to show a dependent phenomena on the type and duration of stressor agent employed. Methods : For this reason, this study was performed to observe the effects of repeated stress on the radioligands binding to GABAA/benzodiazepine receptors of discrete brain regions. The author also examined the activity of GABA transaminase and the concentration of endogenous GABA. Male Sprague-Dawley rats, weighing 150-200g were forced to suffer an immobilization stress for 2 hours during 14 consecutive days. Results : Repeated immobilization stress decreased the binding of [3H]flunitrazepam on the benzodiazepine receptor in the cortex, hippocampus and hypothalamus. Saturation experiments followed by scatchard analyses of the results showed decreased density of benzodiazepine receptor and the affinity remained unchanged. Repeated immobilization stress did not affect the binding of [3H]muscimol on the GABAA receptor, the activity of GABA transaminase, and the concentration of endogenous GABA in the brain regions. Conclusions : From these results, it can be concluded that repeated immobilization stress modulated GABAergic neurotransmission via downregulation of the benzodiazepine receptor in the brain.

        A Double-Blind, Sham-Controlled, Pilot Study to Assess the Effects of the Concomitant Use of Transcranial Direct Current Stimulation with the Computer Assisted Cognitive Rehabilitation to the Prefrontal Cortex on Cognitive Functions in Patients with Stro

        Objective : To examine the synergistic effects of both computer-assisted cognitive rehabilitation (CACR) and transcranial direct current stimulation (tDCS) on cognitive function in patients with stroke. Methods : The current double-blind, sham-controlled study enrolled a total of 11 patients who were newly diagnosed with stroke. The patients of the tDCS group (n=6) completed sessions of the Korean computer-assisted cognitive rehabilitation program five times a week for 30 minutes a session during a mean period of 18.5 days concomitantly with the anodal tDCS over the bilateral prefrontal cortex combined with the CACR. The patients of the control group (n=5) also completed sessions of the sham stimulation during a mean period of 17.8 days. Anodal tDCS over bilateral prefrontal cortex (F3 and F4 in 10-20 EEG system) was delivered for 30 minutes at an intensity of 2 mA. Cathode electrodes were applied to the non-dominant arm. All the patients were evaluated using the Seoul Computerized Neuropsychological Test (SCNT) and the Korean Mini-Mental State Examination. Results : Mann-Whitney U test revealed a significant difference between the two groups. The patients of the tDCS group achieved a significant improvement in the post/pre ratio of auditory continuous performance test and visual continuous performance test on the SCNT items. Conclusion : Our results indicate that the concomitant use of the tDCS with CACR to the prefrontal cortex may provide additional beneficial effects in improving the cognitive dysfunction for patients with stroke.

        우측 어깨에 발생한 모종 1예

        Trichilemmoma is a benign, solitary, small tumor that might often be misdiagnosed as a basal cell carcinoma or verruca. It is usually seen on the face, and especially on the nose and cheeks. A 79-year-old Korean man presented with a 2.0×2.0 cm-sized nodular mass on his right shoulder and the lesion was histopathologically diagnosed as a trichilemmoma. The atypical clinical appearance and localization of his neoplasm did not help us diagnose the lesion, and the histological findings were the only clues for making the diagnosis of trichilemmoma.         색소성 섬유상피종 와 문헌 고찰

        Fibroepithelioma of Pinkus is generally accepted as a low-risk variant of basal cell carcinoma. A 66-year-old man was presented with a solitary dark red nodule with superficial erosion on his right groin. It tended to bleed, but was otherwise asymptomatic. Histopathologically, the tumor was composed of long, thin, branching and anastomosing strands of basaloid cells, which extends from the epidermis into the fibromucinous stroma. Basaloid tumor cells were palisaded at the periphery of the strands, and small buds were seen projecting from the epithelial strands. Significant amounts of melanin pigment were deposited in the papillary and reticular dermis, and the stroma between the tumor cords and also inside the tumor cells. The lesion was completely excised with clear margin. We report a case of pigmented fibroepithelioma of Pinkus with review of the literature in this communication.         Novel Polyhydroxybutyrate-Degrading Activity of the Microbulbifer Genus as Confirmed by Microbulbifer sp. SOL03 from the Marine Environment

        Ever since bioplastics were globally introduced to a wide range of industries, the disposal of used products made with bioplastics has become an issue inseparable from their application. Unlike petroleum-based plastics, bioplastics can be completely decomposed into water and carbon dioxide by microorganisms in a relatively short time, which is an advantage. However, there is little information on the specific degraders and accelerating factors for biodegradation. To elucidate a new strain for biodegrading poly-3-hydroxybutyrate (PHB), we screened out one PHB-degrading bacterium, Microbulbifer sp. SOL03, which is the first reported strain from the Microbulbifer genus to show PHB degradation activity, although Microbulbifer species are known to be complex carbohydrate degraders found in high-salt environments. In this study, we evaluated its biodegradability using solid- and liquid-based methods in addition to examining the changes in physical properties throughout the biodegradation process. Furthermore, we established the optimal conditions for biodegradation with respect to temperature, salt concentration, and additional carbon and nitrogen sources; accordingly, a temperature of 37℃ with the addition of 3% NaCl without additional carbon sources, was determined to be optimal. In summary, we found that Microbulbifer sp. SOL03 showed a PHB degradation yield of almost 97% after 10 days. To the best of our knowledge, this is the first study to investigate the potent bioplastic degradation activity of Microbulbifer sp., and we believe that it can contribute to the development of bioplastics from application to disposal.

        Association between Dementia and Clinical Outcome after COVID-19: A Nationwide Cohort Study with Propensity Score Matched Control in South Korea

        Objective Despite a high prevalence of dementia in older adults hospitalized with severe acute respiratory syndrome coronavirus 2 infection (SARS-CoV-2), or so called COVID-19, research investigating association between preexisting diagnoses of dementia and prognosis of COVID-19 is scarce. We aimed to investigate treatment outcome of patients with dementia after COVID-19. Methods We explored a nationwide cohort with a total of 2,800 subjects older than 50 years who were diagnosed with COVID-19 between January and April 2020. Among them, 223 patients had underlying dementia (dementia group). We matched 1:1 for each dementia- non-dementia group pair yielding 223 patients without dementia (no dementia group) using propensity score matching. Results Mortality rate after COVID-19 was higher in dementia group than in no dementia group (33.6% vs. 20.2%, p=0.002). Dementia group had higher proportion of patients requiring invasive ventilatory support than no dementia group (34.1% vs. 22.0%, p=0.006). Multivariable analysis showed that dementia group had a higher risk of mortality than no dementia group (odds ratio=3.05, p<0.001). We also found that patients in dementia group had a higher risk of needing invasive ventilatory support than those in no dementia group. Conclusion Our results suggest that system including strengthen quarantines are required for patients with dementia during the COVID- 19 pandemic.

      • Characteristics of <i>BRCA1</i> / <i>2</i> mutations carriers including large genomic rearrangements in high risk breast cancer patients

        <P><B>Purpose</B></P><P>We investigated the prevalence of <I>BRCA1/2</I> small mutations and large genomic rearrangements in high risk breast cancer patients who attended a genetic counseling clinic.</P><P><B>Methods</B></P><P>In total 478 patients were assessed for <I>BRCA1/2</I> mutations by direct sequencing, of whom, 306 were identified as non-carriers of <I>BRCA1/2</I> mutation and assessed for large rearrangement mutations by multiplex ligation-dependent probe amplification. Family history and clinicopathological characteristics of patients were evaluated.</P><P><B>Results</B></P><P>Sixty-three mutation carriers (13.2%) were identified with <I>BRCA1</I> mutations (6.3%) and <I>BRCA2</I> mutations (6.9%), respectively. Mutation frequency was affected by familial and personal factors. Breast cancer patients with family history of breast and ovarian cancer showed the highest prevalence of <I>BRCA1/2</I> mutations (67%), and triple-negative breast cancer (TNBC) patients showed high <I>BRCA1</I> mutation prevalence (25%). The three probands of <I>BRCA1</I> deletion (1%) represented both familial risk and personal or clinicopathological risk factors as two with TNBC and one with bilateral ovarian cancer.</P><P><B>Discussion</B></P><P>This is the largest study assessing large genomic rearrangement prevalence in Korea and <I>BRCA1</I> deletion frequency was low as 1% in patients without <I>BRCA1/2</I> small mutations. For clinical utility of large genomic rearrangement testing needs further study.</P>

        Non-sticky silicate replica mold by phase conversion approach for nanoimprint lithography applications

        <P>We have developed a transparent, non-sticky silicate nano mold with high mechanical strength and excellent releasing properties through a simple phase conversion process of polyvinylsilazane (PVSZ) replica mold. This inexpensive inorganic polymer derived silicate mold does not require extra surface modification and could be used as an ideal mold with low adhesion force for nanoimprint lithographic applications. The silicate hard nano mold allows fabrication of sub-100 nm patterns down to 30 nm. The mold can be used for both UV and thermal NIL duplication processes in a repeated manner.
