        Chronic lymphocytic leukemia and concurrent seminoma in the same testis

        A 59-year-old man presented with a painless testicular mass and underwent a radical orchiectomy. The resected specimen showed a 5-cm-sized, white-yellow and homogenous solid mass in the testicular parenchyma. Histologically, the central part of the tumor exhibited typical features of seminoma. The peripheral part of the tumor exhibited diffuse infiltration of small, monotonous lymphoid cells involving the tunica albuginea. The monotonous lymphoid cells were immunoreactive for CD20, CD79a, CD5, and CD23, and negative for CD3, CD10, and cyclin D1. Kappa light chain restriction was detected on flow cytometry using the resected specimen. Considering the circulating lymphoid cell count of >5.0×103/µL, we diagnosed the peripheral component of the tumor as an infiltration of chronic lymphocytic leukemia. This extremely rare combination of seminoma and lymphoid neoplasm should be considered in the differential diagnosis of classic seminoma with extensive lymphoid reaction in tumors arising in elderly patients.

        Efficacy of L-carnitine on ribavirin-induced hemolytic anemia in patients with hepatitis C virus infection

        Background/Aims: L-carnitine not only alleviates hyperammonemia and reduces muscle cramps in patients with liver cirrhosis, but also improves anemia in patients with chronic hepatitis and renal dysfunction. This study prospectively evaluated the preventative efficacy of L-carnitine supplementation against hemolytic anemia during antiviral treatment using ribavirin in patients with hepatitis C virus (HCV)-related chronic liver disease. Methods: A total of 41 patients with chronic hepatitis were consecutively enrolled in this study. Group A (n=22) received sofosbuvir plus ribavirin for 3 months, whereas group B (n=19) was treated with sofosbuvir, ribavirin, and L-carnitine. Hemoglobin concentration changes, the effects of antiviral treatment, and the health status of patients were analyzed using short form-8 questionnaires. Results: A significantly smaller decrease in hemoglobin concentration was observed in group B compared to group A at every time point. Moreover, the prescribed dose intensity of ribavirin in group B was higher than that of group A, resulting in a higher ratio of sustained virological response (SVR) 24 in group B compared with group A. The physical function of patients in group B was also significantly improved compared to group A at the end of antiviral treatment. Conclusions: L-carnitine supplementation alleviates ribavirin-induced hemolytic anemia in patients with HCV and helps relieve the physical burden of treatment with ribavirin-containing regimens. These advantages significantly increase the likelihood of achieving SVR.

        An attempt to establish real-world databases of poly(ADP-ribose) polymerase inhibitors for advanced or recurrent epithelial ovarian cancer: the Japanese Gynecologic Oncology Group

        The development of new treatments for gynecological malignancies has been conducted mainly through collaborative international phase III trials led by the United States and Europe. The survival outcomes of many gynecological malignancies have greatly improved as a result. Recent large-scale genome-wide association studies have revealed that drug efficacy and adverse event profiles are not always uniform. Thus, it is important to validate new treatment options in each country to safely and efficiently provide newly developed treatment options to patients with gynecological malignancies. The Japanese Gynecologic Oncology Group (JGOG) is conducting 5 cohort studies (JGOG 3026, 3027, 3028, 3030, and 3031) to establish real-world data (RWD) of poly(ADP-ribose) polymerase (PARP) inhibitor use in patients with advanced or recurrent epithelial ovarian cancer. The RWD constructed will be used to provide newly developed PARP inhibitors for women with advanced or recurrent ovarian cancer in a safer and more efficient manner as well as to develop further treatment options. In 2022, The JGOG, Korean Gynecologic Oncology Group, Chinese Gynecologic Cancer Society, and Taiwanese Gynecologic Oncology Group established the East Asian Gynecologic Oncology Trial Group to collaborate with East Asian countries in clinical research on gynecologic malignancies and disseminate new knowledge on gynecologic malignancies from Asia. The JGOG will conduct a collaborative integrated analysis of the RWD generated from Asian countries and disseminate real-world clinical knowledge regarding new treatment options that have been clinically implemented.

        Development of HPLC Determination Method for Trace Levels of 1-, 2-Nitropyrenes and 2-Nitrofluoranthene in Airborne Particulates and Its Application to Samples Collected at Noto Peninsula

        1-Nitropyrene (1-NP), 2-NP and 2-nitrofluoranthene (2-NFR) are useful markers for studying the atmospheric behaviors of polycyclic aromatic hydrocarbons (PAHs) and nitropolycyclic aromatic hydrocarbons (NPAHs). However, present methods for measuring trace levels of these compounds are lesssensitive and laborious. Here we describe several improvements to a previously reported high-performance liquid chromatography-chemiluminescence detection system that allows it to determine trace levels of 1-, 2-NPs and 2-NFR. The proposed system was equipped with a reducer column packed with Pt/Rh instead of zinc whose life-time was limited. The combination of Cosmosil MS-II (monomeric ODS) and AR-II (polymeric ODS) columns was used instead of polymeric ODS columns as the separator column to improve the separation. An ethanol mixture with acetate buffer (pH 5.5) was used in place of an acetonitrile mixture with the same buffer to activate the reducer column. The same ethanol mixture was used as the mobile phase for the clean-up column. The switching time of the column switching valve was optimized to concentrate the amino-derivatives of above NPAHs quantitatively on the concentrator column. The concentrations of bis(2,4,6-trichlorophenly) oxalate and hydrogen peroxide in the chemiluminescence reagent solution were optimized to 0.4 mM and 30 mM, respectively, to increase the sensitivity. Under the above conditions, the detection limits (S/N=3) of 1-, 2-NPs and 2-NFR were 1 fmol (0.25 pg), 10 fmol (2.5 pg) and 4 fmol (1 pg), respectively. The proposed system was effectively used to determine trace levels of 1-, 2-NPs and 2-NFR in airborne particulates collected at Noto Peninsula. The atmospheric concentrations of 1-, 2-NPs and 2-NFR were not more than sub pg $m^{-3}$ levels. They were higher in winter (January) than in summer (July). In both seasons, the concentrations were in decreasing order, [2-NFR]>[1-NP]>[2-NP].

      • A Study on Automation of Network Maintenance in Telecom Carriers for Zero-Touch Operations

        In recent years, there are many work efficiency efforts in particular automation of business processes in various industry. The same trend applies to the network operations of telecommunication carriers; however, although automation of service fulfillment, such as developing network services, is progressing, automation of service assurance such as responding to network failure has not progressed. Currently, network maintenance personnel have independently developed tools for automation of network maintenance (such as troubleshooting) process partially and they are manually performing maintenance tasks by coordinating multiple groups of tools. However, in view of the future decrease in the working population, work efficiency must be further improved. Following this situation, this article focuses on further automation of network maintenance process, by developing technologies to orchestrate existing automation tools. In this paper, handling network failures among network maintenance work is targeted, the challenges and the requirements for expansion in the range of automation is discussed, after discussing current automation of network maintenance at telecom carriers in the real world. After that an implementation method that satisfies these requirements using our orchestrator and OSS products is proposed and evaluated. By defining a workflow of network maintenance that makes each process reusable, it becomes easy to create new workflow and update the one, and furthermore it was found that by reviewing how to code the workflow, it was possible to significantly improve processing performance.

        Smad3 deficiency accelerates tongue wound healing via epithelial cell-extracellular matrix interactions

        Aberrant wound closure occurs in a broad range of wounds and scars, and the altered regulation of transcription factors in wound areas can account for both of those conditions. This study aimed to explore the function of the transcription factor Smad3 in wound healing using a tongue wound model in Smad3 knockout (Smad3–/–) mice and with Smad3 small interfering RNA (siRNA) transfected human gingival fibroblasts (HGFs). Smad3 –/–mice were used to examine the extent of repair in tongue wounds. Cell migration was evaluated in HGFs using wound healing assays. The mRNA expression levels of Sox2, E-cadherin, fibronectin, and vimentin were examined in HGFs using reverse transcription-quantitative polymerase chain reaction. Histopathological analysis of wound closure in Smad3–/– mice showed rapid re-epithelialization and remodeling in tongue wound repair compared with Smad3+/+ mice. Increased numbers of neutrophils were identified in the wounds of Smad3 –/– mice. Sox2 and phospho-E-cadherin expression levels were increased in Smad3–/– mice. Smad3 knockdown by siRNA increased cell migration of HGFs. In addition, Sox2, E-cadherin, fibronectin, and vimentin mRNA levels were significantly increased in Smad3 siRNA-transfected HGFs compared with controls. Collectively, these findings demonstrate that a Smad3 deficiency can expedite wound healing and increase immune reactions and extracellular matrix formation after tongue injuries, boosting recovery through Sox2 and E-cadherin. Consequently, Smad3 inhibition would help stimulate tongue wound healing.

        DT Neutronics Benchmark Experiment on Lead at JAEA-FNS

        Lead is one of the most important candidate materials for nuclear fusion reactor blankets. We have carried out an integral benchmark experiment on lead at the DT neutron source facility of JAEA, FNS. A cubic lead assembly on a side of 45.3 cm was set up and was irradiated with the DT neutron source. Reaction rates of the ^(27)Al(n,α)^(24)Na, ^(93)Nb(n,2n)^(92m)Nb, ^(90)Zr(n,2n)^(89)Zr, and ^(115)In(n,n')^(115m)In reactions and neutron spectra above 2 MeV were measured inside the assembly with activation foil and a small NE213 spectrometer, respectively. A Monte Carlo code, MCNP5, was adopted to calculate the reaction rates and neutron spectra. The latest nuclear data libraries, JENDL-3.3 ENDF/B-VII.0, JEFF-3.1 and FENDL-2.1, were used in the calculation. The calculation results with the three libraries except for JENDL-3.3 agreed with the measured reaction rates and neutron spectra. On the other hand, that with JENDL-3.3 underestimated the measured ones with the depth. We found out that the inappropriate evaluation of the (n,2n), elastic scattering and inelastic scattering reactions in the lead isotopes of JENDL-3.3 caused the disagreements.

        Bi-monthly hepatic arterial infusion chemotherapy as a novel strategy for advanced hepatocellular carcinoma in decompensated cirrhotic patients

        Background/Aims: We previously reported the comparable efficacy of bi-monthly hepatic arterial infusion chemotherapy (B-HAIC) to that of sorafenib chemotherapy for the treatment of advanced hepatocellular carcinoma (aHCC) in patients with compensated cirrhosis. In this study, we demonstrate the efficacy of B-HAIC in patients with decompensated cirrhosis. Methods: Forty-five patients with aHCC refractory to transcatheter arterial chemo-embolization (TACE) were treated with B-HAIC and were divided into two groups according to hepatic functional reserve (Child-Pugh grade). Overall survival period, treatment response, and adverse events in each group were analyzed. Results: Efficacy and disease control rates in the Child-Pugh B group (n=24; 21% and 71%, respectively) were not significantly impaired compared the Child-Pugh A group (n=21; 38% and 67%, respectively). Median survival time and survival rate at 12 months in the Child-Pugh B group were 422 days and 58.3%, respectively, whereas those in the Child- Pugh A group were 567 days and 70.8%, respectively. Importantly, the hepatic functional reserve of patients did not worsen in either group during the treatment period. Furthermore, the occurrence rate of adverse events leading to discontinuation of anti-tumor treatment was not significantly increased in the Child-Pugh B group. Conclusions: Given the preservation of hepatic functional reserve afforded by B-HAIC chemotherapy in patients with decompensated cirrhosis, B-HAIC might be an acceptable alternative strategy for aHCC patients who do not respond to TACE.
