        Two Unrecorded Species of the Snapper (Perciformes: Lutjanidae) Collected from Jeju Island, Korea

        Two unrecorded species of the snapper, Lutjanus malabaricus (296.0 mm standard length [SL]) and L. stellatus (350.0 mm SL) belonging to the family Lutjanidae, were first collected from the western coastal waters of Jeju Island, Korea. L. malabaricus is characterized by having a dark marking on the upper half of the caudal peduncle, a band of vomerine teeth that does not protrude posteriorly at the middle, and nine anal soft rays. Compared to its Korean relative, L. malabaricus is distinguishable by having rows of scales that run obliquely in the dorsal-posterior direction above the lateral line (vs. parallel to the lateral line in L. argentimaculatus). L. stellatus can be identified by the absence of wavy blue lines on the head (vs. many blue lines in L. rivulatus) and presence of a white spot above the lateral line (vs. a black blotch on the lateral line in L. rivulatus). We propose new Korean names, Jin-hong-tung-dom” and “Huin-jeom-tung-dom,” for L. malabaricus and L. stellatus, respectively.

        Therapeutic Strategies of the Intracranial Meningioma in Elderly Patients

        Objective : The apparent increase in the incidence of the intracranial meningiomas in the elderly is due in part to improved diagnostic tools and improved span of life. The authors carried out a retrospect study to validate the use of the Clinical-Radiological Grading System [CRGS] as a clinical tool to orientate surgical decision making in elderly patients and to explore prognostic factors of survival. Methods : From January 1997 to January 2006, the authors consecutively recruited and surgically treated 20 patients older than 65 years of age with radiologic findings of intracranial meningiomas and a preoperative evaluation based on the CRGS. Results : High CRGS score was associated with a higher probability of good outcome [p=0.004] and a lower probability of postoperative complications [p=0.049]. Among the different subset items of the CRGS score, larger maximum tumor diameters [$D{\geqq}4cm$] and the presence of a severe peritumoral edema were associated with incidence rate of postoperative poor outcome and complications [p<0.05]. Additionally, the critical location of the tumor was also correlated with poor outcome [p<0.05]. Conclusion : A CRGS score higher than 13 is a good prognostic indication of survival. The CRGS score is a useful and practical tool for the selection of elderly patients affected by intracranial meningiomas as surgical candidates.

        Fatal Aortic Tumor Embolism Presenting as Acute Paraplegia

        We report a case of fatal aortic tumor embolism presenting as acute paraplegia. A four-year-old girl was referred from a local hospital with sudden paraplegia and a poor medical condition. A neighbor had noticed her fall from a bike, and she could not walk. She had no previous illness. Emergency spine MRI revealed no remarkable findings. During the process of evaluation, her general condition deteriorated progressively. Chest and abdominal CT showed a large mass in the left lung field, and a diagnosis of aortic occlusion was made. An emergency transfemoral embolectomy was attempted. However, the patency of the aorta was not recovered. On pathological examination of tissues taken from the embolectomy, a pleuro-pulmonary blastoma was found. The patient died 22 hours after the onset of her symptoms. We describe a possible mechanism for the tumor embolism. To the best of our knowledge, this is the first case report of aortic occlusion caused by an embolic malignancy, presenting as acute paraplegia.

        <P>In this paper, we propose a novel memetic algorithm, which is explorative particle swarm optimization (ePSO), combined with mesh adaptive direct search and apply it to the design of a permanent magnet synchronous machine (PMSM). The ePSO, which is modified from the PSO, drastically improves search time and iteration number at an exploration search stage. Unlike the existing methods, the proposed rule of start point selection takes an advantage of minimizing the search time. By applying the proposed algorithm to PMSM, we clarify the effectiveness of the proposed algorithm.</P>

      • Synthesis of biodegradable poly(ε‐caprolactone)‐organosiloxane hybrid with carboxylate groups

        <P><B>Abstract</B></P><P>Biodegradable poly(ε‐caprolactone)‐organosiloxane hybrid with carboxylate groups was newly synthesized by a sol–gel method with the capacity to conjugate cell adhesion ligands for the potential applications as a bone tissue engineering scaffold material. An intermediate hybrid contained positively charged amine groups was synthesized by end‐capping α,ω‐hydroxyl poly(ε‐caprolactone) with (3‐isocyanatopropyl)triethoxysilane and a subsequent sol–gel reaction with (3‐aminopropyl)triethoxysilane. This resulted in a new hybrid composed of biodegradable organic poly(ε‐caprolactone) segments connected by inorganic siloxane linkages containing positively charged amine groups. The successive succinlylation of amine groups created amide bonds and converted the cationic sites of succinic anhydride into negatively charged carboxylate groups. New formations of amine and carboxylate groups could be directly and indirectly confirmed using Fourier transformed infrared spectrometry, zeta potentials, and atomic force microscopy. Biodegradability of hybrid with carboxylate groups was also examined by weight loss in phosphate buffered saline, and it was about 14 wt % after 8 weeks. Conjugating capacity of a cell adhesion ligand was examined by comparing the relative fluorescent intensities of covalently and noncovalently immobilized synthetic GGGGRGDASSK‐FITC oligopeptides onto the carboxylated groups of the hybrid using confocal laser scanning microscopy; the relative fluorescent intensity of covalently conjugated RGD peptide was about three times higher than that of noncovalently coated RGD peptide. The hybrid covalently conjugated by GGGGRGDASSK oligopeptides improved proliferation and differentiation activities of preosteoblastic MC3T3‐E1 cells. The results suggest that this hybrid possesses an encouraging potential to be used as a bone tissue engineering scaffold material due to its biodegradability and high capacity for conjugating cell adhesion ligands. © 2012 Wiley Periodicals, Inc.       • Proportion and Characteristics of Transient Nodules in A Retrospective Analysis of Pulmonary Nodules

        With the popularity of chest CT scanning, differential diagnosis of pulmonary nodule became a great burden to chest physicians. Pulmonary nodules manifest as pure or mixed ground glass opacities (GGO) as well as solid nodules. We performed this retrospective analysis to observe the proportion and predictive factors of transient lesions in a cohort of patients who visited our institution for the differential diagnosis of pulmonary nodules. Methods: We surveyed 317 cases with pulmonary nodules from June 2004 to March 2011. The age distribution showed 57.1±11.3 years (mean±standard deviation, range 30~86). Male patients comprised 62.1% (n=197), and 53.9% of patients were smokers. Results: Nodules from 114 cases (36%) disappeared or decreased in size during follow up, while 203(64%) cases did not change or enlarged. At the initial CT scan, 63.7% showed solid nodules, while 20.2% had mixed GGOs and 16.1% of cases manifested as pure GGOs. During follow up, more than half of GGOs resolved (66.7% in pure GGOs, 54.7% in mixed GGOs) while only 22.3% of solid nodules resolved. Between transient and persistent pulmonary nodules, there were significant differences in age, gender, presence of smoking history, presence of eosinophilia, size and radiologic attenuation of nodules (solid or GGO). In results of multivariate analysis, age (≤55 years), eosinophilia, size(>15mm) and GGO were significant (p<0.05) independent predictors of transient nodule. The main causes of transient nodules were pneumonia or eosinophilic pulmonary infiltrates. Conclusion: About 54~66% of ground glass opacity nodules revolved spontaneously or with medical treatment. Transient nodules could be predicted with clinical and radiological characteristics.

      • Cytochrome P450-mediated herb-drug interaction potential of Galgeun-tang

        We evaluated the herb-drug interaction potential of Galgeun-tang (GGT) extracts, mediated by cytochrome P450 (CYP) inhibition/induction. Further, the effects of fermentation on the CYP-mediated herb-drug interaction potential of GGT extracts were determined. As measured by LC-ESI/MS/MS, GGT extracts (0-300μg/mL) showed no inhibitory activity toward eight CYP isoforms (1A2, 2A6, 2B6, 2C9, 2C19, 2D6, 2E1, and 3A4) in pooled human liver microsomes, suggesting that GGT may have low potential for herb-drug interactions mediated by CYP inhibition. Hepatic CYP expression and activity in rats treated with GGT extracts twice per day for 1week was examined. Among the tested CYP isoforms (1A1, 1A2, 1B1, 2B1, 2C11, 2E1, 3A1, 3A2, and 4A1), CYP1B1 and 4A1 were increased by GGT extracts. Hepatic activities of 7-ethoxyresorufin-O-deethylase, 7-pentoxyresorufin-O-depentylase, and chlorzoxazone 6-hydroxylase, but not midazolam hydroxylase were also elevated. These results raise the possibility that GGT extracts may increase the toxicity of environmental toxicants through the elevating CYP-dependent metabolic activation. Interestingly, the increases in CYP1B1 and CYP4A1 levels, and 7-ethoxyresorufin-O-deethylase, 7-pentoxyresorufin-O-depentylase, and chlorzoxazone 6-hydroxylase activities were attenuated by fermentation of GGT extract using Lactobacillus plantarum KFRI 402, but not 144. Further studies are needed to identify the CYP regulatory component(s) from GGT and determination its metabolism. ⓒ2012 Elsevier Ltd. All rights reserved.

        First female Korean child with Coffin-Lowry syndrome: a novel variant in RPS6KA3 diagnosed by exome sequencing and a literature review

        Coffin-Lowry syndrome (CLS, OMIM # 303600) is a rare X-linked disorder caused by mutations in RPS6KA3. CLS is characterized by facial dysmorphism, digit abnormalities, developmental delays, growth retardation, and progressive skeletal changes in male patients. Females with CLS are variably affected, complicating diagnosis. Here, we describe the clinical and molecular findings in a female Korean child with CLS and review the associated literature. A 5-year-old girl presented with short stature and developmental delays. She had a coarse facial appearance characterized by a prominent forehead, hypertelorism, thick lips, and hypodontia. She also had puffy tapering fingers and pectus excavatum. We performed exome sequencing and identified a novel, likely pathogenic, heterozygous variant, c.326_338delinsCTCGAGAC (p.Val109Alafs*10), in RPS6KA3 (NM_004586.2). This is the first Korean female genetically diagnosed with CLS. In contrast to the delayed bone age reported in previous studies, our patient showed advanced bone age and central precocious puberty. CLS should be considered as a differential diagnosis of short stature, tapering fingers, and developmental delay. We suggest that molecular techniques can be a useful tool for diagnosis of rare disorders such as CLS because such conditions are not simple, and the associated spectrum of phenotypes can vary.
