        The Impact of N-acetylcysteine on Major Depression: Qualitative Observation and Mixed Methods Analysis of Participant Change during a 12-week Randomised Controlled Trial

        Objective: N -acetylcysteine (NAC) is a novel therapeutic agent with multiple mechanisms of action in the central nervous system and a favourable side effect profile. Clinical evidence indicates that adjunctive NAC may reduce the severity of depressive symptoms in individuals with major depressive disorder (MDD). Methods: A 12-week randomised controlled trial of 2,000 mg/day adjunctive NAC for MDD found no significant improvement at the primary endpoint (week 12) but did see improvements at the post-discontinuation interview (week 16). Within the context of patient-centered treatment, mixed-methods qualitative analysis was also included to explore factors that may determine individual responses to adjunctive NAC treatment. These data were drawn, under blinded conditions, from clinician notes recorded in the case report form. Using the DSM-5 symptom profile for MDD as the initial framework, themes were developed and explored. Frequencies were compared between placebo and NAC groups. Results: Per protocol analysis of individual themes across the six interviews revealed group differences in favour of NAC for overall depressive affect, optimism, relationships and reduced functional impairment. Conclusion: This study provides further evidence for the utility of the mixed methods approach complimenting the primary findings using traditional quantitative analyses, as well as being able to capture additional, often more subtle, evidence of individual symptom-level change that reflects improvement in functional abilities in response to NAC supplementation. The use of mixed methods to explore outcomes from psychiatric studies should be considered in future to work towards improved patient-centred care and both confirm quantitative findings and generate novel hypotheses.

        Diffusion MRI Connections in the Octopus Brain

        Using high angle resolution diffusion magnetic resonance imaging (HARDI) with fiber tractography analysis we map out a meso-scale con- nectome of the Octopus bimaculoides brain. The brain of this cephalopod has a qualitatively different organization than that of vertebrates, yet it exhibits complex behavior, an elaborate sensory system and high cognitive abilities. Over the last 60 years wide ranging and detailed studies of octopus brain anatomy have been undertaken, including classical histological sectioning/staining, electron microscopy and neuronal tract tracing with injected dyes. These studies have elucidated many neuronal connections within and among anatomical structures. Diffusion MRI based trac- tography utilizes a qualitatively different method of tracing connections within the brain and offers facile three-dimensional images of anatomy and connections that can be quantitatively analyzed. Twenty-five separate lobes of the brain were segmented in the 3D MR images of each of three samples, including all five sub-structures in the vertical lobe. These parcellations were used to assay fiber tracings between lobes. The connectivity matrix constructed from diffusion MRI data was largely in agreement with that assembled from earlier studies. The one major difference was that connections between the vertical lobe and more basal supra-esophageal structures present in the literature were not found by MRI. In all, 92 con- nections between the 25 different lobes were noted by diffusion MRI: 53 between supra-esophageal lobes and 26 between the optic lobes and other structures. These represent the beginnings of a mesoscale connectome of the octopus brain.

        Characterization of Blackberry Extract and Its Antiproliferative and Anti-Inflammatory Properties

        Blackberries are rich in polyphenols, including anthocyanins. Polyphenols are hypothesized to have biologi-cal activities that may impact positively on human health. In these studies, an anthocyanin-rich extract from Hull blackber-ries grown in Kentucky was obtained and fully characterized in terms of total anthocyanin and phenolic content, polymericcolor, anthocyanin composition, and total antioxidant capacity. In vitrocell culture studies showed that the blackberry extractinhibited HT-29 colon tumor cell growth in a concentration-dependent manner with 49.2 .g of total anthocyanins/mL in-hibiting HT-29 cell growth up to 66% at 72 hours. Likewise, in a concentration-dependent manner, total anthocyanin con-centrations in the range of 040 .g/mL suppressed both high-dose (10 .g/mL) and low-dose (0.1 .g/mL) lipid A-inducedinterleukin-12 release from mouse bone marrow-derived dendritic cells. These results suggest that Hull blackberry extract(HBE) has potent antioxidant, antiproliferative, and anti-inflammatory activities and that HBE-formulated products may havethe potential for the treatment and/or prevention of cancer and/or other inflammatory diseases.

        Application of Information Value Chain in Gout Management

        Purpose: This study introduces information value chain analysis by identifying essential information for use in gout care management. Part I reviews the essential concepts of information value chain analysis first introduced by Porter. Part II applies the analysis to determine the information values of patient health information and explores ways in which health information technologies can be best utilized to provide that information to patients with gout. Methods: We combined value chain analysis with natural language processing and machine learning techniques to develop algorithms that can identify patients with gout flares using clinical notes. As one of the first signs that the disease was not being controlled, variables found to be associated with gout flares were considered valuable information for patients with gout. Results: The best performing model, in terms of both gout flare prediction and association identification, was the comprehensive model that not only included concepts from all stages of the value chain but also designated natural language processing concepts from every care stage as surrogate variables. Additionally, all administrative codes traditionally associated with gout and its treatment were included as surrogate outcome variables. Conclusion: This study introduced information value chain analysis and applied it to develop a computer-based method with theoretical underpinnings to identify the concepts associated with gout flares. The findings can be used as a starting point for filtering the vast amounts of information patients must go through and identifying the most valuable information for patient with gout to adequately manage their symptoms.

        Morphing anthropomorphism: An update

        This is an update of a 2010 paper we published on anthropomorphic consumer perception of brands and marketer attempts to humanize brands through packaging. Since that time a great deal of academic and business attention to the topic of anthropomorphism has resulted in the related work on brand mascots, brand personality, marketplace mythologies, and anthropomorphism in product design and advertising. In addition, new arenas of anthropomorphism have emerged with developments in projective research methods, digital avatars, robot design, digital self-presentation, and conversational digital assistants like Amazon’s Alexa and Apple’s Siri. Such novel directions have prompted new research questions and further studies. This paper offers a brief update of the evolving issues in the co-creation of anthropomorphic objects and brand interpretations by consumers, designers, roboticists, engineers, and marketers.

        Advances in Research and Forecasting of Tropical Cyclones from 1963-2013

        A review of progress over the past 50 years in observingand forecasting of tropical cyclones is presented. Tremendous progresshas been made in track forecasting in the past 20 years with theimprovement in numerical model guidance and the use of consensusforecasting, and this has contributed to a number of warning centersnow issuing five-day track forecasts that are as accurate as three-dayforecasts of a decade ago. Techniques are now available to specifythe track forecast uncertainty for assessing the risk of a tropicalcyclone. With the advent of five-day forecasts, a focus on improvedunderstanding of formation has led to two field experiments. Arecent advance has been in extended-range (5-30 days) forecasts oftropical cyclone events (formations and tracks) in the western NorthPacific from the ECMWF 32-day ensemble predictions. This advanceis a contribution to a goal of seamless forecasting from one day to aseason for tropical cyclones. Little progress has been made inintensity forecasting, although the Hurricane Forecast ImprovementProject in the United States and recent field experiments may offersome future advances. Some advances in forecasting tropical cycloneimpacts such as storm surge, surface waves, and precipitation havebeen achieved. Future opportunities for continued advances arepossible such that improved warnings can lead to reductions in lossesof lives and minimizing damages from tropical cyclones.
