이 논문은 ‘코로나19’ 대유행의 위기에 의해 영향을 받고 있는 국제적인 관광 환경에서 호주로부터 전라북도로 의료 관광객을 유인하기 위한 가능성을 탐구한다. 이 연구는 대한민국으로 의료 행위를 선택할 가능성이 가장 높은 호주 인구 통계학상의 그룹에 대한 검증결과를 토대로, 전라북도가 지닌 경쟁력 있는 강점들을 분석할 것이다. 전라북도가 지닌 경쟁력 있는 중요한 강점 중의 하나는 한국문화와 지역 문화를 의료 관광 분야에 통합하는 역량에 있다. 의료 관광은 오직 의료 절차와 치료 자체에만 집중되는 단기 체류 방문으로 이루어질 수도 있지만, 적응 기간과 의료 치료 기간 및 회복 기간을 결합하는 장기 체류 방문으로 진행될 수도 있다. 장기 체류 방문은 의료 관광객을 한국문화의 독특한 특성과 정서를 이해하는데 충분한 도움이 된다. 이 연구는 한국의 문화적 특성들이 어떻게 전라북도의 상황에서 호주 여행객들을 유인하는 데 매우 효과적으로 작용할 수 있는지에 대한 논리적 근거를 제공한다. 또한, 이 논문은 대한민국 의료관광 분야에 필요한 의료영어 교육의 중요성이 강조되었다. 특히, 이 논문은 의료기관에게 전문적 의료 경영서비스를 제공하는 MSO(병원경영지원회사)의 역할과 국내에 들어오는 의료 관광객과 의료기관들에게 전적으로 필요한 인적자원인 ‘Medilier(메디리어)’라는 새로운 용어의 개념을 소개한 후, ‘메드리어’의 교육 프로그램과 연계 의료서비스 제공의 중요성이 강조되었다. 또한 관광산업 발전전략 의 분석수단으로 샤넬·헤르메스 조건이 새로이 소개되었다. This paper investigates the potential for the South Korean province of Jeollabuk-do to attract medical tourists from Australia in an international tourism environment impacted by the Covid-19 pandemic crisis. Following identification of the Australian demographic cohort most likely to be receptive to undertaking medical procedures in South Korea, the paper analyses Jeollabuk-do’s competitive advantages in this regard. A key competitive advantage the province has is its capacity to incorporate Korean and local culture into the medical tourism experience. While medical tourism can consist of a short-stay visitation concentrated solely around the medical procedure/treatment itself, it can also consist of a longer-stay visitation which incorporates a settlement period, a medical treatment period and a recovery period. An extended stay as such allows the medical tourist to fully engage with the distinct attributes of South Korea’s culture. This paper seeks to understand how South Korea’s cultural attributes are best able to engage with medical tourists from Australia within the context of Jeollabuk-do. Furthermore, the paper emphasizes the importance of Medical English language education within the Korean medical tourism sector. In particular, the paper emphasizes the role of MSO(Management Services Organization) and introduces the term ‘Medilier’ which relates to the education program and provision of human capital resources which work to professionally facilitate the interaction between incoming international medical tourists and South Korean health care service providers. The new condition of SANEL·HERMES is also introduced as a new instrument of analysis for development strategy of tourism industry.

In effect, what characterized Korean family planning opinion leaders in 1971 was a rather rational and democratic model. Those who had had actually tried methods, discussed them, and knew a lot about them played the leadership role more than others. Place of residence, education, and other such variables mattered relatively little unless the woman had the credibility imparted by actual use and the knowledge imparted by use or discussions with family planning professionals.

CAR links hypoxia signaling to improved survival after myocardial infarction

The coxsackievirus and adenovirus receptor (CAR) mediates homo- and heterotopic interactions between neighboring cardiomyocytes at the intercalated disc. CAR is upregulated in the hypoxic areas surrounding myocardial infarction (MI). To elucidate whether CAR contributes to hypoxia signaling and MI pathology, we used a gain- and loss-of-function approach in transfected HEK293 cells, H9c2 cardiomyocytes and CAR knockout mice. CAR overexpression increased RhoA activity, HIF-1α expression and cell death in response to chemical and physical hypoxia. In vivo, we subjected cardiomyocyte-specific CAR knockout (KO) and wild-type mice (WT) to coronary artery ligation. Survival was drastically improved in KO mice with largely preserved cardiac function as determined by echocardiography. Histological analysis revealed a less fibrotic, more compact lesion. Thirty days after MI, there was no compensatory hypertrophy or reduced cardiac output in hearts from CAR KO mice, in contrast to control mice with increased heart weight and reduced ejection fraction as signs of the underlying pathology. Based on these findings, we suggest CAR as a therapeutic target for the improved future treatment or prevention of myocardial infarction.

<P>Silencing of aberrantly expressed microRNAs (miRNAs or miRs) has emerged as one of the strategies for molecular targeted cancer therapeutics. In particular, miR-21 is an oncogenic miRNA overexpressed in many tumors, including ovarian cancer. To achieve efficient administration of anti-miR therapeutics, delivery systems are needed that can ensure local accumulation in the tumor environment, low systemic toxicity, and reduced adverse side effects. In order to develop an improved anti-miR therapeutic agent for the treatment of ovarian cancer, a nanoformulation is engineered that leverages biodegradable porous silicon nanoparticles (pSiNPs) encapsulating an anti-miR-21 locked nucleic acid payload and displaying a tumor-homing peptide for targeted distribution. Targeting efficacy, miR-21 silencing, and anticancer activity are optimized in vitro on a panel of ovarian cancer cell lines, and a formulation of anti-miR-21 in a pSiNP displaying the targeting peptide CGKRK is identified for in vivo evaluation. When this nanoparticulate agent is delivered to mice bearing tumor xenografts, a substantial inhibition of tumor growth is achieved through silencing of miR-21. This study presents the first successful application of tumor-targeted anti-miR porous silicon nanoparticles for the treatment of ovarian cancer in a mouse xenograft model.</P> [FIG OMISSION]</BR>

BACKGROUND: Cardiac allograft vasculopathy (CAV) is a complication beyond the first-year post-heart transplantation (HTx). We aimed to test the utility of cardiac magnetic resonance (CMR) to detect functional/structural changes in HTx recipients with CAV. METHODS: Seventy-seven prospectively recruited HTx recipients beyond the first-year post-HTx and 18 healthy controls underwent CMR, including cine imaging of ventricular function and T1- and T2-mapping to assess myocardial tissue changes. Data analysis included quantification of global cardiac function and regional T2, T1 and extracellular volume based on the 16-segment model. International Society for Heart and Lung Transplantation criteria was used to adjudicate CAV grade (0–3) based on coronary angiography. RESULTS: The majority of HTx recipients (73%) presented with CAV (1: n = 42, 2/3: n = 14, 0: n = 21). Global and segmental T2 (49.5 ± 3.4 ms vs 50.6 ± 3.4 ms, p < 0.001;16/16 segments) were significantly elevated in CAV-0 compared to controls. When comparing CAV-2/3 to CAV-1, global and segmental T2 were significantly increased (53.6 ± 3.2 ms vs. 50.6 ± 2.9 ms, p < 0.001; 16/16 segments) and left ventricular ejection fraction was significantly decreased (54 ± 9% vs. 59 ± 9%, p < 0.05). No global, structural, or functional differences were seen between CAV-0 and CAV-1. CONCLUSIONS: Transplanted hearts display functional and structural alteration compared to native hearts, even in those without evidence of macrovasculopathy (CAV-0). In addition, CMR tissue parameters were sensitive to changes in CAV-1 vs. 2/3 (mild vs. moderate/severe). Further studies are warranted to evaluate the diagnostic value of CMR for the detection and classification of CAV.

Rents generated by natural resources are usually thought to weaken the quality of institutions, particularly in developing countries. Our hypothesis is that this effect may differ depending on the types of natural resources characterized by their different degree of appropriability. We test this hypothesis using panel data covering 90 developing countries for the period 1970-2010. We find that total rents weaken the quality of institutions. However, while oil rents have a significant negative effect, forest and mineral rents do not, after controlling for the other relevant determinants of institutional quality, institutional persistence, neighbor effect, and endogeneity of rents.

Background and Purpose: Spot sign (SS) on computed tomography angiography (CTA) is associated with hematoma expansion (HE) and poor outcome after intracerebral hemorrhage (ICH). However, its predictive performance varies across studies, possibly because differentiating hyperdense hemorrhage from contrast media is difficult. We investigated whether dual-energy-CTA (DE-CTA),which can separate hemorrhage from iodinated contrast, improves the diagnostic accuracy of SS for predicting HE. Methods: Primary ICH patients undergoing DE-CTA (both arterial as well as delayed venous phase) and follow-up computed tomography were prospectively included between 2014 and 2019. SS was assessed on both arterial and delayed phase images of the different DE-CTA datasets, i.e., conventional-like mixed images, iodine images, and fusion images. Diagnostic accuracy of SS for prediction of HE was determined on all datasets. The association between SS and HE, and between SS and poor outcome (modified Rankin Scale at 3 months ≥3) was assessed with multivariable logistic regression, using the dataset with highest diagnostic accuracy. Results: Of 139 included patients, 47 showed HE (33.8%). Sensitivity of SS for HE was 32% (accuracy 0.72) on conventional-like mixed arterial images which increased to 76% (accuracy 0.80) on delayed fusion images. Presence of SS on delayed fusion images was independently associated with HE (odds ratio [OR], 17.5; 95% confidence interval [CI], 6.14 to 49.82) and poor outcome (OR, 3.84;95% CI, 1.16 to 12.73). Conclusions: Presence of SS on DE-CTA, in particular on delayed phase fusion images, demonstrates higher diagnostic performance in predicting HE compared to conventional-like mixed imaging, and it is associated with poor outcome.
