Role of Cytoreductive Radical Prostatectomy in the Treatment of Metastatic Prostate Cancer

There is controversy regarding the survival benefits of eliminating primary tumors via cytoreductive radical prostatectomy (CRP) in patients with metastatic prostate cancer (mPCa). The purpose of this article is to review the theoretical background of and rationale for CRP, and the current knowledge base. The Scopus and PubMed databases were searched for studies investigating CRP published between January 2000 and October 2019. The retrieved articles were nonsystematically reviewed. Based on preclinical data, retrospective patient case studies, retrospective population-based studies, and prospective studies, CRP has been reported to afford benefits for the treatment and prevention of local symptoms through the removal of primary tumors, and the management of neo-metastatic disease and overall survival. However, despite the results from these studies, the current review mostly addresses small case studies and uncontrolled population-based studies with weak evidence. Based on this weak evidence, therefore, clinical use has not yet been recommended. Further research investigating the role and timing of CRP in patients with mPCa is needed, in addition to studies screening the most suitable populations for CRP.

중년 여성에서 발생한 집합관 신세포암 1 예

Collecting duct renal cell carcinoma (CDRCC) or Bellini duct carcinoma is a rare type of renal cell carcinoma (RCC), accounting for 0.4% to 1.8% of all RCC. CDRCC is considered to originate from the collecting duct of the kidney and it has a poor prognosis. However, the clinical features of CDRCC remain poorly understood, mainly because only studies with a small series or case reports have been published to date. We experienced a case of CDRCC of kidney.

최초 임상증상으로 볼부위의 종괴로 나타난 전이성 신세포암 : 증례보고

신세포암의 골격근으로의 전이는 매우 드문경우로 현재까지 약 20편정도의 증예 보고만이 있을뿐이다. 우리는 이 증례보고에서 매우 드문 경우인 최초 임상증상으로 볼부위의 종괴를 주소로 내원후 전이성 신세포암으로 진단받은 65세 남자의 증례와 전이성 신세포암이 골격근으로 전이된 경우의 문헌들을 리뷰하였다. 환자는 내원 3개월전 왼쪽 볼부위에 무통성 종괴가 있는것을 처음 인지하였다. 절제된 종괴의 병리학적 소견은 전이성 신세포암으로 나타났으며 영상의학검사상 좌측신장에 6x5㎝ 크기의 종괴를 보였고 양측 폐에 다수의 작은 결절들을 보였다. 환자는 근치적 신절제술을 받았으며 이후 폐전이에 대해 5-fluorouracil, interferon-a과 interleukin-2을 이용한 변역요법을 시행하였다. 이후 폐 전산화단층촬영상 암이 진행된 것으로 나타나 Sutene^(?)을 이용한 경구 표적요법을 시행중이다.

Objective : There were variant results for correlation between the postoperative cure rate and valsalva leak point pressure (VLPP) for female stress urinary incontinence (SUI). Therefore the aim of this study was to assess the influence of VLPP on the outcome of suprapubic arch (SPARC) procedure. Materials and Methods : A retrospective analysis was made of 246 women who underwent SPARC procedure for SUI. Based on VLPP, the patients were divided into 2 groups: group A (VLPP<60cmH2O) and group B (VLPP≥60cmH2O). The objective success rate and subjective success rate were assessed. Before surgery, the patients were evaluated with history taking, physical examination, urinalysis and urodynamic studies. The objective success rate was evaluated by clinical examination and the 1-hour pad test postoperatively and the subjective satisfaction rate was assessed using questionnaires for the patients' satisfaction and evaluated by recommendation rates of SPARC procedure to other patients. The objective success rate included cure and improvement, the subjective success rate included satisfaction and fair. Results : The number of patients in each group was 145 patients (group A) and 101 patients (group B). The objective success rate was 99.3% (group A) vs 98.0% (group B) (p=0.285). The subjective success rate was 98.6% (groupA) vs 98.0% (group B) (p=0.500). Recommendation rates of SPARC procedure was 94.5% (group A) vs 95.0% (group B) (p=0.422). Conclusion : It is our belief that SPARC procedure for female SUI is efficacious and safe regardless of preoperative VLPP.

기종성 방광염

Emphysematous cystitis (EC) is a rare complicated lower urinary tract infection (UTI)characterized by air within the bladder wall and lumen. The disease is most common in middle-aged diabetic women. Patients with chronic UTI, indwelling urethral catheters, urinary tract outlet obstruction or neurogenic bladders are predisposed to complicated UTI such as EC. Most patients can be managed by adequate bladder drainage and appropriate antibiotics coverage and operation is needed very rarely. We report a diabetic women with EC who underwent partial cystectomy.

Renal cell carcinoma is the third most common neoplasia of the genitourinary tract. Its most common type, representing 60% of the cases, is the clear cell carcinoma, with an incidence peak between 50 and 70 years. Metastases are present at the time of diagnosis in approximately 30% of the patients, the major sites being lungs, bones, skin, liver, and brain. But distant metastasis of malignant neoplasm to the masseter muscle is extremely rare. We report a case of renal cell carcinoma (RCC) metastasizing to the masseter muscle.

Spindle cell lesions of the urinary bladder are uncommon tumors, and are most often spindle cell (sarcomatoid) carcinomas, non-neoplastic reactive mesenchymal proliferations, or soft tissue sarcomas. Inflammatory myofibroblastic tumors (IMTs) may also occur in this location, but IMTs occurring at bladder sites have rarely been described. We experienced a case of IMT of bladder.

소아의 신손상 35례의 분석

Objective: The purpose of this study was to describe the clinical features of renal injury in the pediatric population. Methods and Materials: We retrospectively reviewed the medical records of 35 children with renal trauma who presented to our hospital between 1994 and 2002. Renal injuries were classified as established by the Organ Injury Scaling Committee of the American Association for the Surgery of Trauma in 1989. Results: Injury of the 35 children with renal trauma, 34 children(97.1%) were injured by blunt trauma. The overall incidence of significant renal injury was 42.8% and the incidence according to the cause was 45.7%, 37.1%, 8.5%, 5.7% in Motor vehicle accidents, falls, assault, and sports. There were 15cases(43.0%), gradeⅠ, 5(14%) grade Ⅱ, 3(8%) gradeⅢ, 5(14%) grade Ⅳ, 7(21%) grade V. Of the 35 patients, 17 cases presented a microscopic hematuria, 16 cases had a gross hematuria, 2 casts were normal. 28 patients(80%) had an associated injury, including Extremity fracture, Liver injury, Thorax injury and Head injury. Surgical exploration was required in 8 patients(23%). Conclusions: Main causes of injury were blunt abdominal trauma. We found that the degree of hematuria does not correlated with the severity of renal injury. Immediate CT is an valuable aid in patients evaluation.

전립선육종

Prostatic sarcoma is rare malignant tumor in adults. It occurs from nonepithelial mesenchymal components of the prostate stroma, which usually present with bladder outlet obstructive symptoms and upper tract obstruction. Historically, long-term survival rates for adult patients with prostate sarcoma are poor. Here we report a case of adult prostatic sarcoma with a review of the previous reports.
