        Correlation between Visual Quality of Life and Visual Function in Mild Glaucoma Patients

        목적 : 초기 녹내장 환자를 대상으로 시기능이 일상생활에서의 시력의 질에 미치는 영향을 알아보고자 하였다. 방법 : 서울시 KU 대학교 병원 안과에서 본 연구에 참여 의사를 밝힌 초기 녹내장 환자 20명을 대상으로 하였다. 녹내장 환자의 시기능을 측정하기 위해 대비시력, 대비감도, 시야결손, 입체시력을 측정하였고, NEI VFQ-25 설문지를 이용하여 시기능을 평가하였다. 모든 측정값은 SPSS 26.0 프로그램을 이용하여 분석하였다. 결과 : 초기 녹내장 환자의 경우 저대비 시력이 정상안보다 저하되었고, 시력저하와 시야결손은 일상생활의 시각적 ‘활동의존성’과 상관성이 있었다(p<0.050). Better eye의 3 cpd 및 12 cpd 대비감도는 ‘원거리 시생활’의 불 편함과, worse eye의 18 cpd 대비감도는 ‘근거리 시력’, ‘주변 시생활’ 및 ‘전반적인 시기능’과, 양안의 12 cpd 대 비감도는 전반적인 시기능저하와 관련이 있었다(p<0.050). 근거리 입체시력은 ‘역할의 제한’과 관련이 있어 취미나 직업 등의 활동에 제한이 있는 것으로 나타났다. 결론 : 시야결손이 적은 초기 녹내장 환자에서 시력, 대비감도 및 입체시력의 저하와 시야감소는 시생활의 질을 저하시키는 것으로 나타났다. Purpose : The correlation between visual quality of life and visual function in mild glaucoma patients was investigated. Methods : The study was conducted on 20 mild glaucoma patients who expressed their intention to participate for this study at K University Hospital in Seoul. Contrast visual acuity, contrast sensitivity, visual field devect and stereoacuity were measured, and quality of life using NEI VFQ-25 questionnaire was assessed. All measurements were statistically analyzed with the SPSS 26.0 program. Results : In the mild glaucoma, the low contrast visual acuity was lower than normal, and decreased visual acuity and visual field loss were related to ‘dependency’ (p<0.050) in the visual life. In the contrast sensitivity, decrease of contrast sensitivity at 3 and 12 cpd in the better eye was related to difficulty of ‘distance vision’, and contrast sensitivity at 12 and 18 cpd in the worse eye was related to the difficulty of ‘near vision’, ‘peripheral vision’ and ‘total score’. Binocular contrast sensitivity at 12 cpd was correlated with the total score of NEI VFQ-25 questionnaires (p<0.050). The stereoacuity at near was related to the ‘role difficulties’ such as limitation of hobby and job activities. Conclusion : In the mild glaucoma patients with less visual field loss, it was revealed that a decrease in visual acuity, contrast sensitivity and stereoacuity and visual field loss could decline the quality of visual life.

      • Meaningful of Pneumothorax in Pneumocystis Pneumonia Patients with Respiratory Failure

        Purpose Pneumocystis jirovecii pneumonia (PCP) with acute respiratory failure can increase the development of pneumothorax during treatment. This study aims to identify the incidence and related factors of pneumothorax in PCP patients with acute respiratory failure and to analyze their prognosis. Method We retrospectively reviewed the occurrence of pneumothorax including clinical characteristics and Results of other examinations in 119 patients with PCP pneumonia and respiratory failure requiring the mechanical ventilator treatment in medical intensive care unit (ICU) at a tertiary-care center between July 2016 and April 2019. Result The 28-day survival rate was 62.2% (N=74). Twenty-two patients (18.5%) developed the pneumothorax during ventilator treatment, and 45 patients (37.8%) were required the tracheostomy due to weaning failure. The cytomegalovirus co-infection (odds ratio 13.9; p = 0.013) were related with the occurrence of pneumothorax. Of 74 patients with 28-day survival, 15 patients (20.3%) were underwent the pneumothorax. In those patients, 5 patients survived (p = 0.048), and only 2 patients finally succeeded to wean from the ventilator (p = 0.037). Conclusion PCP patients with acute respiratory failure who developed pneumothorax were not associated with 28-day mortality, but might have related to difficult and prolonged weaning from the mechanical ventilator.

        Study on the Visual Acuity and Balance Ability in the Elderly without Visual Field Defects

        목적 : 몸의 균형 능력 저하는 낙상과 매우 밀접한 관계에 있어, 본 연구에서는 낙상 위험이 높은 노년층을 대 상으로 시력과 균형 능력간의 상관성을 분석하고자 하였다. 방법 : 성남시 S복지관에서 본 연구에 참여 의사를 밝힌 고령자 중 시야에 문제가 없는 고령자 총 24명(남4, 여20, 평균 나이 74.79±4.85세)을 대상으로 하였다. 시야검사는 OPTOS로 측정하였고, 시력은 ETDRS(early treatment diabetic retinopath study)시표를 이용하여 저대비(low contrast)시력과 고대비(high contrast)시력을 측정하였다. 균형 검사는 Tetra-ataxiometric posturography(Tetrax) 프로그램을 사용하여 안정성 지수, 푸리에 지수, 체중분포율을 산출하였다. 이후 산출된 균형능력 데이터와 시력 간에 상관분석은 SPSS 26.0 프로그램을 사용하여 시행하였다. 결과 : 시력과 관련된 NO자세와 PO자세에서 고대비시력은 푸리에지수(F6~F8)와 유의한 상관성을 보였고, 저 대비시력은 푸리에지수(F6)과 안정성지수와 유의한 상관성을 보였으며(p<0.050), 고대비 및 저대비 시력이 감소 할수록 안정성지수와 퓨리에지수가 증가하여 균형능력이 저하되는 것으로 나타났다. 결론 : 시야이상이 없는 고령자에서는 시력이 낮으면 균형능력이 저하되기 때문에 낙상위험을 낮추기 위해서는 정기적인 시력검사와 굴절교정이 필요할 것으로 사료된다. Purpose : The study was aimed to investigate the effects of visual acuity on the balance ability in the elderly who were at a higher risk of the falls, based on the intimate connection between reduction of balance ability and falls. Methods : Twenty-four elderly subjects without visual field defect (4 males and 20 females, aged 74.79±4.85 in average) voluntarily took part in the study at the S Welfare Center in Seongnam City. The visual field and visual acuity under the low and high contrast were measured with OPTOS and ETDRS (Early Treatment Dia-betical Retinopath Study) charts, recpectively. The balance was identified by the Tetra-ataxiometric posturography (Tetrax) program to evaluate the stability index, Fourier index and weight distribution rate. The correlation of the visual acuity with balance ability was analized by the SPSS 26.0 program. Results : In the NO and PO positions related to vision, the high contrast visual acuity showed a significant correlation with the Fourier Index (F6~F8), while the low contrast visual acuity showed significant correlations with the Fourier Index (F6) and stability index (p<0.050). The lower the high and low contrast visual acuity were, the worse the stability index and the Fourier index the lower the balance ability were. Conclusion : The decrease in vision resulted in reducing the balance ability in the elderly without visual field defect. Therefore, it is thought to be needed regular checkup of visual acuity and the correction to decrease falling risk.

      • Development of the Composite Biologic Scoring as a Predictor of Longterm Prognosis of Lung Transplantation

        Background Survival after lung transplantation (LT) has steadily improved but its prognosis is still lower than other solid organ transplantation. It is important to predict prognosis in patients who underwent LT but currently there is no specific tool for prediction. Methods In the single-center retrospective study, we reviewed data from patients who underwent LT at Severance Hospital in South Korea from October 2012 to September 2020. Patients who underwent multi-organ transplantation (n=5), re-transplantation (n=5), age ≤18 years old (n=4) and early acute-phase patients within 3 months after LT (n=31) were excluded. A total of 249 LT patients were included for constructing the prediction model for prognosis called the Composite Biologic Scoring (CBS), using the numeric biologic markers around 3 months as a reference which were highest among observation period. Cox proportional hazards model was used to calculate the weight of each criterion included in the CBS, and the predictive accuracy and discriminative ability were measured by concordance index (C-index) and Kaplan Meier survival curve. Results Among 249 patients, mean age was 54.5±11.6 years, and 161 (64.7%) were men. Eight independent prognostic factors were identified and points were assigned to each variable based on Cox proportional hazards regression analysis including white blood cells, hemoglobin, platelet, ferritin, fibrinogen, aspartate aminotransferase, triglyceride, and lactate dehydrogenase (Fig. 1). The C-index for cox proportional hazards model was 0.809 and the calibration curves for the probability of 2, 3, and 5-year overall survival showed close approximation between nomogram prediction and actual observation. Three stratified risk groups according to CBS in post LT patients demonstrated significant distinction between Kaplan-Meier survival curves (Fig. 2 & 3; p-value<0.001). Conclusion The CBS is a useful prediction model for long-term prognosis of LT patients. Further validation study is needed to confirm.

        K-doping effects on the characteristics of ZnO thin films synthesized by using a spin-coating method

        Zinc-oxide (ZnO) thin films doped with various amounts of potassium were synthesized on quartz substrates by using the sol-gel method. The effects of K-doping on the structural and optical properties of the resultant ZnO thin films were investigated using X-ray diffraction (XRD), Ultravioletvisible (UV) spectroscopy, and photoluminescence (PL) spectroscopy. From the XRD results, the full width at half maximum (FWHM) for ZnO (002) and the grain size was measured. In the transmittance spectra, all the K-doped ZnO thin films were found to exhibit a high transmittance in the visible region. The optical band gap of the thin films was also determined from the transmittance results. The PL spectra showed near-band-edge (NBE) emission peaks in the UV region and deep-level emission (DLE) peaks of around 640 nm. The PL peak intensities of both the NBE emission and the DLE were found to change with varying concentrations of K.

        시설 내 오이(Cucumis sativus L.) 재배 중 Penthiopyrad 및 Pyriofenone의 잔류특성과 생산단계 잔류허용기준 설정

        본 연구는 시설재배 작물인 오이의 생산단계 중 penthiopyrad 및 pyriofenone의 시간경과에 따른 잔류량을 감소식으로 회귀식을 계산한 후, 생물학적 반감기를 산출함으로써 생산단계 잔류허용기준을 설정하여 농가의 안전한 농산물생산에 기여하고자 수행하였다. Penthiopyrad 및 pyriofenone는 작물보호지침서의 안전사용기준에 준하여 기준량 1회 처리하였고, 각각 포장 1과 포장 2 지역으로 나누어 처리하였다. 약제살포 2시간 후를 0일차로하여 0, 1, 2, 3, 5, 7, 10일차에 시료를 채취하였다. 두 약제 모두 추출 및 정제를 위해 acetonitrile, dichloromethane, SPE NH2 cartridge를 사용하였고 LC-MS/MS를 이용하여 분석하였으며, 분석 정량한계는 0.005 mg/kg이었다. 분석정량한계의 10배, 50배 수준으로 농약을 처리하여 수행한 결과 penthiopyrad 및 pyriofenone의 회수율은 각각 92.8∼95.4%, 81.0∼89.8%이었으며, 표준편차는 두 약제 모두 10% 미만이었다. 오이 중 두 약제에 대한 포장 1, 포장 2의 생물학적 반감기는 penthiopyrad의 경우 각각 2.6일과 2.5일, pyriofenone의 경우 각각 2.5일과 2.4일로 나타났다. Penthiopyrad 및 pyriofenone의 잔류량을 바탕으로 오이 수확 10일 전 잔류량이 3.44 mg/kg, 4.63 mg/kg이하로 나타난다면 수확 시 두 약제의 잔류량은 MRL (Penthiopyrad: 0.5 mg/kg, Pyriofenone: 0.7 mg/kg) 수준 이하일 것으로 사료된다. 본 연구결과를 바탕으로 오이 중 penthiopyrad 및 pyriofenone의 생산단계 잔류허용기준이 설정된다면 오이 재배농가에서 유통단계 뿐만 아니라 생산단계에서 안전한 농산물을 생산하는데 기여할 것으로 사료된다. BACKGROUND: Greenhouse crops are one of agricultural products consumed largely in Korea. Cucumber is a typical example as main vegetables of greenhouse crops. Thus, pesticide residue analysis is an important requirement to guarantee pesticide safety of cucumber. This work was aim to investigate the residues of penthiopyrad and pyriofenone in cucumber after harvest. METHODS AND RESULTS: Cucumber was subjected to treat with penthiopyrad and pyriofenone at a level of recommended dose 0, 1, 2, 3, 5, 7 and 10 days before harvest under greenhouse conditions. The samples were extracted with organic solvent by using a homogenizer and purified on solid phase cartridge column followed by LC-MS/MS analysis. The recovery levels of penthiopyrad and pyriofenoneranged from approximately 81 to 93%with the method limit of 0.005mg/kg and coefficient of variation less than 10%. Penthiopyrad and pyriofenone were detected at a level less than maximum residue limit in cucumber at 10 days before. The half-lives of penthiopyrad and pyriofenone were determined to 2.4 ~ 2.6 days. CONCLUSION: Penthiopyrad and pyriofenone are suggested to use in cucumber 10 days before harvest to reach their levels less than maximum residue limit.

      • Enhanced Light Emission from Monolayer Semiconductors by Forming Heterostructures with ZnO Thin Films

        <P>Monolayer transition-metal dichalcogenides (1L-TMDs) are atomically thin direct band gap semiconductors, from which the emission of light is determined by optical transitions of exciton complexes such as neutral excitons and trions. While the quantum yields of 1L-TMDs are quite low, the ability to control the populations of exciton complexes in 1L-TMDs through various doping processes is an interesting advantage, and provides ample possibilities for engineering the optical properties of these semiconductor monolayers. Here we demonstrate a simple method of controlling the populations of excitons and trions to enhance the light emission of 1L-TMDs by having them form heterostructures with ZnO thin films (TFs). 1Ls of MoS2 or MoSe2 showed up to 17-fold increases in photoluminescence (PL) when they were placed on similar to 50 nm thick ZnO TFs. This enhancement of the PL was due to charge exchanges occurring through the 1L-TMD/ZnO interface. The PL enhancements and changes in the PL spectra of the 1L-TMDs were greater when the 1L-TMD/ZnO heterostructures were subjected to 355 nm wavelength laser excitation than when they were excited with a 514 nm wavelength laser, which we attributed to the onset of energy transfer by photoexcited excitons and/or the additional p-doping by photoexcited holes in ZnO. The p-doping phenomenon and the enhanced light emission of 1L-TMD/ZnO heterostructures were unambiguously visualized in spatially resolved PL and Raman spectral maps. Our approach using the 1L-TMD/ZnO TF heterostructure suggests that a rich variety of options for engineering the optical properties of 1L-TMDs may be made available by carrying out simple and intuitive manipulations of exciton complexes, and these endeavors may yield practical applications for 1L-TMDs in nanophotonic devices.</P>
