Respiratory-induced dynamic tumors render free-breathing cone-beam computed tomography (FBCBCT) imageswith motion artifacts complicating the task of quantifying the internal target volume (ITV). The purpose of thispaper is to study the visibility of the revealed ITV when the imaging dose parameters, such as the kVp and mAs,are varied. The TrilogyTM linear accelerator with an On-Board Imaging (OBITM) system was used to acquirelow-imaging-dose-mode (LIDM: 110 kVp, 20 mA, 20 ms/frame) and high-imaging-dose-mode (HIDM: 125 kVp,80 mA, 25 ms/frame) FBCBCT images of a 3-cm diameter sphere (density=0.855 g/cm3) moving in accordanceto various sinusoidal breathing patterns, each with an unique inhalation-to-exhalation (I/E) ratio, amplitude, andperiod. In terms of image ITV contrast, there was a small overall average change of the ITV contrast when goingfrom HIDM to LIDM of 6.5±5.1% for all breathing patterns. As for the ITV visible volume measurements, therewas an insignificant difference between the ITV of both the LIDM- and HIDM-FBCBCT images with an averagedifference of 0.5±0.5%, for all cases, despite the large difference in the imaging dose (approximately five-folddifference of ~0.8 and 4 cGy/scan). That indicates that the ITV visibility is not very sensitive to changes in imagingdose. However, both of the FBCBCT consistently underestimated the true ITV dimensions by up to 34.8%irrespective of the imaging dose mode due to significant motion artifacts, and thus, this imaging technique isnot adequate to accurately visualize the ITV for image guidance. Due to the insignificant impact of imaging doseon ITV visibility, a plausible, alternative strategy would be to acquire more X-ray projections at the LIDM settingto allow 4DCBCT imaging to better define the ITV, and at the same time, maintain a reasonable imaging dose,i.e., comparable to a single HIDM-FBCBCT scan.

Background: Nintedanib is an approved treatment for idiopathic pulmonary fibrosis (IPF). Pre-clinical data suggest that nintedanib inhibits processes fundamental to progression of lung fibrosis irrespective of the aetiology. The INBUILD trial was designed to investigate the efficacy and safety of nintedanib in patients with various non-IPF chronic fibrosing ILDs with progressive phenotype. Methods: Eligible patients had diffuse fibrosing lung disease of >10% extent on HRCT, FVC ≥45% predicted, DLCO ≥30-<80% predicted, and met ≥1 of 4 criteria for ILD progression (Table) in the 24 months before screening, despite treatment of ILDs in clinical practice. Patients with IPF were excluded. Subjects were randomised to receive nintedanib 150 mg bid or placebo. The primary endpoint is the annual rate of decline in FVC (mL/yr) assessed over 52 weeks. There will be two co-primary analysis populations: all subjects and subjects with a UIP-like fibrotic pattern only on HRCT. Results: 663 patients were randomised and treated (Table). Mean (±SD) age was 65.8±9.8 years, FVC was 69.0±15.7% predicted, DLco was 47.6±32.2% predicted. Conclusions: The INBUILD trial will provide insights into the natural history and role of nintedanib in treating patients with various progressive fibrosing ILDs. Results will be presented at the congress.

Background Several studies have suggested that the progression of fibrosing ILDs is more rapid in patients with a usual interstitial pneumonia (UIP) pattern on HRCT. In the INBUILD trial, nintedanib slowed the rate of decline in FVC in subjects with fibrosing ILDs and a progressive phenotype. We assessed the effect of nintedanib in subgroups by HRCT pattern at baseline. Methods In the INBUILD trial, subjects (n=663) with a progressive fibrosing ILD were randomized to receive nintedanib or placebo. Randomization was stratified by HRCT pattern (UIP-like fibrotic pattern or other fibrotic patterns) based on central review. In pre-specified analyses, we assessed the effects of nintedanib in subgroups of subjects with a UIP-like fibrotic pattern and other fibrotic patterns on HRCT at baseline. Results At baseline, 62.1% of total subjects had a UIP-like fibrotic pattern on HRCT. In subjects who received placebo, the adjusted mean annual rate (SE) of decline in FVC was -209.2 (19.1) mL/year in subjects with a UIP-like fibrotic pattern on HRCT and -155.4 (23.6) mL/year in subjects with other fibrotic patterns on HRCT. The difference between the nintedanib and placebo groups in the annual rate of decline in FVC was 127.8 (95% CI: 74.3, 181.2) mL/year in subjects with a UIP-like fibrotic pattern on HRCT and 75.4 (95% CI: 9.5, 141.4) mL/year in subjects with other fibrotic patterns on HRCT (treatment-by-subgroup-by-time interaction p=0.23) (Table). The effects of nintedanib vs placebo on change from baseline in K-BILD questionnaire total score at week 52, time to acute exacerbation of ILD or death over 52 weeks, and time to death over 52 weeks between the subgroups by HRCT pattern are shown in the Table. Conclusions In the INBUILD trial, the effect of nintedanib on slowing the rate of FVC decline was consistent regardless of fibrotic pattern on HRCT.
Graph and Controller Design for Disturbance Attenuation in Consensus Networks

Disturbance attenuation in consensus networks has been recently studied in the literature. In this paper, we provide illustrative examples of graph and controller design for disturbance attenuation in undirected consensus networks of identical linear time-invariant systems subject to exogenous disturbance. We first summarize some existing results on disturbance attenuation in such consensus networks, and then solve specific graph and controller design examples based on the results.

<P><B>Abstract</B></P> <P>The uptake of nutrients, including metals, amino acids and peptides are required for many biological processes. Pathogenic bacteria scavenge these essential nutrients from microenvironments to survive within the host. Pathogens must utilize a myriad of mechanisms to acquire these essential nutrients from the host while mediating the effects of toxicity. Bacteria utilize several transport proteins, including <U>A</U>TP-<U>b</U>inding <U>c</U>assette (ABC) transporters to import and expel substrates. ABC transporters, conserved across all organisms, are powered by the energy from ATP to move substrates across cellular membranes. In this review, we will focus on nutrient uptake, the role of ABC importers at the host–pathogen interface, and explore emerging therapies to combat pathogenesis. <P><B>Abstract</B></P><P>Herein, we report a general synthetic pathway to various shapes of three‐dimensional (3D) gold nanoframes (NFs) embedded with a Pt skeleton for structural rigidity. The synthetic route comprises three steps: site‐specific (edge and vertex) deposition of Pt, etching of inner Au, and regrowth of Au on the Pt framework. Site‐specific reduction of Pt on Au nanoparticles (NPs) led to the high‐quality of 3D Au NFs with good structural rigidity, which allowed the detailed characterization of the corresponding 3D metal NFs. The synthetic method described here will open new avenues toward many new kinds of 3D metal NFs.</P>
