        Emergence of NDM-1–producing Pseudomonas aeruginosa Sequence Type 773 Clone: Shift of Carbapenemase Molecular Epidemiology and Spread of 16S rRNA Methylase Genes in Korea

        Imipenemase (IMP)-6–producing Pseudomonas aeruginosa sequence type (ST) 235 is a dominant clone of carbapenemase-producing P. aeruginosa (CPPAE) in Korea. As part of the Antimicrobial Resistance Surveillance System in Korea, we found an increase in the carbapenem resistance rate of P. aeruginosa isolates from blood cultures and a shift in the molecular epidemiology of CPPAE. A total of 212 non-duplicated P. aeruginosa blood isolates were obtained from nine general hospitals and two nursing homes. Twenty-four isolates were identified as CPPAE. We observed the emergence of the NDM-1 P. aeruginosa ST 773 clone (N=10), mostly from Gyeongsang Province. The IMP-6 ST 235 clone (N=11) was detected in all provinces. CPPAE isolates showed very high resistance rates to amikacin, and all NDM-1 P. aeruginosa strains carried rmtB. This is the first nationwide surveillance of the recently emerged NDM-1–producing P. aeruginosa ST773 clone in Korea. Continuous surveillance is necessary to prevent the infection and transmission of carbapenem- and amikacin-resistant P. aeruginosa in Korea.

        Discordance between Physician and the General Public Perceptions of Prognostic Disclosure to Children with Serious Illness: a Korean Nationwide Study

        <P><B>Background</B></P><P>It is difficult to decide whether to inform the child of the incurable illness. We investigated attitudes of the general population and physicians toward prognosis disclosure to children and associated factors in Korea.</P><P><B>Methods</B></P><P>Physicians working in one of 13 university hospitals or the National Cancer Center and members of the general public responded to the questionnaire. The questionnaire consisted of the age appropriate for informing children about the prognosis and the reason why children should not be informed. This survey was conducted as part of research to identify perceptions of physicians and general public on the end-of-life care in Korea.</P><P><B>Results</B></P><P>A total of 928 physicians and 1,241 members of the general public in Korea completed the questionnaire. Whereas 92.7% of physicians said that children should be informed of their incurable illness, only 50.7% of the general population agreed. Physicians were also more likely to think that younger children should know about their poor prognosis compared with the general population. Physicians who opposed incurable illness disclosure suggested that children might not understand the situation, whereas the general public was primarily concerned that disclosure would exacerbate the disease. Physicians who were women or religious were more likely to want to inform children of their poor prognosis. In the general population, gender, education, comorbidity, and caregiver experience were related to attitude toward poor prognosis disclosure to children.</P><P><B>Conclusion</B></P><P>Our findings indicate that physicians and the general public in Korea differ in their perceptions about informing children of poor prognosis.</P>

        Reliability of Acinetobacter baumannii Identification with Matrix-Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry

        Background: The purpose of this study was to evaluate the reliability of identification with the strains of Acinetobacter baumannii and non-baumanni Acinetobacter spp, determined with matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS), compared to the molecular genetic method. Methods: A total of 225 blood isolates of A. baumannii and non-baumanni Acinetobacter spp. were included in this study. Species identification was performed using MALDI-TOF MS, blaOXA-51 polymerase chain reaction (PCR), and RNA polymerase β-subunit (rpoB) PCR-sequencing. Results: The MALDI-TOF MS results showed 100% category agreement with that of blaOXA-51 PCR, which clearly distinguished A. baumannii from nonbaumanni Acinetobacter spp. The species-concordance rate of non-baumanni Acinetobacter spp. was 73.7% (14/19) on the basis of rpoB sequence results. Conclusions: MALDI-TOF MS is very useful in distinguishing between A. baumannii and non-baumanni Acinetobacter spp. accurately, which is useful information on the choice of therapeutic agents. However, there was a limitation in correctly identifying non-baumanni Acinetobacter spp.

        배려의 관점에서 누리과정 내용 분석

        본 연구에서는 배려의 개념을 고찰하고, 이에 기초해서 누리과정의 내용을 분석하고자 하였다. 이를 위해 배려를 관계, 동일성과 동등성, 실천으로 개념화하였다. 분석결과, 누리과정은 배려의 관계적 측면, 동일성과 동등성의 측면, 실천의 측면과 관련된 내용들을 비교적 모든 영역에서 골고루 반영하고 있는 것으로 나타났다. 그러나 누리과정의 성공 여부는 현장 교사들의 역량에 의해 좌우된다. 따라서 교사들은 배려를 포함한 인성교육의 가치를 인식하고, 누리과정 구성의 기본방향에 대한 교사의 이해가 선행되어야 할 필요가 있음을 논의하였다. The purpose of this study was to examine the concept of care and to analyze the contents of the Korean National Curriculum, NURI based on this concept. To this end, we conceptualized caring as relation, the same and the equal, practice. As a result of the analysis, it is found that the curriculum reflects the concept of caring evenly in all areas. However, the success of the Korean National Curriculum, NURI depends on the competence of field teachers. Therefore, it was discussed that teachers should recognize the value of humanistic education including caring and the teacher's understanding about the basic direction of the curriculum should be preceded.

        유아의 문제행동과 의사소통능력의 관계

        본 연구의 목적은 유아의 성별과 연령에 따른 문제행동과 의사소통능력의 차이를 확인하고, 문제행동과 의사소통능력의 관계를 알아보는 것이다. 이를 위해 경기도에 위치한 유치원에 재원중인 만 4, 5세 유아 165명을 대상으로 문제행동과 의사소통능력에 대한 검사를 실시하였다. 수집된 자료는 이원변량분석 및 상관관계 분석을 하였으며, 결과는 다음과 같다. 성별과 연령에 따라 유아의 문제행동과 의사소통능력에는 차이가 있었다. 또한 의사소통능력은 유아의 외현적, 내재적 문제행동과 부적으로 상관이 있는 것으로 나타났다. 특히 의사소통능력은 유아의 외현적 문제행동보다 내재적 문제행동과 좀 더 높은 상관이 있었다. 이러한 결과는 유아의 문제행동을 감소시키기 위해 의사소통능력을 증진시킬 필요가 있음을 시사한다. This study investigated the differences in problem behaviors and communication ability according to children’s gender and age, and the relationship between problem behaviors and communication ability of young children. For this purpose, 165 children aged 4 and 5 who were in kindergartens in Gyeonggi-do were examined for their problem behaviors and communication ability. The collected data were analyzed by two-way ANOVA and correlation analysis. The results are as follows. There were significant differences in problem behaviors and communication ability according to children’s gender and age. In addition, the communication ability was negatively correlated with the children’s external and internal problem behaviors. Specifically, communication ability was more negatively related to internal problem behaviors than external problem behaviors. These results suggest that there is a need to improve communication ability to reduce problem behaviors of young children.

        부모의 정서표현 수용태도가 유아의 정서지능 및 놀이성에 미치는 영향

        본 연구는 부모의 정서표현 수용태도가 유아의 정서지능 및 놀이성에 어떠한 영향을 미치는지에 대해 알아보고자 하였다. 이를 위해 본 연구에서는 만 3, 4, 5세 유아 186명과 이들의 부모를 대상으로 부모의 정서표현수용태도, 유아의 정서지능 및 놀이성에 대한 검사를 실시하였다. 본 연구의 결과, 자녀에 대한 부모의 정서표현 수용태도는 유아의 정서지능과 놀이성에 영향을 미치는 것으로 나타났으며, 유아의 정서 지능 중 또래와의 관계, 놀이성 중 사회적 자발성에 많은 영향을 미치는 것으로 나타났다. 특히, 본 연구는 유아의 부정적 정서표현에 대한 부모의 수용태도가 설명력이 높은 것으로 나타났다. The purpose of this study was to investigate the effect of parental attitude toward young children’s emotional expression on their emotional intelligence and playfulness. Subjects of this study consisted of 186 children of 3 to 5 years of age who were in day care centers in Seoul and the surrounding Gyeonggi Province, and their parents. They filled out the questionnaires of Parental Attitude toward Children's Expressiveness Scale(PACES), Emotional Intelligence, and Playfulness. This study showed that parental attitude toward young children’s emotional expression was significant to explain their emotional intelligence and playfulness. Peer relationships were influenced based on young children’s emotional intelligence and their playfulness. Specifically, the explanatory power of parental attitude toward young children’s negative emotional expression was high.

        원장의 감성리더십과 보육교사 이직의도의 관계에서 직무만족도의 매개효과

        본 연구의 목적은 어린이집 원장의 감성리더십과 보육교사 이직의도의 관계에서 직무만족도의 매개효과를 검증하는 것이다. 이를 위해 서울 및 경기도 소재 어린이집에 재직 중인 보육교사 304명으로부터 자료를 수집하였다. 구조방정식을 활용하여 모형을 검증한 결과, 보육교사의 직무만족도는 원장의 감성리더십과 이직의도의 관계를 완전 매개하는 것으로 나타났다. 매개효과에 대한 Bootstrapping 검정 결과, 원장의 감성리더십과 이직의도의 관계에 대한 보육교사 직무만족도의 매개효과는 통계적으로 유의한 것으로 확인되었다. 이러한 결과는 보육교사의 이직의도를 낮추기 위해 원장은 감성리더십을 함양하고, 이를 통해 교사의 직무만족을 향상시킬 필요가 있음을 시사한다. The purpose of this study is to verify the mediating effect of job satisfaction in the relationship between the emotional leadership of the director and the turnover intention of the childcare teacher. To do this, data was collected from 304 childcare teachers at daycare centers in Seoul and Gyeonggi-do province. Using a structural equation model, it was found that job satisfaction completely mediates the relationship between the director’s emotional leadership and childcare teacher’s turnover intention. As a result of the bootstrapping test, it was found that the mediating effect of job satisfaction on the relationship between the director’s emotional leadership and turnover intention was statistically significant. These results suggest that in order to lower the turnover intention, the director needs to cultivate better emotional leadership and thereby improve the teacher’s job satisfaction.

        자유선택활동에서 유아들이 보여주는 배려의 표현

        본 연구는 유아들이 보다 유능한 배려자로 성장할 수 있도록 돕기 위해 자유선택활동 시간에 유아들이 수행하는 배려행동을 배려의 개념 즉 배려의 관계, 동등성, 실천의 측면에 비추어 분석하였다. 이를 위해 약 두 달 간 만4세 학급에서 참여관찰 및 동영상 촬영을 실시하였다. 결과를 제시하면 다음과 같다. 관계적 측면에서 유아들은 전념 및 동기전환의 태도를 보여주었으나 자신의 능력을 과시하면서 배려를 수행하거나 상대방의 배려를 거부하기도 하였다. 동등성 측면에서는 상대방과 자신의 차이를 인정하는 유아도 있었으나 이 시기 유아들의 자아중심성이나 인지적 한계로 인해 상대 유아의 능력을 인정하지 않는 모습도 보여주었다. 마지막으로 실천적 측면에서 유아들은 또래를 배려하는 행동을 수시로 보여주기는 하였으나 주로 간섭의 형태로 배려가 표현되고 있음이 발견되었다. To support young children who can be a good carer, this study explored children's caring from the concept of caring(relationship, equality, practice). In terms of relationship, they showed engrossment and motivational displacement, but they showed off their competence. Also, some children refused other’s caring. In terms of equality, some children recognized the differences between themselves and other people, but they showed that they did not recognize peer’s ability because of their egocentrism and cognitive limitations. Finally, in terms of practice, children frequently demonstrated caring behaviors, but it was mainly in the form of interference.

        Background: CBT is an established alternative donor transplantation for treatment of patients with advanced hematologic diseases. Methods: To assess clinical outcome after CBT and provide future recommendations for alternative donor, we conducted the first retrospective multicenter study of adult CBT in Korea. Results: Between June 2001 and April 2014, 48 patients (male, 19) were enrolled from 16 hospitals. The median age was 36 years (range, 16-65 years). Forty patients (83.3%) were acute leukemia: 33 (68.8%) in remission and 7 (14.6%) in persistence. The others were myelodysplastic syndrome (n=3, 6.3%), severe aplastic anemia (n=3, 6.3%), and lymphoma (n=2, 4.2%). Thirty two patients (66.7%) underwent double unit CBT. Twenty two patients (45.8%) underwent myeloablative conditioning and 26 patients (54.2%), reduced-intensity conditioning. Total body irradiation (TBI)- based conditioning regimens were used in 23 patients (47.9%). The median time to neutrophil >500/μL was 20 days among grafted, and 14 patients (29.2%) failed to recover. During follow-up, 8 patients died of disease and non-relapse mortality (NRM) developed in 23 patients. The 2-year overall survival (OS) was 29.6%. In multivariable analysis, ECOG performance status (PS) =2 (hazard ratio [HR] 6.42, p=0.001) and total nucleated cell dose <3.0x10^7/kg (HR 2.59, p=0.012) were poor prognostic factors for OS. Factors associated with graft failure were ECOG PS 2 (p=0.012) and non-TBIbased conditioning (p=0.026). CBT outcomes after January 2009 were significantly improved compared to those before January 2009 (p=0.012). Conclusions: Although in the past, the clinical outcome of adult CBT was not satisfactory in Korea because of high NRM, it has been improved in recent years. Our data suggest that the clinical outcome substantially depends on the cord blood cell dose and PS.

      • 영유아교사의 공감능력과 교사-유아 상호작용의 관계에서 사회적 지지의 조절효과

        The purpose of this study is to verify the moderating effect of social support in the relationship between early childhood teachers’ empathy and teacher-child interaction. Through this, we tried to find a way to effectively improve the teacher’s interaction ability. To achieve this study purpose, data were collected from 383 teachers working in kindergartens and day care centers nationwide. Data to be used for analysis were collected by distributing questionnaires on mobile to teachers who agreed to participate in this study. Moderating regression analysis was conducted with the collected data to verify the research question. The results of this study are as follows. The main effects of empathy and social support for teacher-child interaction were significant under the control of the general background of the study subjects. While the main effects of empathy and social support were controlled, the moderating effect of social support, a moderating variable, was significant. In other words, it was confirmed that social support perceived by teachers moderates the relationship between empathy and teacher-child interaction. Based on the results, ways to improve teachers’ interaction ability were discussed. 본 연구의 목적은 영유아교사의 공감능력과 교사-유아 상호작용의 관계에서 사회적 지지의 조절효과를 검증하는 것이다. 이를 통해 교사의 상호작용 능력을 효과적으로 향상시킬 수 있는 방안을 모색하고자 하였다. 이러한 연구목적을 달성하기 위해 전국의 유치원 및 어린이집에 재직 중인 교사 383명으로부터 자료를 수집하였다. 분석에 사용될 자료는 연구 참여에 동의하는 교사들에게 모바일로 질문지를 배부하여 수집하였다. 연구문제를 검증하기 위해 수집된 자료로 조절회귀분석을 실시하였다. 연구결과는 다음과 같다. 연구대상의 일반적 배경을 통제한 상태에서 교사-유아 상호작용에 대한 공감능력과 사회적 지지의 주효과가 유의하였다. 공감능력과 사회적 지지의 주효과를 통제한 상태에서 조절변인인 사회적 지지의 조절효과가 유의하였다. 즉, 교사가 지각한 사회적 지지는 공감능력과 교사-유아 상호작용의 관계를 조절하는 것으로 확인되었다. 연구결과에 근거해서 교사의 상호작용 능력을 향상시킬 수 있는 방안에 대하여 논의하였다.
