임상시험 연구나 역학 연구에서 환자들의 예후는 흔히 생존을 추정을 통해 수량화 되곤 한다. 하지만 코호트 분석이나 완전분석에 의한 생존율 추정량들은 수년 전에 진단된 환자에 크게 의존하기 때문에 실제 생존율보다 더 낮게 추정하곤 한다. 본 연구에서는 최근의 생존정보를 잘 반영하는 생존을 추정을 위해 기간분석 방법을 통한누적한계 추정량을 제안하였고, 그 방법을 1993년 1월-1997년 12월 사이에 조사된 서울시 암등록 자료(Ahn등, 2002)에 적용하여 결과를 고찰하였다. Long-term survival rates are the most commonly used outcome measures for patients with cancer. However, traditional long-term survival statistics, which are derived by cohort analysis or complete analysis, essentially reflect the survival expectations of patients diagnosed many years ago. They are often outdated at the time they become available. In this article, we propose a modified product-limit method to obtain up-to-date estimates of long-term survival rates via a period analysis. The proposed method is illustrated with cancer registry data collected from January 1993 to December 1997.

A chemical investigation of the fruits of Piper retrofractum (Piperaceae) has led to the isolation and characterization of a novel piperidine alkaloid, piperoctadecalidine together with three known alkaloids piperine, pipernonaline and guineensine. The structure of the new compound was detemined to be (2E,4E,14Z)-N-(2,4,14-Octadecatrienoyl) piperidine by spectral and synthetic methods.

Pulmonary embolism (PE) is usually associated with deep vein thrombosis (DVT) in the lower extremities. However, foreign bodies in the pulmonary arteries can cause PE, rarely. In this report, we present a case of diffuse PE which was associated with a migrated thread-like structure of the right side heart and pulmonary arteries in a 70 year-old woman. The patient was admitted to the emergency room with a progressive dyspnea for about 2 weeks. On physical examination, there was no evidence of unilateral leg edema. The chest radiography showed high density in T6, T11 and L2 vertebrae suggesting previous percutaneous vertebroplasty (PV). The transthoracic echocardiogram showed about 6cm long hyperechogenic thread like structure, going from the right ventricle (RV) to main pulmonary artery (fig. 1). Multidetector computerized tomography (MDCT) scan demonstrated a long radiopaque thread like structure in the RV, RV out. ow tract, main pulmonary trunk, right main pulmonary artery and several obstructed pulmonary arteries, pulmonary trunk associated with thread like structure (fig. 2). The patient underwent several episodes of PV about 2 years ago to treat compression fractures of vertebral body. Based on patient`s past history and interpretation of the MDCT scan, we identified that the thread-like foreign body was the bone cement injected during the previous PV procedure. After being treated with anticoagulation therapy, the patient`s symptoms were relieved, and follow-up CT scan revealed decreased extent of thrombus in the pulmonary arteries. We present this case study to emphasize that clinicians should consider the possibility of PE as a late complication, especially those with history of PV.

Seventeen dogs were treated with L-ornithin-L-aspartate (LOLA; experimental group). Three dogs were treated with lactulose recognized therapy (control group). Following LOLA administration, 15 dogs experienced a significant decrease in ammonia level (p < 0.05) and showed clinical signs of improvement. However, there were no clinical signs of improvement in two dogs, even though the ammonia level decreased. Conversely, the clinical signs of the control group also improved and the ammonia level decreased, although these changes were not significant (p > 0.05). These results suggest that LOLA is an effective drug to treat hyperammonemia in veterinary medicine.
Elevation of Serum APE1/Ref-1 in Experimental Murine Myocarditis

<P>Myocarditis is an inflammatory disease of the myocardium that causes cardiogenic shock and death. However, endomyocardial biopsy that is, the gold standard for a diagnosis is limited. Apurinic/apyrimidinic endonuclease 1/redox effector factor-1 (APE1/Ref-1) is a multifunctional protein, which is involved in DNA-based excision repair pathway, and in redox signaling, its changes are observed in various cardiovascular diseases including hypertension and coronary artery disease. We analyzed serum APE1/Ref-1 in experimental murine myocarditis. To induce myocarditis, coxsackievirus B3 was injected intraperitoneally to BALB/c mice. The serum APE1/Ref-1, N-terminal pro-B-type natriuretic peptide (NT-proBNP) and troponin I were measured. The histology and virus titers measurements were performed. The troponin I and inflammation were significantly elevated at day 3, peaked to day 7 and decreased at day 10. The NT-proBNP and virus titers were significantly peaked at day 3, and dropped at day 7 and 10. The serum APE1/Ref-1 was gradually raised and its elevation is still maintained until a later time, namely day 10. Also, its level was positively correlated with myocardial inflammation, reflecting severity of myocardial injury. We suggest that serum APE1/Ref-1 can be used to assess for myocardial injury in viral myocarditis without endomyocardial biopsy.</P>
