Intradural extramedullary tumors grow slowly and can have various clinical manifestations depending on their location in the spine. They often mimic other spinal disorders thus delaying the diagnosis. These tumors are usually histologically benign and can be resected with complete extirpation and have excellent prognosis. We report a case of intradural extramedullary schwannoma which was initially diagnosed as spondylolisthesis of the lumbar spine with review of pertinent literature.
흉추와 요추부 방출성 골절의 후방 척추경 나사못 고정술 후 후만각도의 변화 -나사못 고정 범위에 따른 비교-

목적: 흉추와 요추부 방출성 골절에서 척추경 나사못을 이용한 후방 고정술 및 유합술 후 손상된 종판이 위치한 분절의 안정성과 나사못 고정 범위와의 관계를 확인하고자 하였다. 대상 및 방법: 흉추 또는 요추부 방출성 골절 환자에서 척추경 나사못을 이용한 후방 고정술 및 유합술을 시행한 후 6개월간 추시가 가능했던 41명의 환자를 대상으로 후향적 방법으로 연구하였다. 나사못 고정 범위에 따라 골절된 종판 방향으로 척추경 나사못을 1부위만 고정한 16명 (1부위 군)과 2부위를 고정한 25명 (2부위 군)의 두 그룹으로 나누어 수술 직후와 수술 후 6개월째 추시에서 측면 방사선상 후만 변형 각도를 측정하여 비교하였다. 결과: 1부위 군에서는 평균 17.5±2.4도 (범위: 7.2∼32.7도)의 후만 변형을 보였으나, 2부위 군에서는 평균 5.2±0.8도 (범위: 2.1∼7.4도)의 후만 변형을 보여 양 군 간 유의한 차이가 있었다 (p=0.000). 결론: 흉추와 요추부 방출성 골절에서 척추경 나사못을 이용한 후방 고정술 및 유합술을 시행하는 경우 술 후 후만 변형을 방지하기 위해서는 손상된 종판 방향으로 2부위의 나사못을 고정하는 것이 필요할 것으로 생각한다. Purpose: To evaluate the relationship between the level of screw fixation and the stability of the segment of endplate fracture after posterior pedicle screw instrumentation for thoracic and lumbar burst fractures. Materials and Methods: The 41 patients of burst fractures who had been operated with pedicle screw instrumentation were retrospectively evaluated. The patients were divided into two groups by the levels of screw fixation. One group was treated with screws fixed by one-level to the direction of fractured endplate (One-level group, 16 cases). The other group was treated with screws fixed by two-level to the direction of endplate fracture (Two-level group, 25 cases). The two groups were compared by the radiographic changes of kyphotic angle between the day of surgery and 6 months after surgery. Results: At the 6 months, one-level group showed the change of kyphotic angle of 17.5±2.4 degrees, which was different from two-level group of 5.2±0.8 degrees (p=0.000). Conclusion: In posterior pedicle screws fixation for thoracic and lumbar burst fractures, 2 vertebrae to the direction of the endplate fracture should be included to prevent the postoperative kyphotic change.

It is very rare that the bone scan after 72 hours from the trauma doesn`t exhibit the increased radio-nuclide uptake in the patient with fracture. The purpose of this study is to report the case that indicate the false negative finding in the bone scan performed after 78 hours from the trauma in the 56-year-old man with L2 compression fracture, including a review of the relevant literatures.

<P>UDP-glucuronosyltransferase (UGT)–mediated drug–drug interactions are commonly evaluated during drug development. We present a validated method for the simultaneous evaluation of drug-mediated inhibition of six major UGT isoforms, developed in human liver microsomes through the use of pooled specific UGT probe substrates (cocktail assay) and rapid liquid chromatography–tandem mass spectrometry (LC-MS/MS) analysis. The six probe substrates used in this assay were estradiol (UGT1A1), chenodeoxycholic acid (UGT1A3), trifluoperazine (UGT1A4), 4-hydroxyindole (UGT1A6), propofol (UGT1A9), and naloxone (UGT2B7). In a cocktail incubation, UGT1A1, UGT1A9, and UGT2B7 activities were substantially inhibited by other substrates. This interference could be eliminated by dividing substrates into two incubations: one containing estradiol, trifluoperazine, and 4-hydroxyindole, and the other containing chenodeoxycholic acid, propofol, and naloxone. Incubation mixtures were pooled for the simultaneous analysis of glucuronyl conjugates in a single LC-MS/MS run. The optimized cocktail method was further validated against single-probe substrate assays using compounds known to inhibit UGTs. The degree of inhibition of UGT isoform activities by such known inhibitors in this cocktail assay was not substantially different from that in single-probe assays. This six-isoform cocktail assay may be very useful in assessing the UGT-based drug-interaction potential of candidates in a drug-discovery setting.</P>

<OL><LI><P>Ilaprazole is a new proton pump inhibitor, designed for treatment of gastric ulcers, and developed by Il-Yang Pharmaceutical Co (Seoul, Korea). It is extensively metabolised to the major metabolite ilaprazole sulfone.</P></LI><LI><P>In the present study, several <I>in vitro</I> approaches were used to identify the cytochrome P450 (CYP) enzymes responsible for ilaprazole sulfone formation. Concentrations of ilaprazole sulfone were determined by liquid chromatography-tandem mass spectrometry (LC-MS/MS).</P></LI><LI><P>Incubation of ilaprazole with cDNA-expressed recombinant CYPs indicated that CYP3A was the major enzyme that catalyses ilaprozole to ilaprazole sulfone. This reaction was inhibited significantly by ketoconazole, a CYP3A inhibitor, and azamulin, a mechanism-based inhibitor of CYP3A, while no substantial effect was observed using selective inhibitors for eight other P450s (CYP1A2, CYP2A6, CYP2B6, CYP2C8, CYP2C9, CYP2C19, CYP2D6, and CYP2E1).</P></LI><LI><P>In addition, the formation of ilaprazole sulfone correlated well with CYP3A-catalysed testosterone 6β-hydroxylation and midazolam 1′-hydroxylation in 20 different human liver microsome panels. The intrinsic clearance of the formation of ilaprazole sulfone by CYP3A4 was 16-fold higher than that by CYP3A5.</P></LI><LI><P>Collectively, these results indicate that the formation of the major metabolite of ilaprazole, ilaprazole sulfone, is predominantly catalysed by CYP3A4/5.</P></LI></OL>

We have reviewed 15 cases of tibial pilon fractures who have been treated operatively from May, 1990 to May, 1995 at our hospital. 1. The fractures were classified into Type I, II and III according to the system of Mast, Spiegel and Pappas and Type III were subdivided into Type A, B and C according to the system of Ruedi- Allgower. Type I was 1 case(6.7%). Type II were 8(53.3%), Type IIIA were 2(l3.3%), Type IIIB were 3(20%) and Type IIIC was 1 case(6.7%). 2. Type I, II and IIIA were treated with open reduction and internal fixation using screw or plate. 2 cases of Type IIIB were treated with open reduction and external fixation and 1 case was treated with piate. Type IIIC was treated with open reduction and external fixation. 3. Conclusively, we had good results with open reduction and internal fixation in Type I, lI and IIIA. In Type lll8, IIIC, if it was difficult to maintain of reduction with internal fixation due to severe bone loss, comminution and soft tissue injury, we could have goad results with open reduction, bone graft and external fixation including ankle joint.

Purpose : The purpose of this article is to show the efficacy of a cervical epidural steroid injection for cervical disease. Materials and Methods : Twenty-five patients who underwent an cervical epidural steroid infection for subacute cervical sprain. cervical HNP(herniated nucleus pulposus) and degenerative cervical spondylosis from Jan. 1998 to May 1999 were enrolled into the study. The cases were analyzed by verbal analogue percentage scale at 1 months, 6 months and 2 years later. Results : In this study, excellent or good result was shown in 15 patients(60%) one month later, in 10 patients(40%) sixth months later and in 9 patients(36%) 2 years later. Complication was shown in one patients with mild headache. Conclusion : We concluded that the cervical epidural steroid injection was the appropriate method for inflammatory reaction of cervical radiculopathy but not for the mechanical problem.
