Hereditary spherocytosis (HS) is the most common inherited red cell membrane disorder. Its main laboratory finding is anemia with reticulocytosis. However, in the case of an aplastic crisis, there may be no reticulocytosis, making the diagnosis of HS difficult. We present the case of a 4-year-old boy who initially presented with persistent fever and sore throat. His 8-year old brother also had anemia of unknown etiology, and his father had a history of splenectomy in his 20s. Physical examination revealed anemic conjunctivae and hepatosplenomegaly, and laboratory findings showed anemia with decreased reticulocyte count and elevated ferritin and lactate dehydrogenase levels. A peripheral blood smear showed microcytic hypochromic anemia with severe poikilocytosis (spherocytes, acanthocytes, schistocytes), and bone marrow examination revealed decreased erythroid cells and increased hemophagocytosis. Increased osmotic fragility was observed, and parvovirus B19 was detected using polymerase chain reaction. Hence, we established the diagnosis of hereditary spherocytosis manifested as an aplastic crisis caused by parvovirus B19 infection.
A Case with Multiple Fungal Coinfections in a Patient who Presented with Pancoast Syndrome

소아암 환자에서 발생하는 침습성 진균 감염은 사망과 후유증에 이르는 중대한 감염이다. 18세 남자 환자가 호중구감소 기간 동안 입원하여 치료받던 중 우측 견관절과 우측 팔에 감각이상과 신경쇠약을 호소하였고, 우측폐상엽의 폐렴이 진단되었다. 기관지의 조직학적 소견과 폐 수술검체에서 시행한 polymerase chain reaction (PCR)로 털곰팡이증을 확진하였으며, 흉수액의 PCR로 페니실리움 디쿰벤스 감염, 갈락토마난 항원법으로 아스페르길루스증을 추정하였다. 환자는 백혈병이 치료되지 못하고 Staphylococcus epidermidis 패혈증이 합병되어 사망하였다. 본 증례에서는 판코스트 증후군의 증상을 보인 환자에서 진단된 다발성 폐진균증을 보고 하는 바이다. Invasive fungal infection (IFI) is a serious threat to pediatric patients with cancer given high morbidity and mortality. We present an 18-year-old male with precursor T-cell lymphoblastic leukemia who developed Pancoast syndrome, presented with paresthesia and numbness in the right shoulder and arm during a neutropenic fever period. He was diagnosed with pneumonia in the right upper lung field. He was later found to have an invasive pulmonary fungal infection caused by multiple fungi species, including Rhizomucor, confirmed by histology and polymerase chain reaction (PCR) (proven infection), Penicillium decumbens diagnosed by PCR, and Aspergillus suspected from galactomannan assay (probable infection). Unfortunately, the patient's condition further worsened owing to the aggravation of leukemia, chemotherapy-induced neutropenia, and bacterial coinfection, leading to multiorgan failure and death. Here, we report a case of IFI caused by multiple fungal species that presented as Pancoast syndrome.

Drug-induced immune hemolytic anemia (DIIHA) is a rare side effect of drugs. DIIHA may cause a systemic inflammatory response that results in acute multi-organ failure and death. Ceftizoxime belongs to the class of third generation cephalosporins, which are the most common drugs associated with DIIHA. Herein, we present a case of a 66-year-old man who developed fatal DIIHA after receiving a second dose of ceftizoxime. He was admitted to receive photodynamic therapy. He had a history of a single parenteral dose of ceftizoxime 3 months prior to admission. On the day of the procedure ― shortly after the infusion of ceftizoxime ― the patient`s mental status was altered. The blood test results revealed hemolysis. Oliguric acute kidney injury developed, and continuous renal replacement therapy had to be applied. On the suspicion of DIIHA, the patient underwent plasmapheresis. Diagnosis was confirmed by a detection of drug-dependent antibody with immune complex formation. Although his hemolysis improved, his liver failure did not improve. He was eventually discharged to palliative care, and subsequently died.
하천 및 호소 퇴적물 입도분석 방식의 비교와 입도에 따른 중금속물질의 분포경향

한강, 낙동강, 영산강, 금강의 하천 및 호소퇴적물을 대상으로 건식체질법, 습식체질법, 광산란법(PSA) 등의 입도분석법을 비교연구 하였다. 건식체질법의 경우 건조과정에서 발생하는 엉김현상에 의해 오차가 크게 발생하며 이러한 현상은 호소시료의 경우 두드러지게 나타났다. 습식체질법은 건식체질법에 비해 복잡하고 노동력이 보다 많이 필요하지만 정확한 분석이 가능하며 PSA의 경우에도 습식체질과 유사한 경향을 나타내었다. 엉김현상을 방지하기 위한 동결건조 및 과산화수소수에 의한 유기물의 산화/동결건조의 경우 엉김현상을 일부 개선할 수 있었으나 여전히 습식체질에 비하여 미세입자의 분율이 적게 나타났다. 입도별 중금속의 용출량 및 함유량이 조사되었으며 예상된 바와 같이 호소 및 하천시료 모두 미세입자에서 높은 농도의 중금속이 용출되었다. 퇴적물의 관리에 있어서 정확한 입도분석법의 사용과 미세입자에 대한 관리가 보다 중요하다. Dry sieving, wet sieving and photoscattering analyzer were tested as particle size analyzing methods for sediments from rivers and lakes of Han river, Nakdong river, Youngsan river, and Kumgang river area. Dry sieving showed a big error due to coagulating effect over drying process and this phenomena was severe for lake sediment. Wet sieving and PSA showed an accurate results though wet sieving needs more labor and complicate processes. Freeze-dry or freeze-dry after oxidation of organics with hydrogen peroxide showed improved results but gave lower portion of fine particles in comparing with wet sieving. Heavy metal contents and extractable metal contents were investigated for the sediments and high heavy metal content and extractable amount were obtained from fine particles as expected. Using of proper particle size analyzing method is important and the sediment management should be focused on the fine particles.

Background: As the need for expansion of the external quality assessment (EQA) scheme for transfusion medicine in Korea has increased, we performed a pilot project of extended antigen typing for Lewis, Duffy, and Kidd antigens. Methods: We sent EQA materials to participant laboratories for Lewis, Duffy, and Kidd antigen typing in October 2020. Participants were asked to enter the agglutination titers, methodologies, and results of Lewis, Duffy, and Kidd antigen typing on the Korean Association of External Quality Assessment Service website. Results: For Lewis antigen typing, the intended result was Le(a–b–); 20 participants (95.2%) responded as Le(a–b–), and only one (4.8%) responded as Le(a–b+). For the Duffy antigen typing test, the intended result was Fy(a+b–), and all 20 participants (100%) responded as Fy(a+b–). For the Kidd antigen typing test, the intended result was Jk(a+b+), and all 22 participants (100%) responded as Jk(a+b+). Conclusions: As the pilot project was successfully executed, extended antigen typing for Lewis, Duffy, and Kidd antigens has been included in the Korean Association of External Quality Assessment Service on transfusion medicine since 2021.

Background: Many test items in transfusion medicine that are still not included in the Korean Association of External Quality Assessment Service. We performed a pilot project of anti-D titer test for the expansion of proficiency testing program in transfusion medicine. Methods: Proficiency testing materials with low (BBS2-21-01) and high (BBS2-21-02) anti-D titer were sent to 53 laboratories that agreed to participate in the pilot project for the 2nd Korean Association of External Quality Assessment Service on Transfusion Medicine in 2021. The results were entered on the website of the Korean Association of External Quality Assessment Service (http://eqas.keqas.org/). Results: A total of 47 laboratories submitted the results. For BBS2-21-01, the intended response of anti-D titer was 4–16 for tube technique with antihuman globulin (AHG) and 16–64 for column agglutination technique (CAT) with AHG. Meanwhile, for BBS2-21-02, the intended response was 8–32 for tube technique with AHG and 32–128 for CAT with AHG. The proportion of acceptable results was 90.9%–95.5% for tube technique with AHG and 95.8%–100.0% for CAT with AHG. Conclusions: The pilot project of anti-D titer test was successfully performed. This project will be expanded to a biannual proficiency testing program for anti-D antibody titer in the Korean Association of External Quality Assessment Service starting in 2022.

This report summarizes the 2019 survey results of the external quality assessment (EQA) scheme for the Transfusion Medicine Program (TMP) in Korea. Proficiency testing materials were prepared at the Asan Medical Center for the biannual distribution to participating laboratories. The average accuracy rates and number of participants (in parenthesis) for ten survey items were as follows: ABO typing, 99.4%–99.9% (N=875); RhD typing, 99.8%–100% (N=864); crossmatching, 90.8%–99.6% (N=760); ABO subtyping, 98.2% and 100% (N=58); Rh CcEe antigen testing, both 100% (N=55); weak D test, 97.9%–100% (N=232); antibody screening, 99.7%– 100% (N=316); direct antiglobulin test (DAT) using a polyspecific reagent, 99.6%–100% (N=273); DAT using an immunoglobulin-G monospecific reagent, both 100.0% (N=67); DAT using a C3d monospecific reagent, 95.6%–98.5% (N=67); antibody identification, 87.9%–99.2% (N=132); and ABO Ab titration, 85.7%–100% (N=134). The number of participants showed an average increase of 14% across the ten survey items, with the ABO antibody titration showing the highest increase at 83.6%. While results were generally excellent, antibody identification and ABO antibody titration results showed room for improvement. The 2019 EQA scheme for TMP has contributed to the improvement and maintenance of the participating laboratories to the program.
