Carcinoma ex pleomorphic adenoma is a rare malignant salivary gland tumor. The carcinomatous component of the carcinoma ex pleomorphic adenoma is mostly one type such as adenocarcinoma NOS, salivary duct carcinoma and undifferentiated carcinoma. We present a case of carcinoma ex pleomorphic adenoma including two carcinomatous components. The tumor occurred in the palate of a 70-year-old man. Histopathologically, the tumor was composed of both benign pleomorphic adenoma and the carcinoma area that showed adenocarcinoma NOS and squamous cell carcinoma. Finally this case was diagnosed as carcinoma ex pleomorphic adenoma including two carcinomatous components.

A microscopic magnetic phase diagram as functions of temperature and magnetic .eld was mapped for multiferroic compounds of RMn2O5 (R = Er, Ho) by using neutron diraction mea- surements under high magnetic .eld. The unambiguous relation between the ferroelectric phase transition and the magnetic phase transition from a two-dimensional incommensurate to a com- mensurate phase, where both the transitions are concomitantly induced by the magnetic .eld, indicates that the electric polarization is magnetically induced and the commensurate spin state is important for stabilizing the ferroelectricity in the multiferroic RMn2O5 system. The origin of the .eld-induced magnetic phase transition is discussed based on the magnetic structure. j_si

Training for radiation protection and control requires a visual understanding of radiation, which cannot be perceived by the human senses. Trainees must also master the effective use of measuring instruments. Traditionally, such training has exposed trainees to radiation sources. Here, we present a novel e-training strategy that enables safe, exposure-free handling of a radiation measuring tool called a survey meter. Our mixed reality radiation-training system merges the physical world with a digital one. Collaborating with a mixed reality headset (HoloLens 2), this system constructs a mock-up of a survey meter in real-world space. The HoloLens 2 employs a browser-based application to visualize radiation and to simulate/share the use of the survey meter, including its physical movements. To provide a dynamic learning experience, the system adjusts the surveymeter mock-up readings according to the operator’s movements, distance from the radiation source, the response time of survey meter, and shielding levels. Through this approach, we expect that trainees will acquire practical skills in interpreting survey-meter readings and gain a visual understanding of radiation in real-world situations.
Effectiveness and Approach of Rehabilitation in Patients With Acute Heart Failure: A Review

Acute heart failure is associated with high mortality and frequent rehospitalization, resulting in enormous healthcare costs and declining physical function, activities of daily living, and quality of life. Cardiac rehabilitation has been recommended as one of the non-pharmacologic treatments for patients with heart failure. However, much of the evidence for cardiac rehabilitation interventions reported to date has been limited to chronic heart failure. In recent years, the effectiveness of rehabilitation intervention in patients with acute heart failure has been reported, led by the Rehabilitation Therapy in Older Acute Heart Failure Patients (REHAB-HF) trial. This review overviews the recent evidence of rehabilitation in patients with acute heart failure.

ObjectiveaaElectroconvulsive therapy (ECT) is a reasonable option for intractable depression or schizophrenia, but a mechanism of action has not been established. One credible hypothesis is related to neural plasticity. Three genes (Oct4, Sox2, c-Myc) involved in the induction of induced pluripotent stem (iPS) cells are Wnt-target genes, which constitute a key gene group involved in neural plasticity through the TCF family. Klf4 is the other gene among Yamanaka’s four transcription factors, and increases in its expression are induced by stimulation of the canonical Wnt pathway. MethodsaaWe compared the peripheral blood gene expression of the four iPS genes (Oct4, Sox2, c-Myc, and Klf4) before and after modified ECT (specifically ECT with general anesthesia) of patients with intractable depression (n=6) or schizophrenia (n=6). Using Thymatron ten times the total bilateral electrical stimulation was evoked. ResultsaaBoth assessments of the symptoms demonstrated significant improvement after mECT stimulation. Expression of all four genes was confirmed to increase after initial stimulation. The gene expression levels after treatment were significantly different from the initial gene expression in all twelve cases at the following treatment stages: at the 3rd mECT for Oct4; at the 6th and 10th mECT for Sox2; and at the 3rd, 6th and 10th mECT for c-Myc. ConclusionaaThese significant differences were not present after correction for multiple testing; however, our data have the potential to explain the molecular mechanisms of mECT from a unique perspective. Further studie should be conducted to clarify the pathophysiological involvement of iPS-inducing genes in ECT.
