        전문대학 현장실습 영향요인의 상대적 중요도와 만족도에 관한 연구 -비서학 전공을 중심으로-

        본 연구는 전문대학 비서학 전공의 현장실습 영향요인의 상대적 중요도와 만족도를 밝히고자 전문대학 비서학 전공생(12개 대학, 184명)을 대상으로 수행되었다. 현장실습 영향요인으로 학생요인(책임감, 적극적 태도, 전공 몰입 및 만족도, 전공자신감, 산업체에 대한 자료조사), 학교요인(학교의 사전교육, 담당교수의 정기적 방문과 관심, 실습수업 충실도), 기관요인(실습환경, 실습기간, 실습비 지원, 점심제공, 근무시간, 체계적 프로그램, 의미 있는 근무, 실습지도자의 관심, 지도 및 평가공정성, 업체 분위기) 등 총 20개 항목을 선정하였다. 연구 결과는 다음과 같다. 첫째, 비서학 전공 현장실습생들이 지각하는 영향요인의 중요도와 만족도는 적절한 근무시간, 적극적으로 배우려는 태도, 업체의 분위기와 직원과의 원만한 관계 순으로 높게 나타났고, 담당교수의 정기적 방문, 산업체에 대한 사전자료 수집 및 이해 등은 낮게 나타났다. 둘째, 현장실습 영향요인의 IPA분석 결과 실습환경, 실습비 지원, 점심식사 제공, 체계적인 프로그램 등이 집중개선영역으로 나타나 이에 대한 개선책 마련이 요구된다. 본 연구는 현장실습의 효과성을 높이기 위해 현장실습 영향요인의 중요도와 만족도를 분석함으로써 비서와 사무직에 적합한 현장실습 환경구축에 대한 개선방향을 제시했다는 점에서 실무적으로 의미가 있다. For college students majoring in secretarial studies, the opportunity of field practice can be described as a training course to establish an identity as a professional secretary as well as learn practical skills and work-based experience required in the field. Nevertheless, studies on the field practice of the secretary major are limited, and related research on the importance and satisfaction of field practice-influencing factors has been rare. Therefore, this study targeted 184 college students, majoring in secretarial studies located in Seoul, Gyeonggi, and Incheon, to investigate the factors influencing field practice and to administer comparative analysis of the importance and satisfaction level. The statistical package of SPSS WIN 18.0 was used to analyze the collected data. After a theoretical review, the factors influencing field practice were derived from 20 factors regarding the student, college, and company aspects through reliability, validity, and factor analysis. IPA(importance-performance analysis) showed that proper working hours, proactive attitude for field practice, and the atmosphere and relationships in the company were important factors influencing field practice. On the other hand, prior orientation, professor’s field visiting, and professor’s interest were not important factors influencing field practice. In particular, it is necessary to design a working environment, training fee, and systematic program in detail between the training company and college.

        Clinical features and treatment outcomes of Hodgkin lymphoma: A retrospective review in a Malaysian tertiary hospital

        BackgroundClassical Hodgkin lymphoma (cHL) is a clinicopathologically unique, aggressive lympho-ma arising from germinal center B-cells and is one of the most curable hematological malignancies. This study aimed to determine the clinical course, treatment regimens, re-sponse rates, and survival data of patients diagnosed with cHL in a tertiary center.MethodsA retrospective review was conducted to include patients with a diagnosis of cHL from 2013 to 2017. Data of demographic and clinical characteristics, treatment regimens, and outcomes were collected and analyzed. ResultsWe recruited 94 patients with a median age of 27.0 [interquartile range (IQR), 12] years. Most of the patients were male (61.7%) and 73.4% were ethnic Malay. Nodular sclerosis was the most common histology (77.6%), followed by mixed cellularity (6.4%) and others (16%). The median follow-up time was 28.0 (IQR, 32) months. All patients received che-motherapy but only 13.8% received radiotherapy as consolidation. The doxor-ubicin-bleomycin-vinblastine-dacarbazine regimen was the most common (85.1%), fol-lowed by the escalated bleomycin-etoposide-doxorubicin-cyclophosphamide-vincristine- prednisolone-procarbazine regimen (14.9%). Following treatment, 76.1% of patients achieved complete response. The 2-year overall survival (OS) and progression-free sur-vival (PFS) of the entire cohort were 96.5% and 71.1%, respectively. The 2-year OS and PFS for advanced-stage disease were 93.9% and 62.8%, compared to 100% and 82.7% for early-stage disease, respectively (P=0.252 and P=0.052, respectively).ConclusionThis study provides insight into the clinical presentation and treatment outcomes among patients with cHL in Malaysia. A longer study duration is required to identify OS and PFS benefits and treatment-related complications for different chemotherapeutic regimens.

        Clinical Research Article : A comparison of infraclavicular and supraclavicular approaches to the brachial plexus using neurostimulation

        Background: A prospective, double blind study was performed to compare the clinical effect of vertical infraclavicular and supraclavicular brachial plexus block using a nerve stimulator for upper limb surgery. Methods: One hundred patients receiving upper limb surgery under infraclavicular or supraclavicular brachial plexus block were enrolled in this study. The infraclavicular brachial plexus block was performed using the vertical technique with 30 ml of 0.5% ropivacaine. The supraclavicular brachial plexus block was performed using the plumb bob technique with 30 ml of 0.5% ropivacaine. The block performance-related pain was evaluated. This study observed which nerve type was stimulated, and scored the sensory and motor block. The quality of the block was assessed intra-operatively. The duration of the sensory and motor block as well as the complications were assessed. The patient`s satisfaction with the anesthetic technique was assessed after surgery. Results: There were no significant differences in the block performance-related pain, frequency of the stimulated nerve type, evolution of sensory and motor block quality, or the success of the block. There were no significant differences in the duration of the sensory and motor block. There was a significant difference in the incidence of Horner`s syndrome. Two patients had a pneumothorax in the supraclavicular approach. There were no significant differences in the patient`s satisfaction. Conclusions: Both infraclavicular and supraclavicular brachial plexus block had similar effects. The infraclavicular approach may be preferred to the supraclavicular approach when considering the complications. (Korean J Anesthesiol 2010; 58: 260-266)

      • THE INFLUENCE OF CUSTOMERS RESISTANCE INNOVATION TOWARDS CUSTOMERS ATTRITION BASED ON FSQCA: A CASE STUDY OF SMARTPHONE APP

        Introduction The study chooses smartphone applications which have update experience as the study case to analysis if it will cause customer attrition. Application updates aims to make up the bugs which may cause bad effects on using, while in fact, most changes will be resisted by customers. We adapt three elements to collect data about the attitude of customers toward applications update, the conclusion illustrates, customers will resist changes in some conditions and cause customers attrition. Therefore, application developers should focus on the personal emotional reaction and cognitive evolution as well as predict what may influence the customers normal use after the applications update to make correct update decisions. In order to define the relationship between variable and result., we take smartphone application which had experienced update as examples and choose fsQCA as the methodology to analysis. Literature Review Resistance to change is a psychological process, Brehm(1966)claims customers resist to change origins from their believe that they have rights to control free, when this rights be challenged, this resistance behavior will happen due to they want to protect their own rights. Some scholars distinguished the concept of resistance to change: when receptors in the low involvement or limited cognize, their resistance behaviors is likely to be considered as the passive resistance; otherwise, the behaviors would be recognized as active resistance (Nabih,1997). Bagozz and Lee(1999) think that functional barrier is an important factors which influence the degree of innovation receptance, when customers think products after update cannot meet their function needs will cause resistance to change. Resistance always occur passively, bemuse of the disturb of the former habits of using (Ram &Sheth 1989), and former studies also indicate that prejudice towards changes is one of significant factors. Because changes are uncertainty, Fiske and Taylor(1991)claims that uncertainty can make people hate changes and Whitson and Galinsky(2008)consider aversion will be the strong driver of resistance to change to reduce uncertainty and risk. In terms of a company, the cost of maintaining a old customer is much lower than these of develop a new user, Actually, customer attrition is response of entrepreneur revenue and the vital section in development of companies. The famous american honesty management scholar Frederic Reichheld’s research indicates that when the extension of the commercial relationship of entrepreneur and customers, companies can acquire fatter profit through the relationship. Conclusions This study chooses fsQCA as methodology to define the relationship between resistance to change and customer attrition instead of using structural equation modeling. Sometimes, even customers have right cognitive evolution towards changes, when they find their efficiency be influenced by it, they also resist to change; while adverse emotional reaction also has an adverse effect on customer retention though they build positive cognitive evolution about changes. there is a remain circumstance of lossing of customers that when customers hate changes and they find their efficiency be influenced by update, the honesty of this product will also decrease. This three condition all contribute to customers attrition which should be paid more attention by companies.

      • Shear Adhesion of Tapered Nanopillar Arrays

        <P>Tapered nanopillars with various cross sections, including cone-shaped, stepwise, and pencil-like structures (300 nm in diameter at the base of the pillars and 1.1 μm in height), are prepared from epoxy resin templated by nanoporous anodic aluminum oxide (AAO) membranes. The effect of pillar geometry on the shear adhesion behavior of these nanopillar arrays is investigated via sliding experiments in a nanoindentation system. In a previous study of arrays with the same geometry, it was shown that cone-shaped nanopillars exhibit the highest adhesion under normal loading while stepwise and pencil-like nanopillars exhibit lower normal adhesion strength due to significant deformation of the pillars that occurs with increasing indentation depth. Contrary to the previous studies, here, we show that pencil-like nanopillars exhibit the highest shear adhesion strength at all indentation depths among three types of nanopillar arrays and that the shear adhesion increases with greater indentation depth due to the higher bending stiffness and closer packing of the pencil-like nanopillar array. Finite element simulations are used to elucidate the deformation of the pillars during the sliding experiments and agree with the nanoindentation-based sliding measurements. The experiments and finite element simulations together demonstrate that the shape of the nanopillars plays a key role in shear adhesion and that the mechanism is quite different from that of adhesion under normal loading.</P> [FIG OMISSION]</BR>

        Pemetrexed Continuation Maintenance in Patients with Nonsquamous Non-small Cell Lung Cancer: Review of Two East Asian Trials in Reference to PARAMOUNT

        Purpose A recent phase III study (PARAMOUNT) demonstrated that pemetrexed continuation maintenance therapy is a new treatment paradigm for advanced nonsquamous non-small cell lung cancer (NSCLC). The majority of patients enrolled in PARAMOUNT were Caucasian (94%). We reviewed efficacy and safety data from two clinical trials, which enrolled East Asian (EA) patients, to supplement data from PARAMOUNT on pemetrexed continuation maintenance therapy in patients with nonsquamous NSCLC. Materials and Methods Study S110 was a phase II, multicenter, randomized, controlled, open-label trial in never-smoker, chemonaïve, EA patients (n=31) with locally advanced or metastatic nonsquamous NSCLC (n=27). Study JMII was a multicenter, open-label, single-arm, postmarketing, clinical trial in Japanese patients (n=109) with advanced nonsquamous NSCLC. PARAMOUNT was a multicenter, randomized, double-blind, placebo-controlled trial in patients with advanced nonsquamous NSCLC. Results In EA patients with nonsquamous NSCLC, the median progression-free survival (PFS) for pemetrexed continuation maintenance therapy was 4.04 months (95% confidence interval [CI], 3.22 to 5.29 months) in study S110 and 3.9 months (95% CI, 3.2 to 5.2 months) in study JMII. The median PFS for pemetrexed continuation maintenance therapy in PARAMOUNT was 4.1 months (95% CI, 3.2 to 4.6 months). Pemetrexed continuation maintenance therapy in EA patients in studies S110 and JMII did not lead to any unexpected safety events, and was consistent with PARAMOUNT’s safety profile. Conclusion The efficacy and safety data in the EA trials were similar to those in PARAMOUNT despite differences in patient populations and study designs. These data represent consistent evidence for pemetrexed continuation maintenance therapy in EA patients with advanced nonsquamous NSCLC.

      • Molecular Imaging of Colorectal Tumors by Targeting Colon Cancer Secreted Protein-2 (CCSP-2)

        <P>A versatile biomarker for detecting colonic adenoma and colon cancer has yet to be developed. Colon cancer secreted protein-2 (CCSP-2) is a protein specifically expressed and secreted in colon adenomas and cancers. We developed a fluorescent imaging method based on CCSP-2 targeting for a more sensitive and specific detection of colorectal tumors. CCSP-2 expression was evaluated in human colon adenoma and colorectal specimens. Anti–CCSP-2 antibody was labeled with a near-infrared fluorescent dye, FPR-675, and molecular imaging of surgical human colorectal tumors was performed. Immunohistochemistry identified CCSP-2 expression in 87.0% of colorectal cancer specimens and 89.5% of colon adenoma specimens. Fluorescence imaging of surgical human colon specimens after spraying treatment with the probe permitted a clear distinction of cancer from paired normal colon tissue (target-to-background ratio, 4.09 ± 0.42; <I>P</I> < .001). CCSP-2 targeting imaging was also evaluated in patient-derived colon cancer xenograft mouse and liver metastasis murine models. CCSP-2–positive colon cancer xenografts and liver metastases were visualized by near-infrared fluorescence imaging after intravenous injection of the probe, which showed significantly higher fluorescence. Our results show that CCSP-2 is a promising marker for colorectal tumor detection in clinical settings and that a CCSP-2–targeting molecular imaging strategy might improve the diagnosis of colorectal tumors in metastatic or recurrent cancers and aid in early colonoscopic detection of premalignant lesions.</P>
