        Qualitative study of the fractional order nonlinear chaotic model: electronic realization and secure data enhancement

        In this work, we explore the superposition of two well-known chaotic oscillators, namely, the Duffing double-well and the forced van der Pol with the fractional order derivative. The proportional fractional derivative has been taken for numerical simulations and highly chaotic solution to improve some information of security systems has been found. The existence and the uniqueness of a super system are stated in the form of theorems using the Lipschitz condition locally. The qualitative properties of chaotic dynamics are described by mean of Lyapunov exponent (LE), eigenvalues, bifurcation and Poincaré maps. The analog circuit is also intended, with the help of different physical instruments, to validate the superposition of chaotic systems. The randomness level of a superposition chaotic system is tested via standard test suite, and the qualified set of a 32-bit array with high haphazardness is used for encryption as well as decryption. Furthermore, a security analysis is performed using different parameters, such as the uncertainty, similarity etc. The outcomes for the properties, time evolution, phase portrait, and oscilloscopic views are presented in tabulated and graphical forms.

        Defining the hemodynamic response of hypertensive and normotensive subjects through serial timed blood pressure readings in the clinic

        Background: Every third patient in the clinic is misdiagnosed due to white-coat phenomenon, necessitating needless and costly treatment. We aimed to study the hemodynamic response of the physician’s visit on hypertensive and normotensive patients by investigating the trend of blood pressure (BP) before, during and 15 min after the physician-patient encounter. Methods: A descriptive, cross-sectional study was conducted over a period of 8 months in the cardiology clinics at the Aga Khan University Hospital, Karachi. Both hypertensive and normotensive patients, aged ≥18 years, were recruited. Pregnant females or those with a history of volume loss were excluded. BP readings were taken using an automated, validated device (Omron-HEM7221-E) at three points: pre-clinic BP by the assessment nurse, in-clinic BP by the attending physician and post-clinic BP 15-min after the physician-patient encounter by a research assistant. Independent samples t-test was used to calculate the statistical difference between hypertensive and normotensive BP values. Results: Of 180 participants, 71% (n = 128) were hypertensive and 57% (n = 103) of all were males. The mean age of the participants was 57 ± 15 years. The mean and standard deviation(±SD) systolic BP (SBP) taken pre-clinic, inclinic and 15-min post-clinic for hypertensive population was 128.7 ± 20 mmHg, 137.1 ± 21 mmHg and 127.9 ± 19 mmHg. The mean and standard deviation(±SD) SBP taken pre-clinic, in-clinic and 15 min post-clinic for normotensive population was 112 ± 16 mmHg, 115.8 ± 20 mmHg and 111.8 ± 15 mmHg. The hypertensive SBP values showed statistically significant difference from the normotensive values (difference in pre-clinic SBP: 16. 7 mmHg, p-value < 0.001; in-clinic SBP: 21.3 mmHg, p-value < 0.001; and 15 min post-clinic: 16.1 mmHg, p-value < 0.001). Conclusions: Hypertensive and normotensive patients display congruent hemodynamics upon visiting the physician, the alert response being accentuated amongst the hypertensive group. In-clinic BP readings are higher for both hypertensive and normotensive patients making them unreliable for screening and management of hypertension amongst both the groups.

      • Reinforced fibrous recycled aggregate concrete element subjected to uniaxial tensile loading

        In this study, effect of recycled aggregates and polypropylene fibers on the response of conventionally reinforced concrete element subjected to tensile loading in terms of tension stiffening and strain development was experimentally investigated. For this purpose, concrete prisms of 100 × 100 mm cross section and 500 mm length having one central deformed steel re-bar were cast using fibrous and non-fibrous Recycled Aggregate Concrete (RAC) with varying percentages of recycled aggregates (0%, 25%, 50%, 75% and 100%) and tested under uniaxial tensile load. For all fibrous RAC mixes, polypropylene fibers were used at constant dosage of 3.15 kg/㎥. Effect of recycled aggregates and fibers on the compressive strength of concrete was also explored in this study. Through studying tensile load versus global axial deformation of composite and strain development in concrete and steel, it was found that replacement of natural aggregates with recycled aggregates in concrete negatively affected the cracking load, tension stiffening and strain development, and this negative effect was observed to be increased with increasing contents of recycled aggregates in concrete. The results of this study showed that it was possible to minimize the negative effect of recycled aggregates in concrete by the addition of polypropylene fibers. Reinforced concrete element constructed using concrete containing 50% recycled aggregates and polypropylene fibers exhibited cracking behavior, tension stiffening and strain development response almost similar to that of concrete element constructed using natural aggregate concrete without fiber.

        Treatment of Pantothenate-Kinase Neurodegeneration With Baclofen, Botulinum Toxin, and Deferiprone: A Case Report

        Pantothenate kinase-associated neurodegeneration (PKAN) is a rare autosomal recessive disorder characterized by progressive motor symptoms, such as dystonia and spasticity. Classical PKAN is the most common subtype of neurodegeneration with brain iron accumulation (NBIA). Currently, there is no established treatment for PKAN. However, baclofen and botulinum toxin have been reported to improve motor symptoms and ease care in these patients. Additionally, Deferiprone is a well-tolerated iron chelator that has been shown to be effective in reducing brain iron accumulation. In this case report, we present the case of a seven-year-old boy who presented to our ward with spastic gait and extrapyramidal signs. Brain magnetic resonance imaging was performed, which showed features of neurodegeneration secondary to brain iron accumulation with a specific appearance of the eye-of-the-tiger sign. Genetic testing was positive for a homozygous mutation in PANK2, and the diagnosis of early-stage classical PKAN was made. This case report highlights the potent efficacy of baclofen, botulinum toxin, and deferiprone in slowing down the disease progression at an early stage and improving the severity of symptoms.

        Borjeson-Forssman-Lehmann Syndrome: Clinical Features and Diagnostic Challenges

        Borjeson-Forssman-Lehmann syndrome (BFLS) is an X-linked recessive disorder resulting from mutations in the PHF6 gene. The syndrome is characterized by short stature, obesity, hypogonadism, hypotonia, intellectual disability, distinctive facial features, fleshy ears, and finger and toe abnormalities. However, the diagnostic challenge in identifying BFLS remains a topic of interest. In this case report, we present the clinical characteristics of a proband with BFLS, highlighting the additional features of hypotonia, intellectual disability, and distinctive facial features. While no definitive treatment exists for BFLS, patients benefit from specialized education and ongoing supervision from early childhood through adulthood. Symptomatic treatment, including close follow-up, may be necessary for complications such as seizures and hearing problems. Mastectomy or testosterone replacement therapy may be considered on a case-by-case basis. Genetic counseling for X-linkage should be offered to affected families.

      • Association of rs1042522 Polymorphism with Increased Risk of Prostate Adenocarcinoma in the Pakistani Population and its HuGE Review

        Prostate adenocarcinoma is one of the leading causes of cancer related mortality in men but still limited knowledge is available about its associated functional SNPs including rs1042522 (Pro72Arg). The present study was undertaken to explore the association of this SNP with susceptibility to prostate adenocarcinoma along with its structural and functional impacts in the Pakistani population in a case-control study. Three-dimensional structure of human TP53 with Pro72Arg polymorphism was predicted through homology modeling, refined and validated for detailed structure-based assessment. We also carried out a HuGE review of the previous available data for this polymorphism. Different genetic models were used to evaluate the genotypes association with the increased risk of PCa (Allelic contrast: OR=0.0.34, 95%CI 0.24-0.50, p=0.000; GG vs CC: OR=0.17, 95%CI 0.08-0.38, p=0.000; Homozygous: OR=0.08, 95%CI 0.04-0.15, p=0.000; GC vs CC: OR=2.14, 95%CI 1.01-4.51, p=0.046; Recessive model: OR=0.10, 95%CI 0.05-0.18, p=0.000; Log Additive: OR=3.54, 95%CI 2.13-5.89, p=0.000) except the Dominant model (OR=0.77, 95%CI 0.39-1.52, p=0.46). Structure and functional analysis revealed that the SNP in the proline rich domain is responsible for interaction with HRMT1L2 and WWOX. In conclusion, it was observed that the Arg coding G allele is highly associated with increased risk of prostate adenocarcinoma in the Pakistani population (p=0.000).

      • A 33.3% Power Efficiency RF Energy Harvester with -25dBm Sensitivity using Threshold Compensation Scheme

        This paper presents an ultra-low power RF-DC voltage multiplier using auxiliary transistors block to dynamically control the threshold voltage of the main chain transistors. The proposed scheme enhances the forward conduction current and reduces the reverse current by controlling the gate to source voltage of the main chain transistors. The proposed 5-stage scheme attains maximum post-layout simulated power conversion efficiency (PCE) of 33.3% at -16dBm input level and delivers 2.63 V output DC voltage to 1MΩ load. The proposed RF-DC voltage multiplier has been designed in a standard 180 nm CMOS technology.

        Active Contours Level Set Based Still Human Body Segmentation from Depth Images For Video-based Activity Recognition

        Context-awareness is an essential part of ubiquitous computing, and over the past decade video based activity recognition (VAR) has emerged as an important component to identify user`s context for automatic service delivery in context-aware applications. The accuracy of VAR significantly depends on the performance of the employed human body segmentation algorithm. Previous human body segmentation algorithms often engage modeling of the human body that normally requires bulky amount of training data and cannot competently handle changes over time. Recently, active contours have emerged as a successful segmentation technique in still images. In this paper, an active contour model with the integration of Chan Vese (CV) energy and Bhattacharya distance functions are adapted for automatic human body segmentation using depth cameras for VAR. The proposed technique not only outperforms existing segmentation methods in normal scenarios but it is also more robust to noise. Moreover, it is unsupervised, i.e., no prior human body model is needed. The performance of the proposed segmentation technique is compared against conventional CV Active Contour (AC) model using a depth-camera and obtained much better performance over it.

      • Design of an efficient RF-DC voltage multiplier for RF Energy Harvesting Applications

        In this paper, a RF-DC voltage multiplier is presented to efficiently convert RF signals to DC voltages. The proposed circuit uses an internal threshold voltage cancellation (IVC) scheme with auxiliary block to reduce the threshold voltage of forward-biased transistors and minimizes the reverse leakage current of the reverse-biased by dynamically controlling the gate-source voltage of the transistors in the main rectification chain. The proposed circuit is designed in 0.18 um CMOS technology. A three-stage voltage multiplier results in a maximum power conversion efficiency (PCE) of 49.1% at input power level of 0 dBm and at frequency of 900 MHz. The proposed circuit produces an output voltage of 4.94 V at 50 KΩ load.

      • Various Aspects, Patterns and Risk Factors in Breast Cancer Patients of Balochistan

        Purpose: Breast cancer is the commonest malignancy of females throughout the world with one million new cases each year. In Pakistan, the burden of breast cancer disease is high with late stage presentation being a common feature, more than half being stage III or stage IV. The objective of this study was to study various aspects, patterns and risk factors in breast cancer patients of Balochistan. Method: Present study was performed on 134 patients of breast cancer who were registered in CENAR. The patients were interviewed by providing a questionnaire. Informed consent was taken from all the patients who took part in this study after explanation of the study aims. Body mass index (BMI) was calculated andbiopsy reports were obtained from patients files. All the cases were classified with respect to age, gender, ethnic group (Baloch, Pashtoon, Punjabi, Afghani, Hazara) BMI, cancer type, cancer grade, hormonal status, side of the cancer, fertility and marital status. Results: Out of 134 patients, the most common ethnic group was Pashtoon with a total of 42 and the common age group was 41-50 years with a total of 51. Invasive ductal carcinoma (IDC) was the most common type, accounting for in 128 patients (95.5%) followed by invasive lobular carcinoma (ILC). Conclusion: Pashtoon was the most common ethnic group, IDC was common type and most of the patients had an ER/PR positive hormonal status.
