        Different roles of surveillance positron emission tomography according to the histologic subtype of non-Hodgkin’s lymphoma

        Background/Aims: Although the use of surveillance 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) is discouraged in patients with diffuse large B-cell lymphoma, its usefulness in different subtypes has not been thoroughly investigated. Methods: We retrospectively evaluated 157 patients who showed positive results on surveillance FDG-PET/CT every 6 months following complete response for up to 5 years. All of the patients also underwent biopsies. Results: Seventy-eight (49.6%) of 157 patients had true positive results; the remaining 79 (50.3%), including eight (5.1%) with secondary malignancies, were confirmed to yield false positive results. Among the 78 patients with true positive results, the disease in seven (8.9%) had transformed to a different subtype. The positive predictive value (PPV) of FDG-PET/CT for aggressive B-cell non-Hodgkin’s lymphoma (NHL) was lower than that for indolent B-cell or aggressive T-cell NHL (p = 0.003 and p = 0.018, respectively), especially in patients with a low/low-intermediate international prognostic index (IPI) upon a positive PET/CT finding. On the other hand, indolent B-cell and aggressive T-cell NHL patients showed PPVs of > 60%, including those with low/low-intermediate secondary IPIs. Conclusions: The role of FDG-PET/CT surveillance is limited, and differs according to the lymphoma subtype. FDG-PET/CT may be useful in detecting early relapse in patients with aggressive T-cell NHL, including those with low/low-intermediate risk secondary IPI; as already known, FDG-PET/CT has no role in aggressive B-cell NHL. Repeat biopsy should be performed to discriminate relapse or transformation from false positive findings in patients with positive surveillance FDG-PET/CT results.

        Myeloid-derived suppressor cells (MDSCs) is a heterogenous cell population, consisting of myeloid progenitor cells and immature myeloid cells and having an ability to suppress T-cell function. Due to their suppressive effects on immunity, MDSCs can lead to facilitate tumor development and growth, and has been known to be associated with poor prognosis in various solid cancers. However, the role of MDSC in myeloid neoplasm are unclear. So, we elucidated the role of leukemic subpopulation showing CD11b+CD33+HLA-DR- immunophenotype, which resembles MDSCs. CD11b+CD33+HLA-DR- blast were isolated using flow-cytometry from bone marrow mononuclear cell sample, which were collected at the time of diagnosis by Acute Leukemia Cohort Study in Severance. The clinical impact of MDSC-like blasts was retrospectively reviewed in 63 patients. CD33+HLA-DR- leukemic cells from each patients showed various range of expression for CD11b (Mean±SD, 21.02±22.19%). MDSCs can be divided into two subtypes, monocytic MDSC expressing CD14 and granulocytic MDSC expressing CD15, and CD14 expression on MDSC-like blast expression was more frequent than CD15 expression (67.5% vs. 39.3%). To figure out the immunosuppressive activity in MDSC-like blasts, Arg1 and iNOS expression were checked, and MDSC-like blasts showed significantly higher expression of Ara1(77.1% vs. 38.5%, p<0.001) and iNOS (33.0% vs. 1.1%, p<0.0001) compared to non-MDSC-like blasts. Compared to ‘Lower MDSC group’, WBC count, serum LDH, fraction of blasts in bone marrow, and frequency of adverse cytogenetics were significantly higher in ‘Higher MDSC group’. Patients in ‘High MDSC group’ had significantly shorter overall survival (331±42 vs. 758±114 days, p=0.027). On multivariate analysis, higher fraction of CD11b+CD33+HLA-DR- leukemic cells (HR 2.966, 95%CI 1.086-8.095, p=0.034), old age (p=0.001), adverse cytogenetics (p=0.013), and FAB classification (p=0.035) were poor prognosis factors. CD33+HLA-DR- MDSC-like blasts subgroup is existed in myeloid leukemic blasts with various range. Because they suppressed T cell immunity, also showed adverse prognostic effect on survival, MDSC-like blasts might play a certain role in immune-tolerance in leukemia.

      • S-553 Primary low grade duodenal-type follicular lymphoma : A study of 10 cases in South Korea

        In the revised 2016 World Health Organization (WHO) classification, the duodenal-type follicular lymphoma (FL) was newly added. It is a low grade B-cell lymphoma characterized by indolent clinical course. For the rarity of disease, little is known about optimal treatment. We described clinical characteristics of 10 patients with primary low grade duodenal-type FL. A total of 10 patients who diagnosed with duodenal-type FL at 3 hospitals in South Korea from 2009 to 2016 were retrospectively reviewed. The low tumor burden (LTB) criteria by the Groupe D’Etude des Lymphomes Folliculaires (GELF) was assessed for all patients. All patients were clinical stage IE and scored 0 by GELF criteria. Preferential segment was 2nd portion of duodenum (n=8, 80.0%). 9 patients (90.0%) found incidentally the disease during routine health examination. Fold swelling and thickening (n=4, 40.0%) was the most common endoscopic finding. All patients were positive for CD20, CD10, and Bcl-2 in immunohistochemical analysis. Treatment strategies including watch and wait (n=5, 50.0%) and radiotherapy (n=5, 50.0%) were performed. Of the 5 patients who did not receive any treatment, only 1 patient underwent progression of disease with median time of 19.4 months. All patients are still alive with median follow-up duration of 32.7 months (range, 2.5-63.9 months). Primary duodenal-type FL is characterized a remarkable indolent disease. The watch and wait policy may be an acceptable initial strategy for LTB duodenal-type FL. Further prospective investigations are needed to determine the optimal treatment.

      • Application of antihelix antibodies in protein structure determination

        <P><B>Significance</B></P><P>Antibodies have been found to be helpful for structural studies of challenging proteins by X-ray crystallography and electron microscopy. They are being used to construct useful protein nanostructures as well. Antibodies suitable for structural study should recognize the 3-dimensional conformations of target proteins. However, generating such antibodies and characterizing structures of their complexes with antigens take months or even years of research. Here, we show that we can expand the application of well-characterized antibodies by “transplanting” the alpha-helical epitopes that they recognize to proteins with completely different structures and sequences. Systematic screening of more antihelix antibodies will greatly expand the scope of the method.</P><P>Antibodies are indispensable tools in protein engineering and structural biology. Antibodies suitable for structural studies should recognize the 3-dimensional (3D) conformations of target proteins. Generating such antibodies and characterizing their complexes with antigens take a significant amount of time and effort. Here, we show that we can expand the application of well-characterized antibodies by “transplanting” the epitopes that they recognize to proteins with completely different structures and sequences. Previously, several antibodies have been shown to recognize the alpha-helical conformation of antigenic peptides. We demonstrate that these antibodies can be made to bind to a variety of unrelated “off-target” proteins by modifying amino acids in the preexisting alpha helices of such proteins. Using X-ray crystallography, we determined the structures of the engineered protein–antibody complexes. All of the antibodies bound to the epitope-transplanted proteins, forming accurately predictable structures. Furthermore, we showed that binding of these antihelix antibodies to the engineered target proteins can modulate their catalytic activities by trapping them in selected functional states. Our method is simple and efficient, and it will have applications in protein X-ray crystallography, electron microscopy, and nanotechnology.</P>

      • 시각 장애인을 위한 골프 중계 동영상 정보의 실시간 정보 추출 및 촉각화

        본 연구에서는 동영상 콘텐츠에 대한 시각 장애인의 접근성 향상을 위한 자동화된 청-촉각 변환 시스템을 구현하고 이를 시각 장애인 피험자를 대상 평가를 진행하였다. 일차 목표로 설정한 골프중계는 시각 장애인의 수요는 높은 반면 시각 외 제공되는 청-촉각 정보가 부족한데, 이를 해결하기 위해 본 연구에서는 컴퓨터 비전 알고리즘을 활용하여 청-촉각으로 변환할 요소를 추출하고 이를 TTS 및 촉각 디스플레이 닷 패드를 활용하여 변환, 출력하였다. 사용자 평가 결과 전반적으로 높은 만족도를 보였으며, 시각 장애의 정도나 경험 여부에 따른 개인화나 정보의 다양화 등 추가적 지향점도 확인할 수 있었다.

        텍스트 제시 유형에 따른 초등학교 2학년 발달성 난독 학생과 일반 학생의 읽기유창성 특성

        목적 초등 저학년군(1-2학년)의 국어 교과 목표로 유창한 읽기 능력을 포함하고 있다. 따라서 발달성 난독 학생의 해독 능력뿐만 아니라 유창한 읽기 능력에 대한 지원이 필요하다. 본 연구에서는 발달성 난독 학생이 시공간적 요인을 고려한 텍스트 제시유형에 따라 읽기유창성 능력에 어떠한 차이를 보이는지 살펴보고, 지원 방법을 논의하고자 하였다. 방법 인지 및 언어 능력에 어려움이 없으나 읽기에 어려움을 보이는 발달성 난독 학생 13명과 일반 학생 13명을 대상으로 읽기유창성 능력을 정확도, 속도, 운율 측면에서 살펴보았다. 해독 난이도와 언어적 난이도를 조절한 텍스트를 문장의 구두점을 경계로 문단을 구성한 문장 줄 바꿈형 텍스트와 줄글 형식의 일반교과형 유형으로 구성하였고, 각 유형별로 두 가지 텍스트를 순서효과와 반복효과를 배제하여 제시하였다. 결과 정확도면에서는 발달성 난독 집단이 일반집단보다 낮은 정확도를 보였으나 각 집단 내에서 텍스트 제시 유형에 따른 정확도 차이를 보이지 않았다. 읽기 속도에서는 발달성 난독 집단이 일반 집단보다 텍스트를 느리게 읽는 것으로 나타났으며, 문장 줄 바꿈형 텍스트보다 일반교과형 텍스트에서 속도가 더 느린 것으로 나타났다. 운율면에서는 발달성 난독 집단이 일반 집단보다 운율 총점이 낮았지만, 텍스트 제시 유형에 따른 차이를 보이지 않았다. 결론 발달성 난독 학생에게 어느 정도 해독 가능한 텍스트를 제시해주었을 때 정확도 면에서 발달성 난독 학생이 일반아동처럼 텍스트 제시 유형에 따른 차이가 나타나지 않아 텍스트의 조절이 중요함을 확인하였다. 반면 속도 면에서는 문장 줄 바꿈형보다 일반교과형 글에서 더욱 느린 속도로 읽어내어 일반아동과 격차가 벌어지고 있는데, 발달성 난독 아동에게 처리속도에 대한 지원이 매우 필요함을 시사한다. Objectives Reading fluency is an important factor that connects decoding ability and reading comprehension ability. Given that fluent reading is essential for language classes in lower grades of elementary school in Korea, it is necessary to support not only the decoding ability but also student’s reading fluency. This study investigated the differences in reading fluency ability of two groups of children according to text presentation types. Methods Thirteen students with developmental dyslexia(DYS) and 13 typically developing students(TD) read two types of decodable texts, sentence-broken type and general type. Reading accuracy, speed and prosody were measured. Results DYS revealed lower accuracy and prosody scores than TD, while there were no differences in accuracy and prosody according to text presentation type. DYS took longer time to finish reading texts than TD, while only DYS took longer time to read general type text than sentence-broken type text. Conclusions When presenting a text that is decodable to a certain extent to DYS, the text presentation type didn’t seem to matter for reading accuracy, although reading speed was influenced by the presentation type. It seems very important to consider the visuo-spatial presentation format as well as the decodability of the texts to support DYS’s reading fluency meaningfully.

      • Prognostic significance of interventricular septal thickness in patients with AL amyloidosis

        <P><B>Abstract</B></P> <P>The major prognostic determinant of immunoglobulin light chain (AL) amyloidosis is cardiac involvement. However, the role of interventricular septal thickness (IVST), which reflects the extent of cardiac involvement, remains unclear. Therefore, we analyzed 77 patients with newly diagnosed AL amyloidosis and evaluated the prognostic role of IVST. Fifty patients (64.9%) had cardiac involvement and 17 patients (22.1%) showed IVST >15mm. Among all patients, the revised Mayo Clinic Stage III–IV and IVST >15mm were independently associated with inferior overall survival (OS) in a multivariable analysis. IVST >15mm was also adversely prognostic for OS in a subgroup of advanced-stage (revised Mayo Clinic stage III–IV) patients in a multivariable analysis (<I>P</I> <0.001). Furthermore, advanced-stage patients with IVST >15mm did not show survival benefit from treatment with bortezomib-based regimens and/or autologous stem-cell transplantation (ASCT). Our study demonstrated that IVST >15mm is adversely prognostic independent of the revised Mayo Clinic staging system in patients with AL amyloidosis.         준설토 일괄처리시스템을 위한 진공사이로 설계에 관한 연구

        본 논문에서는 우수관로부터 발생되는 퇴적토의 준설처리를 위한 소형의 이동식 준설일괄처리 시스템을 제안하고, 특히 진공사이로와 1차 선별분리기 그리고 컨베이어를 일체화하는 진공사이로 선별분리장치를 설계하였다. 진공사이로 선별분리장치는 저장조, 이송 스크류, 진공 게이트, 스크린 바, 스크린 바 클리너, 진공배출장치로 구성된다. 특히 저장조 내에서 흡입공기의 흐름은 선별분리 성능의 주요 요소임을 고려하여 CFD 유동해석을 통해 최적형상을 도출하였다. 또한 CAE 구조해석을 통해 판재로 이루어지는 저장조의 안전성을 검토하였다. 그리고 3D-CAD 모델링을 통한 구동 메커니즘의 기구적, 역학적 시뮬레이션을 수행하여 사양을 결정하였다. 본 연구에 이어서, 드럼 스크린형 2차선별기, 데칸터형 탈수장치 그리고 보조장치인 오수탱크와 펌프 등을 설계할 것이다. 그리고 본 설계를 바탕으로 시제품을 제작하고 필드테스트를 수행할 것이다. In this study, a small movable batch treatment system for dredging soil deposited in a rain water tube is proposed; further, a vacuum silo sorting separation device with a vacuum silo, first-stage sorting separator, and conveyor is designed. The vacuum silo sorting separation device also consists of a storage tank, transferring screw, vacuum gate, screen bar, screen bar cleaner, and vacuum discharging device. In view of the fact that the flow of drawn air in the storage tank is a major factor influencing the sorting separation performance, the optimum shape of the tank is determined by CFD flow analysis. In addition, by using CAE structure analysis, the safety of a storage tank made of boards is examined. The specifications of the vacuum silo sorting separation device are determined by conducting mechanical and dynamic simulations of the driving mechanism of the vacuum silo sorting separation device through 3D-CAD modeling. Following this study, we will design a drum-screen-type second sorter, a decanter-type dehydration device, and waste water tank and pump as a secondary device. Further, on the basis of this design, we will construct a prototype model and carry out a field test.
