        매복된 상악 전치의 교정적 치료 : 증례보고

        상악 전치의 매복은 제3대구치,상악 견치,상하악 제2소구치에 비해서 드물지만,정중 과잉치,외상,치근만곡 등의 치근형성의 변이가 높은 빈도로 나타나기 때문에 다른 전치부에 비해서는 매복되는 비율이 높고,보통 약 8세 이후의 초기 혼합치열기의 아동에서 관찰된다. 상악 중절치의 매복으로 인한 인접치아의 이동으로 공간상실과 함께 정중선 변이가 나타날 수 있고 낭종 등이 발생할 수 있으므로,정확한 위치를 확인하고 적절한 치료 계획을 세우는 것이 중요하다. 치료 방법으로는 외과적 발거,자발적 맹출의 관찰,교정적 견인 등이 있으나,매복치의 병적 변화가 없고,치근의 발육이 양호하다면 기능과 심미성의 회복을 위해 교정적 견인을 시행한다. 이에 매복된 상악 전치들을 교정적 견인을 통해 정상적인 맹출방향으로 유도하여 양호한 치료결과를 얻었기에 보고하는 바이다. Impaction of maxillary incisor is rare than the third molar and canine, but its rate is higher than the other anterior teeth due to frequent mesiodens, trauma and variation of root formation (root dilaceration etc.). It is often observed in the dental age of about eight years and over. It will be occurred that the space loss, midline deviation and cyst formation due to the impaction of maxillary incisor. So it is important to evaluate the precise location of impacted tooth and to make appropriate treatment plan. Treatment would be surgical extraction or expectation for spontaneous eruption. Ifthe impacted tooth has no pathologic change and development ofthe root is favorable, orthodontic traction is recommended for recovery offunction and esthetics. In these cases, we performed orthodontic traction for the eruption ofimpacted maxillary incisors, and obtained satisfactory results.

        Serotype Distribution and Antimicrobial Resistance of Streptococcus pneumoniae Causing Invasive Pneumococcal Disease in Korea Between 2017 and 2019 After Introduction of the 13-Valent Pneumococcal Conjugate Vaccine

        Background: Streptococcus pneumoniae is a serious pathogen causing various infections in humans. We evaluated the serotype distribution and antimicrobial resistance of S. pneumoniae causing invasive pneumococcal disease (IPD) after introduction of pneumococcal conjugate vaccine (PCV)13 in Korea and investigated the epidemiological characteristics of multidrug-resistant (MDR) isolates. Methods: S. pneumoniae isolates causing IPD were collected from 16 hospitals in Korea between 2017 and 2019. Serotyping was performed using modified sequential multiplex PCR and the Quellung reaction. Antimicrobial susceptibility tests were performed using the broth microdilution method. Multilocus sequence typing was performed on MDR isolates for epidemiological investigations. Results: Among the 411 S. pneumoniae isolates analyzed, the most prevalent serotype was 3 (12.2%), followed by 10A (9.5%), 34 (7.3%), 19A (6.8%), 23A (6.3%), 22F (6.1%), 35B (5.8%), 11A (5.1%), and others (40.9%). The coverage rates of PCV7, PCV10, PCV13, and pneumococcal polysaccharide vaccine (PPSV)23 were 7.8%, 7.8%, 28.7%, and 59.4%, respectively. Resistance rates to penicillin, ceftriaxone, erythromycin, and levofloxacin were 13.1%, 9.2%, 80.3%, and 4.1%, respectively. MDR isolates accounted for 23.4% of all isolates. Serotypes 23A, 11A, 19A, and 15B accounted for the highest proportions of total isolates at 18.8%, 16.7%, 14.6%, and 8.3%, respectively. Sequence type (ST)166 (43.8%) and ST320 (12.5%) were common among MDR isolates. Conclusions: Non-PCV13 serotypes are increasing among invasive S. pneumoniae strains causing IPD. Differences in antimicrobial resistance were found according to the specific serotype. Continuous monitoring of serotypes and antimicrobial resistance is necessary for the appropriate management of S. pneumoniae infections.

      • Periventricular white matter hyperintensities and the risk of dementia: a CREDOS study

        <B>ABSTRACT</B><B>Background:</B><P>Cerebral white matter hyperintensities (WMH) are prevalent incident findings on brain MRI scans among elderly people and have been consistently implicated in cognitive dysfunction. However, differential roles of WMH by region in cognitive function are still unclear. The aim of this study was to ascertain the differential role of regional WMH in predicting progression from mild cognitive impairment (MCI) to different subtypes of dementia.</P><B>Methods:</B><P>Participants were recruited from the Clinical Research Center for Dementia of South Korea (CREDOS) study. A total of 622 participants with MCI diagnoses at baseline and follow-up evaluations were included for the analysis. Initial MRI scans were rated for WMH on a visual rating scale developed for the CREDOS. Differential effects of regional WMH in predicting incident dementia were evaluated using the Cox proportional hazards model.</P><B>Results:</B><P>Of the 622 participants with MCI at baseline, 139 patients (22.3%) converted to all-cause dementia over a median of 14.3 (range 6.0-36.5) months. Severe periventricular WMH (PWMH) predicted incident all-cause dementia (Hazard ratio (HR) 2.22; 95% confidence interval (CI) 1.43-3.43) and Alzheimer's disease (AD) (HR 1.86; 95% CI 1.12-3.07). Subcortical vascular dementia (SVD) was predicted by both PWMH (HR 16.14; 95% CI 1.97-132.06) and DWMH (HR 8.77; 95% CI 1.77-43.49) in more severe form (≥ 10 mm).</P><B>Conclusions:</B><P>WMH differentially predict dementia by region and severity. Our findings suggest that PWMH may play an independent role in the pathogenesis of dementia, especially in AD.</P>

        Learning Communication by Communicating : A Literature-Based Approach in an EFL Setting

        본 연구는 영어를 외국어 학습상황에서 배울 때의 효과적인 의사소통 능력을 개발하기 위해서 문학중심 접근법을 이용하며, 영어를 교실 상황에서 외국어로 학습할 때에 적용할 수 있는 다양한 의사소통 교수법의 원리들을 이론적 배경으로 도입한다. 어린이를 위한 문학 작품을 이용한 문학중심 접근법을 외국어 교실에 도입했을 때, 학생들은 재미있고 쉬우면서도 실제적인 언어로 쓰여진 작품을 통해서 목표어의 의사소통 능력을 개발한다. 구두 의사소통 능력은 교재의 의미를 구축하는 학급토론을 통해서, 문어에 의한 의사소통 능력은 대화저널을 씀으로써 이루어 졌으며, 이 두 가지의 의사소통 방법은 각각 영어 학습으로 연결되었다. 즉 학생들은 미리 읽어 온 작품의 내용을 토대로 해서, 작품의 이해를 돕기 위해 이용된 다양한 활동에 참가할 때 실제적인 언어를 경험하게 된다. 이들은 등장 인물 분석, 문제점과 해결책을 찾는 과정에서 자신의 경험을 얘기함으로써 목표어의 구어 능력을 향상시켰다. 또한 작품과 자신의 경험을 대화 저널을 통해서 좀더 깊이 있는 반응을 함으로써 목표어의 쓰기 능력을 향상시켰다. 따라서 본 연구는 학생들이 문학 중심 접근법을 통해서 영어를 학습할 경우, 단편적인 문법 지식이나 어휘의 기계적인 암기에서 벗어나, 목표어의 네 가지 언어 능력을 골고루 향상시키면서 의사소통 능력을 향상시키는 총체적인 언어 학습의 가능성을 보여 주었다.

        Exploring EFL Learners’ Responses through Literature within a Transactional Framework

          This study investigated Korean EFL students’ literary experience within a framework of response-oriented literature-based approach. The researcher set up a literature-based program in which language learners read and responded to diverse genres of literature written for children. Data collection consisted of audio-recordings of classroom conversations and interviews, both students’ and the teacher’s journals, a survey, the teacher’s field notes, and other classroom documents. Two research questions were set up to explore diverse aspects of response-oriented EFL instruction. Data analysis revealed (1) that students’ perspectives on transactional experience were positive; (2) that students transacted with literature through the connection of the world; and (3) that the teacher in a response-oriented language classroom undertook different roles other than the traditional teacher-centered instruction. Considering EFL students’ enriched experience, the researcher suggests that other EFL teachers utilize this study and further explore the value of response-oriented language instruction through the use of literature.

        Confirming Genetic Abnormalities of Hypokalemic Periodic Paralysis Using Next-Generation Sequencing: A Case Report and Literature Review

        Hypokalemic periodic paralysis (hypoPP) is a disorder characterized by episodic, short-lived, and hypo-reflexive skeletal muscle weakness. HypoPP is a rare disease caused by genetic mutations related to expression of sodium or calcium ion channels. Most mutations are associated with autosomal dominant inheritance, but some are found in patients with no relevant family history. A 28-year-old man who visited the emergency room for paralytic attack was assessed in this study. He exhibited motor weakness in four limbs. There was no previous medical history or family history. The initial electrocardiogram showed a flat T wave and QT prolongation. His blood test was delayed, and sudden hypotension and bradycardia were observed. The blood test showed severe hypokalemia. After correcting hypokalemia, his muscle paralysis recovered without any neurological deficits. The patient’s thyroid function and long exercise test results were normal. However, because of the history of high carbohydrate diet and exercise, hypoPP was suspected. Hence, next-generation sequencing (NGS) was performed, and a mutation of Arg669His was noted in the SCN4A gene. Although hypoPP is a rare disease, it can be suspected in patients with hypokalemic paralysis, and iden tification of this condition is important for preventing further attacks and improving patient outcomes. Diagnosing hypoPP through targeted NGS is a cost-effective and useful method.

      • An open label, split-face study of the efficacy of soothing purifying cream for facial seborrheic dermatitis

        Background: Seborrheic dermatitis(SD) is a multifactorial disease, but Malassezia species is regarded an important role in the pathogenesis of SD.Objectives: This study aimed at determining whether a soothing purifying cream(SPC) containing climbazole and piroctone olamine, antifungal agents with expected efficacy against Malassezia species, could improve SD symptoms. Methods: A total of 24 patients with SD were instructed to apply the SPC on the right side of the face and emollient cream on the left side for 4 weeks. The amounts of sebum and erythema were measured at baseline and after 4 weeks using MPA 5. The minimal inhibitory concentration (MIC) was determined to demonstrate the antifungal activity of the SPC. Results: The amount of sebum on the forehead and cheek was reduced by 30.67% (p<0.0001) and 31.23% (p<0.0004), respectively, on the right side compared to 9.08% (p=0.2870) and 14.35% (p=0.1713), respectively, on the left side. Erythema was reduced by 14.24% (p<0.0001) and 14.29% (p<0.0001) on the forehead and cheek, respectively, on the right side compared to a 1.48% (p=0.6476) increase and a 4.03% (p=0.3069) decrease, respectively, on the left side. As for the SPC’s antifungal activity, the MIC was 0.625mg/mL, 5 mg/mL, 0.625mg/mL, and 2.5mg/mL for Malassezia restricta, M. globosa, M. sympodialis, and M. slooffiae, respectively. Conclusion: This SPC with antifungal activity against the Malassezia species seems helpful for the treatment of patients with mild-to-moderate SD.

      • A case of lupus erythematosus panniculitis

        Lupus erythematosus panniculitis(LEP) is a rare variant of lupus erythematosus, which primarily affects the subcutaneous adipose tissue. LEP may appear as the sole manifestation of lupus erythematosus or may occur prior to or after the onset of discoid lupus erythematosus or systemic lupus erythematosus(SLE). The incidence of SLE in patients with LEP has been reported to range from 10% to 41%. The incidence of LEP occurs in only 1-5% of patients with SLE. Why LEP develops is still lacking. We report a case of LEP in 25 year old women who presented depressed lipoatrophic areas withoutinflammatory skin changes on the both upper arm for a year ago. Histopathology showed lobular panniculitis without vasculitis. The patient was weak positive for antinuclear antibodies, borderline positive for anti-double stranded DNA and low C4 level. Herein we report a rare case of LEP with a systemic lupus erythematosus.
