      • Successful treatment of recurrent focal segmental glomerulosclerosis with a low dose rituximab in a kidney transplant recipient

        <P>Recurrence of focal segmental glomerulosclerosis (FSGS) is a major therapeutic challenge in kidney transplantation (KT). Although intensive plasmapheresis and high-dose rituximab have been introduced to treat recurrent FSGS, the most effective dosage and regimen of rituximab have not been determined. Herein we reported the first case of successful treatment of recurrent FSGS with a low-dose rituximab. The patient showed marked proteinuria (3.5 g/d) and oliguria 2 d after KT. Two courses of plasmapheresis and immunoglobulin were applied to the patient, however, nephrotic range proteinuria persisted and creatinine level increased to 3.56 mg/dL. Five months post-transplant, the patient received injection with only one dose of rituximab 100 mg, without further plasmapheresis, which resulted in immediate reduction of serum creatinine and full remission of proteinuria during the following 18 months. This case suggested that recurrent FSGS, which frequently relapses after plasmapheresis, could be treated successfully with a low-dose rituximab even without plasmapheresis.</P>

        2000년대 문학에 나타난 도시적 삶과 공간

        해방 이후 급속한 경제발전과 함께 성장해 온 도시는 한국문학에서 빼놓을 수 없는 요소이며 그 자체로 한국 사회의 명암을 압축적으로 제시하는 중요한 기호다. 따라서 현대인의 삶을 깊이 있게 통찰하기 위해서는 도시에 대한 탐구가 필수적이다. 하지만 그럼에도 불구하고 도시 문제를 본격적으로 다룬 연구가 충분히 이루어졌다고 보기는 어려우며, 1980년대 이후 도시가 보편적 삶의 공간으로 자리 잡으면서 문학에 도시성이 내재화되면서 2000년대 문학에 재현된 도시공간의 양상과 의미에 대한 해석은 새로운 접근이 요청된다고 생각된다. 이에 본고는 2000년대 문학작품에 나타난 도시적 삶과 공간을 주목하여 분석함으로써 현대 사회에 나타나는 개성 상실과 순환적 세계인식의 문제성을 분석하고자 한다. 본고는 2000년대 한국문학에 나타난 도시와 도시민의 삶을 분석하기 위한 방법으로 게오르그 짐멜과 발터 벤야민의 도시 이론들을 원용하여 분석대상작의 공간들을 ‘개성 상실의 고독한 공간’과 ‘순환의 공간’으로 분류하였고 그에 나타난 현대 사회의 특성과 도시민들의 사회인식을 분석하였다. 분석의 결과, 도시에 살아가는 현대인들은 ‘거리두기’와 ‘속내 감추기’, ‘지성화’와 ‘둔감함’이라는 특성을 내보이며, 2000년대 한국문학은 이 도시민의 특유한 고독을 재현하는 데에 집중하고 있다는 점을 밝히고 있다. 도시는 화려한 볼거리들과 그것들이 만들어내는 환등상으로 가득 찬 곳으로 나타나지만, 도시생활과 냉정하게 거리를 유지하면서도 그것들을 전유하는 인물들은 공통되게 개성을 거세당한 채 물리적으로는 가깝지만 정신적인 거리를 좁히지 못하고 불완전한 산책자의 형상으로 등장한다. 2000년대 문학에 나타난 가난한 도시민들은 보다 나은 삶으로의 계층이동을 욕망하면서도 그것이 환등상에 불과하다는 것을 인식하는 불완전한 산책자로, 환등상에 홀린 군중이 되지도 못하고, 철저한 관조자도 되지 못한 채 도시를 산책하며 끝없이 순환하는 부조리한 세계를 인식하고 있는 것이다. 이상의 분석을 통해 2000년대 한국문학작품에 나타난 도시적 삶과 공간이 보여주는 도시민의 문제와 세계 인식이 우리 시대를 반영하는 독특한 창작방법론이자 실천적 기법이라는 점을 본고를 밝히고 있다. A city that has grown with rapid economic development since the 1930s is an important symbol of the light and shade of Korean society. It is also a must-explore space for insight into lives of modern people. However, considering the fact that the foundation of modern literature is a city, it is difficult to say that enough research has been done on urban issues. In fact, studies have been reduced since the 1990s when the city has become a universal place of life. In literature, the space called a city with more than a simple background needs to be approached in various ways to analyze the modern society. Therefore, the main textbook has significance in that it approaches the problem of loss of identity and the cyclical perception of the world by analyzing urban life and space shown in literary works of the 2000s. The main objective of this study was to review urban theories of Georg Simmel and Walter Benjamin as a way of analyzing lives of urban and urban. Through this, the spaces shown in each literary work were categorized as ‘unanimous space of solitude’ and ‘space of circulation’. Characteristics of modern society and social perception of urbanites in various spaces were analyzed. Results showed that modern people living in cities, the center of money economy, as Simmel theorized, had the characteristics of “Being at a Distance,” “reserve,” “intellectualization”, and “Bluntness,” while urbanites were physically obsessed with loneliness that was stronger than ever, physically close but spiritual. Also, urbanites became strollers who wanted to move to a better life but realized that it was nothing more than a fantasy. Walter Benjamin, while keeping his distance from city life, saw those who passed them on as flaneurs. However, it turned out that the flaneurs in literary in the 2000s recognized the absurd world in which they walked endlessly through the city, unable to become a phantasmagoria or control their ill-conceived fantasies due to their poverty. Through this analysis, we were able to learn about problems of urbanites and their world perceptions that urban life and space in literary works showed. We expect a wider understanding of modern urbanites through follow-up studies of more diverse urban lifestyles and spaces.

        비만 관련 SNP genotype-phenotype 정보기반의 맞춤 식단옴 추천

        전 세계적으로 비만인구의 증가로 인해 경제적 부담이 확대되고 있으며, 그 원인으로 육체적 활동의 감소 및 식이관리의 실패가 손꼽히고 있다. 영양성분 및 칼로리를 기반으로 한 맞춤 식단정보 제공 시스템과는 차별적으로 본 연구는 개인 맞춤형 기능성 식품을 추천하기 위해 비만 관련 SNP (single nucleotide polymorphism) 정보를 활용하였다. 본 연구를 위해 GWAS (Genome-wide association study) 분석을 수행하여 한국인 특이적인 비만 관련 SNP을 발굴하고, 이를 활용하여 유전적 정보를 입력하여 SNP genoype-phenoype 정보에 따른 맞춤 식단옴을 추천하였다. 또한 USDA (The United States Department of Agriculture) 식품 정보를 활용할 수 있도록 식품 통합 Database를 구축하여 식단 추천에 적용하였다. 그 결과, 표현형 정보 BMI (Body Mass Index)는 정상 수치를 가지고 있으나, 비만 관련 SNP 정보를 가지고 있는 샘플은 유전적 비만 위험도를 나타내어 식이관리가 필요하다는 정보를 확인하였으며, 관련 식품 정보를 제공하였다. 따라서 표현형에 따른 비만에 관한 정보와 유전형 정보가 일치하는 것은 아니며, 이는 표현형적 정보만을 이용한 비만 관리 식이 추천에는 한계가 있음을 의미하며 이러한 결과는 비만외 다른 성인병들에도 적용이 필요하며 이를 위해서는 표현형-유전적 통합정보를 기반 한 맞춤식이 추천이 필요함을 나타내었다. Obesity extends the global economic burden and it causes that the failure of a reduction of physical activity, and diet management. In this work, nutritional information and personalized diet based on calorie supply system and is discriminatory utilized the obesity-related SNP information in order to recommend a personalized functional foods. This study performed a GWAS analysis for the excavation of a Korean-specific and obesity-related SNP, which utilizes genetic information were recommended by entering a personalized diet in accordance with the SNP genotype-phenotype information. In addition, we integrated Database with relation of nutrient for utilizing the USDA Food information and it was applied to recommend with Sickdanome. As a result, the obesity-related SNP information was confirmed in the sample which has the normal value BMI. In this study, we have recognized that the phenotype information related obesity, BMI is inconsistent with the SNP genotype information. This result is shown that it is necessary to provide the personalized dietary SickdanOme recommendation based on the both pheotype-genotype information.

        Treatment of rituximab in patients with idiopathic membranous nephropathy: a case series and literature review

        Background/Aims: Membranous nephropathy (MN) is a major cause of nephrotic syndrome in adults. This study aimed to evaluate the effect of rituximab (RTX) in patients with idiopathic MN (iMN) who have a high risk of progression. Methods: We retrospectively analyzed data of 13 patients with iMN, who received RTX treatments from January 2014 to July 2020. RTX was indicated in patients with iMN with severe proteinuria and decreasing estimated glomerular filtration rate (eGFR) in the previous 6 months despite other immunosuppressive therapies. Results: The patients were predominantly males (n = 11) and with a mean age of 55.3 years; median eGFR, 37.0 mL/ min/1.73 m<sup>2</sup> (interquartile range [IQR], 26.3 to 66.5); serum albumin level, 2.6 g/dL (IQR, 1.9 to 3.1); and spot urine protein-to-creatinine ratio at baseline, 6.6 g/g (IQR, 5.7 to 12.9). In a median follow-up of 22 months, eight patients (61.5%) achieved complete or partial remission. In responder group (n = 8), median eGFR increased from 31.5 to 61.5 mL/min/1.73 m<sup>2</sup> (p = 0.049) and serum albumin level increased from 2.3 to 4.2 g/dL (p = 0.017) from RTX initiation to last follow-up. Antiphospholipase A2 receptor antibody (anti-PLA2R-Ab) was positive in six among seven tested patients, which markedly decreased in the responder group. There were no adverse events after RTX. Conclusions: This study suggests that RTX is a safe and effective treatment option for patients with iMN who have a high risk of progression. Individualized therapy based on anti-PLA2R-Ab titer would be needed for better outcomes.

      • Partially Hydrolyzed Poly (Vinyl Acetate)-Borax Complex for the Removal of Simulated Nuclear Fallout Particles

        A Partially hydrolyzed poly (vinyl acetate) (PHPVA)-borax complex-based gel-like coating was successfully developed for the decontamination of Simulated nuclear fallout (SFO) from surfaces. The sprayable coating was self-generated on the surface by borate-diol ester bonds after simultaneously mixing two solutions of borax and PHPVA. The SFO particles, synthesized at 1,200°C for melting, were glassy while some crystalline phases (e.g., SiO2 and Fe2O3) existed together. The SFO particles were fixed onto the Stainless steel (SS) substrate by dropping and evaporating water. for examination of the dust-removal performance of PHPVA-borax based coating. The dusts on the SS surface was successfully removed by casting the PHPVA-borax based coating within 1 minute, demonstrating the excellent dust-removal property of the PHPVA-borax based coating. The used PAB complex in wet state was recovered by using vacuum suction machine in short time. The solid-state PHPVA-borax based film was self-delaminated from the SS substrate after fully drying the used PHPVA-borax coating but this requires long period of time.

        Comparison of Lipid Profiles in Head and Brain Samples of Drosophila Melanogaster Using Electrospray Ionization Mass Spectrometry (ESI-MS)

        Drosophila melanogaster (fruits fly) is a representative model system widely used in biological studies because its brain function and basic cellular processes are similar to human beings. The whole head of the fly is often used to obtain the key function in brain-related diseases like degenerative brain diseases; however the biomolecular distribution of the head may be slightly different from that of a brain. Herein, lipid profiles of the head and dissected brain samples of Drosophila were studied using electrospray ionization-mass spectrometry (ESI-MS). According to the sample types, the detection of phospholipid ions was suppressed by triacylglycerol (TAG), or the specific phospholipid signals that are absent in the mass spectrum were measured. The lipid distribution was found to be different in the wild-type and the microRNA-14 deficiency model ($miR-14{\Delta}^1$) with abnormal lipid metabolism. A few phospholipids were also profiled by comparison of the head and the brain in two fly model systems. The mass spectra showed that the phospholipid distributions in the $miR-14{\Delta}^1$ model and the wild-type were different, and principal component analysis revealed a correlation between some phospholipids (phosphatidylethanolamine (PE), phosphatidylinositol (PI), and phosphatidylserine (PS)) in $miR-14{\Delta}^1$. The overall results suggested that brain-related lipids should be profiled using fly samples after dissection for more accurate analysis.
