Colon cancer usually spreads the regional lymph nodes, liver, lung, bone and brain. Metastasis to mediastinal lymph node without pulmonary and hepatic metastasis from colon cancer is very rare. Here, we report a case with multiple metastasis to mediastinal lymph nodes without any other organ involvement after curative resection of colon cancer. A 47 year-old man presented with abdominal pain, and colonoscopic biopsy were consistent with the diagnosis of colon adenocarcinoma. Subsequently, right hemicolectomy was performed, and histopathology revealed moderately to poorly differentiated adenocarcinoma of right colon with perineural and lymph vascular invasion, and metastatic involvement of 9 out of 22 regional lymph nodes. There was no distant metastasis on computed tomography scans of chest and abdomen, and the patient received chemotherapy with folinic acid, fluorouracil and oxaliplatin (FOLFOX). Two and a half years later, multiple enlarged mediastinal lymph nodes without lung and liver metastasis were observed on chest and abdomen computed tomography, and were extirpated by video-assisted thoracoscopic surgery. Pathology was consistent with the diagnosis of metastatic colonic primary adenocarcinoma. This case report suggests that the mediastinum should be considered as a possible metastatic site from colon cancer, even without evidence of lung and liver metastasis.

Background/Aims: The objective of this study was to evaluate the efficacy and feasibility of the pediatric-inspired regimen of the adult acute lymphoblastic leukemia (ALL) Working Party, the Korean Society of Hematology. Methods: Data of 99 patients with newly diagnosed ALL, who were treated with the KALLA 1406/1407 protocol, were retrospectively analyzed. All patients equally received age-adjusted daunorubicin, vincristine, and prednisolone. L-asparaginase was additionally administered to Philadelphia (Ph)-negative patients according to age, whereas Ph-positive patients received 600 mg/day of imatinib. Results: A total of 99 patients were enrolled in this study, of whom 62 (62.6%) were diagnosed with Ph-negative ALL and 37 (37.3%) were diagnosed with Ph-positive ALL. The median age of patients in the Ph-negative ALL group was 46 years, and that of patients in the Ph-positive ALL group was 49 years. In patients with Ph-negative ALL, 57 (92%) patients achieved complete remission (CR) and CR with incomplete hematologic recovery (CRi). Disease-free survival (DFS) and overall survival (OS) rates at 2 years were estimated to be 42% and 63%, respectively. In patients with Ph-positive ALL, 32 (86%) patients achieved CR/CRi, and 2-year DFS and OS were 31.2% and 49.1%, respectively. Patients who were able to proceed to the allogeneic hematopoietic cell transplantation and younger patients showed significantly superior survival in both Ph-negative ALL and Ph-positive ALL. Neutropenic fever and bacterial infection were the most common and severe adverse events. Conclusions: The KALLA 1406/1407 protocol showed tolerable toxicities in adult ALL patients. Especially, younger patients had more survival benefits with KALLA 1406/1407 protocol.

Invasive pulmonary aspergillosis (IPA) is rarely reported in patients with normal immune function. Recently, nonimmunocompromised hosts such as patients with chronic obstructive pulmonary disease and critically ill patients in intensive care units are reported to be at risk of IPA. Infiuenza infection is also believed to be associated with IPA among immunocompetent patients. However, most reports of IPA with infiuenza infection involved infiuenza A infection including pandemic infiuenza H1N1, and IPA associated with infiuenza B infection was scarcely reported. Here, we report probable IPA with a fatal clinical course in an immunocompetent patient with infiuenza B infection. We demonstrate IPA as a possible complication in immunocompetent patients with infiuenza B infection. Early clinical suspicion of IPA and timely antifungal therapy are required for better outcomes in such cases.

Background: Recently, the composition of lipid in liver is more important than the total amount of lipid in NAFLD. Methods: To evaluate the effect of lipoic acid (LA) on lipotoxicity and on the intracellular lipid partitioning, HepG2 was cultured with palmitic acid (PA) or LA. We used TUNEL assay for apoptosis, mitochondrial DNA copy number for mitochondrial function, electromicroscopy for the structure of the mitochondria, fluorescent microscope for the ROS activity, gaschromatography for lipid partitioning. Results: PA significantly increased apoptosis of the cell line (0.5% vs. 19.5%, p<0.05), and LA significantly suppressed apoptosis (19.5% vs. 1.6%, p<0.05). The number of mitochondrial DNA copy was decreased in PA group, and recovered after adding LA (Control: PA: PA+LA= 553± 33.8: 291±14.55: 421±21.05 copies). Electromicroscopy showed significant difference between the groups. ROS activity was significantly increased in the PA group, and decreased in LA group (4180 vs. 4350 vs. 4220 relative fluorescence unit respectively, p<0.05). LA decreased the proportion of saturated fatty acid and increased the proportion of unsaturated fatty acid. Free cholesterol was showed on the cell membrane in LA group. But in PA group, they located in the cytoplasm and cell membrane. Conclusions: LA prevented FFA-induced hepatic lipotoxicity via reversal intracellular lipid composition.

Purpose: Spontaneous isolated superior mesenteric artery (SMA) dissection is a rare disease. Many such patients are overlooked in the emergency department (ED), and are sent home with a diagnosis other than SMA dissection. In this study, we analyzed clinical findings for patients with isolated SMA dissections, and attempted to identify those ED patients for whom the diagnosis should be considered. Methods: A retrospective observational case series of nineteen patients diagnosed with spontaneous isolated SMA dissection in the ED was done. Demographic data, information regarding pain, the nature and number of cardiovascular risk factors, and laboratory and physical examination findings were analyzed. Results: The median age at which SMA was diagnosed was 54 (44-71) years; 78.9% were male. The most common symptom was sudden onset of severe epigastric pain with cramping. Variations in location of pain included the periumbilical area (31.6%), and the whole abdomen and lower abdomen (10.5% each). Besides cramping (57.9%), dull ache (31.6%) and abdominal fullness (10.5%) were other symptoms. Pain radiation was not noticed in 13 (68.4%) subjects, but 3 complained of their pain radiating to their back. Direct tenderness was present in 9 (47.4%) patients and rebound tenderness in 3 (15.8%). In 10 (52.6%), there was no tenderness. Conclusion: Physicians who assess patients with severe abdominal pain, especially men in their fifties, should generate a high index of suspicion regarding SMA dissection. Contrast enhanced CT scans should be performed in patients with sudden onset of severe epigastric pain, regardless of its nature, radiation, or associated symptoms, and especially whose abdominal physical examination was unremarkable.

Background: The increased risk of pulmonary tuberculosis (PTB) infection has been revealed among the patients with rheumatic diseases. However, it is not clear whether rheumatic diseases affect the risk of EPTB. We aimed to investigate the prevalence of both pulmonary tuberculosis (PTB) and extrapulmonary tuberculosis (EPTB) in patients with rheumatic diseases. Methods: We performed analysis from the Korean Health Insurance Review and Assessment Service (HIRA) database, spanning the period from January 2012 to December 2012. The database (HIRA-NPS-2012-0090) was based on random sampling of outpatients from whole population. Patients with PTB, EPTB, and rheumatic diseases were identified based on the International Classification of Disease-10 (ICD-10) diagnostic codes. Results: We identified 30,854 (2.17%) patients with rheumatic diseases of 1,422,551 cases, and the prevalence of all forms of TB in patients with rheumatic diseases was about 3.5 times higher than those without rheumatic diseases (1.72% vs. 0.49%, respectively). In addition, prevalence of PTB and EPTB were 1.34% and 0.38% in patients with rheumatic diseases, which were almost 3-5 times higher prevalence of PTB and EPTB in those without rheumatic diseases. Conclusions: This study found that the prevalence of both PTB and EPTB were higher among the patients with rheumatic diseases. Physicians should be alert to the possibility of TB in patients with rheumatic diseases, especially EPTB.
Professional Council Model Using Google Forms for Child Injury Surveillance

Background: The authors of this study developed and operated a web-based system model capable of early detection and on-going revision of child injury problem solving in the emergency room. Methods: The modulators carried out an on-line poll using text messages and e-mails. The child injury cases were divided into severe injury and frequent injury types. Ten professors of emergency physicians and pediatricians participated in the inspection of the web-based system model. The study was carried out regardless of time: on days, nights, weekdays, and holidays. We defined answers as valid which when they were returned within 2 hours. Among the valid answers, when more than 70% of the professors agreed on public announcement, the result was approved. Results: Twenty cases in the on-line poll were divided into 17 cases of severe type and 3 cases of frequent type. Sixteen cases occurred on weekdays, 4 on holidays, 16 during the daytime, and 4 during the night. The total number of answers was 195. The answer return rate was 97.5%. The average answering time was 181.2 minutes and the median value was 6 minutes. The number of valid answers within 2 hours was 174(84%). Among these 174 valid answers, the number of approvals was 127, and rejections were 47. Fourteen cases were approved for public announcement among the 20 cases of the on-line poll. Conclusion: This web-service based system could resolve, child injury matters by professionals without delay, even at night or during holidays.
