This work investigated issues for an efficient and reliable embedding and use of Fiber Bragg Grating (FBG)sensors for strain monitoring of composite structures with particular regard to the manufacturing processof components in the nautical field by means of the vacuum bag technique in autoclave. CFRP materiallaminates with embedded FBGs were produced and the effect of the curing process parameters on the lighttransmission characteristics of the optical fibers was initially investigated. Two different types of coating,namely polyimide and acrylate, were tested by measuring the light attenuation by an Optical Time DomainReflectometer. Tensile specimens were subsequently extracted from the laminas and instrumented also with asurface-mounted conventional electrical strain gage (SG). Comparison between the FBG and SG measurementsduring static tensile tests allowed the evaluation of the strain monitoring capability of the FBGs, inparticular of their sensitivity (i.e., gage factor) when embedded.
A clinical and pathological update on hepatocellular carcinoma

It is estimated that more than 1 million individuals will be affected annually by hepatocellular carcinoma (HCC) by 2025. HCC can be broadly grouped into two major molecular subgroups, each of which is characterized by specific morphological and phenotypic features that mirror the genetic background. The use of these tissue biomarkers in the daily practice of pathologists promises to better allocate patients with HCC with adequate treatments. In turn, this will likely boost the attitude of clinicians toward obtaining a pre-treatment biopsy. (J Liver Cancer 2022;22:14-22)

Objective: To assess shear bond strength and failure mode (Adhesive Remnant Index, ARI) of orthodontic brackets bonded to polymethylmethacrylate (PMMA) blocks for computer-aided design/manufacture (CAD/CAM) fabrication of temporary restorations, following substrate chemical or mechanical treatment. Methods: Two types of PMMA blocks were tested: CAD-Temp® (VITA) and Telio® CAD (Ivoclar-Vivadent). The substrate was roughened with 320-grit sandpaper, simulating a fine-grit diamond bur. Two universal adhesives, Scotchbond Universal Adhesive (SU) and Assure Plus (AP), and a conventional adhesive, Transbond XT Primer (XTP; control), were used in combination with Transbond XT Paste to bond the brackets. Six experimental groups were formed: (1) CADTemp ®/SU; (2) CAD-Temp®/AP; (3) CAD-Temp®/XTP; (4) Telio® CAD/SU; (5) Telio® CAD/AP; (6) Telio® CAD/XTP. Shear bond strength and ARI were assessed. On 1 extra block for each PMMA-based material surfaces were roughened with 180-grit sandpaper, simulating a normal/medium-grit (100 mm) diamond bur, and brackets were bonded. Shear bond strengths and ARI scores were compared with those of groups 3, 6. Results: On CAD-Temp® significantly higher bracket bond strengths than on Telio® CAD were recorded. With XTP significantly lower levels of adhesion were reached than using SU or AP. Roughening with a coarser bur resulted in a significant increase in adhesion. Conclusions: Bracket bonding to CAD/CAM PMMA can be promoted by grinding the substrate with a normal/ medium-grit bur or by coating the intact surface with universal adhesives. With appropriate pretreatments, bracket adhesion to CAD/CAM PMMA temporary restorations can be enhanced to clinically satisfactory levels.

In the framework of the Future Internet, the aim of the Quality of Experience (QoE) Control functionalitiesis to track the personalized desired QoE level of the applications. The paper proposes to perform such a taskby dynamically selecting the most appropriate Classes of Service (among the ones supported by the network), thisselection being driven by a novel heuristic Multi-Agent Reinforcement Learning (MARL) algorithm. The papershows that such an approach offers the opportunity to cope with some practical implementation problems: in particular,it allows to face the so-called “curse of dimensionality” of MARL algorithms, thus achieving satisfactoryperformance results even in the presence of several hundreds of Agents.
Microbial Electrochemical Process for Bioremediation of Recalcitrant Contaminants in Soil and Water

Environmental pollution that has exceeded the self-purification capacity of nature requires artificial support, which incurs a lot of economic cost and labor. For the potential solution, a bioremediation process based on ecofriendly, sustainable, and low-cost microbial electrochemical system could be used. In the study, we introduce a microbial electrochemical bioremediation for contaminated soil and water. For bioremediation of contaminated soil by pesticide, a soil microbial fuel cell which can harvest electricity while promoting biodegradation of a selective pesticide called Atrazine was developed, and the direction for scale-up for field application of the proposed system was discussed. For water, a study for concurrent harvesting electrical energy and recovering useful resource from a mineral rich wastewater was developed with linked process of microbial fuel cell and chemical precipitation. The study aims to explain the potential of microbial electrochemical bioremediation for contaminated environment with generation of renewable energy and removal of recalcitrant pollutant, towards a promising sustainable environmental biotechnology.

Background Conventional therapies (CTs), pharmacological (PH) and non–pharmacological (NPH), do not always achieve benefits in the treatment of chronic low back pain (CLBP). We assessed efficacy and safety of acupuncture for CLBP as alternative or addition to CT. Methods We included randomised controlled trials (RCTs) comparing acupuncture alone or in combination with CT to CT. We searched Medline, Cochrane Library, Embase up to May 2022. We assessed risk of bias with the original Cochrane tool and GRADE certainty of evidence. Results were pooled through meta-analysis. Results Ten RCTs (2122 participants) were included comparing acupuncture versus CT and 6 (374 participants) comparing acupuncture plus CT to CT alone. Comparing acupuncture with NPH or PH, no differences were found for pain and disability. Comparing with combined PH and NPH, pain and disability were reduced (SMD=-0.50, 95%CI-0.62 to -0.37; SMD=-0.71, 95%CI-1.17 to -0.24). Comparing acupuncture plus NPH with NPH alone, pain and disability were reduced (SMD=-0.70, 95%CI-0.94 to -0.46; SMD=-0.95, 95%CI-1.36 to -0.54). Comparing acupuncture plus PH with PH alone, pain and disability were reduced (MD=-0.21,8 95%CI-433.28 to -10.42; MD=-3.1, 95%CI-4.87 to -1.83). Comparing acupuncture plus combined treatment versus combined treatment alone, no differences were found in pain, while disability was reduced (MD=-3.40 95%CI-5.17 to -1.63). No studies assessed adverse event. Certainty of evidence ranged from moderate to very low. Conclusion We are uncertain whether acupuncture is more effective and safer than CT. In the comparisons without estimates’ imprecision, acupuncture showed promising results. Acupuncture could be an option based on patients’ preferences.
Outcome of Colorectal Surgery in Elderly Populations

Purpose: The aim of this study is to investigate the impact of age on short-term outcomes after colorectal surgery in terms of the 30-day postoperative morbidity and mortality rates. Methods: The subjects for the study were patients who had undergone colorectal surgery. Patients were divided into 2 groups according to age; groups A and B patients were ≥80 and <80 years old of age, respectively. Both groups were manually matched for body mass index, American Society of Anesthesiologists score, Charlson Comorbidity Index and procedure performed. Results: A total of 200 patients, 91 men (45.5%) and 109 women (54.5%), were included in this retrospective study. These patients were equally divided into 2 groups. The mean ages were 85 years in group A (range, 80 to 104 years) and 55.3 years in group B (range, 13 to 79 years). The overall 30-day postoperative mortality rate was 1% of total 200 patients; both of these 2 patients were in group A. However, this observation had no statistical significance. No intraoperative complications were encountered in either group. The overall 30-day postoperative morbidity rate was 27% (54 of 200) for both groups. The 30-day postoperative morbidity rates in groups A and B were 28% (28 of 100) and 26% (26 of 100), respectively. However, these differences between the groups had no statistical significance importance. Conclusion: Age alone should not be considered to be more of a contraindication or a worse predictor than other factors for the outcome after colorectal surgery on elderly patients.

Purpose: We evaluate the role of transanal tube drainage (TD) as a conservative treatment for patients with anastomotic leakage (AL). Methods: Patients treated for AL who had undergone a low or an ultralow anterior resection with colorectal or coloanal anastomosis for the treatment of rectal cancer between January 2013 and January 2017 were enrolled in this study. The data were collected prospectively and analyzed retrospectively. The primary outcomes were the diagnosis and the management of AL. Results: Two hundred thirteen consecutive patients, 122 males and 91 females, were included. The mean age was 66.91 ± 11.15 years, and the median body mass index was 24 kg/m2 (range, 20–35 kg/m2). The median tumor distance from the anal verge was 8 cm (range, 4–12 cm). Ninety-three patients (44%) received neoadjuvant therapy for nodal disease and/or locally advanced rectal cancer. Only 13 patients (6%) developed AL. Six patients developed subclinical AL as they had a defunctioning ileostomy at the time of the initial procedure. They were treated conservatively with TD under endoscopic guidance in the endoscopy unit and received intravenous antibiotics. Six weeks after discharge, these 6 patients underwent follow-up flexible sigmoidoscopy which showed a completely healed anastomotic defect with no residual stenosis. Seven patients developed a clinically significant AL and required reoperation with pelvic abscess drainage and Hartmann colostomy formation. Conclusion: These results suggest that TD for management of patients with AL is safe, cheap, and effective. Salvaging the anastomosis will help decrease the need for Hartmann colostomy formation. Proper patient selection is important.
