        Interventional Procedures in Superficial Lesions: The Value of 2D with Additional Coronal Reformatted 4D Ultrasonography Guidance

        Objective: We wanted to assess the usefulness of four-dimensional (4D) ultrasonography (US), i.e., real-time three-dimensional US, as an adjunct for performing various US-guided interventional procedures in superficial lesions. Materials and Methods: Thirty-three patients were referred for US-guided interventional procedures for superficial lesions, including core biopsy in 19, fineneedle aspiration in eight, therapeutic drug injection in four and needle puncture in two. The procedures were performed under 4D US guidance. We reviewed the pathologic/cytologic results of the core biopsies or needle aspirations, and also the outcomes of drug injection or needle puncture. Results: For all the patients who underwent 4D US-guided core biopsy, the specimens were adequate for making the pathological diagnosis, and specimens were successfully obtained for those patients who underwent 4D US-guided aspiration. The patients treated with 4D US-guided therapeutic drug injection or needle puncture had a good response. No major procedure-related complications occurred. The procedural times were similar to those procedural times with using two-dimensional US. Conclusion: Combining the two dimensional and 4D US techniques aids the physician when performing US-guided interventional procedures for the superficial lesions.

      • Validation of a Palliative Prognostic Index to Predict Life Expectancy for Terminally Ill Cancer Patients in a Hospice Consultation Setting in Taiwan

        Background: The aim of our study was to assess the practical utility of the palliative prognostic index (PPI) as a prognostic tool used by nurse specialists in a hospice consultation setting in Taiwan. Methods: In total, 623 terminal cancer patients under hospice consultation care from one medical center in northern Taiwan were enrolled between January 1 and June 30, 2011. PPI was assessed by a nurse specialist at first hospice consultation and patients categorized into groups by prognosis (good, intermediate, poor). Patient survival was analyzed retrospectively to determine significance of between-group differences. Results: By PPI sum score, 37.2% of patients were in the good prognosis group, 18% in the intermediate prognosis group and 44.8% in the poor prognosis group. The death rates were 56%, 81.2% and 89.6% and median survivals were 76, 18 and 7 days, respectively. The hazard ratio was 0.19 (95% confidence interval [CI] 0.10-0.24, p<0.001) for the poor versus good prognosis group and 0.54 (95% CI 0.43-0.69, p<0.001) for the poor versus intermediate prognosis group. The sensitivity and specificity for the poor prognosis group was 66% and 71%; the positive predictive value and negative predictive value were 81% and 52%, respectively, to predict patient death within 21 days (area under the curve of the receiver operating characteristic was 0.68). Conclusions: Assessment by PPI can accurately predict survival of terminal cancer patients receiving hospice consultation care. PPI is a simple tool and can be administered by nurse members of hospice consultation teams.

      • Polypoid Lesions of the Gallbladder: A Long-Term Follow-Up of 1204 Patients

        Aims: Polypoid lesions of the gallbladder (PLG) are common, and most of them are benign. Few lesions are found malignant, but cannot be distinguished preoperatively by using common image modalities. Therefore, we compared characteristics of benign and malignant PLGs. Methods: We enrolled 1204 consecutive patients diagnosed with PLG at Taipei Veterans General Hospital during January 2004 to December 2013. Patients underwent either surgery or regular follow-up with various imaging modalities for at least 24 months. The mean follow-up duration was 72 ± 32 months. Results: Amoung our 1204 patients, 194 underwent surgical treatment and 1010 underwent regular follow-up. In addition, 73% patients were asymptomatic. The mean PLG size was 6.9 ± 7.7 (range, 0.8-129) mm; the PLGs of 337 patients (28%) grew during their follow-up periods. The majority of PLGs (90.4%) were single lesions, and 10.5% of patients had associated gallstones. The PLGs of 20.1% of surgical patients were malignant. Malignant PLGs were found in 32.4% of patients ≥50 years old and in 4.7% of those <50 years old (p < 0.001). Right quadrant abdominal pain, epigastric pain, and body weight loss were the three most common symptoms associated with malignancy. Malignant PLGs were significantly larger than benign lesions (means: 27.5 ± 18.4 mm vs. 12.3 ± 12.3 mm respectively, p < 0.001). Notably, the size of 5% of malignant PLGs was 3-5 mm, and that of 8% was 5-10 mm. The negative predictive value for gallbladder malignancy was 92.8% based on a size ≥10 mm and 100% based on a size ≥3 mm. Conclusions: Our study reassesses the PLG size that warrants more aggressive intervention. Cholecystectomy remains mandatory for PLGs >10 mm, but should also be considered a definitive diagnostic and treatment modality for PLGs with diameters of 3-10 mm.

        Predicting the surgical reparability of large-to-massive rotator cuff tears by B-mode ultrasonography: a cross-sectional study

        Purpose: This study aimed to compare the ability of B-mode ultrasonography and magnetic resonance imaging (MRI) to predict the repairability of large-to-massive rotator cuff tears (RCTs). Methods: This cross-sectional study included participants with large-to-massive RCTs who underwent arthroscopic repair. B-mode ultrasonography and MRI were conducted prior to arthroscopic repair. B-mode ultrasonography was used to evaluate the echogenicity of the rotator cuff muscle using the Heckmatt scale. Intra-rater and inter-rater reliabilities were examined for two independent physicians. MRI was used to evaluate the degrees of tendon retraction, fatty infiltration of rotator cuff muscles, and muscle atrophy. Finally, two experienced orthopedic surgeons performed surgery and decided whether the torn stump could be completely repaired intraoperatively. Results: Fifty participants were included, and 32 complete repairs and 18 partial repairs were performed. B-mode ultrasonography showed good intra-rater reliability and inter-rater reliability for assessment of the muscle echogenicity of the supraspinatus and infraspinatus muscles. The correlation coefficients between B-mode ultrasound findings and MRI findings showed medium to large effect sizes (r=0.4-0.8). The Goutallier classification of the infraspinatus muscles was the MRI predictor with the best discriminative power for surgical reparability (area under the curve [AUC], 0.89; 95% confidence interval [CI], 0.81 to 0.98), while the Heckmatt scale for infraspinatus muscles was the most accurate ultrasound predictor (AUC, 0.85; 95% CI, 0.74 to 0.96). No significant differences in AUCs among the MRI and ultrasound predictors were found. Conclusion: B-mode ultrasonography was a reliable examination tool and had a similar ability to predict surgical reparability to that of MRI among patients with large-to-massive RCTs.         <P>Three benzo[<I>d</I>,<I>d</I>′]thieno[3,2-<I>b</I>;4,5-<I>b</I>′]dithiophene (<B>BTDT</B>) derivatives, end-functionalized with benzothiophenyl (<B>BT-BTDT</B>; <B>2</B>), benzothieno[3,2-b]thiophenyl (<B>BTT-BTDT</B>; 3), and benzo[<I>d</I>,<I>d</I>′]thieno[3,2-<I>b</I>;4,5-<I>b</I>′]dithiophenyl (<B>BBTDT</B>; <B>4</B>), were prepared for bottom-contact/bottom-gate organic thin-film transistors (OTFTs). An improved one-pot [2 + 1 + 1] synthetic method of <B>BTDT</B> with improved synthetic yield was achieved, which enabled the efficient realization of new <B>BTDT</B>-based semiconductors. All of the <B>BTDT</B> compounds exhibited high performance p-channel characteristics with carrier mobilities as high as 0.34 cm<SUP>2</SUP>/(V s) and a current on/off ratio of 1 × 10<SUP>7</SUP>, as well as enhanced ambient stability.         BRCA1/2 mutation status in patients with metachronous breast and ovarian malignancies: clues towards the implementation of genetic counseling

        Objective: The characteristics of patients with metachronous breast and ovarian malignancies and the pathogenic role of BRCA1/2 mutations remain poorly understood. We investigated these issues through a review of hospital records and nationwide Taiwanese registry data, followed by BRCA1/2 mutation analysis in hospital-based cases. Methods: We retrospectively retrieved consecutive clinical records of Taiwanese patients who presented with these malignancies to our hospital between 2001 and 2017. We also collected information from the Data Science Center of the Taiwan Cancer Registry (TCR) between 2007 and 2015. Next-generation sequencing and multiplex ligation-dependent probe amplification were used to identify BRCA1/2 mutations and large genomic rearrangements, respectively. When BRCA1/2 mutations were identified in index cases, pedigrees were reconstructed and genetic testing was offered to family members. Results: A total of 12,769 patients with breast cancer and 1,537 with ovarian cancer were retrieved from our hospital records. Of them, 28 had metachronous breast and ovarian malignancies. We also identified 113 cases from the TCR dataset. Eighteen hospital-based cases underwent BRCA1/2 sequencing and germline pathogenic mutations were detected in 7 patients (38.9%, 5 in BRCA1 and 2 in BRCA2). All BRCA1/2 mutation carriers had ovarian high-grade serous carcinomas. Of the 12 patients who were alive at the time of analysis, 5 were BRCA1/2 mutation carriers. All of them had family members with BRCA1/2-associated malignancies. Conclusions: Our results provide pilot evidence that BRCA1/2 mutations are common in Taiwanese patients with metachronous breast and ovarian malignancies, supporting the clinical utility of genetic counseling.

        Effect of a vertical guide plate on the wind loading of an inclined flat plate

        Wind tunnel experiments were performed to study the wind loads on an inclined flat plate with and without a guide plate. Highly turbulent flow, which corresponded to free-stream turbulence intensity on the flat roof of low-rise buildings, was produced by a turbulence generation grid at the inlet of the test section. The test model could represent a typical solar collector panel of a solar water heater. There are up-stream movements of the separation bubble and side-edge vortices, more intense fluctuating pressure and a higher bending moment in the turbulent flow. A guide plate would result in higher lift coefficient, particularly with an increased projected area ratio of a guide plate to an inclined flat plate. The value of lift coefficient is considerably lower with increased free-stream turbulent intensity.

      • Enhanced Performance of Pseudo-Bilayer Organic Photovoltaic Devices via Small Molecule Doping

        <P>Controlling both the film crystallinity of the active layer for better charge transport and the interdiffusion between donor and acceptor materials for optimal bicontinuous networks is essential in producing pseudo-bilayer polymer solar cells. In this work, we investigated the influence of a doping solution-processable small molecule with high carrier mobility, 5,11-bis(triethylsilylethynyl) anthradithiophene (TES-ADT), on the performance of pseudo-bilayer polymer solar cells made of an underlayer of poly(3-hexylthiophene) (P3HT) and an upper layer of [6,6]-phenyl-C61-butyric acid methyl ester (PCBM). By analysis of the X-ray diffraction and UV–vis absorbance spectra of P3HT:TES-ADT blend films it was demonstrated that the film crystallinity was enhanced by TES-ADT doping in the P3HT underlayer. The hole mobility extracted from the current density–voltage curves of hole-only devices based on P3HT:TES-ADT demonstrated an optimized value with proper TES-ADT doping and thermal annealing. An intermixed photoactive layer was observed for the annealed device, indicating the occurrence of interdiffusion with a large interfacial area. With improved film crystallinity and interdiffusion, the optimal device performance was obtained when 5% TES-ADT was blended with P3HT and a thermal annealing treatment at 150 °C for 1 min was conducted. At that optimal condition, the mean crystallite size was increased by 35%, and hence the enhancement of 8% and 14% in power conversion efficiency and short-circuit current density was observed, respectively.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/jpccck/2014/jpccck.2014.118.issue-19/jp502331x/production/images/medium/jp-2014-02331x_0007.gif'></P><P><A href='http://pubs.acs.org/doi/suppl/10.1021/jp502331x'>ACS Electronic Supporting Info</A></P>
